If the problem came on suddenly—especially with trouble walking, dizziness, weakness, vision changes, confusion, or speech difficulty—seek emergency help. If it is recurring or has changed over time, contact a health professional rather than trying to identify the cause with a balance test at home.
First decide whether this is an emergency
Call emergency services for sudden balance problems
Sudden trouble walking, dizziness, loss of balance, or lack of coordination can be signs of stroke. The U.S. Centers for Disease Control and Prevention says to call 9-1-1 immediately for possible stroke symptoms. Other warning signs include sudden vision trouble, numbness or weakness on one side, confusion or difficulty speaking, and a severe headache with no known cause. 9-1-1 is the U.S. emergency number; elsewhere, use your local emergency number. Arrange medical advice for a recurring or worsening change
If the difficulty is not sudden but keeps happening, is getting worse, or comes with dizziness or falls, contact a health professional. Note when it began, how often it occurs, whether you feel unsteady or as if the room is spinning, and whether you have fallen. These details help describe the problem without assuming that a one-leg stand identifies its cause. National Institute on Aging: Older Adults and Balance Problems (reviewed September 12, 2022); MedlinePlus: Balance Problems.
Why one-leg balance cannot tell you what is wrong
Balance relies on signals and function across multiple systems. Difficulty may be related to medicines, the inner ear, vision, nerves, circulation, strength, or another health condition. A single observation—such as wobbling or needing to put a foot down—cannot establish which, if any, of these is responsible. Avoid diagnosing yourself from how long you can hold a pose. When describing what you notice, it can help to distinguish “Do I feel unsteady?” from “Do I feel dizzy or as if the room is spinning around me?” You might also note whether you feel as if you are moving while sitting or standing still, or whether you lose your balance and fall. These are useful descriptions, not a checklist that diagnoses a disorder. National Institute on Aging: Older Adults and Balance Problems.
What a health professional may do
A clinician may ask about your symptoms and medicines, examine you, and choose tests based on the possible cause. Balance evaluations can include hearing, blood, imaging, eye-movement, or movement-response tests; not everyone needs every test. The goal is to investigate the pattern and likely contributors rather than treat a one-leg stand as a diagnosis. Stay safer while you arrange care
- If you feel unsteady, do not test yourself by standing unsupported on one leg or start unsupported balance exercises. Ask a health professional what activity is appropriate for you.
- If you have been advised that exercise is suitable, start slowly and use stable support. MedlinePlus describes using a sturdy chair for support during standing balance exercises. MedlinePlus: Exercises to Help Prevent Falls.
- Consider practical fall precautions suited to your needs: nonskid, rubber-soled, low-heeled shoes; avoiding walking in the dark; and using handrails or a cane or walker if appropriate. These measures do not replace evaluation of a new or recurring balance problem. National Institute on Aging: Older Adults and Balance Problems.
If you are concerned about falling
Falls are an important safety concern, particularly for older adults. The National Institute on Aging reports that more than one in four people age 65 or older fall each year; the cited page does not state a year for that figure. It describes falls among older adults, not the share of people who cannot balance on one leg. National Institute on Aging: Falls and Fractures in Older Adults: Causes and Prevention.
