Call the child’s healthcare professional promptly and have the complete laboratory report ready. The meaning of a “high” result depends on the metal, number, units, specimen (capillary or venous blood, urine, or hair), collection date, symptoms, and likely exposure. A lead result and a mercury result require different follow-up; neither should be interpreted from the number alone.
Do not start chelation, “detox” products, supplements, or home treatment without clinical direction. For severe symptoms, a very high result, or uncertainty about urgency, seek emergency assessment or call Poison Control at 1-800-222-1222.

What to do today
- Contact the child’s clinician. Send the exact report, including the numeric result, units, specimen type, date, reference interval, and laboratory comments. Ask what was measured and whether the result needs confirmation.
- Describe symptoms and timing. Report abdominal pain, vomiting, confusion, weakness, seizures, unusual sleepiness, breathing problems, or any recent spill, renovation, occupational take-home exposure, unusual food or supplement, or contaminated product.
- Ask about the test method. For lead, determine whether the sample was a finger-prick (capillary) screen or venous blood. For mercury, ask whether the laboratory measured total mercury or identified a specific species.
- Stop an obvious ongoing exposure safely. Do not disturb peeling paint, vacuum suspected lead dust with an ordinary household vacuum, handle a mercury spill, or discard potentially useful evidence before receiving local health-department or hazardous-material guidance.
- Use urgent services when needed. Severe neurologic symptoms, seizures, confusion, coma, significant breathing problems, or rapidly worsening illness require emergency care. Poison Control can coordinate specialist advice for both metals.
How to interpret a child’s lead result
The Centers for Disease Control and Prevention (CDC) uses a blood lead reference value (BLRV) of 3.5 µg/dL. It identifies children ages 1–5 whose results are higher than those of most U.S. children; it is not a line between safe and unsafe blood or a regulatory standard. The value represents the 97.5th percentile of the blood-lead distribution in that age group, using NHANES 2015–2018 data. CDC lowered the BLRV from 5.0 to 3.5 µg/dL in 2021. See the current clinical guidance at CDC’s Recommended Actions Based on Blood Lead Level and the update at CDC Updates Blood Lead Reference Value.

CDC states, “There is no safe level of lead in a child’s blood.” A result below 3.5 µg/dL does not prove that a child had no exposure or that no health effect is possible. State and local programs may use different action thresholds or provide different services.

- It's the Vinegar-Activated Test that's Most Accurate: The 30 Swabs lead paint test kit must be dipped in vinegar to get accurate results; this is much more accurate than using water; take a bit of vinegar from the kitchen, put a lead test swabs in the vinegar, and wet it all down to test
- Results in 30 Seconds: Dip a lead test kit for dishes into white vinegar for a while, the tip of the lead swab turns orange yellow, then wipe the surface of the object with the tip of the lead swab for 30 seconds; when the swab turns red/purple or pink, it indicates the presence of lead, while Yellow signifies no lead detected
- Suitable for Many Surfaces Containing Lead: Caution is needed if you want to test valuable items such as antiques, as vinegar itself is acidic and can affect solid surfaces; at home lead testing kit are suitable for various painted metal & ceramic surfaces, such as dishes toys pottery jewelry water bottle woodwork cups glazes and plumbing

If the first test was a finger-prick
Capillary samples can be contaminated by lead on the skin. CDC therefore recommends a confirmatory venous draw according to the initial capillary result. A venous sample used for the initial screen does not need another venous draw solely for confirmation.
| Initial capillary blood lead result | Recommended timing for confirmatory venous sample |
|---|---|
| 3.5–9 µg/dL | Within 3 months |
| 10–19 µg/dL | Within 1 month |
| 20–44 µg/dL | Within 2 weeks |
| 45 µg/dL or higher | Within 48 hours |
These are CDC recommendations; the child’s clinician and the jurisdiction’s public-health program set the actual appointment and collection plan. Do not wait for a routine annual visit when the report falls in a shorter interval.

When a high lead result is an emergency
A child in the highest result band needs urgent assessment even without obvious symptoms. Hospital admission is indicated when symptoms such as confusion, weakness, seizures, coma, nausea, vomiting, or abdominal pain occur. It may also be considered when the home cannot yet be made lead-safe or the source is unidentified and exposure is continuing. Treatment decisions belong to an experienced clinician with toxicology support.
Find and reduce the lead source
Ask the clinician and local health department to help investigate likely sources. Possibilities include deteriorating lead-based paint and dust, lead service lines or household plumbing, contaminated soil, certain foods or consumer and cultural products, folk remedies, and dust carried home from an adult’s job or hobby. A blood test does not identify which source is responsible.

