If you have fever, cough, shortness of breath, chest pain, marked weakness, or bloody or watery mucus after a plausible plague exposure, seek emergency medical care now. Tell the receiving clinician exactly what happened and when. If you have no symptoms but had close, sustained contact with a person or animal who may have pneumonic plague, contact a healthcare provider and your local or state public-health department promptly for an exposure assessment. Do not wait for a test result and do not start leftover or borrowed antibiotics.

Pneumonic plague is uncommon, but it can progress rapidly. The right response depends on the type of contact, how long it lasted, whether respiratory secretions or infected tissues were involved, and whether symptoms have begun. The guidance below explains what to report and what professionals may recommend; it is not a way to diagnose yourself or select a drug.

If symptoms are present, treat this as an emergency

Primary pneumonic plague can begin with fever, headache, weakness and rapidly developing respiratory illness. Cough, shortness of breath, chest pain, and bloody or watery sputum are especially concerning when there has been a plausible exposure. Call your local emergency number or go to an emergency department immediately. Do not stay home to see whether symptoms improve.

  • Tell the dispatcher, emergency department or clinician that pneumonic plague is a concern, who or what the possible source was, and when the contact occurred.
  • Give the time symptoms started and whether they are worsening.
  • If it can be done without delaying care, call ahead so the facility can prepare its infection-control response. Do not postpone emergency treatment to make that call.
  • Follow instructions from emergency personnel about transport and contact with other people.

Centers for Disease Control and Prevention (CDC) clinical guidance says, “Begin appropriate therapy as soon as plague is suspected.” CDC diagnosis guidance likewise says that when plague is strongly suspected, antibiotics should begin as soon as samples are collected, without waiting for final laboratory results. Testing is handled by clinicians; a person with symptoms should not wait for a laboratory confirmation before seeking treatment.

If you have no symptoms but may have been exposed

Contact a healthcare provider and the local or state health department as soon as possible. Explain that you may have had exposure to pneumonic plague and ask for an individualized assessment. Public-health staff may coordinate the evaluation, monitoring and any preventive treatment.

Information to provide

  • The date and exact setting of the contact.
  • Approximate distance from the ill person or animal and how long you were together.
  • Whether the source was coughing or producing respiratory secretions at the time.
  • Any contact with saliva, sputum, blood, other body fluids, or animal tissues.
  • Whether the contact occurred after the source developed a cough or other respiratory symptoms.
  • What respiratory or other personal protective equipment you used, and whether it stayed in place.
  • Any flea exposure, animal illness or death, travel, or handling of carcasses.
  • Your current symptoms, temperature readings and relevant medical circumstances that your clinician asks about.

These details help professionals decide whether the event fits a known-exposure definition. They are not a self-scoring tool: a brief encounter at a distance cannot be classified safely without context, and ordinary contact should not be assumed either to transmit plague or to be risk-free.

What CDC means by a potentially significant exposure

Pneumonic transmission occurs through infectious droplets coughed by an infected person or animal and generally requires direct, close contact. CDC clinical-care material gives examples of known exposure as close (less than 6 feet), sustained contact with a pneumonic-plague patient or animal, or direct contact with infected fluids or tissues. Close-contact material discusses people who were in close contact during the preceding seven days when considering antibiotics.

Situation Why the distinction matters What to do
Close, sustained contact while a person or animal was coughing More consistent with the respiratory-droplet exposure described by CDC Contact a healthcare provider and public health promptly, even if you feel well
Brief or more distant contact May involve less respiratory exposure, but distance and duration alone do not settle the risk Report the event and let professionals determine whether follow-up is needed
Direct contact with infected sputum, blood, other fluids or tissues Direct material contact is a separate exposure pathway that requires clinical assessment Describe exactly what touched your skin, eyes, mouth or clothing
Handling a sick or dead animal, carcass or animal tissues Animal-tissue exposure can involve plague routes other than person-to-person droplets Contact public health and avoid further handling until instructed
Flea exposure or bites in an area where plague occurs Fleas are a route to other forms of plague, not the same event as inhaling respiratory droplets Report the bites, location and animal contact for a tailored assessment
Contact with appropriate protective equipment Protection may reduce exposure but does not prove that none occurred Tell the assessor what equipment was used and whether it was intact

Being in the same building does not automatically establish close exposure. Conversely, a short event involving a cough at close range or contaminated secretions can be important. Professionals need the circumstances rather than a simple “yes” or “no” label.

How quickly illness can appear

Timing cannot rule plague in or out by itself. A CDC public symptoms page states that inhalation incubation can be as short as one day. A 2021 CDC clinical guideline describes primary pneumonic-plague symptoms developing within one to three days after inhalation. These figures describe reported ranges, not a guaranteed deadline after which illness is impossible.

When you contact a clinician or health department, give the exposure time and keep a written record of symptoms and temperature readings. Do not use a symptom-free interval at a particular hour as permission to ignore instructions or skip an exposure assessment.

