Do not try to eliminate cortisol. Cortisol is essential for the stress response, blood-pressure control, inflammation regulation and energy use. Symptoms sometimes attributed to “high cortisol” are not enough to diagnose a disorder. The safe approach is to review steroid exposure, assess symptoms with a clinician, use validated testing when indicated, and treat confirmed Cushing syndrome according to its cause.
Cortisol is necessary, so “zero cortisol” is not a health goal
Cortisol is a hormone made by the adrenal glands. It helps the body respond to physical and psychological stress, supports blood pressure, helps regulate inflammation, affects cardiovascular function and makes stored energy available. You need an appropriate amount at the appropriate time; both excessive and insufficient cortisol can cause illness.

Because cortisol has useful jobs, treatment aimed simply at lowering a number is unsafe when no disorder has been established. The Endocrine Society’s clinical practice guideline states: “We recommend against treatment to reduce cortisol levels or action if there is not an established diagnosis of CS.” In practice, that means a supplement, restrictive diet or unvalidated home test should not be used to force cortisol down.

Clinicians generally intervene to normalize cortisol or block its effects when overt Cushing syndrome (CS) has been diagnosed. Until then, the priority is finding out whether symptoms reflect hypercortisolism at all, another medical condition, a medication effect or ordinary variation in stress and sleep.

Symptoms that can justify an evaluation
Cushing syndrome can produce a recognizable pattern, but no single symptom proves that cortisol is high. Mayo Clinic lists central or truncal weight gain with relatively thin arms and legs, a rounded face, a fatty lump between the shoulders, pink or purple stretch marks, thin skin that bruises easily, slow wound healing, acne, muscle weakness, mood changes, sleep problems, high blood pressure, headaches, infections and bone loss. The NHS also describes increased fat around the neck, upper back, chest and abdomen, a red and rounded face, easy bruising, stretch marks, weakness in muscles close to the trunk, reduced libido, menstrual changes, mood changes and excess hair growth.
These findings overlap with common conditions such as weight change from diet or medication, type 2 diabetes, high blood pressure, depression, anxiety, sleep disorders, menopause, polycystic ovary syndrome and ordinary deconditioning. A symptom checklist can tell you what to discuss; it cannot establish hypercortisolism. A clinician will look for combinations, progression, physical signs and risk factors rather than interpreting one complaint in isolation.
Patterns that deserve prompt discussion
- Rapid or otherwise unexplained change in body shape, especially increasing abdominal, facial or upper-back fat with thinner limbs.
- Wide pink or purple stretch marks, fragile skin, frequent bruising or wounds that heal unusually slowly.
- New difficulty climbing stairs, rising from a chair or lifting objects because of weakness near the hips or shoulders.
- Several new problems together, such as high blood pressure, elevated blood glucose, menstrual or sexual changes, osteoporosis or repeated infections.
- Symptoms that began or accelerated after starting, increasing or repeatedly receiving glucocorticoid medication.
Review steroid exposure before assuming an adrenal or pituitary problem
The NHS identifies prolonged, high-dose steroid medicines as the most common cause of Cushing syndrome. Examples include hydrocortisone and prednisolone, but other glucocorticoids can also contribute. Steroids may be prescribed as tablets, injections, inhalers, creams, eye preparations or other formulations; cumulative exposure and dose matter.

Make a complete list of current and recent medicines, including inhalers, creams, injections, “burst” courses, doses and dates. Tell the clinician why each steroid was prescribed and whether doses were missed. Do not stop a prescribed glucocorticoid suddenly. Long-term treatment suppresses the body’s own cortisol production, and an abrupt stop can cause adrenal insufficiency or a flare of the illness the steroid was treating. Any reduction must be planned and supervised, with the taper determined by the underlying condition, dose, duration and clinical response.
If no medication explains the pattern, endogenous CS can result from a cortisol-producing adrenal growth, a pituitary growth or, less commonly, cortisol stimulation from another tumor. The cause cannot be identified safely from symptoms or a single cortisol reading.
Know when symptoms require urgent care
Arrange routine medical assessment for a persistent pattern of possible CS, especially when several signs are developing together. Seek urgent medical help for severe or rapidly worsening complications, including very high blood pressure with symptoms, chest pain, shortness of breath, one-sided leg swelling, signs of stroke, severe infection, confusion, suicidal thoughts or acute psychosis. Excess cortisol can worsen cardiovascular risk, infection risk, blood sugar, potassium balance, bone health and psychiatric symptoms. Emergency services are appropriate when symptoms are severe, sudden or life-threatening.

