Dryness, redness, peeling, or stinging can happen with benzoyl peroxide, salicylic acid, topical retinoids, and isotretinoin. Irritation by itself does not usually mean you are allergic or that treatment has permanently damaged your skin. The safest response is to simplify your routine, moisturize correctly, avoid adding more irritating products, and contact the prescribing dermatologist before changing a retinoid or isotretinoin plan.
Start with a quick severity check
Pause and assess what your skin is doing rather than adding another “soothing” active. Mild tightness, flaking, or manageable redness can be a medication effect. More severe symptoms need medical advice, particularly when they do not settle after gentle care.

- Contact a dermatologist if dryness persists, becomes painful, cracks, bleeds, stings, or is very irritating.
- Arrange professional acne care for deep, painful spots or acne that is leaving scars. Those situations may need a different treatment plan, not simply more moisturizer.
- Ask a clinician to assess the diagnosis if the reaction seems out of proportion or does not fit the product’s usual effects. Irritation, an allergic reaction, and another skin condition can look similar without an examination.
Do not assume that burning means the medicine is “working better.” More irritation is not a reliable measure of acne improvement.
Why acne medicines can make skin feel worse before it feels better
Many effective acne ingredients affect the outer skin barrier while they unclog pores or reduce acne-causing inflammation. The American Academy of Dermatology (AAD) describes benzoyl peroxide, salicylic acid, topical retinoids, and isotretinoin as treatments that can dry or irritate skin. The reaction may include tightness, flaking, redness, or stinging.

| Treatment | What irritation can look like | Important next step |
|---|---|---|
| Benzoyl peroxide | Dryness, redness, peeling, or burning, especially when starting or using a stronger product. | For over-the-counter adult acne treatment, AAD recommends beginning with 2.5% to help reduce dry, irritated skin or burning. Significant irritation still warrants individualized advice. |
| Salicylic acid | Dry, tight, flaky, or stinging skin. | Use a gentle routine while irritated and avoid stacking it with other exfoliating products. |
| Topical retinoids | Dryness, peeling, redness, and sensitivity that can be most noticeable early in treatment. | Ask the prescribing dermatologist before changing the amount or schedule. |
| Isotretinoin | Marked dryness and sensitivity that can affect the face and other areas. | Follow the treating dermatologist’s plan and avoid additional irritating ingredients unless the clinician specifically tells you otherwise. |
These categories do not identify the cause of an individual reaction. Product strength, frequency, other products, the condition of your skin, and the acne diagnosis all matter.
Reset your routine while the skin settles
A short, simple routine removes common sources of extra irritation without abandoning acne care on your own. AAD advises avoiding irritating products such as toners and exfoliants, and not scrubbing with tools.
- Wash gently. Use a mild cleanser and your fingertips. Rinse with lukewarm water and pat dry rather than rubbing. Do not use a brush, rough cloth, scrub, or peeling treatment.
- Stop the extras. Put toners, facial scrubs, exfoliating acids, and newly added active ingredients aside while the reaction is active. Trying several new acne treatments too often can irritate skin further.
- Moisturize immediately. Apply an acne-friendly moisturizer after washing, while a little water remains on the skin. This helps retain water and can make an acne treatment easier to tolerate.
- Keep the routine stable. Do not introduce multiple replacement products at once. If your skin improves, reintroduce only what your treatment plan requires and change one variable at a time.
A gentle routine does not mean using a heavy, pore-clogging product. The label guidance below helps you choose a formula that supports comfort without deliberately adding comedogenic ingredients.

Choose a moisturizer that supports acne treatment
Look for “oil-free,” “non-comedogenic,” or “won’t clog pores” on the package. These terms are useful screening labels for acne-prone skin, although they do not guarantee that every formula will suit every person.
- Apply after cleansing while damp: leave a small amount of water on the skin, then spread moisturizer over the face.
- Use it whenever skin feels dry: AAD recommends moisturizer during acne treatment, not only after a reaction becomes severe.
- Choose a texture you can use consistently: a lotion, cream, or gel may all be reasonable if it is comfortable and does not worsen congestion for you.
- Keep the rest of the formula uncomplicated: when skin is stinging, avoid experimenting with fragranced or exfoliating products. People taking isotretinoin should specifically avoid fragrance and dyes as advised by AAD.
Moisturizer is supportive care, not a replacement for discussing medication side effects with the prescriber. In studies cited by AAD, most patients saw less acne after four to eight weeks while following a complete plan that included dermatologist-selected medication, cleanser, and moisturizer. That time frame is not a guarantee that irritation will resolve in four to eight weeks, and it does not show that moisturizer alone treats acne.
Medication-specific decisions
Topical retinoids: ask before changing the schedule
If a retinoid is causing dryness or redness, contact the dermatologist who prescribed it before changing the amount or frequency. AAD notes that clinician-guided adjustments can include using a smaller amount, applying it less often, or putting moisturizer on first. The appropriate adjustment depends on the specific retinoid, your acne, and the severity of the reaction.