- Keep the child away from peeling paint, renovation dust, and visibly contaminated soil until the area is assessed.
- Use cleaning and renovation methods recommended by the local lead program; ordinary dry sanding, scraping, or unsafe dust removal can spread contamination.
- Discuss drinking-water testing or plumbing evaluation with the local authority rather than buying a filter without confirming that it is certified for the suspected contaminant and fits local requirements.
- Ask about iron deficiency, iron-rich foods, and adequate calcium. Nutrition supports care but does not remove lead or replace environmental control and repeat testing.
- Track developmental milestones and accept referrals for early-intervention or other supportive services when the clinician recommends them.
What a high mercury result means
Mercury is not one chemical. Elemental, inorganic, and organic forms behave differently, so a result cannot be interpreted like a lead level. The specimen, timing, exposure route, and whether the laboratory speciated the sample all matter. Neither the lead BLRV nor a single universal pediatric mercury cutoff should be applied to a mercury report.
The Agency for Toxic Substances and Disease Registry (ATSDR) explains the clinical use of biomarkers in Evaluating Mercury Exposure and its October 2024 Toxicological Profile for Mercury. These resources distinguish biomarkers for methylmercury from those associated with elemental and inorganic exposure and caution that a total measurement may not identify the form.

Match the specimen to the suspected exposure
| Specimen or report | What it can suggest | Important limitation |
|---|---|---|
| Blood mercury | Can be informative soon after a substantial acute exposure and may reflect dietary methylmercury. | Blood mercury has a short half-life; timing and recent seafood intake can change the result. Ask whether the test was total or speciated. |
| Urine mercury | Most useful for assessing elemental mercury and useful for acute or chronic elemental or inorganic exposure. | It is not a general test for every mercury form and must be interpreted with exposure history. |
| Hair mercury | Primarily reflects organic, especially methylmercury, exposure over a longer period. | It is not useful for assessing a recent exposure and can be affected by sampling and interpretation problems. |
Ask the ordering clinician what chemical form the laboratory measured, when the sample was collected relative to the suspected exposure, and whether another specimen or repeat test is appropriate. A report labeled “total mercury” does not, by itself, reveal the species or dictate treatment.
Questions that help identify a mercury source
- What fish or shellfish did the child eat, in what amounts, and when? Follow species-specific and local advisories; do not eliminate all fish without individualized advice.
- Could the child or household have had a mercury-containing product, a broken thermometer or bulb, or a household spill?
- Do adults work with mercury or use it in a hobby, and could contaminated clothing or equipment enter the home?
- Are there cultural products, remedies, skin-lightening products, supplements, or imported items that need review?
These questions identify possibilities, not diagnoses. For a suspected acute exposure, symptoms, or an uncertain result, call the child’s clinician or Poison Control. Severe symptoms require emergency care. ATSDR advises that chelation decisions be made with Poison Control or a medical toxicologist experienced in chelation; commercial “detox” products are not a substitute.
Lead and mercury: why the next steps differ
| Question | Lead | Mercury |
|---|---|---|
| What does the number mean? | Compare a blood result with CDC’s 3.5 µg/dL reference value, while explaining that it is a follow-up reference and not a safety boundary. | Interpretation depends on chemical form, specimen, timing, and exposure; the cited authorities do not establish one universal child cutoff. |
| Is confirmation standardized? | Yes. A capillary screen should be confirmed with venous blood on the schedule shown above. | No single protocol covers every form. The clinician determines the appropriate specimen, speciation, and timing. |
| Main next step | Confirm when needed, identify and reduce the source, assess nutrition and development, and repeat testing according to the confirmed level. | Identify the form and likely source, interpret the exact biomarker with a clinician or toxicologist, and stop ongoing exposure safely. |
| Who can help? | Child’s clinician, local or state lead program, PEHSU, and Poison Control. | Child’s clinician, Poison Control, and a medical toxicologist. |
Prepare for the appointment
Bring the original report or a clear image of every page. Write down the child’s age, symptoms, medicines and supplements, recent renovations or moves, water source, foods and imported products, hobbies and occupations in the household, and the date and location of any spill. Include prior lead or mercury tests so the clinician can assess the trend rather than one isolated value.

Because state programs and laboratory methods differ, local instructions take precedence over a generic timetable.
What not to do
- Do not call 3.5 µg/dL a “safe/unsafe” lead line or assume a lower result proves there was no exposure.
- Do not use a lead capillary result as the final diagnosis when CDC recommends venous confirmation.
- Do not apply lead thresholds to mercury or infer mercury species from a total-mercury number.
- Do not begin chelation, laxatives, supplements, home testing, filtration, or detox products solely because a report is flagged high.
- Do not delay urgent care while trying to identify the source yourself.
Bottom line
Get the report to a clinician, verify the specimen and units, and act on the metal-specific pathway. Lead follow-up uses CDC’s reference value, venous confirmation schedule, exposure control, and result-based monitoring. Mercury requires identification of the chemical form and exposure route before the result can be interpreted. Poison Control and qualified toxicology specialists can help when the result is very high, symptoms are present, or the next step is unclear.
