An AI medical scribe produces a draft, not an authoritative account of an appointment. The responsible clinician should compare the draft with what happened, correct mistakes and omissions, and review it before saving or signing. If you discover an error later, tell the clinician or provider exactly what is wrong and ask for a review or amendment under that provider’s process. If the error could affect a medicine, allergy, diagnosis, treatment, or follow-up, contact the treating team promptly; use local urgent or emergency services if there is immediate danger.
Why a fluent AI note can still be wrong
Ambient scribes listen during a consultation and generate a proposed clinical note. Because the result is grammatical and well organized, it can look more reliable than it is. NHS England identifies three central risks: inaccurate wording, loss of context, and missing information. A system may struggle with complex terminology, abbreviations, rapid speech, accents, dialects, or speech impairments.

Errors are not limited to individual words. A note can change the clinical meaning by losing a negation (“no chest pain” becoming “chest pain”), reversing chronology, attaching a statement to the wrong person, or turning a hypothetical comment into a confirmed diagnosis. An omission can be just as serious as an invented detail when it removes a medication change, allergy, symptom, refusal, safety-net instruction, or follow-up plan.

The first decision: where is the note in the workflow?
The practical response depends on whether the text is still a draft or has already been placed in the official record. The distinction is important because correction is simplest before filing, while a saved record may require a provider’s formal amendment process.
| Situation | Who should act | What to do |
|---|---|---|
| Unreviewed draft | The clinician using the scribe | Compare it with the encounter, correct inaccuracies and omissions, then review before adding it to the record. |
| Draft being finalized | The clinician responsible for the encounter | Check clinically sensitive details, edit the text, and confirm that the final note represents what was actually said, decided, and done. |
| Saved or signed record | The clinician and provider’s records team | Report the exact discrepancy and ask how the organization handles review or amendment requests. |
| Error that may affect current care | The patient or care team immediately | Contact the treating team promptly rather than waiting for a routine records request; seek urgent or emergency care for immediate danger. |
Clinicians remain responsible for the accuracy of documentation even when software produced the initial text. NHS England’s professional guidance and American Psychological Association guidance both direct the professional to review and edit AI-generated documentation before it is signed or entered. Great Ormond Street Hospital describes the same local practice: the clinician checks and edits the draft and adds relevant details manually before saving.
What the clinician should check before signing
A review should be a comparison with the encounter, not a quick spellcheck. The clinician should use the conversation, examination findings, orders, prescriptions, and agreed plan to verify the draft. The following checks translate the error types identified in NHS England guidance into a practical review sequence.
Negation and clinical meaning
- Confirm every “no,” “not,” “without,” and “denies.” A missing negation can reverse the assessment.
- Check that uncertainty, possibility, and differential diagnoses remain uncertain rather than becoming established diagnoses.
- Make sure a patient’s question or hypothetical example is not recorded as a symptom, diagnosis, or treatment decision.
Medicines, allergies, and treatments
- Verify drug names, doses, routes, timing, duration, and whether a medicine was started, stopped, continued, or merely discussed.
- Check allergy status and the reaction described. Do not assume that a familiar medication name was transcribed correctly.
- Confirm referrals, investigations, procedures, and follow-up instructions, including who is expected to arrange them.
People, chronology, and attribution
- Check who made each statement: patient, parent or carer, clinician, or another professional.
- Confirm dates and sequence. A past illness, planned test, and completed test must not be collapsed into one event.
- Ensure that information from a previous encounter has not been presented as a new finding.
Completeness and context
- Look for symptoms, examination findings, risk factors, consent or refusal, safety-net advice, and follow-up details that were discussed but omitted.
- Check terms, abbreviations, names, and measurements against the source information available to the clinician.
- Ask whether a reader who was not in the room could understand the clinical reasoning and the next step.
This is a safety-oriented checklist, not a validated scoring instrument. Its purpose is to focus attention on changes that can alter care.

If you are the patient and find a mistake
1. Identify the exact passage
Write down the wording that appears to be wrong, where it appears, and the date of the encounter. Quote the relevant sentence or describe the missing detail precisely. “The note is inaccurate” is harder to investigate than “It says I am taking 20 mg, but we discussed stopping that medicine” or “It records chest pain, but I said I had no chest pain.”
2. State what happened instead
Give the correction in plain language and include the context that makes it clinically important. If something was omitted, explain what was discussed, what decision was made, and whether any action was expected. Keep the request factual; you do not need to diagnose the technical cause of the transcription error.
3. Contact the right point of care
Start with the treating clinician or the provider’s records or health-information team. Ask them to review the note and explain the organization’s process for correcting or amending records. A clinician may be able to correct a draft or add an addendum; a finalized record may follow a different procedure.

