What to Do If a Doctor Says Your Symptoms May Be Psychological

Being told that symptoms may have a psychological component does not mean they are imagined, fabricated or “all in your head.” It means the clinician is considering how stress, mood, trauma, attention, sleep or other factors might affect your symptoms, often while the medical explanation is still uncertain. Ask exactly what the doctor means, what has been assessed, what remains possible, and when you will be reviewed. Psychological support can be considered alongside appropriate medical care. New, severe or rapidly worsening symptoms still need urgent assessment.

What “psychological” can mean in a medical appointment

The word is not a diagnosis by itself. A clinician might be describing a possible contribution from anxiety, depression, stress, past trauma, sleep disruption or the way the nervous system processes sensations. They might also be using it to describe symptoms that remain medically unexplained after an initial assessment. Different clinicians and health systems use different terms, so ask for the specific working explanation rather than trying to interpret one word.

The NHS uses the phrase “medically unexplained symptoms” for persistent physical symptoms where no clear medical cause has been identified. Its patient guidance states: “This doesn’t mean the symptoms are faked or ‘all in the head’ – they’re real and can affect your ability to function properly.” The same page reports that about one in four people who see a GP have physical symptoms that cannot be explained. That figure refers only to people consulting a GP in the context of that NHS guidance; it is not a global prevalence estimate or a measure of psychological illness.

A suggestion that psychological factors may be involved also does not prove that every physical cause has been excluded. An initial examination or blood tests may be reassuring without answering every question, and symptoms can have more than one cause. Treat the conversation as a prompt to clarify the plan, not as a reason to stop reporting changes.

Start by asking what the doctor means

You are entitled to a plain-language explanation. Calm, specific questions make it easier to distinguish a diagnosis from a possibility:

  • “When you say psychological, what do you mean in my case?” Ask whether the clinician is considering a named condition, a possible contribution to physical symptoms, or uncertainty after the assessment so far.
  • “What evidence supports that explanation?” Invite the doctor to describe findings, symptom patterns or other information that led them there.
  • “What remains uncertain?” Ask which alternatives are still open and what would make the clinician reconsider the working explanation.
  • “What physical assessment has been completed?” Request a summary of the examination, tests or medication review already done, without assuming that a particular test battery is required.
  • “Could a medicine or another health condition be contributing?” Give a complete list of prescription drugs, over-the-counter medicines, supplements and recent changes.
  • “What would prompt more investigation or a specialist referral?” Ask which changes, examination findings or persistence of symptoms would alter the plan.

Writing these questions down before the visit can prevent an important concern from being lost when the appointment is short. You can ask the clinician to record the agreed working explanation and safety-net instructions in your notes or after-visit summary.

Check what has been assessed without demanding a fixed test list

The appropriate assessment depends on your symptoms, history, examination and risk factors. There is no universal set of tests that every person should receive simply because a psychological explanation was mentioned. The NHS describes a GP examination and, when indicated, blood tests; a clinician may also review medicines, assess associated anxiety or depression, and refer to a specialist such as a neurologist.

  1. Ask which symptoms and signs the clinician believes have been explained and which have not.
  2. Confirm what examinations or tests were performed, what they showed, and whether any results are still pending.
  3. Discuss whether medicines, withdrawal, sleep problems or another diagnosed condition could affect the symptoms.
  4. Ask whether a specialist opinion is appropriate now or would become appropriate if the symptoms persist, change or interfere more with daily life.
  5. Agree on which developments should trigger an earlier review rather than waiting for the routine appointment.

A normal result can reduce the likelihood of some conditions, but its meaning depends on the test, timing and clinical context. Ask the doctor to explain what a result rules out, what it does not rule out, and whether another assessment is needed if your pattern changes.

Prepare for the next appointment

At follow-up, describe the symptom rather than only its label. NHS guidance recommends discussing when it began, what it feels like, what makes it better or worse, what you think may be causing it, how it limits activities and how upsetting it is. A short written record can organize those observations; a “medical symptom journal notebook” is an optional format, not a diagnostic device or an NHS endorsement.

