What to Do After an Abnormal ECG Screening Result

Contact the clinician or screening service that ordered the ECG and ask for the exact finding, its significance, and the follow-up timeline. “Abnormal” is a report label, not a diagnosis. If you have acute chest pain or a comparable emergency symptom, seek emergency care rather than waiting for a routine callback.

Start with the clinician who ordered the ECG

An abnormal screening ECG should be reviewed by a healthcare professional who can see the tracing, the written report, your symptoms, medical history and medicines. Contact the ordering clinician or screening service and request the actual result, not only the word “abnormal.” Ask how quickly it needs review and what happens next.

The NHS describes the expected process this way: “The GP, nurse or specialist should talk to you about your results and explain what happens next.” That conversation is important because the same broad label can lead to different follow-up depending on the finding and the reason the ECG was performed.

Know what an “abnormal” label does—and does not—tell you

An ECG records the heart’s electrical activity. A report may flag a rhythm, rate, conduction pattern, electrical axis, waveform or other feature as outside the device’s reference range. Automated software can generate preliminary labels, and a clinician must interpret them in context. The label alone does not establish the cause, severity or urgency of a problem.

Do not try to diagnose yourself from a printed tracing or an automated statement. Ask the clinician to translate the report into plain language and explain whether the finding is confirmed, possibly incidental, or in need of additional investigation. A previous ECG can also be useful: a longstanding unchanged pattern may be viewed differently from a new change, but only the treating team can make that comparison.

Questions to ask at the follow-up

Write down your questions before calling or attending the appointment. These focus the discussion on the information needed to make a safe plan:

  • What exact ECG finding was reported?
  • Was the report reviewed or confirmed by a clinician?
  • Does it need prompt review, or is a routine appointment appropriate?
  • Could my symptoms, prior ECGs, medicines or other health conditions affect the interpretation?
  • What follow-up is recommended, and on what timeline?
  • What is each proposed test intended to clarify?
  • Which symptoms should make me seek care sooner?

Bring a list of medicines and supplements, relevant diagnoses, prior ECG reports if available, and a short record of symptoms. Note when symptoms began, what you were doing, how long they lasted and whether they recurred. This information helps the clinician decide whether the ECG was a screening finding or part of an evaluation for an active problem.

When to seek emergency help

Do not wait for a routine appointment if you develop acute chest pain or symptoms that could represent a chest-pain equivalent. The American Heart Association advises calling 911 in the United States for these emergencies; elsewhere, use the local emergency number. Follow the emergency service’s instructions and do not drive yourself if emergency dispatch or local guidance advises otherwise.

An abnormal screening result is not automatically an emergency, and the absence of chest pain does not prove that it is harmless. The urgency cannot be determined from the label alone. If symptoms are new, severe, worsening or otherwise concerning, tell the clinician or emergency service what is happening and mention the abnormal ECG result.

Screening ECG versus a diagnostic ECG

“Screening” generally means testing someone without symptoms to look for a condition. The U.S. Preventive Services Task Force (USPSTF) addresses resting and exercise ECG screening for asymptomatic adults. It recommends against screening low-risk asymptomatic adults and says evidence is insufficient to determine the balance of benefits and harms for asymptomatic adults at intermediate or high risk.

That recommendation does not mean an ECG is inappropriate when a person has symptoms or when a clinician has another reason to investigate. An ECG obtained because of palpitations, fainting, shortness of breath, chest discomfort, a known condition or an abnormal examination is being used diagnostically, not as routine screening. Tell the clinician why the ECG was ordered; that context changes what an abnormal result may mean and what follow-up is reasonable.

What follow-up might involve

The next step depends on the exact finding and clinical context. A clinician may arrange a review appointment, compare the tracing with an earlier ECG, repeat the ECG under different circumstances, or order another evaluation. The purpose should be explained before you proceed: for example, whether a test is checking a rhythm, assessing heart structure or clarifying whether a pattern is genuine.

Do not select tests based only on the word “abnormal,” and do not assume that every flagged feature requires the same work-up. Ask what question a proposed test is expected to answer, how soon it is needed, what preparation is required and how the result will change management. If you do not understand the plan, ask the clinician to restate it in plain language.

