What to Do If You Test Negative for COVID but Still Feel Sick

A negative COVID-19 test means the test did not detect the virus; it does not rule out COVID-19. If you have symptoms and used an at-home antigen test, follow the test instructions and take another test 48 hours after the first negative result. If you remain ill or concerned—or have risk factors for severe illness—contact a healthcare provider. Seek emergency medical care immediately for an emergency warning sign.

What a negative COVID test does—and does not—tell you

A negative result is a finding about that test at that time: it did not detect the virus in the sample. It is not proof that you do not have COVID-19. Testing can happen before enough virus is detectable, and a test can miss an infection. The CDC makes this distinction in its Testing and Respiratory Viruses guidance, dated August 18, 2025: “A negative COVID-19 test means the test did not detect the virus, but this doesn’t rule out that you could have an infection.”

That is why your symptoms still matter after a negative result. If you feel sick, do not use one negative home test as the sole reason to dismiss COVID-19 or to delay a conversation with a clinician, especially if you may be at higher risk for severe illness. At the same time, COVID-19 is not the only possible explanation: flu, RSV, or another illness can cause similar symptoms. A COVID test does not identify those other causes.

The right next step depends partly on which kind of test you took. The 48-hour repeat recommendation below is for people with symptoms who used an at-home antigen test and received a negative result. Follow the directions for your particular test; collection and interpretation errors can make a result less reliable. If you have symptoms, FDA guidance says to test immediately; if an antigen result is negative, repeat it according to the schedule below.

What to do after a negative at-home antigen test

1. Check the test and how you used it

Read the instructions for the specific test, including how to collect the sample, how much sample to use, when to read the result, and what the test’s result indicators mean. Do not substitute another test’s directions or interpret the result outside the instructed reading window. Check the package information, including the expiration date, and follow any current instructions provided with that test.

If you are unsure whether you collected the sample or read the result correctly, treat the result cautiously. The test’s instructions are the authority for its use. A correctly performed negative result is still not a guarantee that you are uninfected, but following the procedure helps avoid preventable errors. If you use another test, read that product’s instructions too; procedures and result indicators may differ.

2. Repeat the antigen test 48 hours later

For a person with COVID-like symptoms and an initial negative at-home antigen test, the FDA recommends another antigen test 48 hours after the first negative result, for at least two tests total. The FDA states: “If you have COVID-19 symptoms, test again 48 hours after the first negative test, for a total of at least two tests.” Use the timing and procedure in the test instructions. A repeat test is a second check, not a promise that infection is impossible.

The interval matters because an early test may not detect enough virus. Testing again after 48 hours gives the virus more time to become detectable if infection is present. Do not treat the two-test recommendation as applying automatically to every test type or every situation; the FDA guidance specifically addresses repeat testing after a negative antigen result, and individual test labeling may give additional directions.

If you are buying or using an at-home antigen test for this step, check its current authorization and expiration information and follow the package directions. A rapid result is useful, but it does not change the need to repeat a negative test when you have symptoms. If you cannot follow the test’s directions or have questions about which test is appropriate, ask a healthcare provider or pharmacist for guidance.

3. Consider whether a molecular test is appropriate

Molecular tests, including NAATs and PCR tests, are generally more sensitive than antigen tests and are more likely to detect infection, particularly early in illness. CDC guidance says a symptomatic person with a negative antigen result can repeat the antigen test according to FDA guidance or have the result confirmed with a NAAT. A healthcare provider can advise whether a molecular test makes sense for your symptoms and circumstances.

If you need a more definitive answer for a clinical decision, ask a healthcare provider which test to use rather than assuming that repeating a home test is the only option. The choice can depend on the reason for testing, timing, and your health situation. Tell the provider when symptoms began, what kind of test you took, and when you took it; those details help frame the next step.

How the test options differ

Test option What a negative result means What to do next if you have symptoms
At-home antigen test The test did not detect virus in the sample. Antigen tests are less likely than molecular tests to detect infection, especially early in illness. Repeat 48 hours after the first negative test, for at least two tests total, and follow the test’s instructions. Consider asking a clinician whether NAAT confirmation is appropriate.
Molecular test, such as NAAT or PCR The test did not detect the virus, but a negative test does not by itself establish that COVID-19 is impossible. Discuss persistent symptoms or continuing concern with a healthcare provider, who can consider whether further evaluation or testing is needed.

This comparison describes the general guidance, not a guarantee about any individual test. The type of test, when it was taken, and how the sample was collected all affect how to interpret a result. Neither a negative antigen result nor a negative molecular result tells you whether you have flu, RSV, or another respiratory illness.

In practical terms, an antigen test offers a rapid result, but it is less likely than a molecular test to detect infection early in illness. A molecular test may be an alternative or a way to confirm a negative antigen result, depending on your circumstances. The right choice is not simply “one test is always best”: consider the timing, reason for testing, and advice from a healthcare provider.

When to contact a healthcare provider

Contact a healthcare provider if you still feel ill after repeat testing, your symptoms worsen, or you remain concerned that you could have COVID-19. CDC advises people whose test results are negative but who remain concerned to talk to a healthcare provider. Its testing guidance says, “If all of your test results are negative and you are still concerned you have COVID-19, talk to your health care provider.” A clinician can consider your symptoms and circumstances and advise whether you need additional COVID testing or evaluation for another illness.

