What to Do After a Positive TB Skin Test or Blood Test

A positive tuberculosis (TB) skin test or blood test does not by itself show whether you have inactive (latent) TB infection or active TB disease. Arrange a medical evaluation promptly. A clinician will review your history, examine you, obtain a chest X-ray, and order sputum or other laboratory tests when indicated. Do not assume that feeling well means no follow-up is needed, and do not assume that a positive screening result means you are contagious.

The guidance below is based on U.S. Centers for Disease Control and Prevention (CDC) recommendations. Reporting rules and referral pathways vary by state and country, so your clinician or local TB program can explain the requirements where you live.

1. Contact a clinician or local TB program

Call a health care provider, infectious-disease clinic, or local health department and say that you had a positive TB skin test (tuberculin skin test, or TST) or TB blood test (interferon-gamma release assay, or IGRA). CDC clinical guidance says that anyone with a positive result should be medically evaluated for TB disease. See the CDC clinical and laboratory diagnosis guidance.

When you make the appointment, provide:

  • Which test was positive: skin test or blood test.
  • The date it was performed and, for a skin test, the measured reaction in millimeters if it is recorded.
  • Any known contact with someone who had TB disease, recent travel or residence in an area where TB is common, or prior TB treatment.
  • Your current symptoms, medical conditions, medicines, pregnancy status, and any condition or treatment that weakens your immune system.
  • Whether you received Bacille Calmette-Guérin (BCG) vaccination.

Do not wait for a routine workplace appointment if you have symptoms that could indicate TB disease. Tell the scheduling staff about them so the clinical assessment can be prioritized.

2. Report symptoms promptly

TB disease can affect the lungs or other parts of the body. CDC lists cough, chest pain, fever, weight loss, and tiredness among symptoms of TB disease. A clinician should begin diagnostic evaluation when TB disease is suspected rather than waiting for a screening-test result. The CDC explains the diagnostic approach at Diagnosing Tuberculosis and in its clinical testing and diagnosis guidance.

When you call, describe when each symptom began and whether it is worsening. Also mention coughing or other respiratory symptoms to help the clinic arrange an appropriate evaluation. A positive test with no symptoms still requires follow-up because latent infection and active disease cannot be separated by the screening result alone.

3. Understand what the positive result does—and does not—tell you

A TST or IGRA detects an immune response associated with TB infection. Neither test can determine on its own whether bacteria are inactive or causing disease. CDC summarizes the point this way: “All persons with signs or symptoms of TB disease, or a positive result from a TB blood test or skin test should be medically evaluated for TB disease.”

Test What happens Important limitation after a positive result
TB skin test (TST) A small amount of tuberculin is placed in the skin. The site must be examined by a trained health worker two to three days later. The reaction can be affected by prior BCG vaccination and still cannot distinguish latent infection from active disease.
TB blood test (IGRA) A blood sample is analyzed for an immune response to TB bacteria. A positive result still requires an evaluation for disease; it does not establish that disease is active or that you can spread TB.

CDC patient information on the two methods is available for the skin test and blood test.

4. Expect an evaluation for active TB disease

The purpose of the follow-up is to decide whether you have active disease, latent infection, or another explanation for the result. The exact work-up depends on your symptoms, examination, exposure history, immune status, and earlier medical records.

Medical history and examination

The clinician will ask about symptoms, previous TB tests or treatment, close contacts, travel or residence, BCG vaccination, and conditions that increase the risk of progression. A physical examination helps identify findings that require additional testing. Bring any prior results, vaccination records, and treatment documentation you have.

Chest X-ray

A chest radiograph is generally part of the evaluation. It can show findings that raise concern for pulmonary TB, but an X-ray alone cannot definitively diagnose TB disease. A normal image also has to be interpreted with your symptoms and risk factors.

Sputum and other laboratory tests

If you have respiratory symptoms or concerning examination or X-ray findings, the clinician may collect sputum. Possible studies include smear microscopy, nucleic acid amplification testing, culture, and drug-susceptibility testing. Culture is the microbiologic gold standard for confirming TB, while the other tests provide complementary information and can help guide immediate decisions. The tests selected and the number of samples depend on the clinical situation. CDC describes these methods in its clinical and laboratory diagnosis guidance.

