Do not pull or probe under a lifting nail. This separation, called onycholysis, means the nail plate has come away from the nail bed. Trim only a fully detached edge that can be clipped without pain, keep the nail short and protected, avoid irritants and repeated trauma, and arrange an examination—especially when the cause is unclear or the nail is painful, discolored, or inflamed.
What a lifting nail means
Onycholysis is the medical term for separation of the hard nail plate from the skin beneath it. The detached area often looks sharply defined and opaque white because air is under the nail. Separation can begin at the free edge, along one side, or, less commonly, near the cuticle. It may affect one nail or several. A fingernail or toenail can lift only partly, or the separation can progress toward the base.

Onycholysis is a sign, not a diagnosis. The appearance alone cannot identify why it happened or which treatment is appropriate. A nail can lift after a single injury, but it can also reflect repeated irritation, infection, inflammatory skin disease, a systemic illness, or a medicine effect. A loose edge may catch on clothing or footwear, but catching does not mean the nail should be pulled off.

What to do immediately
Leave the attached nail alone
Do not pull, peel, bend, or force the nail back onto the nail bed. Do not slide a pin, file, cuticle tool, toothpick, or other sharp object underneath it. Trying to clean the gap can tear tissue that is still attached, worsen the separation, and create an entry point for infection. Do not glue the nail down or tape it tightly in an attempt to make it reattach.

Trim only a safely detached edge
If a portion is hanging free, you may gently clip only the part that is no longer connected, provided the cut can be made without pain and without cutting skin. Use clean basic nail clippers and make small, controlled cuts. Stop if the nail resists, bleeds, feels tender, or appears attached at the point you are trying to cut. Do not cut into the pink or firmly attached portion. The American Academy of Dermatology recommends this conservative trimming approach for an injured nail. Basic nail clippers are a task aid, not a treatment for the underlying problem.
After clipping, smooth a sharp corner carefully with a file held across the edge rather than digging beneath the nail. If the loose piece cannot be clipped without discomfort, cover it lightly to reduce snagging and arrange professional advice instead of forcing it.
Keep the nail short and prevent snags
Once a loose edge has been clipped, keep the nail short with careful, regular trimming as it grows. A shorter edge is less likely to catch on clothing, bedding, hair, gloves, or footwear. Avoid activities that repeatedly strike, pinch, bend, or scrape the affected finger or toe until it has been assessed. Choose shoes with enough room for the toes and avoid pressure over a lifted toenail.

While the nail is separated, avoid nail enamel, remover, solvents, and detergents that can irritate the nail unit. Pause manicures, artificial nails, aggressive cuticle work, and scraping under the nail until it has recovered or a clinician advises otherwise. For wet work, wear protective gloves; for prolonged tasks, a cotton glove under a vinyl glove can reduce moisture and friction against the skin. Change damp gloves when practical. Dry the hands and feet well after washing, without pushing anything under the nail.
Arrange a clinical assessment
Book an appointment with a primary-care clinician or dermatologist, particularly when there was no obvious injury, the lifting persists or worsens, more than one nail is involved, or the nail has pain, odor, color change, or inflamed surrounding skin. The American Academy of Dermatology states: “A dermatologist should examine any nail that’s lifting up.” Assessment is important because several different problems can look alike and require different treatment.
Why a fingernail or toenail can separate
Several unrelated problems can produce a similar-looking gap. Possibilities include:
- A single blow, crush, pressure injury, or sports-related impact.
- Repeated minor trauma, such as aggressive manicures, scraping beneath the nail, tight footwear, or activities that strike or bend the nail.
- Prolonged exposure to water, detergents, solvents, or other chemicals.
- Nail cosmetics or their removers irritating the nail unit.
- Fungal or yeast infection. A lifted nail can also permit a secondary infection beneath it.
- Psoriasis and other inflammatory skin diseases.
- Systemic conditions, including illnesses that affect nail growth.
- Medicines that alter nail growth or cause photosensitivity.
These are possibilities rather than a checklist for self-diagnosis. One nail lifting after a clear injury points toward trauma, but does not prove it. Several nails lifting, or separation without an identifiable injury, makes a broader assessment more important. A greenish or dark color, tenderness, or redness can occur with infection or injury, but visual clues are not reliable enough to select an antifungal, antibiotic, steroid, or other medicated product. Do not start or stop such treatment without appropriate medical advice. If you take a prescription medicine that might be contributing, speak with the prescriber rather than stopping it yourself.
When a lifting nail needs urgent care
After an injury
Seek prompt professional or emergency care after trauma if any of the following applies:
- You cannot bend the injured finger or toe.
- Blood covers more than half of the nail.
- The nail is black or purple.
- The injury is particularly painful.
These findings can accompany a significant nail-bed injury or a fracture and should not be managed by clipping at home. A crush injury may need assessment even when only part of the nail has lifted.

Without a clear injury
Arrange medical advice for an unexplained change in nail shape or color, nail loss, or skin around the nail that becomes sore, red, swollen, or warm. Increasing pain, warmth, swelling, drainage, or spreading discoloration should be reported rather than watched indefinitely. A new or changing dark streak under a nail also warrants prompt dermatology evaluation. Do not assume that a changing streak is harmless, but do not try to diagnose its cause from photographs or color alone.
How clinicians work out the cause
A clinician will ask when the separation began, whether there was trauma or chemical exposure, which nails are affected, and whether you have skin symptoms or other health changes. Mention manicures, artificial nails, wet work, tight shoes, new medicines, psoriasis, and any color or odor change. The nail and surrounding skin are examined for the pattern of separation, inflammation, debris, discoloration, and signs of injury.

