Don’t assume new or worsening weakness is simply part of getting older. If it is sudden, affects one area, has no clear cause, persists, or follows a viral illness, contact a healthcare provider rather than trying to fix it with exercise alone. Once possible medical causes have been considered, a plan adapted to your health can include gradual strength work, aerobic and balance activity, and attention to nutrition.
Why muscle weakness deserves a checkup
Age-related muscle loss is one possible explanation, but weakness can have other causes. Sarcopenia describes a decline in muscle mass, strength, and function; it may be associated with fatigue, lower energy, or difficulty standing, walking, or climbing stairs. Symptoms alone cannot establish that diagnosis. Chronic illness, poor nutrition, and inactivity can also be relevant factors. The National Institute on Aging (NIA) staff scientist Eric Shiroma, Sc.D., advises: “If you or a family member is feeling general weakness, talk with a doctor.” NIA: How can strength training build healthier bodies as we age?

The NIA reported in 2022 that about 30% of adults over 70 have trouble walking, getting up from a chair, or climbing stairs, in the context of findings from its Baltimore Longitudinal Study of Aging. That figure describes those functional difficulties; it does not mean that every person experiencing them has sarcopenia. NIA
When to contact a healthcare provider
Contact a provider about weakness that is sudden, limited to one area, unexplained and persistent, or occurs after a viral illness. Unexplained or long-term muscle loss also merits an appointment. MedlinePlus: Weakness

A clinician can ask when the weakness began, whether it is changing, and whether you have had a recent illness, other symptoms, or medication changes. They may examine you and decide whether tests are appropriate. If weakness is affecting your balance or you have had falls, mention that and ask whether your medicines should be reviewed; do not stop or change a medicine on your own. Sarcopenia, balance or gait problems, and certain medication effects can all contribute to fall risk. NIA: Falls and Fractures in Older Adults
Build activity around your ability and health
For adults 65 and older, CDC guidance recommends at least 150 minutes of moderate-intensity aerobic activity each week—or 75 minutes of vigorous activity or an equivalent mix—plus muscle-strengthening activity on at least two days and balance activity. These are general targets, not a requirement to begin at full volume: if you cannot meet them, CDC advises being as active as your abilities and conditions allow. Check with a healthcare provider about appropriate activity if you have a chronic condition, have been inactive, or are unsure what is safe for you. Resistance bands are an option, not a treatment or a prerequisite for getting stronger. CDC; MedlinePlus

Include aerobic, balance, and flexibility activities
Strength is only one part of an activity plan. Brisk walking is a CDC example of moderate aerobic activity. MedlinePlus groups older-adult exercise into endurance, strength, balance, and flexibility categories. Activities such as tai chi or water aerobics may combine activity types; if pain, limited mobility, falls, or a health condition makes choosing difficult, ask a healthcare professional about adapting the plan. CDC; MedlinePlus
Movement can support practical functions such as rising from a chair or bed, doing household chores, and getting around a neighborhood. Exercise may help protect against age-related declines in muscle mass, strength, and function, but it is not a way to diagnose the cause of weakness or a guaranteed cure. Talk about nutrition as part of the plan
Poor nutrition may contribute to the odds of sarcopenia, but there is no single protein target or supplement recommendation that applies to everyone. Some older adults need more protein, while needs and circumstances vary. Appetite changes, medication effects, chewing or swallowing difficulties, and limited resources can make eating well harder. Tell a clinician or registered dietitian if you are eating less than usual, losing weight, or having trouble getting enough food; do not assume a supplement will address unexplained weakness.
