First response: contain the agent before investigating
Speed matters because an agent may continue running on a schedule, retry a failed tool call, follow instructions in newly retrieved content, or use a still-valid token. The U.S. Department of Energy’s GEAR AI Security and Safety guidance treats access to another user’s or project’s data and an unapproved agent action as incident examples. Its sequence is practical for both completed and attempted events.

Use an administrative or infrastructure control that does not depend on the model’s cooperation. Disable the job, pause the orchestration workflow, stop the container or worker, disable the connector, or remove the agent from the queue. If the agent is embedded in a business application, use that application’s emergency-disable or feature-flag control. Do not rely on a prompt such as “stop,” a model-generated confirmation, or a request that the agent reverse its own action; the model may be compromised, confused, or unable to see all of its active processes.

Record the exact time, operator and control used. If there is no tested kill path, note that as a control failure for remediation rather than improvising by deleting evidence or shutting down unrelated systems.
2. Prevent additional actions
Isolate the implicated service and its pathways. Remove network access to the affected API, revoke a connector session, disable a webhook, detach a shared drive, or place the account and job in a quarantine group. If a physical device, industrial controller or other equipment connection could be affected, have the responsible safety or operations team place it in a safe state.
Contain the smallest set of systems that can still cause harm, while preserving forensic data. A broad shutdown may prevent damage but can destroy volatile evidence or interrupt unrelated operations; a narrow isolation may preserve continuity but leave a route open. The incident owner should make that trade-off explicitly and document it.
3. Revoke or rotate credentials
Invalidate tokens, API keys, session cookies, certificates and delegated grants that the agent or its tools could use. Rotate replacement secrets through the organization’s approved secrets-management process, and update only the services that need the new values. If a credential is shared by several jobs, identify every dependent workload before revocation and prioritize a controlled replacement.
Never paste a secret into a ticket, chat transcript or incident timeline. Record a non-secret identifier, the revocation time and the system that performed it. Keep the old credential unavailable to the agent, including through cached environment variables, connection pools or long-lived workers.
4. Preserve the action trail
Collect evidence before routine cleanup, retention jobs or redeployments overwrite it. Preserve:

5. Report through the established process
Open the organization’s incident record and notify its incident-response and cybersecurity functions. Include the containment actions already taken, the suspected identity, systems, data categories and whether the event is ongoing. Escalate to privacy, legal, safety, compliance, communications or equipment owners when their criteria are met.
NIST SP 800-61 Revision 3, published in April 2025, provides general organizational incident-response recommendations and supersedes Revision 2; it does not replace procedures specific to your system. Reporting deadlines depend on the organization, contract, incident type and jurisdiction. Neither that publication nor the DOE guidance supplies one universal deadline, so follow the applicable policy and legal advice rather than guessing.
Build a precise picture of what happened
Containment answers “how do we stop more harm?” Investigation answers “what happened, what can still happen, and what must be repaired?” Assign an incident lead and a technical investigator, and use the affected system owner to authorize changes.
Identify the acting identity
Distinguish the model from the identity that had authority. Determine which service account, user delegation, workload identity, connector account or temporary token signed each request. Check whether the agent inherited a human session, assumed a role, used a shared key or reached a second system through a downstream service.
Map tools, resources and data
Create an inventory of every tool invocation and resource touched. For each item, record the requested operation, the effective permission, the result and whether data left the original boundary. Separate attempted calls from successful calls; an authorization denial is evidence of intent and a control working, while a successful read or write requires impact analysis.

Classify the data involved: public, internal, confidential, personal, regulated, proprietary or safety-critical. Identify whose data was accessed and whether it was merely displayed to the model, stored in a context window, written to a file, sent to an external service or exposed to a human. Check downstream systems for copies, exports, notifications and irreversible side effects.
Reconstruct the sequence
Align prompts, retrieved content, approvals, tool calls and system events on one timeline. Account for clock drift and asynchronous queues. Comprehensive telemetry that records tool invocations and context changes makes it possible to distinguish an instruction the agent generated from one inserted into the data it consumed.
Preserve uncertainty explicitly. If logs cannot establish whether a file was downloaded, describe it as potentially accessed and investigate the network and storage records. Do not claim that no exposure occurred merely because the agent did not produce the data in its final response.
Do these 3 things before closing this tab:
1Clear out junk files and repair common Windows errors2Scan for outdated or missing drivers - takes under a minute3Repair Windows errors before they cause bigger problemsInvestigate possible causes without jumping to a conclusion
Several mechanisms can produce the same visible failure. Treat each as an investigative lead, not a diagnosis.
Indirect prompt injection and agent hijacking
NIST’s Center for AI Standards and Innovation describes agent hijacking as indirect prompt injection in which malicious instructions are inserted into data an agent ingests, causing unintended harmful actions. A poisoned document, web page, email, ticket or database field can therefore influence behavior even when the user’s original prompt was benign.
Compare the agent’s trusted instructions with every retrieved or user-supplied item in the relevant context. Look for instructions that attempt to override priorities, request secrets, change the destination of an export, or persuade the agent to bypass approval. Do not erase the source item; preserve it for analysis and threat hunting.

