If you were close to someone diagnosed with pneumonic plague, contact your local or state health department and a healthcare professional promptly. Describe exactly what happened, including your distance, the length of contact, whether the person was coughing, any contact with body fluids, and the mask or other protective equipment you used. Ask whether you need post-exposure prophylaxis (PEP); do not wait for symptoms, take leftover antibiotics, or use someone else’s prescription.
Seek immediate medical attention if you develop fever, cough, chest pain, shortness of breath, weakness, headache, or bloody or watery mucus. Tell the health service or emergency department about the possible plague exposure before you arrive when possible.

Why you should call now
Pneumonic plague can progress quickly. After inhalation, symptoms may begin after an incubation period as short as one day, so feeling well immediately after an encounter does not settle whether you need preventive treatment. The Centers for Disease Control and Prevention (CDC) recommends seven days of post-exposure prophylaxis for people with a known exposure. A clinician or public-health authority must decide whether that recommendation applies to your encounter and select the medicine for you.
Do these 3 things before closing this tab:
1Clear out junk files and repair common Windows errors2Scan for outdated or missing drivers - takes under a minute3Repair Windows errors before they cause bigger problemsDo not wait until you have a fever to ask for exposure guidance. Public-health officials need the details while they can still identify other contacts, arrange appropriate monitoring, and determine whether PEP is indicated. If you become ill, treatment for suspected plague should not be delayed while waiting for laboratory results.

Step-by-step: what to do after the contact
- Call public health promptly. Contact your local or state health department. If you cannot immediately reach it, call a healthcare provider and explain that you may have had close contact with a person with pneumonic plague. CDC guidance calls for immediate health-department notification when plague is suspected.
- Give a precise exposure history. Report the date, time, and location; your approximate distance from the patient; how long you were together; whether the patient was coughing; whether you touched saliva, sputum, blood, or other body fluids or tissues; and what mask, gloves, eye protection, or other PPE you wore. Include whether you were providing care, transporting the person, or present only briefly.
- Ask specifically about PEP. Use the words “post-exposure prophylaxis” and ask whether you meet the definition of a known exposure. CDC clinical-care guidance published May 15, 2024, states: “Post-exposure prophylaxis should be given for 7 days.” That is a duration recommendation, not permission to choose an antibiotic without medical advice.
- Follow the monitoring plan you are given. Public health may ask you to watch for fever or cough and to report symptoms during a specified period. Write down the instructions and the telephone number to use after hours. If officials tell you to check your temperature, a digital thermometer can help record readings, but it cannot diagnose plague or show that PEP is unnecessary.
- Escalate any symptoms immediately. Call the health service or seek urgent care for fever, headache, weakness, cough, shortness of breath, chest pain, or bloody or watery mucus. Tell responders about the possible exposure before arrival when possible so they can direct you to the safest evaluation route.
Does your contact count as a significant exposure?
Exposure assessment is fact-specific. CDC describes known exposure as close, sustained contact within 6 feet of a patient or animal with pneumonic plague, or direct contact with infected body fluids or tissues. Distance alone is not a self-diagnosis test: duration, coughing, the timing of the encounter, the task you were performing, and your PPE all affect the assessment.
| Encounter detail | Why officials ask about it | Your next step |
|---|---|---|
| Close, sustained contact within 6 feet | This is a CDC-recognized situation in which PEP should be considered, particularly when adequate PPE was not worn. | Report the approximate distance and duration promptly and ask for an individualized PEP decision. |
| Direct contact with infected fluids or tissues | Contact with material from the patient or an infected animal can change the risk assessment even if the encounter was not prolonged. | Explain what material was involved, how contact occurred, and whether skin, eyes, nose, or mouth were exposed. |
| Brief or non-close contact | A short interaction farther away may not meet the close-contact definition, but officials still need the circumstances to decide whether monitoring is appropriate. | Give the exact details rather than assuming you are clear or automatically need antibiotics. |
| Standard and droplet precautions maintained by a responder or healthcare worker | CDC’s 2021 MMWR recommendation says first responders and healthcare providers who maintained those precautions while caring for a pneumonic-plague patient do not need PEP under that recommendation. | Still report the encounter and PPE used. Do not generalize that occupational guidance to household visitors or other contacts. |
| Accidental laboratory exposure or close/direct contact with an infected animal | CDC says PEP may also be considered in these circumstances. | Use the facility’s exposure-reporting process and contact public health or occupational health immediately. |
These categories help public-health professionals triage an encounter; they are not a scoring system you can use to rule plague in or out. Do not infer that every close contact has been infected, and do not infer that every person without symptoms needs the same drug.

