What to Do After an AI Agent Makes an Unauthorized Change

Stop the agent through a control it cannot override, revoke the credentials it used, preserve the evidence, and determine the full blast radius before changing anything back. Do not rely on a prompt asking the agent to undo its work. Treat the event as a security incident until you can show what changed, which identities and systems were involved, and whether data or access left the intended boundary.

The sequence below applies to coding agents, workflow automations, support agents, cloud operators and multi-agent systems. Exact restore commands depend on the affected platform, so use this as an incident procedure rather than a product-specific runbook.

1. Stop further execution from outside the agent

Use an operator-controlled runtime or infrastructure control first. Pause the run, terminate the worker, disable the write-capable tool or integration, or isolate the workload at the network, job-queue or orchestration layer. An agent may ignore a natural-language instruction, lose context, or continue through a queued task; an external stop control does not depend on model output.

OWASP recommends interruption and rollback mechanisms, while the U.S. Department of Energy’s GEAR guidance places authorization in the application or infrastructure enforcement layer. The DOE’s warning is direct: “A prompt such as ‘never delete files’ is not an access control.” DOE GEAR: AI Security and Safety

Contain every route that could still write

  • Pause the current agent run and any retry or scheduled job.
  • Disable the tool, API integration or deployment credential that performed the change.
  • Check queues, webhooks, event triggers and background workers for already-submitted actions.
  • Identify connected agents or services that may have received instructions from the first agent, and contain them if necessary.
  • Do not delete the agent, workspace or logs while the event is being investigated.

If the change is actively damaging production, prioritize isolation and service safety. Record who made the containment decision and the exact time before making emergency changes.

2. Revoke the identity and access used for the change

Disable or revoke the agent’s token, session, delegated OAuth grant, API key or service credential, as applicable. If the agent used a shared human account, involve that account’s owner: review active sessions, rotate exposed secrets and determine which unrelated users or automations might be interrupted.

Prefer agent-specific, short-lived and scope-limited identities in future. An identity that is unique to one agent or workflow lets you cut off that workflow without disabling an entire team. OWASP’s excessive-agency guidance recommends least privilege and user-context identity; DOE GEAR likewise emphasizes knowing how credentials can be revoked. See OWASP LLM06:2025 Excessive Agency.

Access-revocation checklist

  • Record the credential identifier, owner, scopes, issue time and expiry time.
  • Revoke refresh tokens as well as current access tokens where the platform supports both.
  • Inspect service accounts, role bindings, SSH keys, signing keys and secrets available to the agent.
  • Check whether credentials were copied into logs, prompts, tickets or chat; redact them and rotate them if exposure is possible.
  • Confirm that revocation took effect by testing from a controlled, non-destructive request.

3. Preserve evidence before you repair the state

Capture a defensible timeline while the original state and logs are still available. Preserve the agent and model identifiers (if exposed), workflow and tool versions, source inputs, instructions, approvals and denials, every tool call, errors, affected targets, resulting state and relevant timestamps. Include the operator who stopped the run and every subsequent containment action.

Protect copies with access controls and normal retention rules. Minimize sensitive content and redact credentials; copying secrets into an incident note creates a second exposure. Keep an original, access-restricted copy where possible and make investigative working copies from it. DOE GEAR lists provenance, workflow versions, actions, approvals, timestamps, outputs and human review as useful evidence: https://gear.doe.gov/rfa-teams/ai-security-and-safety.

What to record

  • The first known unauthorized action and the last possible action.
  • Agent, model, workflow, integration and policy versions.
  • Identity, permissions, network location and session information.
  • Prompt or input context needed to understand why the action was selected, with secrets removed.
  • Targets, parameters, tool responses, failures, approvals and human overrides.
  • Snapshots, checksums, audit-log locations and the people who collected them.

4. Establish the blast radius

Do not roll back the first visible change until you know what else happened. Compare the current state with a trusted baseline, change history or independent audit source. Scope both direct and indirect effects.

Inspect these impact categories

  • Data: created, edited, deleted, exported or disclosed records; altered retention or backup settings; and access to sensitive data.
  • Permissions: new users, roles, policies, keys, group memberships or delegation grants.
  • External actions: messages, posts, tickets, purchases, deployments, domain changes or other requests sent outside the system.
  • Infrastructure: modified code, configuration, dependencies, jobs, containers, databases, routing or security controls.
  • Financial and operational effects: spend, resource creation, service outages, customer-visible changes and irreversible physical actions.
  • Cascades: downstream workflows, webhooks, connected agents and services that acted on the original output.

OWASP identifies data exfiltration, privilege escalation, high-impact changes and cascading failures as agent risks. Microsoft’s guidance recommends post-execution logs and isolation to reduce blast radius: Reduce autonomous agentic AI risk. If you cannot establish the scope, treat all resources reachable by the identity during the time window as potentially affected until disproved.