Excessive agency and weak authorization
An agent may have had more tools or broader permissions than its task required. OWASP’s guidance recommends least-privilege tools, separate read and write capabilities, and authorization enforced by downstream systems rather than by the model’s judgment. A mailbox summarizer, for example, may need read access but not send or delete permission.
Check whether the tool itself verified the caller, resource, operation and target. A model deciding “this looks safe” is not an authorization boundary. Verify policy decisions in the API gateway, database, file service or device controller that ultimately performed the action.
Unsafe tool behavior or operational error
Review tool schemas, argument validation, default values, retries and error handling. A harmless-looking empty or ambiguous argument may have selected a broad scope. A retry may have repeated a write. A framework upgrade may have changed a tool name or permission mapping. Also consider a human approval mistake, an expired policy, a misconfigured connector or a compromised service account.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.Recover in the affected system owner’s process
There is no universal rollback command for an AI-agent incident. Recovery depends on the resource and on whether the action was reversible.
Validate or reverse high-impact changes
The owner of the affected system should approve restoration. Use normal backups, object versions, database point-in-time recovery, change-management records or vendor-supported reversal procedures. Before applying a rollback, preserve the current state so investigators can compare before and after versions.
Recommended Free Tools
For messages or external transactions, contact the responsible business and legal teams before attempting cancellation or notification. A compensating action may create additional records or obligations. For deleted or overwritten data, determine whether retention copies, audit archives or downstream replicas exist.
Best Value
Confirm that containment holds
Test the revoked token, disabled connector and isolation rule using a controlled request. Check that cached workers, scheduled retries and alternate credentials cannot reach the same resource. Monitor for attempted access after containment; those attempts may reveal additional compromised identities or hidden automation.
Set conditions for resumption
Document the root cause or the remaining uncertainty, the measured scope, corrective controls, data-owner decisions and required approvals. The DOE guidance states: “Do not resume until the cause, impact, corrective action, and required approvals are reviewed.” Resume in stages, beginning with read-only or low-impact operations, and watch telemetry for unexpected tool calls.
Choose containment actions by risk, not convenience
When several controls are available, compare them on four practical questions: how quickly they prevent another action, whether they revoke the exact credentials and integrations at risk, whether they preserve evidence, and whether the affected system owner authorizes recovery.
| Response choice | Strength | Risk or limitation | Use when |
|---|---|---|---|
| Pause the specific job or workflow | Fast and narrow; preserves unrelated service | Other jobs may share the same identity or connector | The execution path is known and independently controllable |
| Disable the connector or token | Stops access at the credential boundary | May interrupt legitimate workloads and require coordinated rotation | The credential or integration is clearly implicated |
| Isolate the service or network segment | Blocks multiple tool paths and retries | Can affect evidence collection and unrelated dependencies | Scope is uncertain or ongoing actions could be high impact |
| Shut down the host or equipment | Strongest immediate stop for active physical or destructive risk | May lose volatile state and create operational or safety consequences | Safety, destructive change or uncontrolled execution requires it |
Prevent a repeat incident
Constrain permissions and tools
Design each agent identity for one accountable purpose. Grant only the resources and operations required for that purpose, use separate identities for read and write tasks, and make high-impact tools unavailable by default. Enforce resource-level authorization in the downstream service, including tenant, project, record and recipient checks.
Require approval where consequences are high
Insert a human approval step before sending external messages, changing access, deleting or exporting data, moving money, modifying production systems or controlling equipment. Show the approver the exact target, arguments, affected records and expected effect. An approval that merely says “continue?” without those details is difficult to audit and easy to misuse.
Make actions attributable and observable
Give every agent and connector a distinct identity. Log the effective permissions at the time of each tool call, not only the configured role. Capture context changes, retrieved sources, policy decisions, approvals, retries and outputs in tamper-resistant storage with a defined retention period.
Test the kill path and recovery
Exercise emergency disablement in tabletop sessions and controlled tests. Verify that an operator can stop a scheduled run, revoke a token, isolate a connector and obtain the evidence needed for investigation. Test restoration with the owner of each critical system, and record which changes are reversible, which require manual reconstruction and which create external obligations.
Quick Recap
Common mistakes that make an incident worse
- Asking the agent to stop or clean up: it may lack authority, may be manipulated, and can overwrite evidence.
- Rotating one key and assuming containment: cached sessions, alternate credentials or a second connector may remain active.
- Deleting the suspicious prompt or file: removal destroys evidence and may conceal an injection path.
- Equating a quiet final answer with no exposure: data may have been read, stored or transmitted during tool use.
- Restoring before impact assessment: a rollback can erase clues, reintroduce a vulnerable configuration or trigger more side effects.
- Blaming prompt injection automatically: excessive permissions, authorization bugs, unsafe tools and human error require the same attention.
- Inventing a reporting deadline: apply the organization’s policy, contract and jurisdiction-specific rules instead of relying on a generic number.