Post-exposure prophylaxis: what the seven-day recommendation means
Who decides
A clinician working with public health should decide whether PEP is appropriate. The decision can depend on the patient’s confirmed or suspected diagnosis, the nature and timing of your contact, PPE, your medical history, allergies, pregnancy or breastfeeding, kidney function, age, and local protocols. Tell the prescriber about all medicines and medical conditions before taking anything.
What to expect
For people whom officials classify as having a known exposure, CDC guidance recommends seven days of PEP. Take the prescribed course exactly as directed and ask what to do if you vomit, miss a dose, or develop symptoms. PEP is preventive treatment after exposure; it is not a substitute for urgent evaluation if illness begins.

Why you should not self-medicate
- Different antibiotics and doses may be selected for different people and circumstances.
- Leftover or borrowed medicine may be the wrong drug, dose, formulation, or duration.
- Taking an antibiotic can cause adverse effects and may complicate a clinician’s assessment.
- Symptoms during or after PEP still require immediate medical attention; prophylaxis does not justify waiting.
Symptoms to watch for and when to seek emergency care
Pneumonic plague symptoms can include fever, headache, weakness, and rapidly developing pneumonia. Respiratory warning signs include shortness of breath, chest pain, cough, and sometimes bloody or watery mucus. CDC’s symptoms guidance says: “If you are experiencing symptoms like those listed here, seek immediate medical attention.” In the context of a possible exposure, treat these symptoms as urgent rather than waiting for them to become severe.
When you call or arrive for care, state clearly that you had possible contact with pneumonic plague and give the date of the encounter. If you are coughing or having trouble breathing, ask before entering a clinic or emergency department whether staff want you to use a particular entrance or wait in a separate area. Follow the instructions from the receiving facility and health department.

How to monitor yourself safely
Use the plan from public health
Some lower-risk contacts may be asked to monitor for fever or cough and to notify their healthcare provider and state or county health department if symptoms occur. The monitoring period and reporting method should come from officials handling your exposure. Keep a written log of temperature readings and symptoms only if they ask you to do so.

A normal temperature does not prove that you were not exposed, does not rule out early disease, and does not determine whether PEP is indicated. Likewise, the absence of cough on the first day does not replace professional advice. A thermometer is simply a way to document a measurement during an instructed fever watch.

Limit avoidable uncertainty
Keep your phone available, respond to calls from public health, and tell officials if your contact information changes. If someone in your household is helping you monitor symptoms, make sure that person knows the exposure details and the number to call if you become ill. Do not conceal the possible exposure when seeking care.
A real-world example—and what it does not prove
In a CDC report published October 1, 2026, investigators described a fatal pneumonic-plague case in Coconino County, Arizona, in July 2025. They identified 46 potentially exposed people two days after the patient’s death, and all began a seven-day oral ciprofloxacin PEP course. This illustrates how public health can identify contacts and provide a defined preventive course after an investigation. It does not establish ciprofloxacin as the correct medicine for every exposure, nor does it mean that every person who was nearby automatically requires antibiotics.
Common mistakes after a possible exposure
- Waiting for symptoms: inhalational disease can begin as soon as one day after exposure, and PEP decisions are time-sensitive.
- Assuming distance alone answers the question: officials also need duration, coughing, direct fluid contact, and PPE details.
- Taking someone else’s antibiotics: only a clinician or public-health authority should choose and prescribe PEP.
- Overgeneralizing healthcare-worker guidance: the no-PEP recommendation for personnel who maintained standard and droplet precautions does not automatically apply to unprotected household or community contacts.
- Relying on a normal temperature: a thermometer cannot diagnose plague or clear you from follow-up.
- Arriving unannounced while symptomatic: call ahead when possible so the facility can give safe arrival instructions.
Questions to have ready when you call
Prepare a short timeline: when you first met the person, the closest distance, total time together, whether the person was coughing, any contact with respiratory secretions or other body fluids, and exactly which PPE you wore. Tell the official whether the patient’s diagnosis was confirmed, suspected, or unknown to you, and whether you have developed any symptoms. This information lets public health distinguish a close, sustained exposure from a brief or protected encounter without asking you to make the medical decision yourself.