5. Decide whether and how to recover

Recovery is not automatically “undo.” Choose a platform-supported restore, revert or compensating action only after evidence is preserved and dependencies are understood. A rollback can overwrite legitimate work performed after the unauthorized change, trigger another automation, or destroy the very state you need to analyze.

Compare recovery approaches

Approach Best fit Evidence and dependency concerns Control to require
Versioned revert A repository, document or configuration with a trusted change history May remove legitimate later edits or invoke deployment hooks Review the exact diff and dependent jobs before applying
Snapshot or backup restore Corruption or broad changes where a known-good point-in-time copy exists Can lose valid updates after the snapshot and may restore vulnerable settings Validate the snapshot, restore in an isolated path when possible, then reconcile changes
Compensating change An external action cannot be technically reversed, or only selected records should be corrected Requires complete impact knowledge; a second change can compound the error Approve exact targets and parameters and record the operator
Quarantine and forensic copy Scope is uncertain or legal, security or safety review is required Delays normal service recovery but preserves the most evidence Define an owner, preservation period and decision deadline

These are patterns, not universal commands. The reviewed guidance supports rollback capability but does not define a recovery procedure for a particular cloud provider, IDE, repository, database or agent product. Use the affected service’s current official recovery documentation and your incident-response owner for exact commands, ordering and validation.

Validate the restored state

  1. Confirm the unauthorized identity can no longer write.
  2. Check that the restored objects, permissions, integrations and secrets match the intended baseline.
  3. Run dependent tests or health checks in a controlled environment before reopening traffic.
  4. Review logs for new actions caused by the recovery itself.
  5. Document what could not be restored and any compensating controls still required.

6. Escalate and communicate at the right threshold

Contact your security or incident-response function when the agent touched production, sensitive data, credentials, access controls, customer-facing services or an external system. Escalate as well when the scope is uncertain, even if the visible change appears small. Give responders the timeline, identity details, preserved logs, impact assessment and containment status rather than an unverified conclusion.

Notify system owners whose services or data were involved, and coordinate one accountable communications owner. Legal or regulatory notification duties depend on jurisdiction, sector, contracts and the facts of the event. General AI-security guidance cannot determine those duties for you. CISA’s May 1, 2026 announcement encourages monitoring, threat modeling, layered defenses and security assessment, but it is not a breach-notification rule: CISA and Partners Release Guidance on Adopting Agentic AI Services.

7. Re-enable the workflow with independent controls

Do not restart the same configuration after merely asking the agent to be careful. Assign a human owner who can stop the workflow and approve high-impact actions. Start with read-only access, then add one narrowly defined write capability at a time.

Controls to put in place

  • Remove tools, destinations, data sets and permissions the task does not need.
  • Enforce authorization in deterministic application or infrastructure policy, independently of the model’s text.
  • Require approval for destructive, financial, access-control, production and external-communication actions.
  • Bind an approval to the exact actor, tool, target, parameters, time window and expiry. Require fresh approval if any of those change.
  • Use separate identities for development, staging and production; avoid shared human credentials.
  • Set rate, volume, destination and spend limits, with a hard operator-controlled stop path.
  • Log proposed actions, approvals, executions, denials and results in a tamper-resistant, access-controlled system.
  • Test interruption, credential revocation and rollback procedures before granting broader autonomy.

Microsoft’s risk-management guidance and OWASP’s agent-security guidance both emphasize limiting autonomy, isolating workloads and maintaining post-execution visibility. Read the OWASP checklist at AI Agent Security Cheat Sheet.

Common mistakes that make the incident worse

  • Asking the agent to undo itself: the same untrusted execution path may make another unauthorized change.
  • Deleting logs or rebuilding immediately: you may destroy the timeline and lose the ability to prove what happened.
  • Rotating one password only: refresh tokens, delegated grants, service accounts or copied secrets may remain valid.
  • Restoring the first file you notice: permissions, external actions and downstream systems may be the real impact.
  • Re-enabling all tools at once: it prevents you from identifying which capability caused or amplified the event.
  • Promising that no data was exposed: absence of an obvious changed record is not proof that a read or export did not occur.

A compact incident checklist

  1. Stop the run and contain queues, integrations and connected agents.
  2. Revoke the agent’s tokens, sessions, grants and shared-account access.
  3. Preserve logs, inputs, approvals, tool calls, state and timestamps with secrets redacted.
  4. Compare against a trusted baseline and map data, permission, external and cascading effects.
  5. Select a reversible, platform-appropriate recovery or compensating action.
  6. Escalate to security, system owners and incident response when the threshold is met.
  7. Restore service only after validation, then resume with least privilege, exact-action approval and an independent stop path.