Understand the possible downsides of further testing

Follow-up can be useful, but testing also has potential harms. The USPSTF notes that abnormal screening results may lead to unnecessary radiation exposure and harms from invasive procedures. Those risks are reasons to discuss the expected benefit of each test, not reasons to ignore a medically indicated evaluation.

Figure What it describes Important qualification
0.6%–2.9% Angiography rates reported after abnormal exercise-ECG screening in asymptomatic adults Older studies, mostly from the 1980s and 1990s, summarized by the USPSTF in its 2018 recommendation; not a current individual risk estimate
0.1% and 0.5% Rates of subsequent revascularization reported in those older studies Historical study figures for asymptomatic screening populations, not a prediction for a particular patient today

These numbers show why an abnormal screening result should lead to a reasoned clinical conversation rather than an automatic cascade of procedures. They do not tell you what will happen in your case. Your age, symptoms, risk factors, ECG finding, local practice and other test results all affect decisions.

If you are waiting for the result or a callback

Ask when and how the service will communicate. Some ECG results are available the same day; others take longer because a specialist must review them or because the ECG is part of a broader assessment. NHS guidance says that if you have not heard back after a few weeks, contact the referring doctor. Use any shorter deadline given by your own service, and seek help sooner if symptoms develop.

When you call, state the date and location of the ECG, the name of the ordering clinician and any symptoms you are having. Ask whether the result has been reviewed and whether an appointment is already scheduled. Keep a note of the call, the advice provided and the date of planned follow-up.

How to prepare for the review appointment

  1. Gather records. Bring the ECG report, prior ECGs, discharge letters and recent test results that the service has given you.
  2. List medicines and conditions. Include prescription drugs, over-the-counter products, supplements, implanted devices and known heart or other medical conditions.
  3. Describe symptoms precisely. Record timing, duration, triggers, associated symptoms and whether they stopped or returned.
  4. Clarify the plan. Before leaving, confirm the next appointment or test, who will contact you, the expected timeframe and the symptoms that require earlier care.
  5. Check understanding. Repeat the plan in your own words and ask for written instructions if the information is complex.

Situations that need different conversations

No symptoms and a routine screening ECG

Ask why the screening was performed, what exact feature was flagged and whether the clinician considers the finding clinically meaningful. For asymptomatic adults, the USPSTF’s recommendations distinguish low-risk people from those at intermediate or high risk, and they address screening rather than diagnostic testing. Your clinician can explain how your own risk profile affects the decision.

Symptoms were present when the ECG was obtained

Tell the ordering clinician what you felt, when it occurred and whether it is continuing. An ECG obtained during a symptom evaluation has a diagnostic purpose, so the follow-up plan may differ from that for an incidental screening label. Acute chest pain or an equivalent emergency symptom requires emergency care rather than a routine result call.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

A previous ECG was available

Ask whether the new tracing was compared with the earlier one and whether the finding is new, unchanged or uncertain. Do not infer reassurance or danger from the comparison yourself; the clinician must assess both tracings and your current condition.

The report uses unfamiliar terms

Ask the clinician to define every term you do not understand and to separate a machine-generated suggestion from a confirmed interpretation. Write down the explanation and the follow-up date. A glossary or consumer device cannot replace review of the clinical ECG.

What not to do

  • Do not assume “abnormal” means a heart attack or another specific diagnosis.
  • Do not assume it is harmless because you feel well.
  • Do not delay emergency care while waiting for a screening provider to call.
  • Do not start, stop or change medicines based solely on an ECG label.
  • Do not buy a home ECG device to explain an existing screening result; a consumer recording cannot establish the cause of the reported finding.

Bottom line

Get the exact ECG finding and have the ordering clinician explain its significance, urgency and follow-up plan. A screening label is not a diagnosis, and the appropriate next step depends on why the ECG was done, your symptoms, history and the actual tracing. Ask what each proposed test is meant to clarify, follow the stated timeline, and use emergency services immediately for acute chest pain or a comparable emergency symptom.
