What to Do When an AI Coding Tool Adds a Malicious or Vulnerable Dependency

Stop the change, determine whether the package was merely suggested, written to a manifest, installed, or executed, then contain any possible exposure before replacing it. A vulnerable dependency has a known security flaw; a malicious dependency is intentionally harmful or compromised. Either label demands investigation, but neither proves that your project was exploited.

Do not approve or merge the AI-generated change while the package identity, version, provenance, and execution status are unresolved. Preserve the evidence, check advisories and repository alerts, rotate credentials that may have been accessible, restore a known-good dependency state, and verify the result.

First establish what the AI tool actually did

An assistant can stop at a code suggestion, edit a manifest, update a lockfile, run an install command, or trigger code during a build. Those are different security events. Your first task is to place the change in the correct category before deciding how severe the incident is.

Observed state What it means Immediate response
Suggested only The package appears in chat or generated text, but no repository or machine state changed. Do not run the command. Verify the package name, project identity, version and advisories before considering it.
Written to files A manifest, lockfile, build script or configuration now references the package, but installation is unconfirmed. Pause review and preserve the diff. Search every changed file and remove the reference only after recording what was introduced.
Downloaded or installed The package manager fetched files or placed them in a local, CI or build environment. Identify the host and time window, inspect install activity and determine whether credentials or scripts were available.
Executed Install scripts, build steps, tests or application code loaded the package. Use your incident process, contain the affected environment, investigate persistence and rotate potentially exposed credentials.

Record the exact package name, version, registry or source, files changed, introducing commit, and timestamps. Keep the manifest and lockfile diff, relevant assistant transcript, package-manager output and CI logs. Cleaning up before preserving these details can make it harder to establish whether execution occurred.

GitHub’s incident guidance emphasizes that “Incident response is not a linear process.” You may return to earlier steps as new evidence appears.

Validate the package before treating the alert as fact

Confirm identity and provenance

AI systems can invent a package name, select a similarly named project, or choose an abandoned package with a familiar-looking description. Open the package registry entry and the project’s official repository. Compare the spelling, namespace, maintainer or organization, publication history and repository links with the dependency your team intended to use. A package that exists is not automatically the right package.

Check the exact version, not just the project name. Review when that version was published, whether ownership or maintainers changed unexpectedly, and whether the release is available from the registry your builds use. Compare the lockfile’s resolved source and integrity information with the manifest. If the package came from a URL, local path or private registry, verify that source separately.

Separate a vulnerability from malicious behavior

A vulnerable package contains a known security weakness that may be exploitable under particular conditions. A malicious package is intentionally harmful or has been compromised to perform actions such as stealing data, altering files or establishing persistence. A vulnerability notice does not show that an attacker used the flaw in your project. A malware alert does not show that the package ran on your systems. Your response must establish exposure rather than infer it from the label.

Check authoritative advisories and repository alerts

Search the relevant vulnerability databases and malware notices for the exact package and version. Review your repository’s dependency graph, security alerts, recent commits and code-search results for the package name. GitHub documents Dependabot malware alerts for npm repositories; do not assume that this specific alert type covers every language ecosystem or registry.

Look for fixed versions, affected version ranges, remediation guidance and the date the advisory was issued. A package can be flagged after you first installed it, so compare advisory timing with your lockfile and build history. Treat an AI-generated claim that a package is “safe” or “latest” as unverified until these checks agree.

Scope where the dependency traveled and whether it ran

Inventory repositories and artifacts

Search manifests, lockfiles, vendored directories, container definitions, build scripts and generated artifacts. Use the dependency graph to find direct and transitive paths, then identify every branch, release and project that contains the affected version. Check local workstations, shared development environments, CI runners, build agents, containers and deployment jobs that could have consumed the lockfile.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Establish a time window: when the AI change was created, when it was committed, when installation occurred, and when builds or tests ran. Review pull requests, commit history, package-manager logs and CI job records. The available evidence differs by ecosystem and logging configuration, so document what you can prove and what remains uncertain.

Determine execution, not just presence

Presence in a lockfile is not execution. Look for package-manager install events, lifecycle-script output, import or require statements, test runs, build logs and runtime traces. Check whether the package was included in a published artifact or merely cached and never used. If a package’s install script could run automatically in your environment, treat that possibility as an execution lead until logs or policy settings resolve it.

Assess credentials and persistence

Consider what the install, build or runtime process could read: environment variables, repository and organization secrets, cloud credentials, signing keys, tokens, local configuration files and mounted volumes. Do not state that a credential was stolen without evidence. If there is any possibility that a credential was exposed, GitHub’s incident guidance recommends rotating it.

Inspect suspicious workflow runs and changes to workflow files, webhooks, deploy keys, applications, self-hosted runners, scheduled jobs and unfamiliar binaries. These are persistence routes an attacker could use after package code executes. Compare current configuration with a known-good revision and preserve suspicious files for investigation rather than deleting them without a record.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Contain the change and protect affected environments

Pause approval and preserve evidence

Block the pull request or deployment and tell reviewers why the dependency is under investigation. Do not allow an automated update bot, assistant or merge rule to reintroduce the same version while the case is open. Save the package metadata, diffs, logs and alert details in the incident record.

