Act as though you have an incident to investigate, not as though every machine is already compromised. Stop the package from running, preserve evidence, identify the exact package and version, determine where its code executed and what that process could access, revoke exposed credentials, then remove the dependency and rebuild from a known-clean state. Finally, report confirmed malware to npm and coordinate with your security or incident-response team when organizational systems or data are involved.

Downloading a package is not the same event as executing it. A package that was fetched but never run has a different risk profile from one whose install script ran on a developer laptop, CI runner, release host, or production server. Your response should therefore be based on execution, access, current activity, and scope.

Take these first 15 minutes seriously

  1. Stop using the affected environment for sensitive work. If the package may still be running, or the machine has access to production, cloud, source-control, signing, or payment systems, pause that access and involve your security team. Do not automatically shut down every system or revoke every credential: GitHub recommends making containment proportionate to the evidence and scope because disruptive actions can break legitimate automation. See GitHub’s security-incident response guidance.
  2. Do not delete files yet. Record the package name, exact version, installation and execution times, affected repositories and machines, the alert or advisory that triggered the investigation, and every action already taken.
  3. Prevent further propagation. Pause builds, releases, scheduled jobs, or developer workflows that would reinstall the dependency until you know which versions are affected and have a reviewed replacement.

If the package was installed only in an isolated, disposable environment and no code or scripts ran, the likely impact is narrower. If it ran with access to credentials or sensitive files, treat the event as a possible credential-exposure incident even before you find proof of theft.

Preserve evidence before cleanup

Cleanup can destroy the clues needed to determine whether code ran and what it touched. Before removing the dependency, preserve a snapshot of the environment and your decisions.

  • Copy the relevant package.json, lockfile (for example, package-lock.json), and dependency-tree output.
  • Save suspicious package files, install scripts, build artifacts, and hashes in a location that will not be overwritten by a reinstall. Do not execute copied files merely to inspect them.
  • Export available shell history, package-manager logs, CI logs, host logs, source-control audit events, cloud audit trails, and identity-provider records before their retention window expires.
  • Write a time-ordered incident log: who discovered the issue, which machines and repositories were checked, what was isolated, which credentials were revoked, and why each decision was made.
  • Keep the original alert, advisory, package-page URL, and any references to suspicious commits or code.

GitHub’s incident guidance emphasizes capturing evidence and recording findings and decisions. Its investigation overview also identifies repository activity, workflows, dependency graphs, alerts, and commit history as useful evidence sources: Common security incident investigation areas.

Find every copy of the package and its exact versions

A direct dependency in one application may also be present transitively in several repositories, cached in CI, or pinned in a lockfile after the manifest has changed. Search all of those locations.

Repository and workstation checks

  • Search every package.json and lockfile for the exact package name and version range.
  • Inspect the resolved tree, not just top-level dependencies. In a project directory, npm ls --all can reveal which direct dependency brought a package into the tree; save the output as evidence.
  • Review recent manifest and lockfile changes, pull requests, release branches, tags, and commits. A malicious version may have been introduced by an apparently routine update.
  • Check local npm caches, build workspaces, container layers, artifact repositories, and any archived source tree that could be rebuilt later.

CI, release, and production checks

  • Search organization repositories and dependency graphs for the package and all affected versions.
  • Review workflow definitions, reusable actions, self-hosted runner logs, build logs, and deployment records for the period in which the package was installed or executed.
  • Identify images, release bundles, serverless artifacts, and production hosts built from an affected lockfile. A clean source repository does not automatically make an already-built artifact safe.

GitHub documents these investigation surfaces in its incident-investigation areas guidance. Record both positive and negative findings; “not found” is useful only when you can state where and how you searched.