4. Keep a record of your request
Save the message you sent, the date, and any response. If the provider directs you to a particular form or department, follow that local instruction. The available guidance supports asking the provider to review and correct a discrepancy, but it does not establish one universal form, deadline, or appeal route for every country or health system.

When an error is urgent
Do not wait for a routine records request if the wrong or missing information could change a current decision. Contact the treating team promptly when the discrepancy involves:
- a medication, dose, interaction, or discontinuation;
- an allergy or previous adverse reaction;
- a diagnosis or a test result;
- a treatment, procedure, referral, or follow-up deadline;
- symptoms or warning signs that should have triggered safety-net advice.
Tell the team both what the note says and what you believe is correct, and say that you are concerned it could affect care. If you have immediate danger, severe symptoms, or an imminent medication risk, use your local urgent or emergency service rather than waiting for a records correction.
The Agency for Healthcare Research and Quality has discussed potential diagnostic-safety implications when AI-generated documentation is inaccurate, inconsistent, or biased. That does not mean every wording error causes harm; it does mean that a clinically consequential discrepancy deserves timely human attention.

There is no single amendment process that applies everywhere. Rules and procedures vary by jurisdiction, provider, record system, and type of document. A provider may correct an entry, append a clarification, preserve the original text with an amendment, or route the request through a records office. The safe assumption is that the original entry may remain visible as part of the audit history; do not expect an inaccurate note simply to disappear.

Ask the provider:
- which team reviews correction requests;
- whether the request must be submitted in writing or through a portal;
- what identifying information and encounter date they need;
- how the decision and any amendment will be recorded; and
- how the correction will be communicated to clinicians who may rely on the note.
Those questions seek the provider’s actual process without assuming that a form, deadline, or legal right is identical in every location. If you are unsure which rules apply, consult the provider, your local health-information authority, or a qualified adviser in your jurisdiction.

NHS England’s patient guidance says a patient can tell the professional at the start of an appointment that they do not want an ambient scribe. Policies and applicable rules differ, so ask how the service handles that preference. You can also request clarification during the visit if the clinician reads back an important detail or if you notice the system is capturing something incorrectly.
A refusal or request for clarification should not be treated as a substitute for reviewing the final note. It is an additional way to reduce the chance that an error enters the record.

Clinicians
The professional who uses the scribe should know when and how the draft is generated, check its accuracy and completeness, edit it before signing, and remain accountable for the final documentation. If the tool is unreliable for a particular accent, speech pattern, language, specialty, or clinical setting, that limitation should inform whether it is used and how closely the output is checked.

Organizations
NHS England’s version 3 guidance for England, published on 27 April 2025 and updated on 29 July 2026, advises organizations to assess product accuracy, context, completeness, safety processes, and reporting arrangements. Organizations should define who may use an ambient scribe, what patient information is provided, how consent or objection is handled under local rules, how drafts are reviewed, and how safety concerns are escalated.
Relevant incidents should be reported through the appropriate local channels. The exact route depends on the country and organization; the England guidance is not a universal legal rule.
Common mistakes to avoid
- Assuming fluent language means accurate content. Read for clinical meaning, not just grammar.
- Waiting for the next appointment. Escalate sooner when a medicine, allergy, diagnosis, or follow-up plan may be affected.
- Requesting a vague correction. Identify the exact text and supply the replacement wording or omitted fact.
- Claiming a universal legal entitlement or deadline. Patient-record procedures differ by jurisdiction and provider.
- Editing the patient’s copy without telling the care team. The treating team needs the correction in the clinical workflow so future decisions use the right information.
- Using an unofficial recording or transcript as the only proof. A recording may raise privacy, consent, and local-law issues; follow the provider’s process for documenting what occurred.
A concise action plan
- Decide whether the text is an unfiled draft or an already-saved record.
- Mark the exact inaccurate wording or missing detail.
- Describe what was actually said, decided, or omitted.
- Check whether the issue concerns a medicine, allergy, diagnosis, treatment, or follow-up.
- Ask the clinician or provider’s records team to review and correct it under their local process.
- If care could be affected now, contact the treating team promptly; use urgent or emergency services for immediate danger.
The central safeguard is human accountability: an AI scribe can help prepare documentation, but the responsible professional must verify the final note, and patients should raise specific discrepancies rather than relying on a polished draft to correct itself.
