Together they can review how long you have taken it, the dose and schedule, missed doses or other barriers, changes in symptoms, side effects, and every other medicine or supplement you use. Do not stop, increase, decrease, or switch the antidepressant on your own.
Some antidepressants take three to six weeks to show a therapeutic response, and clinicians commonly start at a low dose and adjust gradually. That waiting period is not a reason to ignore worsening depression, troublesome adverse effects, or symptoms that interfere with cancer treatment. The National Cancer Institute (NCI) advises seeking further help when those problems occur. NCI’s patient guidance explains the general process, while its health-professional summary covers treatment and interaction considerations.

First, tell the right clinicians
Send a message or make an appointment with the antidepressant prescriber and notify the oncology team. If different health systems are involved, ask them to communicate directly or give you a shared medication plan. Describe the problem in concrete terms:

- How long you have taken the antidepressant, the current dose, the prescribed schedule, and any doses you have missed.
- Any new or worsening adverse effects, including effects that make eating, sleeping, traveling to treatment, or taking medicines difficult.
- Whether low mood, anxiety, loss of interest, hopelessness, or poor concentration is making cancer treatment harder to complete.
- Every prescription, over-the-counter medicine, as-needed product, vitamin, herbal product, supplement, and medication patch you use.
This is a clinical reassessment, not a test of willpower. Cancer treatment can change sleep, appetite, energy, pain, concentration, and stress, and those changes can overlap with depression. Your clinicians need the whole pattern before deciding whether the antidepressant is ineffective, needs more time, is causing problems, or is being affected by another treatment.

Why an antidepressant may appear not to work
It may be too early to judge the response
NCI states that some antidepressants take three to six weeks to work. A low starting dose may be increased gradually rather than changed all at once. The time to benefit varies by medicine and person, so the prescriber should set the next review point. Do not treat three to six weeks as a promise that you will improve by a particular day, and do not delay a call if symptoms are severe or worsening.
The dose, schedule, or daily routine may need review
A clinician will check whether the prescribed dose has been reached, whether doses are taken consistently, and whether nausea, fatigue, treatment appointments, cost, memory problems, or other practical issues are interfering. Tell the clinician what actually happened rather than trying to compensate with an extra dose. An accurate account helps distinguish a medication that needs adjustment from one that has not yet had a fair, safe trial.
Another medicine or treatment may be affecting the plan
Cancer care often involves several prescriptions and supportive-care drugs. NCI notes that interactions, as well as cancer-related changes in absorption or metabolism, can matter. The appropriate antidepressant in one treatment regimen may be unsuitable with another. This is why the review must include medicines used only occasionally and products that do not require a prescription.

Depression may need more than one kind of support
Medication is only one possible part of care. Psychological treatment, practical support, social-work assistance, psychiatric consultation, or a coordinated combination may be appropriate. The choice depends on symptom severity, prior response, tolerability, interactions, availability, and your preferences. A lack of improvement does not mean you have failed treatment; it means the plan needs professional review.
Use the timing guidance correctly
Several time points appear in cancer-depression guidance, but they answer different questions. Use them as prompts for communication, not as rules for self-management.

The ASCO eight-week recommendation is specifically framed for adult cancer survivors. It should not be interpreted to mean that every person receiving active cancer treatment must wait eight weeks before asking for help. Worsening symptoms, adverse effects, or depression that interferes with treatment justify earlier contact.

