If your organization loses access to an institutional crypto wallet, first identify who controls the keys and what recovery route was configured. There is no universal reset: a custodian-managed account, a provider-managed non-custodial wallet, and a self-custodied smart-contract account can have very different recovery options. If a key or device may have been stolen, treat the event as a security incident—not just an access problem.
What to do first
- Identify the wallet and account model. Record the chain, wallet address, account type, signer set, approval threshold, and which devices or credentials are still available. Establish whether the wallet is custodial, provider-managed but non-custodial, or a self-custodied smart-contract account; recovery authority depends on that distinction.
- Contact the provider through its verified support or incident channel. Preserve the affected addresses, timestamps, relevant approvals, and steps already attempted. For Coinbase Token Manager, the support guidance asks organizations for their name, inaccessible admin wallet address, account type, and attempted steps. Do not send support a seed phrase, private key, or recovery phrase.
- Decide whether access was lost or a compromise is suspected. If a key, device, or account may have been exposed or stolen, use your organization’s incident process and involve the appropriate security and treasury decision-makers. Do not sign an unverified transaction or move assets to an address supplied through an unverified channel.
- Follow the exact product’s documented recovery procedure. Do not assume that a provider can recover a key held by another service, replace an on-chain owner, or reverse a confirmed blockchain transaction. Coinbase says its support cannot recover an admin wallet’s private key or seed phrase or override on-chain ownership without a valid signature.
- Bring in relevant internal teams. Security, treasury, finance, legal, and compliance may need to assess authorization, operational and client impact, and whether notifications are required. The applicable duties depend on the jurisdiction and circumstances; the provider guidance cited here does not establish a universal reporting deadline or legal requirement.
Which recovery route applies?
Use the provider’s instructions for the specific account rather than treating all institutional wallets alike. Coinbase’s Prime Onchain Wallet is described as non-custodial, and its recovery steps depend on which signer devices, key shards, and recovery passphrase remain available.

| What remains available | Documented Coinbase Prime Onchain Wallet route |
|---|---|
| One signer loses a device or app, but another signer retains a key shard | An existing signer can reprovision that user’s access. |
| All signers lose access to their devices, but the recovery passphrase is available | The documented route uses the portfolio’s 12-word recovery passphrase, an administrator to initiate recovery, a signer to complete it, and portfolio consensus approval. The passphrase encrypts the recovery backup; Coinbase says it is not the wallet’s private key. |
| The recovery passphrase is lost, but a signer can still access a key shard | Coinbase documents replacing the recovery backup and invalidating the previous passphrase. |
| All signers lose their key shards and the recovery passphrase is unavailable | Coinbase says no recovery option is available and wallet access may be permanently lost. |
These are product-specific instructions, not a general recovery procedure for other MPC, custody, or wallet systems. The documented Prime flow also requires an administrator to authenticate with a YubiKey; confirm the required hardware and model with the provider for your organization’s setup.

For Coinbase Token Manager self-custodied smart-contract accounts, the owner or admin wallet controls account-level actions such as configuration changes, escrow withdrawals, and ownership transfers. Coinbase says it cannot recover the key, sign for the organization, or replace the on-chain owner without a valid signature from the current owner. If that key is permanently inaccessible and no recovery route exists, the organization’s options may be very limited.
#1 Best Overall
- Effortlessly build your crypto portfolio via the all in one Ledger Wallet app: buy, sell, send, receive, swap, stake and more across popular blockchains. 15,000+ coins & tokens in a single dashboard. Keep a close eye on the market. Compare service providers. Track performance. Get timely alerts. Build your portfolio with confidence.
- Enjoy Bluetooth connectivity, iOS access, and hours of battery use with this mobile-first, secure backup signer. Freedom you can depend on.
- Genuine Check: confirm your signer is authentic during setup with the Ledger Wallet app.
- Protect your signer: keep it in mint condition at all times with a bespoke Pod or Case to avoid scratches and everyday wear and tear.
Consequences can extend beyond the admin’s ability to make changes. Depending on account type and network support, some vested and unlocked stakeholder tokens may remain claimable, while unvested or future vesting may no longer be administratively adjustable. Ask legal and finance teams to assess the specific account and communicate clearly with affected stakeholders.
Does recovering access mean recovering the assets?
Not necessarily. Restoring a signer or key restores the ability to authorize transactions; a separate recovery design may instead move assets to a destination approved in advance without restoring the lost key.

- Pre-signed asset extraction: Circuit Security describes its Automatic Asset Extraction product as using pre-signed transactions to sweep assets to a backup vault after key loss, system failure, or compromise. This only applies if the organization established and approved the mechanism beforehand; it is a vendor-described feature, not a universal emergency remedy.
- Product-specific recovery tooling: Ripple describes an open-source recovery CLI for its wallet product and says its recovery path does not depend on Ripple or a single third party. Verify the current implementation and your institution’s configuration before relying on it.
Ask the provider whether its procedure restores signing access, transfers assets through a pre-authorized path, or does something else—and which approvals and prior configuration it requires.
How to compare institutional wallet recovery designs
When assessing custody or wallet options, compare how recovery works under the conditions your organization actually needs to withstand. These questions apply across products; the answers and capabilities are provider-specific.

| Decision area | Questions to ask |
|---|---|
| Key control | Who holds or can use key material? Can the provider independently sign or recover assets? |
| Recovery design | Does recovery restore a signer or key, or move assets through pre-approved transactions? What must be configured beforehand? |
| Authorization | How many people or approvals are required to recover or transfer assets? Can thresholds be changed during an incident? |
| Provider dependence | Can the institution recover if its custody provider or primary platform is unavailable? |
| Key technology | Is the system based on an HSM, MPC, multisig, or a combination, and what operational dependencies follow? Ripple describes HSMs as keeping keys within tamper-resistant hardware and MPC as splitting key generation and signing across parties. |
| Operating model | How do the provider’s hot, warm, and cold wallet arrangements balance transaction speed and operational access? |
| Assurance | Are recovery drills, access reviews, audit evidence, and escalation responsibilities documented and tested? |
How to reduce the risk of another lockout
- Do not make one person the sole route to administration. Coinbase recommends multisig or an institutional custody solution for admin access. For its Prime product, Coinbase recommends considering at least three signers if possible; that is product guidance, not a universal institutional standard.
- Keep an approved wallet inventory. Record wallets, chains, addresses, signers, roles, approval thresholds, recovery materials, and escalation contacts. Protect recovery material under the organization’s security policy, with access separated from day-to-day signing where appropriate.
- Define signer lifecycle procedures. Document key backup, signer recovery, role changes, offboarding, ownership transfer, and signer rotation so that access does not depend on undocumented knowledge.
- Check access periodically. Coinbase recommends regular checks that more than one authorized person can access the required signing path.
- Rehearse approved recovery steps. Run controlled drills, record the results, and verify that the documented contacts and approvals work. CoinCover describes regular tests and recovery drills as features of its service; that product description is not independent evidence of provider performance.
- Map dependencies. Identify whether recovery depends on a provider, a quorum, specific devices, independent backups, or an audit trail—and whether the mechanism can still operate if the primary custody platform is unavailable.
