What to Do When an AI Agent Makes a Destructive Mistake: A Recovery Checklist

Stop the agent first. Interrupt the run, cancel pending approvals, disable automations and block further writes or external calls. Then preserve evidence, determine the full blast radius, revoke exposed access, recover from the smallest verified known-good state, and validate it before restoring normal operations. Stopping prevents new damage; it does not undo actions already committed.

1. Freeze the incident before investigating

Use the agent’s stop or interrupt control immediately. If it is running through a queue, CI job, workflow engine or scheduled trigger, pause that system as well. Revoke approvals that have not yet been executed, disable automations that could re-run the task and block tool calls, database writes, deployments and outbound requests where your controls allow it.

Microsoft Visual Studio Code guidance is explicit: “If the agent is about to run a destructive command, deploy a change, expose sensitive information, or modify an external service, stop the request before you investigate further.” Apply that rule even when the visible error appears limited to one file. An agent may have already called a database, cloud API, ticketing system, messaging service or deployment platform.

  • Terminate the active agent run and any parallel runs using the same credentials.
  • Pause scheduled jobs, webhooks, queues and retries that could repeat the action.
  • Block additional writes and external calls without deleting logs or temporary evidence.
  • Tell the system owner and incident coordinator that the environment is frozen.

Do not assume that killing one process reverses downstream writes. A completed API request, deployment, deletion or message remains effective until a separate recovery action changes it.

2. Preserve evidence before cleanup or rollback

Capture an untouched evidence copy before restoring files, rotating credentials or deleting artifacts. Rollback can remove the clues needed to establish what happened, which account acted and whether an attacker or a second automation was involved.

Record the execution context

  • Start and stop timestamps, including the time zone.
  • The complete user prompt, system instructions, tool descriptions and approval decisions.
  • Agent, model, extension, runtime and tool versions.
  • Command history, API requests and responses, job identifiers and correlation IDs.
  • Changed-file lists, diffs, database transaction records, deployment revisions and cloud audit events.
  • Screenshots, console output, error messages and the state of affected systems.

Make the copy defensible

Export logs in their original format, record who collected each item and protect the copy from alteration. Hash exported files when your incident process supports it, restrict access to the evidence store and keep the collection time in the incident timeline. Preserve both successful and failed attempts: an unsuccessful delete or denied API call can explain why the final state differs from the agent’s plan.

3. Map the complete blast radius

Start with the agent’s permissions and tool connections, then trace every system those tools could reach. Inspect what changed, what was only attempted and the earliest known-good checkpoint for each affected asset.

Area to inspect Questions to answer
Source repositories and workspaces Which branches, commits, files, generated artifacts and configuration values changed? Were untracked files removed?
Databases and storage Were rows deleted or modified, schemas migrated, buckets emptied, snapshots removed or retention settings changed?
Deployments and infrastructure Did a build publish, a service restart, a cloud resource change or a network rule update occur?
Credentials and identity Could the agent read, copy, create or revoke keys, tokens, sessions, certificates or service accounts?
External systems Were tickets, messages, customer records, payments, vendor accounts or administrative settings changed?
Data exposure Did prompts, logs, output or outbound requests contain secrets, personal data or regulated information?

Compare audit logs with the agent transcript. The transcript describes intent; authoritative system logs establish what was actually accepted. Mark each event as committed, rejected, unknown or still executing. When records disagree, preserve both versions and have the system owner determine which source is authoritative.

4. Contain identities, secrets and high-impact access

If a secret may have been displayed to the model, written to a log, sent to an external service or included in generated code, treat it as exposed. Revoke or rotate it rather than relying on deletion from a chat or workspace. Include tokens, API keys, cloud credentials, database passwords, signing keys, cookies, sessions and service-account credentials available to the agent.

  1. Disable or revoke the agent identity and any shared account used during the run.
  2. Rotate exposed credentials at the issuing system, then update dependent applications through a controlled change.
  3. Invalidate active sessions and temporary credentials that could still be replayed.
  4. Review access logs for use of the credential between the first possible exposure and revocation.
  5. Apply temporary deny rules or network isolation if compromise cannot be ruled out.

Production deployment, bulk deletion, financial activity, administrative changes and externally visible communications are high-impact operations. They require bounded permissions, explicit human approval and an independent policy check, not merely a conversational confirmation. OWASP’s AI Agent Security Cheat Sheet recommends allowing users to “interrupt and rollback agent operations” and adding controls beyond a simple approval prompt for these actions.

5. Select the smallest safe recovery action

Choose a recovery that limits additional change while returning the affected system to a verified state. Keep the original evidence and current state available until validation is complete.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
Recovery action Best use Advantages Risks and checks
Restore a workspace checkpoint or Git commit Wrong edits, deleted files or configuration changes in code Usually fast, narrow and easy to compare with a known-good diff May omit data written outside the repository; confirm generated files, secrets and dependency locks
Roll back a deployment A bad release or infrastructure change Can return traffic to a previously validated artifact Database migrations, queues and external side effects may not roll back with the application
Restore verified data Deleted or corrupted database, object or customer data Recovers content from a protected point in time Defines a data-loss window; verify integrity, permissions, relationships and writes made after the backup
Isolate or disable a component Unknown compromise or an unsafe service Stops interaction while facts are established Can interrupt dependent services; document the owner, duration and safe fallback
Switch to a safe service or manual process Business continuity when the primary path is untrusted Maintains essential operations without reopening the suspect component Capacity, synchronization and reconciliation must be planned before reactivation

Use the earliest checkpoint that is known to be clean, not simply the most recent snapshot. If a backup was created after the agent acted, it may preserve the mistake. When data integrity is uncertain, restore into an isolated environment first and compare before replacing production.

