What to Do If a Whole-Body MRI Finds an Incidental Abnormality

Get the complete MRI report and review the exact finding and any recommended next step with the clinician who ordered the scan or your primary-care clinician. Ask who will arrange follow-up and how you will receive the results. “Incidental” means the finding was noticed while imaging for another reason; it is not a diagnosis and does not, by itself, say whether the finding is serious.

What does “incidental” mean on an MRI report?

An incidental finding is an abnormality noticed during imaging performed for another reason. Its significance depends on what the report describes, where it is, your medical history and symptoms, and whether earlier images are available for comparison. The word alone does not mean cancer, but a specific recommendation in the report should not be ignored.

Radiology reports may describe a finding as confidently benign, indeterminate, or suspicious, and may recommend comparison with earlier images, additional imaging, consultation, or no further action. Treat those terms as prompts for a clinician conversation, not as a way to diagnose yourself.

What to do next

  1. Get the full written report. Ask the imaging service or ordering clinician for it if you do not have it. Read the impression and any recommendation, not just a brief summary or portal notification.
  2. Note the exact wording and location. Record what body area is mentioned and what the radiologist recommends. Avoid trying to choose a test based on the finding’s name alone.
  3. Contact the ordering clinician or your primary-care clinician. Ask what the finding may represent, how your history and symptoms affect its interpretation, whether follow-up is recommended, and how soon it should happen.
  4. Share relevant context. Tell the clinician about symptoms, relevant risk factors, prior diagnoses, and earlier imaging. A recommendation designed for one finding or patient group may not apply to another.
  5. Close the loop if follow-up is advised. Confirm who will place the order or make the referral, when it is expected to happen, who will review the result, and how that result will be communicated.

If the report or a clinician describes the finding as urgent, follow their instructions promptly. New severe or rapidly worsening symptoms also warrant prompt contact with a healthcare professional or local emergency services as appropriate. Without the specific finding and clinical context, no general article can determine its urgency.

Why there is no single follow-up test or timetable

Whole-body MRI can show abnormalities in different organs and tissues, so the right next step depends on the particular finding and the person’s clinical context. A clinician may recommend comparing prior images, targeted imaging, a specialist assessment, another diagnostic test, or no additional work-up.

The American College of Radiology’s incidental-findings resources are organized around particular findings and explain the patient populations and context for their recommendations. The ACR also notes that when effective management guidance is lacking, very low-risk findings can lead to over-testing and over-treatment. A measured, clinician-led plan is more useful than either assuming the worst or pursuing tests indiscriminately.

When discussing possible next steps, ask what question each test or consultation is meant to answer, whether it fits this finding and your history, what timing is appropriate, what burdens or risks it may involve, and how the result could change care. The clinical team can apply the report and relevant guidance to your situation.

Does an incidental MRI finding mean cancer?

No. “Incidental” describes how the finding was discovered, not what it is. The term alone cannot establish whether a finding is harmless, needs clarification, or requires further evaluation. The report’s specific description and a clinician’s interpretation in light of your health history are needed to understand it.

Does whole-body MRI replace routine screening?

No. Whole-body MRI should not be treated as a substitute for primary care or screening recommended for your age and risk profile. A normal scan should not be used to cancel established screening without discussing that decision with a clinician.

In an April 17, 2023 statement, the American College of Radiology said it did not find sufficient evidence to recommend total-body screening for people without symptoms, risk factors, or family history suggesting disease or serious injury. It also raised concern that nonspecific findings may prompt follow-up tests or procedures that do not improve health, with added expense. This position concerns screening in that population; it does not determine what should happen after a specific finding in an individual report.

Questions to take to your clinician

  • What exactly did the radiologist see, and where?
  • Does the report recommend comparison with older images, another test, a referral, or no further action?
  • How do my symptoms, medical history, and prior imaging affect the recommendation?
  • Who is responsible for arranging the next step and reviewing the result?
  • How and when will I hear about the result, and whom should I contact if I do not?

The ACR describes its performance measures as supporting completion of evidence-based follow-up recommended by radiologists. For an individual, the practical safeguard is to leave the conversation knowing who owns the next step and how the result will get back to you.
