If you may have been exposed to someone with plague, contact a healthcare provider and your local or state health department promptly. If you have symptoms that could fit plague, seek medical attention immediately and tell the clinician about the possible exposure. Being nearby does not by itself establish that you had a qualifying exposure; the type, closeness and duration of contact matter.
What to do now
- Call a healthcare provider and local or state public-health department. Explain that you may have been near a suspected plague case and ask what to do next. The CDC says health departments should be notified immediately when plague is suspected. They can help assess the encounter and advise on monitoring or preventive treatment.
- If you feel ill, seek immediate medical attention. Call ahead if practical so the facility can prepare. Clearly say that you may have had plague exposure and describe when and how it happened. Do not wait to see whether symptoms worsen.
- Write down the exposure details. Note the time and place, approximate distance and duration, whether the person was coughing, any protective equipment you wore, and any contact with body fluids, tissue, fleas or animals. Also note symptoms and relevant health information such as pregnancy, medication allergies and existing conditions.
- Follow individual instructions. Do not start leftover antibiotics or decide on isolation based only on proximity. A clinician or public-health official should determine whether your contact warrants monitoring, preventive antibiotics or other steps.
What counts as a possible exposure?
Plague does not spread between people through ordinary proximity in the same way as many common airborne infections. Person-to-person spread is associated with respiratory droplets from someone with pneumonic plague, generally during direct, close contact. CDC clinician recommendations identify close contact—less than 6 feet—that is sustained and occurred without adequate protective equipment as an exposure for which preventive antibiotics may be considered. That definition is an assessment factor, not a self-scoring rule.

Other routes matter too. Plague can spread through bites from infected fleas, and handling infected animal tissue or contact with infected body fluids can pose a risk. If your encounter involved a sick or dead animal, a flea bite, or time in an area where plague occurs, report that as well. The CDC describes plague in parts of the western United States and in other endemic areas; baseline risk is not the same everywhere.
Tell the health department or clinician whether the suspected case was believed to have pneumonic plague, whether they were coughing, how close you were and for how long, and what protection you used. Also report direct contact with infectious material, animal handling, possible flea bites and any relevant laboratory exposure. These details help professionals assess risk; no single detail lets you rule exposure in or out on your own.

Symptoms that need urgent attention
Plague symptoms can resemble other illnesses, so do not try to diagnose yourself. Seek immediate medical attention for symptoms that could fit plague, and disclose the possible exposure.
- Bubonic plague: fever, chills, weakness and a painful, swollen lymph node.
- Pneumonic plague: fever, cough, chest pain or shortness of breath; sputum may be bloody or watery.
- Septicemic plague: fever, chills, weakness or abdominal pain; severe illness can involve shock, bleeding or tissue damage.
CDC describes the usual incubation period as 2 to 8 days for bubonic plague and 1 to 3 days for pneumonic plague. The timing for septicemic plague is less well defined and is thought to be within days. These are typical ranges, not a way to determine whether an individual is infected.

- USE FOR: Relief from certain non-harmful particles and annoying odors
- KIT INCLUDES: One facepiece and one pair of replaceable organic vapor/P100 combination cartridges (#60921)
- 3M BAYONET CONNECTION: Makes it compatible with a wide range of 3M cartridges and filters
Monitoring and preventive antibiotics
Monitoring
CDC’s contact handout advises close contacts of someone with pneumonic plague to take their temperature twice daily for seven days and see a physician if fever develops. It advises people with some, but not close, contact to watch for fever or cough and contact a healthcare provider and state or county public-health department if symptoms appear. Follow any current instructions from your health department or clinician if they differ.
Preventive antibiotics
CDC recommends that clinicians consider post-exposure prophylaxis for certain defined exposures, including close, sustained contact with a pneumonic-plague patient without adequate protective equipment, accidental laboratory exposure, and some close or direct contacts with infected animals. The recommended course is seven days, but whether it is appropriate and which medicine to use depend on professional assessment and individual factors such as allergies, pregnancy, age and health conditions. Do not take antibiotics without that assessment.

- NIOSH APPROVED: For at least 95 percent filtration efficiency against certain non-oil based particles
- COMPATIBLE: With a variety of protective eyewear and hearing protection
- ADJUSTABLE NOSECLIP: Reduces potential for eyewear fogging and helps provide a custom and secure seal
- TWO-STRAP DESIGN: With welded dual point attachement that helps provide a secure fit
- APPLICATIONS: Gringing, sanding, sweeping, bagging, and other dusty operations. Metalworking, chemical processing, construction, and metal processing
Isolation
The CDC materials cited here do not establish one universal self-isolation rule for every asymptomatic person who was merely nearby. Ask public-health officials what precautions apply to your specific contact, and follow their directions rather than assuming that distance alone settles the question.

For future outdoor activities where flea exposure is possible, CDC advises using repellent according to its label; it identifies DEET products for skin and clothing and permethrin products for clothing. Wear gloves when handling animals that may be infected, and ask local authorities how to handle a dead animal. These are prevention measures, not treatment for a possible exposure. CDC says no plague vaccine is available in the United States.

- NIOSH-APPROVED N95 for at least 95 percent filtration efficiency against certain non-oil-based particles and aerosols
- USE FOR: Sanding, drywall cutting, sweeping, sawing, woodworking, fiberglass insulating, yard work, grinding and dusting
- 3M ADVANCED ELECTROSTATIC MEDIA enhances the capture of airborne particles and allows for easy breathing
- 3M COOL FLOW VALVE helps release warm, exhaled breath from inside the respirator
- UP TO 50% EASIER BREATHING Compared to 3M 8200
Plague can be severe, but severity statistics are not a personal estimate of the chance that a particular encounter caused infection. The World Health Organization reports an untreated bubonic-plague case-fatality ratio of 30% to 60%; it says untreated pneumonic and septicemic plague are fatal and that early antibiotic treatment is effective. If you have possible symptoms, prompt medical assessment matters.