How clinicians test for cortisol excess
Testing is selected after a history and examination. The Endocrine Society patient resource describes three common approaches. They measure cortisol behavior over time rather than relying on a casual blood value.
The Tool Desk
Outbyte PC Repair FREERepair Windows errors before they cause bigger problemsFix Now →Outbyte Driver Updater FREEFix the driver behind crashes, sound loss and screen glitchesFind Drivers →- Late-night salivary cortisol. Saliva is collected between 11 p.m. and midnight, when cortisol should normally be low. The clinician may request more than one collection and will explain preparation, contamination precautions and timing.
- 24-hour urinary free cortisol. You collect all urine for a complete 24-hour period in a supplied container. Missing a collection can make the result unreliable, so follow the laboratory’s storage and return instructions exactly. Repeat collections are often needed because output varies from day to day.
- Overnight dexamethasone suppression test. You take dexamethasone at a specified time and have cortisol measured the next morning. In people without CS, dexamethasone should suppress cortisol; failure to suppress prompts further evaluation. Medication interactions, pregnancy, liver disease and other factors can affect interpretation.
A general practitioner may arrange blood, urine or saliva testing and refer someone with abnormal results to an endocrinologist for additional tests of the pituitary or adrenal glands. A consumer test ordered without clinical context, a single random cortisol result or a social-media “cortisol pattern” does not diagnose CS. Ask which test was chosen, how to collect it, whether it must be repeated and what could produce a false result.
Understand the terms: Cushing syndrome and Cushing disease
Cushing syndrome means prolonged exposure to excessive cortisol or cortisol-like activity from any cause, including medication. Cushing disease is a specific form of Cushing syndrome caused by a pituitary tumor that drives the adrenal glands to make too much cortisol.

Cushing disease is rare. The Endocrine Society’s 2022 patient resource says it affects about 10 to 15 people per million each year. Rarity does not make evaluation unnecessary when the physical pattern is convincing, but it does make indiscriminate testing and self-treatment inappropriate.

Treatment is directed at the source of excess cortisol and at the health problems it has caused. The plan depends on whether the cause is medication, pituitary, adrenal or another tumor, how severe the disease is and whether surgery is feasible.

The clinician treats the underlying illness while gradually reducing the glucocorticoid when medically possible. The taper may be slow and may require monitoring for adrenal insufficiency. Some people need temporary replacement dosing or “sick-day” instructions while normal adrenal function recovers. Never design this taper yourself.

When the body is making excess cortisol
For endogenous CS, the Endocrine Society recommends addressing the causal lesion, with surgery generally first-line when feasible. Pituitary or adrenal surgery is selected according to the source. If surgery is not possible, is incomplete or does not control cortisol, specialists may use medicines that reduce cortisol production or block its action, radiotherapy for selected pituitary disease, or additional procedures. Choice and duration require endocrine monitoring; these drugs can affect liver function, electrolytes, blood pressure and other hormones.
Treating complications is part of the treatment
Guidelines call for active monitoring and adjunctive care for psychiatric disorders, diabetes, hypertension, low potassium, infections, abnormal lipids, osteoporosis and poor physical fitness. A person can need blood-pressure treatment, glucose management, bone protection, infection prevention, mental-health care and a supervised rehabilitation plan even after the source of cortisol excess has been addressed.
What you can do safely while an evaluation is arranged
These steps support general health and improve the quality of a clinical assessment; they are not a substitute for treating CS.

Quick Recap
- Keep a dated symptom and medication record. Note weight and waist changes, bruising, stretch marks, muscle tasks that have become difficult, blood-pressure readings, menstrual or libido changes, infections, sleep and mood. Record every steroid product and dose.
- Bring objective information. Take medication containers or an up-to-date list, recent laboratory reports and a family or personal history of diabetes, hypertension, osteoporosis and endocrine disease.
- Protect sleep and regular routines. Consistent sleep and wake times, moderate daytime activity appropriate to your fitness and regular meals can improve energy, blood pressure and mood. They do not “flush out” cortisol.
- Address established risks. Follow prescribed treatment for blood pressure, diabetes, asthma or other conditions. Do not replace it with an over-the-counter cortisol blocker.
- Use caution with supplements. Products marketed as cortisol reducers may contain undisclosed or pharmacologically active ingredients, interact with medicines or delay diagnosis. No supplement has been established as a treatment for CS.
Questions to take to your clinician
- Do my findings suggest CS, or are there more likely explanations?
- Could any tablet, injection, inhaler, cream or supplement be contributing?
- Which screening test is appropriate, and what collection time and preparation are required?
- Will an abnormal result be repeated or confirmed by another method?
- If CS is confirmed, which specialist will determine the source and treatment?
- Should my blood pressure, glucose, potassium, bones, infection risk or mental health be checked now?
- If I use a steroid, what is the safe plan for missed doses, illness and tapering?
Mistakes that can make the situation worse
- Trying to eliminate cortisol. Cortisol is essential; excessively suppressing it can be dangerous.
- Diagnosing yourself from nonspecific symptoms. Weight gain, fatigue, insomnia and anxiety have many causes.
- Stopping steroids abruptly. This can trigger adrenal crisis or a flare of the treated disease.
- Ordering one unvalidated test and treating the result. Timing, collection errors, medicines and medical conditions can distort results.
- Delaying care while using “natural” products. Supplements do not remove a pituitary or adrenal cause and may interfere with testing or treatment.
A practical decision path
- If symptoms are mild, isolated or recent, schedule a primary-care visit and document changes rather than attempting to lower cortisol yourself.
- If several characteristic signs are present or steroid exposure is substantial, request a focused evaluation and bring the complete medication history.
- Complete the clinician-selected saliva, urine or dexamethasone test exactly as instructed; do not interpret a home result in isolation.
- If screening is abnormal, attend endocrine follow-up for confirmation and source testing.
- If CS is confirmed, follow cause-directed treatment and monitoring for complications, with urgent care for severe cardiovascular, infectious, thromboembolic or psychiatric symptoms.