Do not interpret a chest X-ray report or a single laboratory result without the clinician’s assessment. TB diagnosis combines the history, examination, imaging, and microbiology.

5. Learn the difference between latent infection and active disease

Feature Latent (inactive) TB infection Active TB disease
What is happening TB bacteria are present but inactive. TB bacteria are active and multiplying.
How a person may feel Usually no TB symptoms. Symptoms may occur, including cough, chest pain, fever, weight loss, or tiredness, although presentation varies.
Can it spread to others? No. Latent infection is not contagious. It can spread, especially when disease affects the lungs or throat.
Typical evaluation findings Chest X-ray is typically normal and sputum tests are generally negative. Chest imaging or sputum testing may show disease; confirmation relies on the complete clinical and laboratory evaluation.

CDC’s overview states, “People with latent TB infection are infected with TB bacteria, but they do not have TB disease.” Read the agency’s latent TB infection overview and clinical overview of TB for the distinctions clinicians use.

6. Discuss treatment if active disease is excluded

If the evaluation does not indicate active disease and the positive test is consistent with infection, the clinician may diagnose latent TB infection. Active disease must be excluded before preventive treatment is started; treating presumed latent infection without checking for disease can be unsafe and inappropriate.

CDC estimates that about 5% to 10% of people with untreated latent TB infection develop TB disease during their lifetimes. CDC also states that treatment for latent infection is 90% effective at preventing TB disease. That figure describes the preventive benefit across treated populations, not a guarantee for one person. Your clinician chooses the regimen after considering age, other medicines, liver and kidney health, pregnancy, immune status, and the possibility of drug interactions. Do not start, stop, or share TB medicines without clinical supervision.

Ask how monitoring will work, what side effects require a call, and how missed doses should be handled. Keep a written record of the diagnosis and any completed treatment.

7. Account for BCG vaccination and previous results

If you received BCG

Tell the clinician if you were vaccinated with BCG. CDC prefers TB blood tests for people who received BCG because BCG does not affect the blood test, whereas it can affect a skin-test reaction. A positive skin test still needs the same appropriate medical evaluation; BCG history does not eliminate the need to assess for TB disease.

If you have tested positive before

A TB test can remain positive after treatment. Ask for a written copy of the result, the evaluation, and any treatment completion record. When a documented positive result is available, showing it to future clinicians or employers can help avoid unnecessary repeat testing. Follow your workplace or local public-health instructions if they request specific documentation.

8. Know when local public-health guidance matters

State, county, and national rules differ on reporting, contact investigations, work or school clearance, and follow-up. CDC identifies state and local TB programs as sources of assistance. Your clinician or health department can tell you whether the result must be reported, where to obtain imaging, and what documentation an employer or school accepts.

If you may have been exposed recently, live with someone at higher risk, are pregnant, have a weakened immune system, or have a complex medical condition, mention that at the first contact. These circumstances can change the urgency and the tests selected, so individualized advice is important.

Common mistakes to avoid

  • Assuming “positive” means contagious: contagiousness depends on whether active disease is present, not on the screening result alone.
  • Skipping follow-up because you feel healthy: latent infection often causes no symptoms, yet it can progress later without treatment.
  • Using a consumer device as a substitute for diagnosis: home symptom tools or marketed testing equipment cannot replace the clinical evaluation, chest imaging, and laboratory work indicated by a positive result.
  • Repeating a test without records: preserve the original result and treatment history so future providers can interpret later testing correctly.
  • Starting preventive treatment before disease is excluded: the clinician must first assess for active TB.

What to bring to your appointment

  • The laboratory report or skin-test measurement and the date of testing.
  • Prior TB test, chest X-ray, and treatment records.
  • A list of symptoms, their start dates, and known TB exposures.
  • Your medicine and supplement list, allergies, and relevant medical conditions.
  • BCG vaccination information and any employer, school, immigration, or health-department paperwork.

The practical next step is straightforward: contact a qualified health professional or local TB program, report symptoms, complete the recommended evaluation, and then discuss treatment if latent infection is confirmed. A positive skin or blood test is a reason for medical assessment—not a diagnosis of active TB by itself.
