If the same malware alert returns, treat it as either a new reinfection or evidence that a hidden component is reinstalling the detected file. On Windows, save your work and run Microsoft Defender Offline from Windows Security; it restarts the PC and scans outside normal Windows operation. If detections continue, protect known-clean backups and consider a reset, restore, or reinstall. Multiple affected devices or accounts require an organizational incident-response approach rather than repeated consumer scans.

Why the same malware keeps coming back

A repeated detection does not prove that one unchanged file survived removal. Microsoft identifies two broad possibilities: you were reinfected, or another component remained hidden and keeps reinstalling the detected malware. The first can happen when you revisit the website that delivered the malware or receive and open the same malicious attachment again. The second may redetect the threat shortly after a restart because the surviving component launches again. Microsoft describes this latter pattern as an undetected component “constantly, quietly, reinstalling the detected malware.” See Microsoft’s malware-detection troubleshooting guidance.

Do not assume either explanation without checking the device. Stop using the suspected download, website, or attachment while you investigate, and do not sign in to sensitive accounts from a computer that may still be compromised.

First escalation on Windows: run Microsoft Defender Offline

Microsoft Defender Offline is the appropriate next scan when the same detection returns on a Windows PC. It runs outside the normal Windows environment, which can make it easier to detect malware that hides while Windows is running. The scan requires a restart, so save open documents first.

  1. Save your work and close applications. The computer will restart before the offline scan begins.
  2. Open Windows Security.
  3. Select Virus & threat protection.
  4. Choose Scan options.
  5. Select Microsoft Defender Offline scan, then choose Scan now.
  6. Allow the restart and let the scan complete before signing back in and checking the result.

These labels and steps are Microsoft’s Windows instructions; they are not a universal procedure for macOS, Android, iOS, ChromeOS, or every managed-work-device configuration. On a work-managed computer, contact your IT or security team before changing security settings, deleting files, or resetting the device.

An offline scan is a stronger removal step, not proof that every account, browser session, or other computer is clean. If the alert returns again, move to the recovery and escalation choices below instead of running the same quick scan indefinitely.

Prevent an immediate reinfection

Stop reopening the suspected source

Do not revisit the website associated with the original download, and do not reopen an attachment or executable that may have delivered the malware. If an email appears to be the source, leave it unopened and report or delete it according to your mail provider’s procedures. A clean scan followed by the same unsafe download can produce another detection that looks like persistence.

Update software after the device is cleaned

Microsoft advises keeping software updated after recovery because current versions include fixes for known security issues. Apply operating-system, browser, document-reader, and other application updates once the device is stable. If a work administrator controls updates, follow the organization’s process rather than installing unapproved software.

Protect accounts separately

Because an offline scan addresses the device, not every online session, use a different known-clean device to change important passwords if the infected computer was used for email, banking, administration, or work access. For a business incident, credential resets should be coordinated with containment and rebuilding; the organizational sequence is described below.

When scanning is not enough: reset, restore, or reinstall

Microsoft says a reset may be appropriate when malware has caused irreversible changes. A reset, a restoration from a clean backup, and a manual reinstall have different consequences, so choose based on the condition of the system and the recovery material you actually have.

Option When to consider it What to verify first Important trade-off
Reset Damage appears irreversible, or repeated scanning does not return the system to a trustworthy state. Identify files that must be preserved and follow the current Windows reset workflow. Apps, settings, and possibly personal files can be removed depending on the reset choice; read each on-screen option carefully.
Restore from backup You have a backup made before the infection and stored externally or otherwise known to be isolated from the infected PC. Confirm the backup date and that it was not available for malware to modify during the infection. Anything created or changed after that backup may not be included, and restoring a contaminated backup can reintroduce the threat.
Manual reinstall The installation is not trustworthy, or a clean rebuild is preferable to trying to preserve the current system. Installation media, product keys, setup files, and a safe copy of needed personal data. Applications and configuration must be installed again; required media or keys may be unavailable.

