What to Ask Your Oncologist About a New Targeted Cancer Treatment

By Freedom251 Team · · 10 min read

Before agreeing to a newly proposed targeted therapy, ask four things first: what feature of your cancer it is meant to target, what test supports that choice, what benefit and harm are realistic for you, and how the team will monitor and pay for treatment. “Targeted” does not automatically mean appropriate, effective, or less toxic than chemotherapy. The exact cancer, biomarker, medicine, treatment setting, and country determine the answers.

Take a written list to the appointment and, if you want, bring a trusted person to listen and take notes. The National Cancer Institute (NCI) recommends preparing questions about benefits, risks, side effects, medicines, timing, and clinical trials in its Questions to Ask about Cancer Treatment guide.

Start with the treatment’s purpose

Ask your oncologist to state the goal in plain language. A targeted treatment might be intended to cure the cancer, reduce the chance it returns, shrink or control it, slow progression, relieve symptoms, or achieve another defined objective. The goal affects how success is measured and how long treatment may continue.

Questions to ask

  • What is the goal of this treatment for me?
  • Is the goal cure, reducing recurrence risk, control, slowing growth, symptom relief, or something else?
  • What result would make you continue, change, or stop this treatment?
  • How soon might we know whether it is helping?

Ask which evidence applies to people with your diagnosis, stage, prior treatments, and overall health. A result from a different cancer or a different drug may not predict your outcome. There is no reliable benefit percentage for an unspecified cancer and medicine, so ask for evidence that matches your situation rather than accepting a general claim about targeted therapy.

Find out exactly what is being targeted

Targeted therapy is chosen because a cancer cell has a feature—often a gene change, protein, or other substance—that a medicine may act on. Tumor biomarker testing can look for these features, but a finding is not automatically a treatment instruction. NCI explains that some results guide an approved treatment, some may support an off-label treatment or a clinical trial, and some do not guide treatment decisions at all. See Biomarker Testing for Cancer Treatment.

Questions about the biomarker result

  • Which biomarker or cancer feature does this medicine target?
  • What test found it, and can I have a copy of the report?
  • Was the sample taken from the current tumor, an older specimen, or a blood test?
  • How certain is the result, and is it considered actionable for my cancer?
  • Are any findings uncertain, benign, or not useful for choosing treatment?
  • Could the tumor change over time, making repeat testing useful?

A matching biomarker points to a possible treatment; it does not guarantee that the medicine will work. Other tumor characteristics, resistance mechanisms, the amount and location of cancer, previous treatment, and your health can all affect the result. Ask what the test can and cannot establish before treating the report as a promise.

Do not confuse tumor testing with inherited testing

Tumor testing examines cancer cells. It may occasionally raise the possibility of an inherited mutation that could matter to relatives as well as to treatment. That possibility is not the same as a confirmed inherited result. Ask whether confirmatory testing and genetic counseling are recommended, what the result could mean for family members, and who will explain it.

Ask about test limits

Testing may depend on whether a biopsy is safe, whether there is enough usable tissue, laboratory access, trial eligibility, and insurance coverage. Ask whether a new biopsy is necessary, what risks it carries, whether blood-based testing is an alternative, how long results will take, and what happens if the sample cannot be analyzed.

Clarify approval, off-label use, or a clinical trial

Ask how the proposed use is classified in the country where you are being treated. In the United States, ask whether the medicine is approved by the Food and Drug Administration (FDA) for your specific cancer and situation, used off-label, or being offered through a clinical trial. An approved drug may still be used differently from its labeled indication, and an off-label recommendation is not automatically inappropriate; the important issue is the evidence and the reason your oncologist believes it fits.

Questions that reveal the evidence

  • Is this treatment FDA-approved for my cancer, stage, biomarker, and line of treatment?
  • If it is off-label, what evidence supports using it for someone like me?
  • If this is a trial, what phase is it, what is known about risks, and what alternatives remain?
  • Would joining the trial change which tests, visits, or follow-up I need?
  • Who pays for the medicine, trial-related tests, and routine care?

Outside the United States, ask for the equivalent local regulatory status. A trial can provide access to a promising approach but also involves uncertainty and eligibility requirements. NCI explains reasons people consider trials in Why Participate in a Clinical Trial?

Discuss benefits, uncertainty, and alternatives together

Ask your oncologist to compare the proposed therapy with every reasonable alternative, including no immediate treatment when that is clinically appropriate, another standard treatment, a different targeted approach, supportive care, a second opinion, or a clinical trial. The relevant choice is not “targeted therapy versus nothing”; it is the option that best fits your goals and circumstances.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

For each option, ask

  • What benefit do you expect, and how strong is the evidence for my exact cancer?
  • How uncertain is the estimate?
  • What are the common side effects and the most serious risks?
  • How is it given, how often, and for how long?
  • What monitoring, travel, time away from work, or caregiver help will it require?
  • Could it affect fertility, pregnancy, sexual health, or daily activities?
  • How quickly could treatment start?
  • What happens if the cancer does not respond or later stops responding?
  • What would the next option be?

Ask for a recommendation and the reasoning behind it, but also say what matters most to you—longevity, symptom relief, avoiding particular effects, staying at home, preserving fertility, or minimizing visits. A shared decision is clearer when your priorities are explicit.

Get the exact administration and scheduling plan

“Targeted therapy” describes a broad group of medicines, not one schedule or route. Confirm whether treatment is a pill, infusion, injection, or combination; whether it is taken with food; and what to do after a missed or vomited dose. Ask who prescribes it, where it is administered, and whether you need a first-dose observation period.

