If you are losing weight, appetite, or strength during cancer treatment, tell your oncology team and ask for an assessment—not just a calorie target. Cancer cachexia can involve changes in appetite, body weight, skeletal muscle, strength, and daily function. It is not always caused by eating too little, and there is no single treatment proven to reverse it for everyone. Use the questions below to plan a conversation about symptoms, nutrition, medicines, rehabilitation, and how you will judge whether a plan is helping.
Questions to ask at your next appointment
- “What might be causing my weight or muscle loss, and how will you assess it?” Ask how the team will consider your weight and appetite history, strength, daily function, cancer, and current treatment rather than assuming one cause.
- “Could symptoms be making it harder for me to eat?” Mention nausea, pain, mouth sores, swallowing trouble, feeling full quickly, constipation, or changes in mood. Ask which symptoms can be treated and whether you need help from a speech therapist, dentist, or another specialist.
- “Can you refer me to an oncology dietitian?” Ask for advice tailored to your needs, treatment symptoms, and dietary requirements. If you are considering nutrition drinks, ask whether one is useful for you and which type fits your situation.
- “Is a medicine appropriate for me, and what would we be trying to achieve?” If a medicine is proposed, ask about likely benefit, risks, interactions with your other medicines, and when you would stop or review it.
- “Would physical therapy or rehabilitation help me preserve strength or daily function?” Ask what activity is safe for you and whether a physical therapist can adapt a plan to your abilities and treatment.
- “How will we track whether this plan is helping, and when will we review it?” Agree on measures that matter to you, such as appetite, weight trend, strength, daily activities, treatment tolerance, or quality of life.
- “Is a clinical trial relevant to my cancer, treatment stage, and goals?” Ask whether a cachexia trial is available and what participation would involve.
- “What changes should prompt me to contact the team sooner?” Ask for clear instructions specific to your symptoms and treatment.
These are prompts for a clinical conversation, not a diagnosis or treatment plan. Do not start, stop, or change medicines, supplements, feeding approaches, or exercise intensity based only on this article.

What cancer cachexia means—and why assessment matters
Cachexia is a wasting syndrome associated with cancer that can involve loss of appetite, body weight, skeletal muscle, and fat, as well as weakness and fatigue. It can affect strength and the ability to carry out everyday activities. Because it is not simply a matter of eating less, nutrition support alone may not reverse all of its metabolic and disease-related effects. Weight or muscle loss is not a personal failure, and patients and caregivers should not be blamed for it.
The American Society of Clinical Oncology’s 2020 adult guideline estimated that cachexia affects approximately half of people with advanced cancer. That broad estimate is not a rate for every cancer type or stage. Report changes in appetite, weight, or strength early and ask how the team will assess your risks and needs.

What current guidance says about treatment options
Treatment is individualized: the goal might be easing symptoms that interfere with eating, supporting nutrition, improving appetite or weight, or preserving strength and daily function. These outcomes are not interchangeable. An increase in appetite or body weight does not necessarily mean that muscle, strength, function, or survival has improved.
Dietitian counseling and nutrition support
ASCO’s 2020 guideline says clinicians may offer assessment and counseling by a registered dietitian to people with advanced cancer and appetite or weight loss. The guideline describes practical feeding advice and education about high-protein, high-calorie, nutrient-dense foods, and advises against fad or extreme diets. A dietitian may also discuss whether a nutrition supplement drink fits your circumstances; such drinks are individualized support, not proven cachexia treatments.

Medicines, including olanzapine
ASCO’s 2020 guideline found evidence insufficient to strongly endorse a drug as standard treatment for cancer cachexia; clinicians may choose not to prescribe a cachexia-specific medicine. If a medication trial is chosen to address appetite or weight, the 2020 guideline discussed progesterone analogs or short-term corticosteroids.

ASCO’s 2023 rapid update added a clinician-led option: for adults with advanced cancer, clinicians may offer low-dose olanzapine once daily to improve appetite and weight gain. The update’s qualifying statement says most evidence involved lung or gastrointestinal cancers, and the largest study enrolled people receiving cytotoxic chemotherapy. This does not make olanzapine appropriate for everyone or establish that it restores muscle or function. ASCO states that there are currently no FDA-approved medicines to treat cancer cachexia. Ask your oncologist whether any medication is suitable for your situation, including its risks, interactions, intended goal, and review point.

Feeding tubes and intravenous nutrition
ASCO advises against routine tube feeding or parenteral (intravenous) nutrition solely to manage cachexia in advanced cancer outside a clinical trial. A short-term parenteral nutrition trial may be considered for selected people with a reversible obstruction, short bowel, or malabsorption who are otherwise reasonably fit; this requires clinical judgment and is not a general recommendation.
Physical therapy and rehabilitation
Rehabilitation may help support strength, endurance, or daily activities, depending on the person’s condition and goals. However, ASCO’s 2020 guideline did not make a recommendation for exercise as a cachexia treatment because the evidence was insufficient. Ask the oncology team whether physical therapy or another form of rehabilitation is suitable and what level of activity is safe.

How to agree on a plan and judge its value
Before trying an intervention, ask what outcome it is meant to change and how much burden it involves. A reasonable review plan identifies when the team will check progress and what would count as a meaningful benefit for you. Your priorities may differ from someone else’s.
- Goal: Clarify whether the aim is symptom relief, appetite, weight, strength, function, treatment tolerance, or quality of life.
- Fit: Ask how the option’s evidence relates to your cancer, stage, and current treatment.
- Trade-offs: Discuss risks, interactions, practical burden, and cost where relevant.
- Review: Agree on what you and the team will monitor and when to reassess, adjust, or stop the intervention.
What to know about supplements and other approaches
Do not use unproven supplements, high-dose products, or restrictive diets as cancer treatments without discussing them with your oncology team. ASCO’s guidance steers patients away from fad and extreme diets. Ask the team or dietitian before making major changes, especially if treatment symptoms or other health conditions affect what you can eat.
