A low-grade fever after vaccination is often short-lived, but a high, worsening, persistent or otherwise concerning fever needs medical advice. For a baby younger than 3 months, a temperature of 100.4°F (38°C) or higher requires immediate contact with the child’s doctor, even if a vaccine was just given. Breathing trouble, facial or throat swelling, a seizure, difficulty waking, or a child who looks seriously ill warrants emergency assessment rather than home monitoring.
Start with urgency, not the presumed cause
Use the person’s age, measured temperature, duration, trend and other symptoms to decide what to do. The fact that a fever followed a shot does not prove the vaccine caused it; an unrelated infection or another condition can begin at the same time. Do not wait for a routine time limit when emergency symptoms are present.

| Situation | What to do |
|---|---|
| Baby younger than 3 months with 100.4°F (38°C) or higher | Contact the child’s doctor immediately for assessment. |
| Child with a temperature above 104°F (40°C), especially repeatedly | Seek prompt medical advice; use emergency services if the child also has serious symptoms. |
| Any age with trouble breathing, face or throat swelling, seizure, extreme drowsiness or collapse | Call emergency services or go to emergency care now. |
| Fever that is worsening, lasts beyond the expected period or returns after resolving | Contact a healthcare professional for individualized guidance. |
| Mild fever with no warning signs and otherwise comfortable behavior | Offer fluids, keep clothing comfortable, measure accurately and watch the trend. |
The temperature and duration thresholds in this article are primarily U.S. guidance from the Centers for Disease Control and Prevention (CDC) and the American Academy of Pediatrics (AAP). Local services may use different pathways, so use your country’s urgent-care and emergency numbers when needed.
When a post-vaccination fever can be expected
Vaccines activate the immune system. A low-grade fever, sore arm or general discomfort can occur as part of that response. The CDC says, “For the most part these are minor (for example, a sore arm or low-grade fever) and go away within a few days.” The statement appears in its Possible Side Effects from Vaccines guidance, dated July 31, 2024.

Timing differs by vaccine, so there is no single schedule that applies to every product. A fever that starts later than expected, becomes more intense, or continues instead of improving should be reassessed rather than automatically attributed to the shot. Tell the clinician which vaccine was given, when it was given and when the temperature first appeared.
Measure the temperature correctly
A number is useful only when the measurement method and timing are clear. Use a working digital thermometer, follow its instructions and record the reading, time and method. Recheck if a result seems inconsistent with how the person looks, but do not postpone urgent care while repeatedly measuring.
The AAP’s How to Take Your Child’s Temperature explains several thermometer approaches and identifies rectal measurement as the best reading method for infants under 3 months. Tell the clinician whether a reading was rectal, oral, underarm, ear or forehead because methods can produce different results. A thermometer cannot identify the cause of a fever or override an age-based warning threshold.

Infants younger than 3 months
A temperature of 100.4°F (38°C) or higher in a baby younger than 3 months needs immediate contact with the child’s doctor. This applies even when the baby received a vaccine recently and even when the baby otherwise appears reasonably well. Young infants can become seriously ill quickly, and the post-vaccination context should not lower the urgency.

The AAP immunization symptom guidance also says not to give fever medicine to a baby under 12 weeks before the baby has been seen. Call the child’s clinician or the local urgent service for instructions on where to go. If the baby is difficult to wake, has breathing difficulty, turns blue or gray, has a seizure or appears severely ill, call emergency services.
Older babies and children with very high fever
The AAP vaccine-reaction symptom checker advises seeking care now for a fever higher than 104°F (40°C). Its general fever guidance also calls for prompt advice when temperatures above 104°F occur repeatedly. A high number matters, but the child’s appearance and associated symptoms matter just as much.
Seek urgent assessment for high fever accompanied by breathing difficulty, a stiff neck, unusual drowsiness or difficulty waking, an unexplained rash, repeated vomiting or a seizure. A child who looks very ill should be assessed urgently even if the measured temperature is below 104°F.

