Contact a healthcare professional or local public-health authority promptly for advice specific to the suspected infection and how the exposure happened. There is no single protocol for every disease: the right next step depends on the pathogen, exposure route and timing, symptoms, vaccination or immunity, personal health risks, and local rules. If symptoms are severe or rapidly worsening, seek urgent medical care and explain the suspected exposure.

What should you do first?

  1. Get urgent help for severe or rapidly worsening symptoms. Call emergency services or seek urgent medical care, and tell the person assessing you what exposure you suspect and when it happened.
  2. Contact a clinician or public-health authority promptly. Do not wait for symptoms if the suspected exposure might have a time-sensitive intervention. If the exposure happened at work, report it through your employer’s occupational-health or exposure-reporting process as well as seeking clinical care.
  3. Write down the details. Record the suspected disease or source, the date and time, how the exposure happened, and its duration or intensity if known. Note any symptoms, relevant vaccinations or immune status, and health conditions that may affect your risk.
  4. Ask for a specific plan. Find out whether you need testing now or later, what symptoms to monitor and for how long, what precautions to take around others, and whether medication or vaccination is appropriate.

Do not take leftover prescription medicines or start post-exposure prophylaxis (PEP) on your own. A clinician needs to assess the exposure and decide whether any medication or testing is indicated.

What if blood or body fluids were involved?

For a needlestick, puncture, cut, or splash, take immediate first-aid steps and seek prompt medical evaluation. CDC’s NIOSH guidance for healthcare workers advises washing punctures and cuts with soap and water; flushing splashes to the nose, mouth, or skin with water; and irrigating the eyes with clean water, saline, or sterile irrigant. Report the incident and immediately seek medical care to assess the risk.

For a workplace exposure, use the employer’s reporting and occupational-health channels promptly. CDC occupational guidance describes exposure evaluation, assessment of the source, testing when needed, counseling, follow-up, and prophylaxis or treatment when indicated. A clinician can advise which steps fit your circumstances.

When is HIV PEP time-sensitive?

A possible HIV exposure needs rapid clinical assessment. CDC describes HIV exposure as a medical emergency and says to seek PEP evaluation rapidly when care is sought within 72 hours. If nonoccupational PEP is indicated, CDC’s 2025 recommendations say the first dose should be given as soon as possible; treatment should not be delayed while the source’s HIV status is investigated. The 2025 U.S. Public Health Service occupational guideline also emphasizes prompt reporting and PEP initiation when indicated.

These recommendations make prompt assessment important; they do not mean that every suspected exposure qualifies for PEP or that it will be prescribed. Do not wait for symptoms or for source testing before seeking care.

Does every exposure call for the same test or follow-up?

No. A test’s usefulness and timing depend on the infection, the exposure, and the method used. A test taken too early may not answer the question, so ask the clinician whether testing is useful now and whether follow-up is needed.

Example What the guidance says Important limit
Possible HIV exposure Seek rapid PEP assessment when care is sought within 72 hours; if nonoccupational PEP is indicated, CDC’s 2025 recommendations say to give the first dose as soon as possible. This is a prompt-assessment window, not a guarantee that PEP is appropriate or will be prescribed.
Occupational HCV exposure in healthcare personnel CDC’s 2020 occupational guidance recommends baseline testing of the exposed healthcare worker as soon as possible, preferably within 48 hours. In the specified cases requiring follow-up, it recommends HCV RNA testing at 3–6 weeks and later testing at 4–6 months. These intervals apply to the occupational guidance and should not be treated as a universal schedule for everyone exposed to hepatitis C. The cited guidance does not recommend HCV PEP.
Recent COVID-19 exposure CDC says testing may help people decide what to do next, including steps to reduce spread and seeking treatment if at risk for severe illness. A COVID-19 test does not establish whether you were exposed to a different infectious disease.

Should you isolate or avoid work after exposure?

Do not assume that every exposure requires isolation, work restrictions, a fixed monitoring period, or preventive medication. Those decisions depend on the suspected infection, setting, symptoms, and local public-health instructions. Ask the clinician or public-health authority what precautions apply to your specific situation and for how long.

For respiratory viruses, CDC guidance for healthcare personnel says antiviral PEP is generally not routine after an exposure, although it may be considered in certain circumstances, such as risk of severe disease or an outbreak. That is healthcare-worker guidance, not a blanket rule for the public.

What information helps a clinician assess the exposure?

  • Route: for example, respiratory contact, blood or body-fluid splash, needlestick, bite, or contaminated food or water.
  • Timing and circumstances: when it happened, how long it lasted, and how intense it may have been.
  • Source information: the suspected disease or source, if known, and any available test or diagnosis information.
  • Your health context: symptoms, relevant vaccination or immunity, and health conditions or other factors that may affect your risk.

There is no reliable single risk figure for an unspecified exposure. The assessment depends on these details, the pathogen, and the relevant local guidance.