Work out whether the package actually ran

Installation and execution are separate events. Your exposure depends on which of the following occurred:

What happened What it means for triage Priority actions
Package metadata or tarball was downloaded, but no install or application code ran The package may not have had an opportunity to read local files or secrets. Supply-chain and future-build risk still exists. Preserve the lockfile and logs, remove the version through a reviewed change, and check whether the package was copied into other artifacts.
An npm install lifecycle script ran Code may have executed with the permissions of the installing user or CI job. Identify the host, user, environment variables, network access, and files available at that time; begin credential review.
The application, test suite, build, or workflow imported or invoked the package The package had the application’s runtime access and may have run repeatedly. Contain the process or workload, preserve runtime and network evidence, and investigate every credential and service available to it.
The package ran on a shared runner, release system, or production host Blast radius can include other repositories, artifacts, deployments, and tenants reachable from that system. Escalate to incident response, isolate the affected runner or host as evidence permits, and examine cross-system access logs.

Look for npm lifecycle-script output, process and shell history, CI job records, application logs, file and network telemetry, and timestamps that correlate package installation with other activity. Absence of a log is not proof that nothing ran; many environments retain incomplete history.

Assess what the process could access

Make an inventory for each host or job where execution is plausible. List environment variables, files, mounted workspaces, SSH material, cloud metadata, package-manager credentials, source-control tokens, signing keys, deployment secrets, and network services reachable by the process. Access is the key distinction between a suspicious package in a locked-down test container and the same package in a developer account with broad production privileges.

  • Check whether secrets were injected into the job, inherited from the shell, mounted as files, or available through a cloud workload identity.
  • Compare the package’s execution window with unusual outbound connections, new files, changed workflows, unexpected commits, artifact publication, or data transfers.
  • Search for persistence: modified startup scripts, scheduled jobs, altered CI definitions, new deploy keys, or changes to package and release configuration.

GitHub notes that real-world incidents rarely involve a single attack vector. Treat package execution, credential use, repository changes, and data movement as related questions rather than waiting for one definitive indicator.

Use malware alerts as evidence, not as a safety certificate

Dependabot malware alerts can identify affected files and package versions, provide a patched version when one is available, and point to remediation steps. They are generated for known malicious packages represented in the GitHub Advisory Database. New malware can take time to appear, alerts do not cover every malicious package, and only GitHub-reviewed advisories trigger them. An absent alert therefore does not establish that a package is safe.

Check the alert’s affected-version range against your lockfiles and build history, then verify the proposed replacement against the package maintainer’s guidance and trusted advisories. Keep a copy of the alert and its remediation details with your incident record. Documentation: Dependabot malware alerts.

Revoke and rotate credentials that may have been exposed

If the package ran where it could read a token or secret, assume that credential may have been copied until logs establish otherwise. Start with the narrowest credentials tied to the affected process, then expand based on evidence and privilege.

  1. List credentials available to the process during the execution window: npm tokens, source-control tokens, cloud keys, deployment credentials, service-account secrets, database passwords, signing keys, and personal access tokens.
  2. Revoke affected npm tokens. npm documents npm token list for listing tokens and npm token delete <id> for deleting a token by ID. npm warns that revocation can take up to one hour in some cases; plan for that delay rather than assuming access ends instantly. See npm’s token-revocation documentation.
  3. Rotate repository, cloud, CI, deployment, service, and personal credentials when they were present in the environment or could have been reached through it. Replace secrets in the systems that consume them, not only in a local password manager.
  4. Review npm, source-control, cloud, CI, identity, and application audit logs for token use, logins, API calls, repository changes, package publication, and deployments you do not recognize.
  5. Record the revocation time and the replacement credential’s activation time. This lets investigators distinguish activity that occurred before and after containment.

GitHub recommends revoking credentials believed to be exposed or exploited and checking audit logs for token activity. If a credential is shared by many services, coordinate the rotation to avoid creating a second outage while the incident is still active.

