Preserve the cycle records, investigate the indicator and sterilizer, repeat the biological test according to the applicable protocol, and keep the unit out of service until required challenge cycles pass. A single positive steam biological indicator does not automatically require recall of every non-implantable item under the U.S. Centers for Disease Control and Prevention (CDC) general recommendation, but it always requires prompt investigation and documented follow-up.
The exact response depends on whether the load contained implants, whether the result is an isolated positive or part of a pattern, the type of sterilizer and cycle, and your facility’s policy, manufacturer instructions, and local requirements. The sequence below reflects CDC guidance for healthcare and dental settings.

First actions after a positive biological indicator
- Take the sterilizer out of service. CDC Table 12 states: “Take the sterilizer out of service. Notify area supervisor and infection control department.” Attach an out-of-service notice and prevent additional loads until the responsible team authorizes a defined testing pathway.
- Notify the responsible people. Contact the area supervisor, infection-prevention or infection-control department, central processing leadership, and maintenance or the sterilizer manufacturer when technical help is needed.
- Quarantine implantable items. Do not release or use an implantable item from a load associated with a positive indicator while the investigation is pending. CDC recommends a biological indicator for every load containing implantables and, when possible, holding those items until the indicator is negative.
- Secure the records and identify affected loads. Preserve sterilizer and cycle numbers, load identification and contents, exposure time, temperature and pressure, operator, and mechanical, chemical and biological monitoring results. These records allow the team to determine which loads may need retrieval.
- Review the written operating procedure and repeat the test promptly. CDC dental guidance calls for an immediate repeat using the same cycle that produced the positive result. This immediate repeat is an investigation step; it is not the same as the three-cycle challenge used to verify a repaired or inspected sterilizer before return to service.
Decide what to do with processed items
Implantable items
Keep implantable items quarantined until the relevant biological indicator result is negative and the facility’s release criteria are met. If an implant was used before the result was known, notify infection prevention and risk management immediately so they can document the event and determine patient-specific follow-up.

Non-implantable items after one positive steam test
CDC’s general steam recommendation does not require automatic recall of every non-implantable item after one positive spore test when there is no evidence that the sterilizer or procedure is defective and cycle settings were appropriate. The positive result still requires investigation, prompt repeat testing, and a documented disposition decision. Do not release items simply because the mechanical printout looked normal; each monitoring method answers a different question.

When repeat positives or a defect are found
If additional biological tests remain positive, or the investigation identifies an incorrect cycle, equipment malfunction, inadequate steam, or another process defect, treat supplies processed since the last acceptable negative biological indicator as potentially nonsterile. Recall and reprocess suspect loads according to facility policy. CDC dental guidance recommends, when possible, recalling, rewrapping and resterilizing suspect items dating back to the last negative biological indicator. The responsible infection-control professional should document the date range and the rationale.
Non-steam methods
Do not transfer the steam decision rule to chemical-vapor, dry-heat, ethylene-oxide, hydrogen-peroxide or other sterilization methods. CDC recommendations differ by method, and the indicator labeling, sterilizer instructions and facility policy govern the investigation and recall pathway.
If an item was already used
When an instrument cannot be retrieved, the infection-control professional should assess the risk with central processing, surgical services and risk-management staff. Review the mechanical and chemical indicators, sterilizer parameters, cycle and load records, cleaning quality, and the circumstances of use. CDC notes that steam sterilization has a substantial margin of safety when instruments were properly cleaned and the required temperature was achieved, and reports no published studies documenting disease transmission through a nonretrieved surgical instrument after a positive biological-indicator cycle. That evidence supports a structured risk assessment; it is not permission to ignore the failed test or to skip corrective action.

Investigate the sterilizer and the indicator
Confirm operation and cycle settings
- Compare the selected cycle with the load type and the sterilizer’s instructions.
- Check programmed and recorded exposure time, temperature and pressure.
- Look for aborted cycles, unusual alarms, interrupted power, door or gasket problems, and loading errors that could block steam contact.
- Check that packs were prepared, positioned and loaded as required, without overloading the chamber.
Check steam and utilities
Investigate steam quality and delivery, including electrical irregularities and changes in the steam supply. CDC’s steam protocol specifies standard steam of at least 97% steam and less than 3% moisture. Maintenance staff or the manufacturer should evaluate supply lines, drains, valves, chamber condition and any utility abnormality. Report findings and repairs in the sterilizer record.
Verify the biological indicator system
- Confirm that the indicator is FDA-cleared for the sterilizer type and cycle and is within its expiration date.
- Check storage, transport and handling conditions and make sure the indicator was incubated and read at the time and temperature specified by its labeling.
- Confirm that the correct control indicator was processed and that the result was interpreted correctly.
- Investigate possible contamination or damaged packaging.
A suspected false positive should not be labeled false until the entire process has been investigated. CDC discusses improper testing, faulty indicators, contamination, storage problems and resistance variation as possible contributors. When a product issue is plausible, paired indicators from different manufacturers can help the investigation, provided both are compatible with the cycle.
Repeat testing and decide whether the unit stays closed
Immediate repeat
Repeat the biological indicator promptly using the same cycle that produced the positive result, following the indicator’s instructions and your facility protocol. Record the repeat result, the control result, the cycle parameters and who performed the test. A negative immediate repeat does not erase the original event; the supervisor and infection-control lead still need to determine whether a procedural or equipment problem existed.