6. Recover code and configuration safely

For a repository or local workspace

  1. Clone or copy the current affected state into a restricted location so the post-incident state remains available for comparison.
  2. Identify the last reviewed commit, tagged release or workspace checkpoint that predates the mistake.
  3. Restore that checkpoint in a separate branch or directory; do not overwrite the evidence copy.
  4. Review the diff for source files, infrastructure definitions, dependency manifests, generated output, permissions and environment configuration.
  5. Run targeted tests, linters, build checks and security scans before merging or deploying.
  6. Have an independent owner approve the recovery commit and record the decision in the incident timeline.

A clean Git commit does not prove that external state is clean. Check databases, object storage, deployment settings, secrets and third-party services separately.

For a deployment

  1. Stop further promotion and freeze the release pipeline.
  2. Identify the last deployment that passed functional and security checks.
  3. Confirm whether the agent ran migrations, provisioning scripts, data backfills or one-time jobs.
  4. Roll back the application or infrastructure artifact only after checking compatibility with the current data schema.
  5. Route a small, monitored amount of traffic to the restored version, then expand gradually after validation.

7. Restore and verify data integrity

Data recovery is more than putting files back. NIST guidance says organizations must be able to recover quickly from a data-integrity attack and trust that recovered data is accurate, complete and free of malware.

  • Restore a verified backup or point-in-time copy into an isolated environment first.
  • Compare row counts, checksums, object manifests, schemas, indexes, relationships and timestamps with the known-good baseline.
  • Check access controls, retention rules, encryption settings and audit logging.
  • Identify the data-loss window between the recovery point and the incident, and reconcile legitimate transactions that occurred in that period.
  • Scan restored artifacts and dependencies for malicious or unexpected changes.

NIST SP 1800-25 emphasizes backups, secure storage, integrity-checking mechanisms, audit logs and access controls for protecting data from corruption or destruction. NIST SP 1800-26 addresses detecting and responding to destructive events affecting databases, system files, configurations, application code and customer data.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

8. Validate before reopening access

Validation must demonstrate that the restored state is complete, correct, intact and safe. “It starts” is not enough.

Validation checklist

  • Compare restored files, schemas, permissions, dependencies and configuration values with the approved baseline.
  • Run focused functional checks for the workflows affected by the agent.
  • Confirm monitoring, alerting, backups and audit logs are operating.
  • Test authentication, authorization and secret rotation, including service-to-service calls.
  • Check that queues, webhooks, scheduled jobs and integrations will not replay harmful actions.
  • Review outputs for unexpected content, unauthorized changes or signs of malware.

Reopen access in stages. Begin with a restricted owner or test account, observe logs and metrics, then increase traffic or permissions only after the expected behavior is confirmed. Keep enhanced monitoring in place for a period appropriate to the system’s risk and data volume.

9. Escalate according to impact

Use your organization’s incident-response process when production, regulated or customer data, credentials, financial actions or multiple systems are involved. Bring in the people who can make binding decisions, not only the team that operates the agent.

  • Security: suspected credential exposure, unauthorized access, malware or cross-system activity.
  • Engineering and system owners: code, infrastructure, database and deployment recovery.
  • Privacy and legal staff: personal, regulated or contractually protected information.
  • Finance and business owners: payments, refunds, purchasing or other financial actions.
  • Communications and affected stakeholders: customer impact, service interruption or externally visible changes.

Record who authorized containment, rollback, restoration and reactivation. NIST SP 800-61 Revision 3, published in April 2025, treats preparation, detection, response and recovery as an organizational capability rather than an isolated technical task.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

10. Close the incident and prevent a repeat

Write a timeline from the first prompt or trigger through containment, recovery and validation. Include root cause, affected systems, evidence locations, decisions, data-loss window, user impact, residual risk and the person who accepted the recovery.

Turn findings into specific changes:

  • Reduce tool permissions and separate read, write, deploy and administrative identities.
  • Add approval gates and independent policy checks for destructive, financial, administrative and externally visible actions.
  • Make high-risk operations idempotent or require a dry-run preview before execution.
  • Keep immutable or access-controlled backups and test restoration on a schedule.
  • Retain detailed agent, tool and system audit trails with useful correlation IDs.
  • Define a tested rollback path for every production-connected agent.
  • Update prompts, tool descriptions, inventories and runbooks to reflect actual boundaries.
  • Run an incident-response tabletop in which an agent deletes, changes or exposes data.

OWASP SAMM calls for incident scenarios involving unintended or destructive agent actions and rapid credential revocation. The OWASP GenAI Incident Response Guide lists known-good rollback, secure replacement, isolation, enhanced monitoring, dependency review and vendor coordination as recovery options.

Decision rule for the next five minutes

  1. Stop: prevent another write, call, deployment or approval.
  2. Preserve: capture logs, prompts, versions, diffs and system state before cleanup.
  3. Scope: trace every connected repository, database, credential, deployment and external service.
  4. Contain: revoke or rotate potentially exposed access and isolate untrusted components.
  5. Recover: use the smallest action that returns the system to an earlier verified state.
  6. Prove: validate integrity, permissions, behavior and monitoring before reopening access.
  7. Learn: document the incident and improve permissions, approvals, backups and rollback tests.
