Do not take a medicine or choose a dose based on an uncertain AI reading. Ask the prescribing clinician or dispensing pharmacist to check the original prescription and confirm the medicine and directions first. If you already received the medicine and it does not match what you expected, contact the pharmacy or prescriber before using it.
Why an AI reading is not confirmation
An AI transcription is an interpretation, not proof of what the clinician intended. The official guidance cited here does not validate AI systems for reading handwritten prescriptions, so do not use an unclear AI result to infer a medicine, strength, or dose. AHRQ also cautions that if a patient cannot read a doctor’s handwriting, the pharmacist may not be able to either; ambiguity needs to be resolved with the prescriber, not guessed at. AHRQ’s medicine-safety guide recommends getting answers about a new prescription before taking it.

What to do before taking a dose
- Pause. Do not start, change, or take a dose based on a reading you are unsure about.
- Contact the dispensing pharmacist or prescribing clinician. Ask them to compare the original prescription with the intended medication and directions. Keep the paper or image available while you speak with them.
- Ask them to confirm the details explicitly. Check the medicine name, strength, dose, route, and schedule; do not fill in any unclear part yourself. FDA advises patients to ask pharmacists how and when to use medicines and what dose to take. FDA’s Stop – Learn – Go guide also prompts patients to ask about missed or excess doses and other safety questions.
- Compare the answer with the dispensed package. If the medicine or directions on the label differ from what you expected, tell the pharmacist or prescriber and ask what to do before using it. FDA recommends checking the medicine and dose before leaving the pharmacy and asking about differences or uncertainty.
If you have already taken the medicine
Contact a pharmacist or prescriber promptly and explain what you took, when, and what remains unclear; ask for advice specific to the medicine and situation. The U.S. guidance cited here does not establish a single response for every drug, amount, or symptom, so do not try to correct the situation by changing a later dose on your own. If you believe there is an immediate medical emergency, use your local emergency service.
Reporting a medication error in the U.S.
Once the immediate question of what to take has been handled, medication errors can be reported to FDA through MedWatch. Reporting is not a way to verify a prescription or receive treatment. FDA describes reporting to its adverse-event system as voluntary and encourages reports; its consumer guidance also explains how to seek help from pharmacists. FDA’s pharmacist resource describes pharmacists as a source of medication information and questions. The reporting information here is U.S.-specific; people elsewhere should use their local health-service and medication-safety reporting channels.

Reduce uncertainty with the pharmacy label and counseling
When a prescription is filled, read the pharmacy label and ask the pharmacist to explain any direction that is unfamiliar or does not match what the prescriber told you. FDA notes that prescription labeling for patients can also include Medication Guides, Patient Package Inserts, and Instructions for Use; questions about a drug should be discussed with a healthcare professional. See FDA’s prescription-labeling FAQ. Electronic prescriptions are easier to read and have safeguards intended to reduce medication errors, according to MedlinePlus; that general point does not establish how accurately a consumer AI tool reads handwriting.
