If someone may be in immediate danger or is seriously injured, call the emergency service for their location. If they are safe for the moment, stay calm, ask what they need, and help them reach medical care or sexual-assault support without taking decisions away from them.
What to do first if they may be in danger
Check whether the person is safe right now and whether they need urgent medical help. If there is an immediate threat or a serious injury, contact local emergency services. Do not approach a suspected assailant or enter a place if doing so could put you or the survivor at risk; get emergency help instead.

How to support them if they are safe for now
Offer safety and presence
Ask simple, practical questions: “Are you somewhere you feel safe?” and “Do you want me to stay with you?” You can also ask whether they would like help contacting someone they trust or getting to a safer place. Offer options, rather than making arrangements on their behalf without consent when they are able to choose.

Listen without pressing for details
Believe them and let them decide what to tell you. You do not need to investigate what happened. Avoid questions such as why they did not resist, leave, or report. NHS advice for friends and relatives is to listen without asking for details and respect the person’s decisions: Help after rape and sexual assault.
Offer specific practical help
Ask if they want company while they contact a medical provider, specialist service, or trusted person. They may want help making a call, arranging transport, or simply having someone sit with them. A supporter can help with the next step without deciding what that step must be.

Encourage medical care and specialist support
Medical care can address injuries and other health concerns, including infection and pregnancy concerns. Injuries may not be visible, so a person can benefit from speaking with a qualified medical provider even if they do not appear hurt. Specialist sexual-assault services can explain local options for medical care, forensic examination, and emotional support. In England and Wales, Sexual Assault Referral Centres (SARCs) provide crisis care and may offer medical and forensic examinations, emergency contraception, STI testing, and onward emotional support. Availability and procedures vary. The UK government describes these services and related routes in its support guidance for victims of sexual violence and abuse.

Discuss a forensic exam only if it matters to them
A forensic exam may be an option if the survivor wants one; it is not something they must agree to in order to receive medical care or support. A specialist service can explain what an exam involves and what choices are available where they live. RAINN provides US information about the Sexual Assault Forensic Exam (SAFE).
Do not tell someone that washing or changing clothes means it is too late to seek help. NHS guidance says a person can still seek police help after washing; a specialist service can advise about local options and evidence preservation: NHS guidance after rape and sexual assault.

Respect their choices about reporting
Do not pressure the person to report to police or promise that a service will keep everything confidential. In England and Wales, government guidance says SARC and Independent Sexual Violence Adviser (ISVA) support can be accessed without making a police report. Rules, exceptions, and available routes depend on location and circumstances; a local specialist service can explain them. Keep offering support after the first conversation
Check later what kind of contact feels welcome: a message, practical help, or space. Ongoing safety, emotional needs, and access to further support can matter beyond the immediate crisis. The World Health Organization’s 2014 clinical handbook says: “There are simple ways that every health-care provider – including those who are not specialists – can assist a woman subjected to violence.” Its guidance is framed for women and does not directly address men or people under 18: WHO clinical handbook.
