What to do if you think you have dengue: symptoms, testing, and when to seek care

If dengue is possible, contact a healthcare professional promptly, explain where you have recently lived or traveled, and say exactly when symptoms began. Do not wait for a test result before seeking help. Go to a clinic or emergency department immediately for belly pain or tenderness, repeated vomiting, bleeding from the nose or gums, blood in vomit or stool, or unusual extreme tiredness, restlessness, or irritability. These warning signs can start after the fever falls.

Until you receive medical advice, rest, drink water or fluids with added electrolytes, and use acetaminophen (paracetamol) for fever or pain if it is normally safe for you. Avoid aspirin and ibuprofen. Symptoms can resemble other infections, so only a blood test can confirm dengue.

What to do first if dengue is possible

  1. Call a healthcare provider promptly. Tell the clinician about recent travel or residence in an area where dengue occurs, the date symptoms started, your temperature pattern, and any medicines you have taken. Testing and follow-up depend partly on the day of illness and local conditions.
  2. Ask what to do today while testing is arranged. A clinician may manage a stable person without warning signs as an outpatient, but that decision requires assessment and a plan for monitoring. Do not self-diagnose from a symptom list or postpone evaluation while trying to obtain a test.
  3. Use emergency care for warning signs. Go immediately to a local clinic or emergency room if any sign in the callout below appears. Severe dengue is an emergency.
  4. Prevent mosquito bites while you are ill. During the first week of illness, dengue virus can be present in the blood and passed to a mosquito. Use an EPA-registered insect repellent and sleep under a mosquito bed net when appropriate, especially around household members.

There is no specific antiviral medicine for dengue. Clinical care is supportive: clinicians monitor you, manage fluids and symptoms, and escalate treatment if warning signs or severe disease develop.

Recognizing the usual dengue symptom pattern

Dengue commonly causes a fever together with one or more of these symptoms:

  • Muscle, joint, or bone pain
  • Nausea or vomiting
  • A skin rash

Symptoms usually last two to seven days, and many people recover in about a week. This pattern is not diagnostic: influenza, COVID-19, malaria, chikungunya, Zika, other infections, and noninfectious conditions can produce overlapping symptoms. The Centers for Disease Control and Prevention (CDC) says a blood test is needed to confirm dengue.

About one in four people infected with dengue become sick, according to the CDC’s 2025 public information. Among people who become sick, about one in 20 develop severe dengue. Those figures describe population-level risk, not the likelihood for one individual, and they do not make a mild-looking illness safe to ignore.

  • Vomiting at least three times in 24 hours
  • Bleeding from the nose or gums
  • Blood in vomit or stool
  • Feeling extremely tired, restless, or irritable

These signs can indicate progression toward severe dengue. Do not wait for a scheduled appointment, a home test, or a new fever reading. If you are caring for someone else, arrange urgent transport and tell the medical team when the fever and other symptoms began.

Why a falling fever is not always reassuring

Dengue’s most dangerous transition can occur as the fever resolves. CDC public guidance says warning signs usually begin in the 24 to 48 hours after the fever has gone away. CDC clinical guidance describes the critical phase as typically occurring three to eight days after fever onset. These are general timing patterns, not a personal forecast: a person can worsen even when the thermometer looks better.

CDC dengue clinical team leader Dr. Joshua Wong has emphasized that the period when fever drops is precisely when medical assessment matters. Continue monitoring and follow the clinician’s instructions through this window rather than assuming recovery has begun.

How dengue testing works

A blood test is the only way to confirm dengue. Clinicians choose a test according to the illness day, symptoms, local dengue activity, possible co-infections, previous flavivirus exposure, and vaccination status. They should not delay appropriate clinical management while waiting for a laboratory result when dengue is suspected.

Test choice by day of illness

When the sample is taken CDC-recommended approach How results are generally interpreted
Days 0–7 after symptoms begin Either a nucleic acid amplification test (NAAT), such as RT-PCR, plus an IgM antibody test, or an NS1 antigen test plus IgM A positive NAAT or NS1 result confirms acute dengue. A single positive IgM result indicates presumptive recent infection.
After day 7 IgM is the primary test If the acute sample was negative or unavailable, a clinician may consider a convalescent sample to look for evidence that develops later.

A negative NAAT or NS1 result does not rule out dengue. A single IgG blood sample is not recommended for diagnosing acute dengue because IgG can reflect an earlier dengue infection or exposure to another flavivirus. Your clinician may order additional or repeat testing when the first sample does not settle the diagnosis.

The CDC’s “Clinical Testing Guidance for Dengue,” dated August 26, 2025, provides the day-by-day framework above. The World Health Organization’s laboratory interim guidance, published April 28, 2025, also stresses adapting test selection to illness stage, the specimen, regional prevalence, possible co-infection, prior flavivirus exposure, and vaccination status. Those details are why a consumer cannot reliably choose or interpret a dengue test without clinical context.