Microsoft’s recovery guidance covers reset, restore, and reinstall paths and notes that a manual reinstall may require installation discs, product keys, and setup files. Read the current instructions at Microsoft Support for the Windows edition and recovery method you plan to use. A USB flash drive for Windows installation media can be useful for a manual reinstall, but it is optional and depends on that workflow; it is not required to run Microsoft Defender Offline.

Preserve only data you can verify

Before resetting or reinstalling, list the documents, photos, and other files you need. Copying files does not make them safe automatically: executable files, scripts, macros, and unknown installers deserve particular caution. When possible, scan the copy from a known-clean environment and restore only data you can identify.

Use backups carefully

Restore from a backup created before the infection and kept externally when possible. Microsoft warns that backups present on the PC while it was infected may already have been changed by malware. An apparently successful restore can therefore put the same malicious file or altered document back on the rebuilt system. Keep the original backup protected until you have verified the recovered machine, and maintain a separate clean copy rather than overwriting your only recovery point.

What persistence can look like

Persistence means a malicious component arranges to run again after a restart, logon, or other trigger. CISA’s malware-analysis material documents malicious Windows services as one mechanism: a service can start with Windows and repeatedly execute a payload. CISA’s Emotet advisory gives additional examples—Registry Run keys or startup folders, Windows services, and scheduled tasks. These are examples of how persistence works, not a diagnosis of any particular computer, and the older Emotet advisory does not establish current prevalence.

Do not delete services, scheduled tasks, or registry entries at random. Removing the wrong item can make Windows or legitimate software fail while leaving the actual persistence mechanism intact. If you need to inspect these areas, preserve relevant evidence and use a qualified technician or your organization’s security team, especially when the computer contains business data.

When a recurring infection may be a wider compromise

For a personal computer that continues to alert after an offline scan, or for a business where more than one device or account is involved, repeated consumer scans are not a complete incident response. CISA’s #StopRansomware Guide recommends conducting extended analysis to identify both outside-in and inside-out persistence mechanisms, rebuilding cleaned systems from standard images where possible, and resetting affected credentials after the environment has been cleaned and rebuilt.

Contain affected systems

In an organization, isolate affected computers from networks according to the incident plan. Avoid actions that destroy useful evidence, such as wiping every system before logs and artifacts are collected. Secure backups so they cannot be altered or encrypted while the investigation continues.

Review evidence and rebuild consistently

Collect and review relevant logs, endpoint artifacts, authentication records, and signs of persistence. Rebuild systems from a standard, trusted image where possible instead of cloning a potentially compromised installation. After rebuilding and confirming containment, reset passwords, tokens, and other affected credentials.

Bring in outside expertise when scope is uncertain

CISA’s guidance for suspected advanced-threat activity recommends isolating affected systems, securing backups, reviewing logs and artifacts, and considering third-party incident-response assistance. See CISA’s guidance on understanding and mitigating state-sponsored cyber threats. This organizational advice is not a mandatory home-user checklist; it becomes relevant when several systems or accounts, ongoing unauthorized access, or business-critical services may be involved.

A practical decision path

  • One Windows PC, first recurrence: stop using the suspected source, save work, and run Microsoft Defender Offline.
  • Detection returns after the offline scan: preserve needed files cautiously, verify that the source is not being reopened, and evaluate reset, restore, or reinstall rather than repeating ordinary scans.
  • A clean pre-infection backup exists: prefer that known-clean recovery point over a backup that remained connected to the infected computer.
  • No trustworthy recovery path or irreversible damage: prepare for a reset or manual reinstall using the current Microsoft instructions and any required media, keys, or setup files.
  • Several devices, accounts, or signs of continuing access: isolate systems, secure backups, preserve logs and artifacts, and involve qualified incident-response professionals or your organization’s security team.

Sources and scope

The Windows scanning and recovery procedures come from Microsoft Support. Persistence examples are documented in CISA’s Malware Analysis Report 10365227.r2.v1 and Emotet Malware advisory. The containment, rebuilding, and escalation recommendations are from CISA’s organizational guidance, including the #StopRansomware Guide and its advanced-threat alert.