Monitoring during the period public health gives you

If public health or a clinician advises monitoring as a close contact, CDC close-contact material advises measuring your temperature twice daily for seven days and seeking a physician if fever develops. Follow the exact plan you are given, including how and when to report readings.

If fever or respiratory symptoms develop

  • Seek medical attention immediately rather than waiting for the next scheduled check.
  • Report the possible plague exposure at the first point of contact.
  • Also report cough, shortness of breath, chest pain, weakness, or bloody or watery mucus, even if the temperature is not high.

If you were less closely exposed

CDC material for less-close contacts advises watching for fever or cough and notifying a healthcare provider and public health if symptoms arise. Ask whether you need a formal temperature schedule; do not assume that self-monitoring replaces the initial professional assessment.

Post-exposure antibiotics are clinician-directed

When indicated, CDC clinical-care guidance recommends seven days of a single antimicrobial for potentially exposed people. The choice and dose depend on individual circumstances and must be made by a clinician or public-health authority. A preventive course is not something to assemble from an old prescription, a family member’s medication or an online dosing table.

  • Take the medicine exactly as prescribed and report side effects or difficulty obtaining it promptly.

Not every person who was nearby will automatically receive prophylaxis. The decision depends on the exposure assessment, the source’s diagnosis, timing and the person’s medical circumstances. Conversely, feeling well does not mean you should decline an evaluation if you had a qualifying close contact.

What to do while arranging advice

Until a clinician or public-health professional gives you instructions, avoid unnecessary close contact with other people and animals, and do not handle potentially contaminated tissues or fluids. Use ordinary respiratory hygiene and hand hygiene. If you develop symptoms, prioritize emergency care.

An asymptomatic person should not walk unannounced into a crowded waiting room claiming possible plague. Call a healthcare provider or public-health department first for routing instructions. This is not a reason to delay care once symptoms begin: emergency symptoms require immediate attention, with the exposure disclosed to the receiving team.

How clinicians investigate and treat suspected illness

Healthcare teams will assess symptoms, exposure history, vital signs and possible alternative diagnoses. They may collect respiratory or other samples for laboratory testing. CDC guidance emphasizes starting treatment promptly when plague is suspected; final laboratory confirmation is not a prerequisite for beginning appropriate therapy.

Clinical teams, rather than patients, decide what isolation and droplet precautions are needed. If you are directed to a facility, follow its instructions about transport, masking and who may accompany you. Tell staff about every potentially exposed household member or animal contact so public health can decide who needs follow-up.

Keep the exposure routes separate

Plague is caused by Yersinia pestis, but the route of exposure changes the response. Pneumonic plague spreads through droplets from an infected person or animal, usually during direct, close contact. Infected flea bites and handling infected animal body fluids or tissues are separate routes that can lead to other forms of plague. A flea bite is not evidence that you inhaled pneumonic droplets, and the absence of a bite does not settle a respiratory exposure.

Because routes and symptoms differ, tell the assessor whether your concern involves a coughing person, an animal, a carcass, a bite, or direct contact with fluids. Do not assume that a single rule covers all of these situations.

Common mistakes that can delay safe care

  • Waiting for a test: suspected plague is treated promptly; waiting for final results can be dangerous.
  • Self-prescribing: preventive treatment requires a clinician’s choice of antimicrobial and duration.
  • Using a thermometer as a diagnosis: temperature checks support monitoring but cannot confirm or exclude plague.
  • Assuming all contact is equivalent: close, sustained respiratory contact, direct fluid contact, animal handling and flea exposure are different scenarios.
  • Assuming no symptoms means no action: asymptomatic close contacts still need prompt professional assessment.
  • Entering a facility without warning: call ahead when feasible for routing, while never delaying emergency treatment for a phone call.
  • Relying on geography alone: plague is rare and geographically concentrated, but travel and animal contact can change the relevant public-health advice.

A concise action checklist

  1. If you have compatible symptoms, obtain emergency medical care now.
  2. Tell clinicians the possible exposure, timing, distance, duration, source symptoms and any fluid or tissue contact.
  3. If you have no symptoms, contact a healthcare provider and local or state public health promptly.
  4. Follow the monitoring schedule you receive; CDC close-contact guidance uses twice-daily temperature checks for seven days.
  5. If prophylaxis is recommended, use only the prescribed seven-day regimen and follow the prescriber’s instructions.
  6. Report fever, cough or other new symptoms immediately and do not wait for a scheduled check.

Where this guidance applies

The recommendations summarized here come principally from U.S. CDC public-facing pages, CDC clinical-care guidance dated May 15, 2024, and a 2021 CDC MMWR antimicrobial guideline. Medical recommendations can change, and clinical guidance is U.S.-focused. If you are elsewhere, contact the relevant national or local public-health authority and follow the clinician’s individualized instructions.