Remove the dependency and recover from a known-clean state

  1. Choose the replacement. Use the maintainer’s security notice and trusted advisories to select a version known to be clean. If no clean version exists, remove the dependency or replace it with a reviewed alternative.
  2. Make a reviewed manifest and lockfile change. Update or remove the dependency in source control; do not rely on deleting a local directory while leaving the bad version pinned for the next build.
  3. Invalidate affected artifacts. Mark packages, containers, build outputs, caches, and releases produced from the compromised dependency as untrusted until rebuilt.
  4. Rebuild in a clean environment. Use a fresh runner or host with rotated credentials, a reviewed lockfile, and only the minimum network and identity permissions required for the build.
  5. Verify the result. Confirm the bad version is absent from the resolved dependency tree, inspect the new build logs, and test deployment with least-privilege credentials.
  6. Decide whether hosts need isolation or replacement. Deleting node_modules removes files from that directory; it cannot undo stolen secrets, unauthorized commits, or changes elsewhere on a host. Rebuild or reimage a machine when execution evidence, persistence, or uncertainty about host integrity warrants it.

GitHub’s response guidance stresses that remediation depends on evidence and scope. Keep the original affected environment isolated or preserved long enough for investigation, subject to your organization’s incident procedures.

Report suspected malware to npm

Use the package page’s Report malware flow when you believe the package itself is malicious. npm asks for:

  • the package name;
  • all affected versions you identified; and
  • a description with references, commits, code examples, or other evidence that helps confirm the behavior.

npm says it validates malware reports, removes confirmed packages, publishes a security placeholder, and issues an advisory. Report package malware through npm’s malware-reporting procedure. A vulnerability in otherwise legitimate package code is a different category: npm advises reporting vulnerabilities privately to the maintainer rather than using the malware process. Review the npm Security Policy for the distinction and handling expectations.

Coordinate the response when organizational assets are involved

Notify the security lead, incident-response function, and engineering owner as soon as your evidence indicates that a shared runner, repository, cloud account, production service, or sensitive data could be involved. Include legal or privacy contacts when regulated or confidential information may have been accessed.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • Give stakeholders a factual timeline, affected package versions, known execution locations, credential actions, and open questions.
  • Assign owners for host containment, credential rotation, repository review, customer or regulator assessment, and npm reporting.
  • Preserve communications and decisions with the incident record; avoid speculative claims about data theft until logs support them.
  • After containment, review why the package was admitted, which controls failed to flag it, and whether CI permissions, secret exposure, dependency pinning, or review practices should change.

Keep updates scoped to verified facts. A package alert can evolve as new versions, repositories, or credentials are discovered.

Choose containment by execution, access, activity, and disruption

Situation Reasonable first containment Escalate when
Downloaded only; no evidence of execution Block the affected version, preserve evidence, remove it from manifests and caches, and verify no build artifact contains it. The package appears in a shared artifact, or execution cannot be ruled out.
Install script ran on one developer workstation Stop sensitive access from that workstation, preserve logs, rotate credentials available to the process, and rebuild the project elsewhere. There are unusual logins, persistence, repository changes, or broad user privileges.
Executed in CI or on a release runner Pause affected workflows, isolate or replace the runner according to response procedures, invalidate outputs, and rotate injected secrets. Other repositories, signing keys, deployment systems, or production environments were reachable.
Executed in production Engage incident response, preserve runtime and network evidence, contain the workload, and assess customer or data impact. Any indication of data access, persistence, lateral movement, or ongoing execution.

Broad shutdowns, mass credential invalidation, and destructive reimaging can be necessary, but they are not automatic. Match the disruption to the evidence and document why the chosen scope protects systems without destroying investigation data.

After recovery: close the gaps the incident exposed

  • Pin and review lockfile changes, including transitive updates, in code review.
  • Run builds with least-privilege identities and avoid exposing production secrets to ordinary dependency installation.
  • Separate developer, CI, release, and production credentials so one package execution cannot reach every environment.
  • Retain package-manager, CI, source-control, cloud, and identity logs long enough to investigate supply-chain events.
  • Use dependency graphs and malware alerts as signals, while retaining independent review and monitoring because alerts are incomplete.
  • Document an internal playbook covering evidence preservation, credential rotation, artifact invalidation, host isolation, npm reporting, and stakeholder notification.