Persistent positive results
If the repeat remains positive, keep the sterilizer out of service. Request equipment inspection, check steam supply lines, discuss the abnormality with the manufacturer, and consider testing with a biological indicator from another manufacturer. Do not resume routine processing while troubleshooting is incomplete.
Challenge after inspection or repair
CDC dental guidance describes re-challenging a sterilizer after a repeat positive with three consecutive fully loaded chamber cycles. The exact cycle or cycles must match the equipment and the facility’s written protocol. The sterilizer remains out of service until inspection or repair is complete and the required challenge results are acceptable.
CDC cycle-verification pathway
CDC separately describes verification after a failure using three consecutive empty steam cycles with biological and chemical indicators in an appropriate test pack or tray. Test each type of cycle separately. For a prevacuum or dynamic-air-removal sterilizer, also run three consecutive Bowie-Dick tests. Do not return the unit to use until the biological indicators are negative and the chemical indicators show the correct endpoint. Because CDC materials describe both empty-cycle verification and dental full-load challenge wording, use the pathway that applies to the sterilizer, cycle, and facility policy rather than combining them into an improvised rule.

Understand what each monitoring test tells you
| Monitor | What it evaluates | How often CDC recommends it | What it cannot prove |
|---|---|---|---|
| Mechanical | Recorded time, temperature and pressure during each cycle | Every load | It cannot demonstrate that resistant spores were killed or that every pack received adequate steam contact. |
| Chemical indicator | Exposure to one or more specified process conditions; the endpoint depends on the indicator type | Every load | It is not a substitute for a biological indicator and does not by itself prove sterility. |
| Biological indicator | Process lethality using resistant spores | At least weekly, and every load containing implantables | It does not replace correct cleaning, packaging, loading or cycle operation. |
| Bowie-Dick test | Air removal and steam penetration in a dynamic-air-removal or prevacuum sterilizer | As required by the applicable verification protocol | It is not a biological indicator test and cannot substitute for spore testing. |
Steam biological indicators contain spores of Geobacillus stearothermophilus, formerly called Bacillus stearothermophilus. Their resistance makes them a direct challenge to the sterilization process. Chemical indicators are useful exposure checks, but a passing color change cannot overrule a positive biological indicator.

Return to service only after documented clearance
The sterilizer can return to routine use only when the applicable inspection, repair and verification requirements are complete. The release record should identify:
- the cause found, or the conclusion that no cause was identified after investigation;
- maintenance or manufacturer work performed;
- the sterilizer and cycle types challenged;
- each biological-indicator result and control result;
- chemical-indicator endpoints and Bowie-Dick results when applicable;
- the person who reviewed the evidence and authorized return to service.
Follow the sterilizer manufacturer’s instructions, indicator labeling, infection-prevention policy and local regulatory requirements if they are more specific or more restrictive than the general CDC pathway. A unit should not be released merely because it has been idle, because one repeat test was negative, or because the mechanical record appears normal.
Quick Recap
Common mistakes to avoid
- Using an implant load while waiting for results: quarantine it until the biological indicator is negative and release criteria are met.
- Automatically recalling every item after one positive steam test: CDC distinguishes an isolated positive with no identified defect from persistent positives or a confirmed process failure.
- Calling a result a false positive too early: investigate testing technique, storage, contamination, controls, equipment and steam delivery first.
- Confusing Bowie-Dick with spore testing: Bowie-Dick evaluates air removal; it does not replace a biological indicator.
- Mixing challenge protocols: immediate same-cycle repeat, three fully loaded dental challenge cycles, and three empty verification cycles serve different purposes.
- Releasing a repaired unit without chemical evidence: CDC’s cycle-verification description requires negative biological indicators and chemical indicators showing the correct endpoint.
Quick decision guide
| Situation | Immediate disposition | Next action |
|---|---|---|
| One positive steam indicator; no implantables; no known defect | Stop the sterilizer, investigate and document; do not automatically recall every non-implantable item under CDC’s general recommendation. | Repeat promptly using the same cycle and review records and operating conditions. |
| Any implantable item in the positive load | Quarantine and do not use until the indicator is negative and release criteria are met. | Notify infection control, identify related loads and follow the facility’s implant policy. |
| Repeat positive or confirmed equipment/process defect | Treat items since the last acceptable negative indicator as potentially nonsterile; recall and reprocess as applicable. | Keep the sterilizer out of service, inspect or repair it, and complete the required challenge cycles. |
| Item already used and cannot be retrieved | Do not conceal the event or assume transmission occurred. | Have infection prevention, processing, surgical and risk-management staff perform and document a patient-risk assessment. |
