Stop before acting. A fluent, confident chatbot answer can be completely wrong or contain serious errors. If the advice could affect your health, safety, money, legal position, privacy, or another person, treat it as unverified information: assess immediate danger, get a qualified human or authoritative source to check it, and use emergency or crisis services when someone may be at risk.

The World Health Organization (WHO) warns that large-language-model responses can sound authoritative and plausible while being incorrect, particularly for health questions. Its guidance also cautions that sensitive information, including health data, may not be protected by an AI application. WHO guidance, 16 May 2023

First, decide whether this is an emergency

Do not continue a chatbot conversation as though it were a safety check. Move to live help immediately if the response is connected to imminent self-harm, harm to someone else, severe medical symptoms, an overdose, a dangerous physical situation, or instructions that could cause serious injury or death. NIST’s AI Risk Management Framework says risks involving severe injury or death deserve the most urgent prioritization and the most thorough risk management; that framework is general risk guidance, not a substitute for an emergency protocol. In the United States, the American Psychological Association (APA) lists calling 911, calling or texting 988, or going to the nearest emergency department for a mental-health crisis. Those numbers are US-specific. Elsewhere, contact your local emergency number, crisis line, poison center, or emergency department. Do not wait for the chatbot to “clarify” a crisis response.

Situation What to do now Why
Imminent danger, self-harm, violence, or severe distress Contact local emergency or crisis services and stay with the person if it is safe. In the US, APA identifies 911, 988, or an emergency department. A chatbot cannot monitor a person, dispatch help, or make a reliable crisis assessment.
Possible poisoning, severe symptoms, or a dangerous treatment instruction Call emergency services or the appropriate poison/medical service and seek urgent clinical care. Delaying professional assessment can make harm worse.
High-consequence but not immediately dangerous advice Pause the action and verify it with an appropriate professional or primary authority. You have time to check the claim before creating an irreversible consequence.
Low-stakes error Check the underlying source, correct the answer, and avoid passing it on. Even harmless-looking errors can become harmful when repeated or applied in the wrong context.

What counts as dangerous or misleading advice?

Danger is not limited to an obviously extreme instruction. A response deserves a pause when it tells you to stop prescribed treatment, substitute a diagnosis for an examination, combine medicines, ignore a warning, reveal private information, send money, break a law, or take an action whose consequences you cannot easily reverse. Advice can also mislead by omitting a key warning, presenting one possibility as certain, or attaching a citation that does not actually support the claim.

Separate the answer into three kinds of statements:

  • Facts: claims about symptoms, laws, prices, compatibility, deadlines, or what happened.
  • Recommendations: what the chatbot says you should choose or believe.
  • Instructions: steps to take, such as changing a dose, deleting evidence, transferring funds, or configuring equipment.

Instructions and recommendations with high consequences need independent verification even when the factual background sounds credible. There is no reliable universal statistic for how often consumer chatbots give dangerous advice, so do not infer safety from a claimed error rate or from polished wording.

Verify the claim instead of trusting the tone

Read the original source

If the chatbot provides links or citations, open each one. Confirm that the source exists, is current for your country and situation, and actually supports the sentence that cites it. The Centers for Disease Control and Prevention (CDC) tells reviewers of generative-AI output to check citations, and to review for accuracy, completeness, hallucinations, and misleading content. A real citation can still be irrelevant or overstated. CDC considerations for generative AI

Prefer primary and accountable authorities

Use the agency, regulator, manufacturer, professional association, court, bank, or other body responsible for the underlying decision. Check the edition, jurisdiction, date, and any eligibility conditions. For a product safety recall, use the relevant regulator or manufacturer notice; for a tax or benefits question, use the responsible government agency; for a legal deadline, consult the court or a licensed lawyer. The appropriate authority varies by subject, so a general search result or another chatbot is not a substitute for the accountable source.

Ask a qualified person who can see the context

For symptoms, diagnosis, medication, treatment, or mental health, ask a licensed health care practitioner. APA says AI is not an accurate way to diagnose medical or mental-health conditions, recommends verifying information with a practitioner, and suggests telling your care team when you have used AI. A practitioner can consider your examination, history, current medicines, allergies, measurements, and changes over time—information a chatbot may not have or may misinterpret. Do not change a prescribed medicine or treatment solely because a chatbot recommends it.