What to tell the clinician before testing

  • The countries, regions, or locations where you recently lived or traveled
  • The date you returned, if travel was involved
  • The exact day fever or other symptoms began
  • Whether the fever has fallen and when
  • Any bleeding, repeated vomiting, abdominal pain, marked fatigue, restlessness, or irritability
  • Medicines already taken, particularly aspirin or ibuprofen

Do not rely on a purchased consumer test or interpret a result on your own. A clinician must match the result to the timing of the sample and your clinical picture.

Safe interim care while following medical advice

Fluids and rest

Rest as much as practical. Drink plenty of fluids; water and drinks with added electrolytes are options identified in CDC guidance. Take frequent small amounts if a large drink is uncomfortable. Hydration is supportive care, not a substitute for evaluation, and it does not prevent severe dengue.

Fever and pain medicine

CDC advises acetaminophen (also called paracetamol) for fever or pain when it is safe for you under the product label and your clinician’s advice. Do not take aspirin or ibuprofen. These medicines can increase bleeding risk in a person with dengue. Ask a healthcare professional or pharmacist before taking any additional over-the-counter product, especially if you have liver disease, take other medicines containing acetaminophen, are pregnant, or are treating a child.

Mosquito-bite prevention

For the first week of illness, protect yourself from mosquitoes so an insect does not acquire the virus and spread it to someone else. Use an EPA-registered repellent as directed and sleep under a mosquito bed net where appropriate. This is a public-health precaution, not a treatment for the infection.

How clinicians decide between home monitoring and hospital care

A person who is clinically stable and has no warning signs may be managed outside the hospital with instructions for fluids, symptom relief, reassessment, and rapid return if symptoms change. That is a clinician’s disposition decision, not a rule for self-triage.

Warning signs generally call for hospital evaluation and closer monitoring. Severe dengue requires immediate emergency care. In clinical settings, teams may watch for fluid accumulation, changes in mental status, an enlarged liver, bleeding, and laboratory trends in addition to the public-facing warning signs. These findings require professional examination and tests; they are not a checklist for deciding that it is safe to stay home.

The CDC’s “Clinical Care of Dengue,” dated April 15, 2026, explains that supportive treatment and monitoring are adjusted as the illness moves through its phases. The critical phase commonly falls three to eight days after fever begins, so a follow-up plan must cover the period after the temperature improves.

A practical monitoring plan for the first week

  1. Record the start date. Write down the first day of fever or other dengue-like symptoms. This helps the clinician select and interpret testing.
  2. Check for the warning signs at each reassessment. Look specifically for abdominal pain or tenderness, three or more vomiting episodes in 24 hours, nose or gum bleeding, blood in vomit or stool, and extreme tiredness, restlessness, or irritability.
  3. Pay special attention when fever falls. Continue monitoring for at least the next 24 to 48 hours after the fever resolves, because warning signs often begin then.
  4. Follow the agreed contact plan. Ask when the clinician wants a repeat assessment or result review. Return immediately if a warning sign appears rather than waiting for that appointment.
  5. Keep mosquito protection in place. Continue repellent and bed-net precautions during the first week, when virus may be present in blood.

Mistakes that can delay safe care

  • Treating symptom improvement as proof of recovery. The critical phase can begin as fever falls.
  • Taking aspirin or ibuprofen for aches. CDC specifically advises against both when dengue is suspected.
  • Waiting for laboratory confirmation before seeking help. Management should begin based on clinical suspicion when appropriate.
  • Assuming a negative early result excludes dengue. NAAT or NS1 can be negative even when infection is present, so timing and follow-up matter.
  • Using a single IgG result to diagnose a new infection. IgG may reflect past dengue or another flavivirus exposure.
  • Relying on a symptom list alone. Dengue resembles many other febrile illnesses, and only blood testing can confirm it.
  • Buying a test without a plan for interpretation. Test selection and meaning depend on illness day and clinical context.

Questions to ask at the appointment

  • Based on my symptom start date, which test is appropriate now?
  • If today’s result is negative, do I need repeat or convalescent testing?
  • Which symptoms mean I should go directly to an emergency department?
  • How should I monitor the 24 to 48 hours after my fever ends?
  • Which fluids and fever medicine are safe with my other health conditions and medicines?
  • How long should I use mosquito-bite precautions?

Current guidance used for this advice

The recommendations above align with CDC pages on dengue symptoms and testing (updated August 7, 2025), dengue clinical testing (August 26, 2025), dengue management (May 15, 2025), and clinical care (April 15, 2026). They also reflect WHO laboratory guidance published April 28, 2025, and WHO clinical-management guidelines for arboviral diseases published July 4, 2025. For a plain-language warning-sign explanation, see CDC’s severe dengue warning-sign information.

This information cannot diagnose dengue or replace an examination. If dengue is plausible, contact a healthcare professional promptly; if a warning sign appears, seek emergency care immediately.