Fever that lasts or returns
For a child, the AAP immunization guidance says to contact a doctor within 24 hours when fever lasts more than three days or returns after being gone for more than 24 hours. The AAP’s broader fever advice uses additional age-dependent duration rules, so do not apply one duration limit to every child. Have the temperature record and symptom timeline ready when you call.

Emergency signs after any vaccine
A severe allergic reaction is an emergency. Trouble breathing, wheezing, swelling of the face, lips or throat, widespread hives, sudden weakness or collapse can signal anaphylaxis. The CDC’s vaccine side-effect guidance and the AAP immunization symptom checker both direct readers to emergency care for serious reactions.
Seizures, severe confusion, inability to stay awake, blue or gray skin, or rapidly worsening illness also require emergency services. Do not give fluids, fever medicine or a bath as a substitute for emergency evaluation. If the person becomes unresponsive or has severe breathing problems, call emergency services and follow the dispatcher’s instructions.
Adults: when to call about a high or persistent fever
The reviewed CDC material does not establish one universal post-vaccination temperature cutoff for every adult, vaccine and medical condition. Instead, contact a healthcare professional when the fever is high, persists beyond a few days, worsens, or is accompanied by symptoms that concern you. The appropriate response can differ for an older adult, someone who is pregnant, a person with an immune-suppressing condition, or someone with significant heart, lung or neurological disease.

Use the CDC’s Getting Your COVID-19 Vaccine after-shot advice as a general example of the approach: fluids and comfortable clothing can help with mild discomfort, but contact a provider when side effects are worrying or do not seem to be going away after a few days. Fluids and comfort
Offer frequent fluids when the person is awake and able to drink. For children, breast milk or usual formula can be continued unless a clinician says otherwise. Dress the person comfortably; avoid heavy bundling and overheating. Rest is reasonable, but check that a child can be roused and is interacting in a way that is normal for them.
Medicines
For a child, ask the child’s healthcare professional before giving a non-aspirin pain reliever after vaccination. A safe dose depends on age, weight, product concentration, other medicines and medical history. Do not copy a dose from another child, combine products with the same ingredient or give aspirin unless a clinician specifically directs it. The AAP guidance for babies under 12 weeks is stricter: do not give fever medicine before medical assessment.
Do not delay urgent evaluation while trying medicine, fluids or cooling measures. Fever reducers may make someone more comfortable but do not treat a serious infection or allergic reaction.

Monitoring
Write down the vaccine name if available, the injection time, each temperature with its measurement method, medicines given and new symptoms. Note urine output, drinking, alertness and breathing. This information helps a clinician distinguish a brief reaction from an illness requiring examination.
Why a fever may not be from the vaccine
Vaccination and infection can occur close together by coincidence. A respiratory, stomach or urinary infection may begin after the appointment, and the vaccine may not be the explanation. Persistent or returning fever, new cough, sore throat, diarrhea, painful urination, rash or localized worsening should be described specifically rather than summarized only as “a vaccine reaction.”
Do not stop prescribed medicines or skip needed testing because a shot was recently given. A clinician may ask about contacts, travel, chronic conditions, immune status and other recent exposures before deciding on next steps.
What to tell the healthcare professional
Have these details ready:
- Age, pregnancy status when relevant, immune conditions and major chronic illnesses.
- The vaccine name, dose or product if known, and the date and time it was administered.
- Highest measured temperature, measurement method, times recorded and whether the fever is rising, steady, resolving or returning.
- All other symptoms, including breathing changes, rash, vomiting, diarrhea, pain, seizure, unusual sleepiness and drinking or urination.
- Any medicine given, including the exact product, strength, amount and time.
When calling for a baby under 3 months or a child above the AAP high-fever threshold, say the temperature and age first so the service can triage appropriately.
Bottom line for deciding what to do
Mild fever that is improving within a few days can usually be managed with fluids, comfortable clothing and observation. A baby under 3 months at or above 100.4°F (38°C), a child above 104°F (40°C), fever that lasts or returns, or any breathing problem, seizure, severe drowsiness, facial or throat swelling or seriously ill appearance requires medical advice or emergency care at the level described above. Because adult thresholds are not universal, adults should seek individualized guidance for a high, persistent, worsening or worrying fever.