Benzoyl peroxide: reduce unnecessary exposure
For someone starting an over-the-counter benzoyl peroxide product for adult acne, AAD recommends 2.5% as a starting strength to help avoid dry, irritated skin or burning. That general starting point does not override individualized instructions, and it is not a reason to continue a product that is causing substantial pain, cracking, bleeding, or persistent stinging.
Do not layer benzoyl peroxide with several other new actives while your skin is already inflamed. Ask a clinician or pharmacist how your particular product should fit with a prescription regimen.

Salicylic acid and other exfoliating products
Salicylic acid can be useful for acne but may add to dryness when combined with a retinoid, benzoyl peroxide, scrubs, or other exfoliants. During an irritation flare, remove nonessential exfoliating steps and ask the prescriber which acne treatment should remain. Avoid using a rough tool to “remove” peeling skin; that can increase irritation.
Isotretinoin: follow the treating dermatologist’s plan
Isotretinoin requires the closest adherence to the treating dermatologist’s instructions. AAD recommends a gentle cleanser and moisturizer and advises avoiding additional potentially irritating ingredients, including topical retinol, benzoyl peroxide, sulfur, exfoliants, fragrance, and dyes. Do not add or stop products independently if the clinician gave you a specific skin-care plan; contact the practice for an adjustment.

AAD advises people using topical retinoids to seek shade, wear sun-protective clothing, and apply a broad-spectrum, water-resistant sunscreen with SPF 30 or higher to uncovered skin. Sun protection is especially practical when the skin is already dry or irritated, because additional exposure can make discomfort harder to manage.

- Apply sunscreen to uncovered areas as the final morning skin-care step.
- Use shade and clothing as well as sunscreen rather than relying on one measure alone.
- If a sunscreen stings, stop testing multiple new actives at the same time and ask a clinician or pharmacist for a better-tolerated option.
What not to do when skin is irritated
- Do not scrub away flakes. Peeling is not dirt that needs to be removed with a brush or abrasive cloth.
- Do not add several “calming” products at once. A new ingredient can be another source of irritation and makes it difficult to identify the problem.
- Do not switch a prescription retinoid schedule without guidance. Ask the prescriber about a smaller amount, less frequent use, or moisturizer-first application.
- Do not use toothpaste as an acne treatment. AAD dermatologist Julie Harper, MD, FAAD, advises relying on clinically proven acne ingredients instead; toothpaste can irritate facial skin.
- Do not interpret severe burning as a required step. Pain, cracking, bleeding, or persistent stinging is a reason to seek advice.
A practical troubleshooting path
If symptoms are mild and improving
Continue the gentle cleanser, damp-skin moisturizer, and prescribed plan as directed. Keep toners, scrubs, and extra exfoliants out of the routine. Monitor whether comfort improves rather than trying to accelerate acne treatment.
If symptoms are mild but not improving
Contact the prescribing office or a dermatologist. Bring the product names, strengths, application schedule, start date, and a list of every other skin-care product you use. This information helps the clinician distinguish medication irritation from an allergy or another condition and decide whether the regimen needs adjustment.
If symptoms are painful or severe
Seek dermatology care for painful, cracked, bleeding, stinging, or very irritating dryness. The same applies when acne is deep, painful, or scarring. A clinician can decide whether to modify treatment, diagnose another skin problem, or provide a different approach rather than leaving you to experiment.
Do these 3 things before closing this tab:
1Scan for outdated or missing drivers - takes under a minute2Clear out junk files and repair common Windows errors3Fix the driver behind crashes, sound loss and screen glitchesQuestions to take to your dermatologist
- Which product is most likely causing the dryness or redness?
- Should I use a smaller amount, apply it less often, or moisturize first?
- Which active ingredient should I pause, and which part of the acne plan should continue?
- Is my cleanser or moisturizer appropriate for this reaction?
- Could this be an allergic reaction or another skin condition rather than expected irritation?
- Does my acne need a different treatment because it is deep, painful, or leaving scars?
Bottom line for irritated acne-prone skin
Use a gentle cleanser, stop scrubs and unnecessary actives, and apply an oil-free, non-comedogenic moisturizer while your skin is damp. Protect uncovered skin with shade, clothing, and broad-spectrum water-resistant SPF 30 or higher when using a topical retinoid. Ask the prescriber before changing a retinoid regimen, follow the treating dermatologist’s plan for isotretinoin, and obtain professional care when dryness persists or becomes painful, cracked, bleeding, stinging, or severe.
