Start by confirming the exact date your employer plan ends, then compare three paths: a Marketplace plan, COBRA continuation, and Medicaid or CHIP. Losing job-based coverage generally opens a 60-day Marketplace Special Enrollment Period (SEP). Marketplace coverage usually begins the first day of the month after the old plan ends, while COBRA can preserve the same employer plan but usually requires you to pay its full premium plus an administrative fee. Apply promptly, check the effective date, submit any requested proof, and pay the first premium before relying on new coverage.
First, pin down when your current coverage ends
Your last day at work and your last day of health coverage are not always the same. Ask the former employer’s human-resources department or plan administrator for written confirmation of:
As an Amazon Associate I earn from qualifying purchases.
- The exact termination date of medical, prescription, dental, and vision benefits.
- Whether dependents lose coverage on the same date.
- A COBRA notice, if the employer plan and employer are subject to COBRA.
- The COBRA premium, election instructions, covered dependents, and expected end date.
Keep the termination letter, benefits statement, and any notice showing the coverage end date. The Marketplace may request documents proving that you lost coverage and when it ended.

Marketplace Special Enrollment Period
HealthCare.gov says leaving a job for any reason and losing the job-based plan can qualify you for a Marketplace SEP. The general window lasts 60 days after the employer coverage ends. Submit an application as soon as you know the end date rather than waiting for the final weeks, and use the deadline shown in your eligibility notice.

A Marketplace plan is not normally same-day insurance. Coverage generally starts on the first day of the month after the previous plan ends, provided you select a plan, complete any required verification, and pay the first premium. Your eligibility and enrollment notices control the actual effective date.
Annual Open Enrollment
The current federal Marketplace Open Enrollment period is November 1 through January 15. For the coming coverage year, enrolling by December 15 generally supports a January 1 start; enrollment from December 16 through January 15 generally supports a February 1 start. Payment and eligibility requirements still apply. If you lose job-based coverage outside that period, the loss-of-coverage SEP is generally the route into Marketplace coverage.
Medicaid and CHIP timing
Medicaid and the Children’s Health Insurance Program (CHIP) accept applications year-round for people who meet applicable state and household rules. There is no annual Marketplace-style enrollment window for these programs, but eligibility and effective dates vary by state. Do not assume unemployment alone qualifies you.

The three coverage routes to compare
| Route | What it does | Timing and eligibility points | Main cost issue |
|---|---|---|---|
| Marketplace plan | Individual or family coverage selected through HealthCare.gov or your state Marketplace | Usually available through a 60-day loss-of-coverage SEP; generally starts the first day of the month after the old plan ends | Premium may be reduced by premium tax credits; additional cost-sharing help may be available |
| COBRA continuation | Temporarily continues the employer group plan for the employee and covered family members | HealthCare.gov describes a usual continuation period of 18 months; confirm whether your plan is covered and follow the administrator’s notice | You usually pay the full group premium plus a small administrative fee |
| Medicaid or CHIP | Public coverage for people meeting state income and other eligibility rules | Applications are accepted throughout the year | Eligibility, premiums, and cost sharing depend on state and household circumstances |
Marketplace coverage: what to enter and what to check
Use full-year household income
The Marketplace application estimates savings using expected income for the entire calendar year, not only the wages you expect after losing the job. Pay received earlier in the year counts. Include income for the tax household and update the application if your expected income, household, or other circumstances change. The application can screen for premium tax credits, extra help with deductibles and other cost sharing, Medicaid, and CHIP.
Compare the total exposure, not just the premium
For every available plan, record the premium after estimated savings, deductible, copayments or coinsurance, annual out-of-pocket maximum, provider network, hospital access, and prescription formulary. A low monthly premium can be a poor fit if your doctors are out of network or a medication is excluded. Prices and plan availability are specific to your location and household; the application’s results are more reliable than a generic national estimate.
Check the effective date before using care
After selecting a plan, read the eligibility and enrollment notices for the start date and any conditions. Coverage generally cannot be used until required eligibility is confirmed and the first premium is paid. Ask the insurer when your member ID and pharmacy information will be available, and retain your payment confirmation.