Bring a complete medication list for an interaction check
Write down the name, strength, schedule, and purpose of each product. Include medicines prescribed by other specialists, treatments taken only when needed, injections, patches, vitamins, herbals, and supplements. If you use a pill organizer or a medication app, bring the list from that system but verify it against the bottles and your oncology instructions.
Ask the prescriber, oncology pharmacist, or another qualified clinician to compare the list with your current cancer drugs and supportive-care regimen. Do not assume that a product is harmless because it is nonprescription or because you have taken it for a long time. NCI specifically emphasizes concomitant medicines and the possibility that cancer-related changes in absorption or metabolism can alter treatment.
A specific tamoxifen example
NCI identifies paroxetine and duloxetine as CYP2D6 inhibitors that are not recommended with tamoxifen because they may reduce formation of tamoxifen’s active metabolite. This is a regimen-specific concern, not evidence that every antidepressant interacts with every cancer medicine. If you take tamoxifen, ask the oncology and prescribing teams to review the combination. Do not stop either medicine or substitute another one without a clinician-directed plan. What a clinician may reassess
Your clinicians may revisit the diagnosis and symptom pattern, how consistently the medicine has been taken, the current dose and duration, adverse effects, other medical conditions, and possible interactions. They may also ask about sleep, pain, anxiety, substance use, cognition, treatment stress, and symptoms that could have another medical explanation. The purpose is to identify the safest next step rather than to label the medicine a failure prematurely.
ASCO says treatment decisions for adult cancer survivors should consider evidence, adverse effects, tolerability, interactions, prior response, and patient preference. Depending on that review, a clinician might continue the medicine longer, adjust it, add psychological care, add another pharmacologic intervention, or change the medication. The sources do not identify one antidepressant or one combination as best for everyone.

Never make the change by yourself. NCI advises checking with the doctor before stopping and describes gradual dose reduction before another antidepressant when appropriate, to reduce problems from sudden discontinuation. The exact taper, transition, or washout depends on the specific drug, dose, duration, other medicines, medical conditions, and cancer regimen. A plan that is safe for one person can be unsafe for another.
Ask for written instructions that state which medicine to change, on what date, what dose to take at each stage, what to do if you miss a dose, and when to report symptoms. Confirm who will monitor you during the transition and how the oncology team will be informed. Do not use leftover tablets, share medication, or follow an old taper from a previous prescription.
Coordinate psychological and psychiatric care
ASCO describes collaborative care that can involve oncology or primary care, a care manager, psychosocial intervention, and psychiatric consultation when needed. Depending on local services, you might be referred to a psycho-oncology clinician, psychiatrist, psychologist, social worker, counselor, or another mental-health professional. Ask what is available through your cancer center, primary-care practice, or insurer, and tell the team if travel, language, cost, disability, or appointment timing limits access.
Psychological care is not a replacement for reviewing the antidepressant when the medicine may be interacting with cancer treatment or causing adverse effects. It is one part of a coordinated plan. Tell the team which forms of support you prefer, whether you have benefited from therapy before, and whether you want individual, family, group, in-person, or remote care where available.

Know when to seek urgent help
Contact the oncology team or prescribing clinician promptly if symptoms are worsening, adverse effects are troublesome, or depression is interfering with cancer treatment. Do not wait for a routine follow-up when the situation is deteriorating.
If you have thoughts of self-harm, feel unable to stay safe, or are in immediate danger, seek urgent help through local emergency services or a crisis service and tell the oncology team. The appropriate number and service depend on your country and location; the clinical sources cited here do not provide one universal crisis number. If possible, stay with a trusted person and remove immediate access to anything you might use to hurt yourself while help is being arranged.
Questions to take to the appointment
- How long should this medicine be given at the current dose before we reassess, and what symptoms should prompt an earlier call?
- Could any cancer medicine, supportive-care drug, supplement, or as-needed product be affecting its safety or effectiveness?
- Are my symptoms consistent with depression alone, or should pain, sleep, treatment effects, or another medical problem be evaluated?
- Would psychological, psychiatric, social-work, or psycho-oncology support be useful now?
- If a change is recommended, what is the exact taper or transition plan, who will monitor it, and whom should I contact about problems?
- How will the oncology team and the prescribing clinician share updates?
Write down the answers and the next review date. A shared plan reduces the risk of duplicate prescriptions, unplanned stopping, and missed interaction checks.