Blood tests may be used when a systemic cause such as thyroid disease is suspected. These tests are directed by the examination and history; they are not routine for every lifting nail. Testing can prevent a person from treating the wrong condition for months.
Treatment depends on the cause
Trauma or irritant exposure
If repeated impact, wet work, chemicals, or cosmetics are responsible, treatment centers on removing the trigger, protecting the nail, and allowing undamaged nail to grow forward. Trimming the free edge reduces snagging but does not make the separated portion reattach. There is no evidence-backed consumer product that can be compared as a universal treatment before the cause is known.
Confirmed infection
A clinician may recommend a targeted treatment after examining the nail and, when needed, testing a clipping or scraping. Do not choose an antifungal or antibiotic solely because the nail is discolored; several conditions look alike, and the wrong product can delay the correct care. A secondary infection beneath a separated nail is possible, so new pain, warmth, swelling, drainage, or worsening odor deserves medical review.
Associated skin, systemic, or medicine-related disease
Management may involve treating an inflammatory skin condition, evaluating a systemic illness, or reviewing a potentially contributing medicine with the prescriber. Never discontinue a prescribed medicine on your own. Treating the associated condition and protecting the growing nail are often both necessary.
What recovery looks like
The detached section cannot fuse back onto the nail bed. DermNet summarizes this plainly: “The detached portion of the nail will not reattach.” Recovery means new nail grows from the base and remains attached as it advances. The old separated part gradually moves outward and is trimmed away. Clipping the loose edge is therefore a safety measure, not a way to cure onycholysis.
DermNet’s December 2020 guidance gives typical full-regrowth estimates of 4–6 months for a fingernail and about twice as long for a toenail. These are estimates, not deadlines. The cause, how long the nail has been separated, the condition of the nail bed, and whether the trigger continues all affect the result. Long-standing separation is less likely to reattach normally as new growth emerges. A toenail may therefore remain visibly abnormal for many months even after the cause has been corrected.
Use the pattern to decide how quickly to seek advice
| Pattern | What it can suggest | Practical response |
|---|---|---|
| One nail lifted after a clear recent injury | Trauma or nail-bed damage is possible | Protect and trim only a painless, fully detached edge; seek prompt care if severe-injury signs are present. |
| Several nails lifting or no clear injury | An irritant, infection, inflammatory disease, systemic condition, or medicine effect may be involved | Arrange a clinical examination rather than treating empirically. |
| Lifting with greenish or dark discoloration, pain, or inflamed skin | Infection or injury is possible, but appearance is not diagnostic | Obtain medical advice; do not start an antibiotic or antifungal solely from the color. |
| New or changing dark streak beneath a nail | A nail change requiring dermatology assessment | Seek prompt evaluation and do not reassure yourself that it is harmless. |
| Black or purple nail, blood over more than half the nail, inability to bend, or severe pain after trauma | Potentially significant injury | Seek prompt professional or emergency care. |
Habits that protect the new nail
- Trim straight and carefully, without cutting into attached nail or surrounding skin.
- Choose footwear with enough room to avoid repeated toe pressure.
- Use gloves for wet or chemical work and change damp gloves when practical.
- Pause manicures, artificial nails, polish, remover, and aggressive cuticle work until the nail has recovered or a clinician advises otherwise.
- Do not clean under the nail with sharp tools.
- Monitor for increasing pain, warmth, swelling, redness, drainage, or spreading discoloration and report those changes.
- Do not use a cosmetic coating to hide a changing color or dark streak before it has been examined.
Protective care limits additional damage while the cause is identified. It cannot reattach the portion that has already separated, and it should not replace an examination when the change is unexplained, persistent, progressive, or associated with warning signs.

Frequently Asked Questions
Should I cut off a loose nail?
Only clip the portion that is completely detached and can be cut without pain or contact with skin. Stop if it resists, bleeds, or is tender. Do not cut attached nail or pull the piece away.
Can a lifted nail reattach?
The detached portion will not reattach. New nail must grow from the base and remain attached as it advances.
How long does a lifted nail take to grow out?
Typical full regrowth is about 4–6 months for a fingernail and roughly twice as long for a toenail, according to DermNet’s December 2020 guidance. The timing varies with the cause and nail-bed condition.
Should I put antifungal cream or antibiotic ointment under it?
Do not choose a medicated product based only on color or appearance. Infection is only one possible cause, and a clinician may need to examine or test the nail before recommending treatment.
When is a lifting nail an emergency?
After an injury, seek prompt care if you cannot bend the finger or toe, blood covers more than half the nail, the nail is black or purple, or the pain is particularly severe.

What if several nails are lifting?
Several affected nails or separation without a clear injury can reflect an irritant, infection, inflammatory disease, systemic condition, or medicine effect. Arrange a clinical examination rather than self-treating.
Can I wear nail polish or artificial nails?
It is safer to pause polish, remover, artificial nails, and aggressive manicure work while the nail is separated, because these can irritate or traumatize the nail unit and hide changes that need assessment.
Who should examine a lifting nail?
A primary-care clinician or dermatologist can assess it. A dermatologist is particularly appropriate when the cause is unclear, the change persists or worsens, several nails are involved, or there is a new or changing dark streak.
The Bottom Line
Protect rather than pull. Clip only a painless, fully detached edge, keep the nail short, avoid trauma and irritants, and arrange medical assessment to identify the cause. Seek prompt care for severe injury signs, infection-like inflammation, unexplained progression, or a new or changing dark streak.