Ask whether testing for flu, RSV, or another respiratory illness is appropriate. Those illnesses can resemble COVID-19, and a COVID test does not distinguish among them. A clinician can determine which evaluation is useful rather than relying on symptoms alone or on a home COVID test to identify another cause. Early diagnosis and treatment can matter for some respiratory illnesses, so do not assume that a negative COVID test means there is nothing else to evaluate.

When you call, be ready to explain what test you used—antigen or molecular—when you took it, whether you repeated it, and how symptoms have changed. That information does not diagnose the illness by itself, but it gives the provider context for deciding whether testing or an appointment is appropriate.

If you may be at higher risk for severe illness, act promptly

If you feel sick and have risk factors for severe respiratory illness, seek healthcare right away for testing and/or treatment—even if a home COVID test is negative. The CDC’s Treatment of Respiratory Viruses guidance, dated August 18, 2025, emphasizes timely care for people at higher risk. The CDC advises: “Seek healthcare right away for testing and/or treatment if you feel sick or tested positive for a respiratory virus and have risk factors for severe illness.” Do not wait for another home result before starting that conversation if you may need prompt evaluation.

A healthcare provider can advise whether testing or treatment is appropriate for your situation. A negative home test does not assess your risk from other respiratory viruses, and it does not answer whether a time-sensitive treatment option may be relevant. If you are uncertain whether a risk factor applies to you, contact a healthcare provider and describe your symptoms and test results. Promptly asking for advice is more useful than trying to decide from a negative antigen result alone whether you can safely wait.

When to seek emergency care

Seek emergency medical care immediately for an emergency warning sign. The CDC’s About Respiratory Illnesses guidance, updated September 10, 2026, calls for immediate emergency care when such a warning sign is present. Do not wait to repeat a test, arrange a routine appointment, or see whether symptoms pass before seeking emergency help.

The particular warning signs can vary with the illness and the person. Use current emergency guidance from a public health authority or emergency medical professional rather than relying on an improvised checklist. If you believe the situation is an emergency, contact local emergency services.

A practical decision path

  1. You have symptoms and took a home antigen test: read that test’s instructions and check that you followed them. Treat one negative result as preliminary rather than conclusive.
  2. Your first antigen result is negative: repeat the test 48 hours later, aiming for at least two tests total, as FDA recommends for people with symptoms. Follow the product’s directions for both tests.
  3. You need clinical guidance, or you remain concerned: contact a healthcare provider. Ask whether a molecular COVID test or evaluation for flu, RSV, or another illness is appropriate.
  4. You feel sick and may be at higher risk for severe illness: seek healthcare promptly for advice about testing and/or treatment. Do not let a negative home result delay the call.
  5. You have an emergency warning sign: seek emergency medical care immediately.

What not to conclude from repeat negative tests

  • Do not conclude that one negative antigen test rules out COVID-19. Antigen tests can miss infection, particularly early in illness.
  • Do not assume that symptoms must be COVID-19. Flu, RSV, or another illness may be responsible, and a COVID test cannot diagnose those conditions.
  • Do not assume a test tells you whether you are contagious. A negative result is about whether that test detected the virus; it is not a definitive contagiousness assessment.
  • Do not ignore ongoing illness because the tests are negative. Persistent or worsening symptoms, continued concern, or risk factors are reasons to contact a healthcare provider.

CDC and FDA recommendations are general public-health guidance, not an individualized diagnosis. If you are unsure how to interpret a result or what to do next, describe your symptoms, the kind of test you took, and when you took it to a healthcare provider.

Frequently Asked Questions

Does one negative COVID test mean I do not have COVID?

No. It means that test did not detect the virus in that sample. Testing may happen before enough virus is detectable, so a negative result does not rule out infection.

When should I repeat a negative at-home COVID antigen test if I have symptoms?

FDA recommends repeating it 48 hours after the first negative result, for at least two tests total. Follow the instructions for the specific test.

Should I get a PCR or other molecular test after a negative antigen test?

Molecular tests such as NAAT or PCR are generally more sensitive than antigen tests. CDC says a symptomatic person can repeat the antigen test according to FDA guidance or have the result confirmed with a NAAT. Ask a healthcare provider which option fits your situation.

What if I take two tests and both are negative but I still feel sick?

Contact a healthcare provider if symptoms persist or worsen, or if you remain concerned. A clinician can consider whether additional COVID testing or evaluation for flu, RSV, or another illness is appropriate.

Should I wait to call a healthcare provider until I repeat the test?

Not if you feel sick and may be at higher risk for severe respiratory illness. CDC advises seeking healthcare right away for testing and/or treatment in that situation; do not let a negative home result delay the conversation.

Can a negative COVID test tell me whether I have flu or RSV?

No. A COVID test does not diagnose flu, RSV, or another respiratory illness. Ask a healthcare provider whether evaluation or testing for another cause is appropriate.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

When is a negative COVID test an emergency?

A negative test does not change the need to seek emergency medical care immediately if you have an emergency warning sign. Do not wait for another test or a routine appointment.

The Bottom Line

A negative COVID-19 test is not a rule-out, especially when it is a single home antigen test taken early in illness. If you have symptoms, repeat a negative antigen test after 48 hours for at least two tests total, following the test instructions. Contact a healthcare provider if you remain ill or concerned, and seek care promptly if you may be at higher risk; seek emergency care immediately for an emergency warning sign.