A practical verification workflow

  1. Freeze the consequential action. Do not take the medicine, stop treatment, send funds, publish an accusation, disclose a password, or alter a safety-critical system until checked. If the action has already happened, tell the relevant professional or service exactly what occurred.
  2. Save the relevant exchange. Keep the prompt, response, timestamp, model or service name, links, and any files or images that affected the answer. Screenshot or export it before the conversation changes. Redact unnecessary personal information before sharing it.
  3. Rewrite the claim in a testable form. For example: “Does this medicine interact with my prescription?” is easier to verify than “Is this treatment safe?” Record the person’s age, location, date, product version, or other context only when the professional or primary source needs it.
  4. Check multiple authoritative points. Compare the claim with the responsible primary source and, where appropriate, a second independent professional source. Look for warnings, exceptions, contraindications, deadlines, and definitions the chatbot omitted.
  5. Ask for an explicit decision. Tell the professional what the chatbot said and ask what action is safe, what signs require escalation, and what information would change the advice. Do not hide the chatbot’s involvement; it may explain a risky decision or a delay.
  6. Act on the verified guidance and monitor. Follow the professional or authority’s instructions. If symptoms, threats, financial loss, or other consequences worsen, escalate through the relevant emergency or complaint channel rather than returning to the chatbot.

Medical and mental-health advice needs special handling

Health questions combine incomplete personal data with consequences that can be immediate. WHO says large-language-model answers may appear authoritative and plausible while being completely incorrect or seriously erroneous, especially in health contexts. WHO caution on health LLMs

Use a chatbot, at most, to help formulate questions for a clinician or explain terminology after a professional has given you information. It should not be the sole basis for a diagnosis, emergency decision, dose change, treatment choice, or decision to delay care. Tell the clinician the exact answer and any action you took. In a mental-health crisis, switch from text generated by the bot to a person who can provide immediate support and emergency intervention.

WHO’s broader governance guidance describes more than 40 recommendations for governments, technology companies, and health-care providers; that count is not a measure of chatbot accuracy or user harm. WHO large multimodal model guidance, 18 January 2024 WHO’s 2021 health-AI guidance also sets out six principles: protect autonomy; promote human well-being, human safety, and the public interest; ensure transparency, explainability, and intelligibility; foster responsibility and accountability; ensure inclusiveness and equity; and promote responsive, sustainable AI. Protect privacy while you investigate

Do not paste more sensitive information into the chatbot just because its first answer was uncertain. Remove names, addresses, account numbers, passwords, identification numbers, exact dates of birth, and another person’s private details unless a qualified service specifically requires them. WHO warns that LLM applications may not protect sensitive data, including health data. WHO privacy warning

Keep enough of the exchange to explain what happened, but store it securely and share it only with the professional, regulator, or platform that needs it. If you uploaded a document or image containing confidential information, check the service’s deletion and account-security controls, change exposed credentials, and notify the affected institution when appropriate.

When and how to report the chatbot

Reporting is issue-specific and depends on your jurisdiction. Preserve the exchange, the date, the service identity, the advice, the harm or near miss, and any supporting documents. Do not publish another person’s private information merely to make a report.

Possible routes include:

  • The chatbot platform’s in-product safety or abuse report channel.
  • A professional licensing board or health regulator when the service appears to provide regulated care.
  • A consumer-protection or data-protection authority when there is deception, financial harm, or privacy misuse.
  • Emergency services or police when there is an immediate threat, rather than a routine platform complaint.

These routes are not interchangeable. For example, Pennsylvania’s Department of State says no AI chatbot is licensed to practice a health-care profession in Pennsylvania and accepts reports about negative experiences or suspected unlicensed chatbot health care. This is a Pennsylvania-specific process, not a universal reporting mechanism. Pennsylvania Department of State chatbot report

In another state or country, identify the regulator whose remit covers the conduct and follow its instructions. If no regulator accepts the complaint, a platform report can still create a record, but it does not replace medical, legal, financial, or emergency help.

Tell the difference between correction, escalation, and abandonment

Problem Best next route What not to assume
An incorrect answer with no immediate harm Verify it, correct your notes, and report it to the platform if useful. That a confident tone means the model “checked” the fact.
Potential harm from regulated advice Contact the relevant licensed professional or regulator; preserve evidence. That the chatbot is licensed because it uses medical or legal language.
Privacy exposure Secure accounts, limit further sharing, and contact the affected organization or data authority as appropriate. That deleting a chat guarantees every copy has been erased.
Immediate danger Use local emergency or crisis services first. That a platform ticket will be answered quickly enough to keep someone safe.

What not to do

  • Do not ask the same chatbot repeatedly until it produces the answer you want.
  • Do not treat a citation, disclaimer, professional-sounding vocabulary, or high confidence as proof.
  • Do not forward an unverified answer to family, patients, customers, or colleagues as an instruction.
  • Do not reveal additional private information to “help” the model diagnose the situation.
  • Do not delay emergency care while collecting a perfect transcript or waiting for a platform response.

The safer role for a chatbot

A chatbot can help you organize questions, summarize information you already trust, translate general terminology, or create a checklist for a conversation with a professional. Keep the boundaries explicit: it is an information aid, not a diagnosing clinician, emergency dispatcher, licensed adviser, or accountable decision-maker. WHO Chief Scientist Dr. Jeremy Farrar summarized the condition for useful deployment: “Generative AI technologies have the potential to improve health care but only if those who develop, regulate, and use these technologies identify and fully account for the associated risks.” WHO statement, 18 January 2024