For one or two weeks, if practical and safe, record:

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • the date and time symptoms start, stop or change;
  • what you were doing, eating or taking beforehand;
  • severity and duration in your own consistent scale;
  • sleep, activity, stress and menstrual or other relevant patterns;
  • what relieved or aggravated the symptom;
  • missed work, disrupted sleep, reduced mobility or other functional effects;
  • new symptoms, side effects or questions for the clinician.

Do not postpone urgent care to complete a log, and do not use the record to prove or disprove a diagnosis yourself. Its purpose is to make patterns and impact easier to discuss.

Agree on a concrete follow-up and safety net

Leave the visit knowing who will contact you, when you should check back and what to do if the plan is not helping. A useful plan states:

  • the next review date or the timeframe for arranging one;
  • which symptoms should be monitored and which changes require an earlier call;
  • how to obtain test results or advice outside the appointment;
  • which symptom-management steps are reasonable to try now;
  • how success will be judged, such as improved sleep, mobility, attendance at work or reduced symptom distress;
  • when the clinician would reconsider the explanation or refer you.

Symptoms can remain real and disabling even when a cause has not been identified. The aim is not to choose between “physical” and “psychological” as mutually exclusive categories, but to manage distress and function while appropriate medical uncertainty is resolved.

Consider psychological support alongside symptom care

Psychological support is not an admission that you are pretending. Depending on your goals and assessment, a GP may discuss lifestyle changes, talking therapy such as cognitive behavioural therapy (CBT), specialist assessment, physiotherapy or occupational therapy. These options differ in purpose: some target fear or low mood, some build activity and coping skills, and some address practical limits in movement or daily tasks.

The NHS notes that antidepressant medicines may sometimes help certain physical symptoms even when a person is not depressed. That does not make them suitable for everyone; potential benefits, interactions and adverse effects require an individual discussion. Painkillers and sedatives can carry dependence risks, so do not start, stop or change them without advice from the prescriber.

If you still think something has been missed

Tell the clinician specifically what worries you: a new sign, a progressive pattern, a family history, a medication exposure or a mismatch between your daily impairment and the explanation offered. Ask what information would change the assessment. Bringing another person, if you want, can help with note-taking and remembering instructions.

You can request another medical opinion when you remain uncertain or believe a condition may have been missed. NICE adult NHS patient-experience recommendations say patients should know they can ask for a second opinion and should have psychological and emotional support needs recognized alongside needs such as pain management. NHS mental-health assessment guidance says someone unsure about a suggested diagnosis or treatment may ask another professional; local NHS arrangements can help in many cases, but that guidance does not create a legal right to a second opinion. Procedures differ outside the NHS, so ask your insurer, clinic or health service how referrals work.

A second opinion is most useful when you bring your symptom timeline, test results, medicine list and the questions you want answered. It is not a guarantee of a different diagnosis, but it can clarify uncertainty and the next safe step.

When to seek urgent help

Do not dismiss new or worsening symptoms as psychological because that possibility was discussed earlier. Call 999 in the UK for a life-threatening emergency. If you are unsure whether emergency help is needed, NHS 111 can assess symptoms and direct you. Elsewhere, use your local emergency number.

NHS stroke guidance uses the warning signs of sudden face weakness, arm weakness or speech problems. Seek emergency help even if those symptoms stop. NHS heart-attack guidance identifies tight or squeezing chest pain, pain spreading to the arms, neck or jaw, severe difficulty breathing and unresponsiveness as emergency warning signs. These examples are UK-specific and not exhaustive; sudden severe symptoms, collapse, major bleeding, severe allergic reaction or rapidly worsening breathing also warrant immediate local emergency advice.

A practical appointment checklist

  1. Write the three symptoms or changes that concern you most.
  2. Bring your medicine and supplement list, including recent starts, stops and dose changes.
  3. Describe onset, pattern, triggers, relievers, activity limits and emotional impact.
  4. Ask what “psychological” means in your case and what evidence supports it.
  5. Ask what has been assessed, what remains uncertain and what would prompt referral.
  6. Agree on follow-up timing, warning signs and who to contact.
  7. Discuss support options by their goals, benefits, burdens and review plan.
  8. Request another opinion if uncertainty remains, using the process available in your health system.

You deserve care that takes both your symptoms and your concerns seriously. A psychological contribution can be explored without denying physical suffering, and medical assessment should remain responsive to new information.