COBRA: when continuity may justify the higher bill
COBRA can let you and covered family members keep the same employer-sponsored group coverage temporarily. That may matter when you are in active treatment, have a specialist who is difficult to replace, rely on a particular hospital, or have already met much of the plan’s deductible. The trade-off is that you usually take over the entire premium that the employer previously helped pay, plus a small administrative fee.
Request the actual COBRA offer from the plan administrator. Confirm the election instructions and deadline, the monthly amount, which dependents are included, whether the deductible and out-of-pocket totals carry over, and the date continuation ends. The cited federal guidance describes 18 months as the usual period, but your notice controls the details and some situations can differ.
COBRA is not automatically cheaper or more expensive than a Marketplace plan. Compare its real premium and cost sharing with Marketplace prices after any savings, while checking networks, prescriptions, and start dates.
Medicaid and CHIP: a year-round check
Submit a Medicaid or CHIP application if your household may qualify under your state’s rules. The Marketplace application can screen for these programs, or you can follow your state agency’s application process. Have household members, addresses, and income information ready. Eligibility is determined under state and household criteria, so job loss by itself does not guarantee enrollment.

Before ending another policy, obtain the program’s approval and effective date. Ask how the program handles care received while an application is pending and when managed-care enrollment, cards, or provider assignments begin.

A practical sequence for the first days after job loss
- Confirm the end date. Obtain written proof of when employer coverage stops and identify every family member affected.
- Get the COBRA figures. Ask the former employer or plan administrator for the notice, premium, election instructions, covered dependents, and plan end date.
- Open a Marketplace application. Use HealthCare.gov or your state Marketplace during the loss-of-coverage SEP. Enter the tax household and expected income for the full calendar year, including wages already received.
- Review public-program results. Follow any Medicaid or CHIP instructions if the application indicates potential eligibility.
- Build a side-by-side comparison. For each Marketplace option and COBRA, write down premium, deductible, other cost sharing, out-of-pocket maximum, doctors, hospitals, prescriptions, and effective date.
- Select coverage and complete verification. If the Marketplace asks for proof of the coverage loss, submit the documents within the period shown in your eligibility notice. HealthCare.gov guidance says requested SEP documents are generally due within 30 days after selecting a plan.
- Pay and verify. Pay the first Marketplace premium as directed, save the confirmation, and verify the insurer’s effective date before scheduling care as an insured patient.
Documents and deadlines that commonly cause problems
Proof of the qualifying event
A termination letter, benefits statement, or employer notice may establish the loss and end date. Upload only what the notice requests, ensure names and dates are legible, and check your application account for an approval or follow-up request.
First-premium failure
Plan selection alone does not activate coverage. If the first premium is not paid according to the insurer’s instructions, the policy may not take effect. Keep the receipt and confirm enrollment directly with the insurer.
Best Value
Unclear employer-plan status
Not every employer or plan is subject to COBRA in the same way. The plan administrator or the US Department of Labor can explain whether the federal COBRA rules apply to your situation and what notice governs you.
Do not cancel COBRA without checking the next plan
If you elect COBRA, you can compare Marketplace coverage before making a change. Voluntarily ending COBRA early generally does not create a new Marketplace SEP outside Open Enrollment. COBRA expiring or becoming unavailable can qualify, but the next policy’s start date must be confirmed before you end continuation coverage. A gap can leave you responsible for the full cost of care.

Favor COBRA when continuity has immediate value
- Your current doctors, hospital, or prescriptions are difficult to replace.
- You are mid-treatment and switching networks could disrupt appointments or approvals.
- Your employer plan’s deductible and out-of-pocket spending are already substantially satisfied.
- The COBRA premium is affordable compared with the Marketplace alternatives available to your household.
Favor a Marketplace plan when savings or long-term cost control matter
- Premium tax credits or other savings materially reduce the monthly bill.
- The plan includes your providers and medicines at acceptable cost-sharing levels.
- You expect COBRA’s full premium to strain your budget.
- You want coverage that can continue beyond the COBRA period, subject to future eligibility and enrollment rules.
Investigate Medicaid or CHIP when household income has fallen
If your projected annual household income is low enough under your state’s rules, public coverage may offer a better financial fit. Apply rather than guessing, and wait for the effective-date information before dropping existing coverage.
Questions to ask before you rely on the new policy
- What is the exact effective date printed in the eligibility or enrollment notice?
- Has the first premium posted, and can the insurer confirm active enrollment?
- Are my primary-care clinician, specialists, hospital, and prescriptions covered?
- Have I submitted every requested loss-of-coverage document within the stated deadline?
- What happens to pending referrals, prior authorizations, and prescriptions during the transition?
Bottom line
Act before the coverage end date if possible: document the loss, open a Marketplace application within the SEP, check Medicaid or CHIP, and obtain the actual COBRA offer. Choose only after comparing after-savings premium, cost sharing, networks, prescriptions, and effective dates. Keep COBRA in place until the replacement coverage is approved, paid, and confirmed unless your plan administrator gives you different instructions.