Isolate systems when execution is possible

If the package may have executed, contain the relevant workstation, runner, container or deployment according to your organization’s incident procedure. Stop additional builds from using the affected cache or lockfile. Restrict network access when appropriate, but coordinate with the incident team so that containment does not destroy volatile evidence.

Rotate potentially exposed credentials

Revoke and replace credentials that the process could access, including repository, environment and organization secret-store values and credentials in related external systems. Prioritize tokens that can write code, publish packages, deploy infrastructure or access production data. Update dependent services after rotation and review authentication logs for use from unexpected locations or times.

Remove the unsafe dependency and restore a known-good state

Choose a safe replacement

Remove the malicious package or vulnerable version from the manifest and lockfile, or replace it with the project’s verified alternative. Pin a known-good version or commit where your ecosystem supports that practice. Do not select a replacement solely because an AI assistant recommends it; repeat the identity, provenance and advisory checks.

Keep manifest and lockfile changes consistent

Use the package manager’s supported remove or update operation so the manifest, lockfile and dependency graph agree. Inspect the resulting diff for unrelated upgrades, registry changes or newly introduced scripts. If the original lockfile is trusted, restoring it can be safer than performing a broad update; if it is not trusted, regenerate it from verified sources under review.

Reinstall from a trusted source

Clear or quarantine caches that may contain the suspect package, then reinstall using the registry and integrity controls approved by your team. GitHub recommends auditing and reinstalling dependencies from trusted sources and pinning to known-good versions or commit SHAs. Rebuild in a clean environment and run the project’s normal tests, static checks and security checks before reopening the change.

Report confirmed npm malware

For a suspected malicious npm package, follow npm’s official malware-reporting instructions. npm Docs states: “Malware is a major concern for npm Security and we have removed hundreds of malicious packages from the registry.” The statement is undated; do not treat it as a dated incident statistic. Reporting helps the registry, but it does not replace local containment, credential rotation or an assessment of whether the package executed.

Verify recovery before resuming normal development

Verification should produce evidence that the unsafe code is gone and that no related compromise remains. Complete these checks:

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • Search every relevant manifest, lockfile, dependency graph and build artifact for the package and affected version.
  • Confirm that CI runners, developer machines, containers and deployment environments no longer use the suspect cache or image layer.
  • Review repository security alerts, malware findings and secret-scanning results after remediation.
  • Examine workflow, webhook, deploy-key, runner, application and binary inventories for unauthorized changes.
  • Review authentication, package-publishing, CI and cloud logs for activity during the exposure window.
  • Rebuild from the restored dependency state and compare outputs with an expected, known-good build.
  • Continue monitoring after closure, especially when a credential was rotated or execution cannot be conclusively ruled out.

Document the decision for each uncertainty: what evidence supports “suggested,” “installed” or “executed,” which systems were checked, which credentials were rotated and why the final dependency version was accepted.

Make AI-assisted dependency changes safer by default

Require human review of package changes

Treat every AI-proposed dependency as untrusted input. Require the reviewer to confirm the package identity, intended use, exact version, license and provenance, then inspect the manifest and lockfile diff. A code review that examines only application files can miss a dependency change that carries the greatest risk.

Automate version and advisory checks

Run dependency checks in CI for every pull request and on a recurring schedule. Alert on known vulnerabilities, malware findings where your ecosystem supports them, unexpected registry sources and lockfile drift. Keep in mind that alert coverage varies by ecosystem, registry and product; document what your chosen checks can and cannot detect.

Control which packages can enter builds

Use approved-package lists, trusted registries, namespace rules and review requirements where they fit your language and release process. OWASP guidance discusses controls for package installation and recommends checking AI-suggested packages against vulnerability databases. These are policy options to adapt, not universal commands that work unchanged for every package manager.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Manage install scripts and build permissions

Review whether package lifecycle or install scripts are needed, and restrict them when your ecosystem permits. Run dependency installation and builds with the least privilege practical: isolated runners, limited network access, short-lived credentials and no unnecessary production secrets. A restriction that breaks required tooling is not a completed control, so test policy changes in representative builds.

Record a software bill of materials

Maintain dependency information in an SBOM or equivalent inventory so you can answer which projects and artifacts contain a package when a new advisory appears. Record direct and transitive relationships, versions and sources. An inventory accelerates scope analysis; it does not prove that a package executed.

How to choose investigation and prevention coverage

When evaluating repository, registry or CI security features, compare them on the dimensions that affect this incident rather than assuming one alert is comprehensive.

Evaluation axis Questions to ask
Ecosystem coverage Which languages, registries, private feeds and lockfile formats are supported?
Detection type Does the feature identify known vulnerabilities, malicious packages, exposed secrets, or only outdated versions?
Path and file mapping Can it show the affected manifest, lockfile, artifact and transitive dependency path?
Alert freshness How quickly do newly disclosed vulnerabilities or newly flagged packages appear?
Workflow fit Can results block pull requests, run in CI and export evidence to your incident process?
Registry alignment Does it inspect the registries and package sources your builds actually use?

There is no evidence here for a neutral ranking of commercial scanners. Select controls based on your repositories, package sources, CI design and response obligations, then test them with a controlled dependency change.