Write down the practical details

  1. Record the medicine’s generic and brand names, dose, route, and schedule.
  2. Ask how long each visit takes and which appointments can be combined.
  3. Confirm required baseline tests, such as blood work, scans, heart tests, or blood-pressure checks.
  4. Ask whether work, driving, exercise, diet, travel, or sexual activity needs to change.
  5. Find out how refills, delivery, missed doses, and treatment delays are handled.

Request the medicine’s patient guide or prescribing information. Keep it with your medication list and the team’s written instructions.

Build a side-effect and urgent-contact plan

Targeted medicines can cause serious harm, and the exact pattern depends on the drug and on how your body reacts. The NCI states: “The side effects that you may have depends on the type of targeted therapy you receive and how your body reacts to it.” Read the NCI overview at Targeted Therapy for Cancer.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Possible effects across targeted therapies include diarrhea, liver problems, blood clots, wound-healing problems, high blood pressure, fatigue, mouth sores, nail changes, changes in hair color, and skin problems. This is not a universal checklist: your specific medication guide is the authority for your treatment.

Ask for symptom thresholds, not vague reassurance

  • Which symptoms are expected and can be reported at the next visit?
  • Which symptoms require a same-day call?
  • Which symptoms mean I should seek urgent or emergency care?
  • What temperature, blood-pressure reading, bleeding, breathing change, pain, rash, diarrhea, or vomiting threshold should I use?
  • What can I do at home to prevent or manage predictable effects?
  • Should I stop, hold, or continue a dose while waiting for advice?

Ask for a daytime number, an evening and weekend number, and instructions for what to do if nobody answers. Follow your own team’s emergency guidance rather than waiting for an online symptom list.

Agree on how response and toxicity will be monitored

Monitoring may include visits, physical examinations, blood tests, imaging, and other tests. Ask what will be checked before each dose and what findings would require a dose change, pause, or switch.

Questions about knowing whether it works

  • When is the first scan, examination, or other response assessment?
  • Which improvement would count as success?
  • What would stable disease mean in my case?
  • How will you distinguish treatment effects from cancer symptoms?
  • How often will blood counts, liver tests, kidney tests, blood pressure, or other safety tests be done?
  • When will we formally reconsider the plan?

Ask how results will be communicated and who will explain an unexpected scan or laboratory value. A treatment can have effects that require action before a scan shows whether the cancer is changing, so clarify both effectiveness and safety monitoring.

Review every medicine, supplement, and health condition

Give the team a complete list of prescription drugs, over-the-counter medicines, vitamins, herbs, supplements, and recreational substances. Include occasional pain relievers, antacids, sleep products, and medicines prescribed by another clinician. Ask whether anything could alter the treatment’s effects or increase toxicity.

Useful medication questions

  • Should I stop or change any medicine before starting?
  • Are there foods, supplements, or over-the-counter products to avoid?
  • Who should review interactions when another clinician prescribes something?
  • What should I do if I cannot keep the medicine down?

Also mention allergies, liver or kidney problems, heart conditions, high blood pressure, previous blood clots, planned surgery, pregnancy, breastfeeding, and fertility goals. These details can change monitoring, timing, or the range of safe choices.

Ask about coverage, testing costs, and support

Before treatment starts, ask whether your insurer covers the medicine, biomarker test, scans, laboratory work, infusion services, and management of side effects. Coverage can differ between the drug and the test that justifies it.

Practical financial questions

  • Which parts of treatment are covered, and what deductibles or co-payments apply?
  • Does the biomarker test require prior authorization?
  • Who can help with an appeal if coverage is denied?
  • Are there hospital, pharmacy, transportation, or lodging costs?
  • Is there a financial counselor, social worker, or patient navigator I can contact?

Ask for written estimates when possible. If treatment is in a clinical trial, ask specifically which costs the study covers and which remain part of routine care.

Decision point Questions to record for each option
Goal and evidence What is the intended outcome, and how directly does evidence apply to my cancer?
Biomarker support Which test supports it, and how certain or actionable is that result?
Expected benefit What improvement is realistic, how soon might it appear, and what is unknown?
Risks What common effects and serious complications matter most for me?
Administration Is it oral, infused, or injected; how often; and for how long?
Monitoring Which visits, laboratory tests, scans, and home measurements are required?
Daily life How could it affect work, travel, caregiving, fertility, or other priorities?
Status Is it approved for this use, off-label, or available through a trial?
Timing How soon can it begin, and what could delay it?
Cost What will insurance cover, and who handles authorization or assistance?

Prepare for the appointment and leave with a written plan

  1. Write your top three goals and the questions you cannot leave unanswered.
  2. Bring pathology, imaging, medication, allergy, and supplement information if the clinic does not already have it.
  3. Ask permission for your companion to take notes or for you to record the discussion if local rules and clinic policy allow.
  4. Request copies of the biomarker report, treatment plan, medication guide, and laboratory or imaging schedule.
  5. Use teach-back: explain in your own words the dose, schedule, warning symptoms, and contact numbers, then ask the clinician to correct anything wrong.
  6. Before leaving, confirm the next appointment, the first monitoring test, the after-hours plan, and who will call with results.

If you need more time, ask whether a second appointment, tumor-board review, genetic counselor, pharmacist, palliative-care clinician, or second-opinion consultation would help. A second opinion can clarify evidence and alternatives without obligating you to change treatment.

Questions that often remain after the visit

Does a biomarker match guarantee that treatment will work?

No. It identifies a possible vulnerability or treatment route, but response also depends on the biology of the tumor, resistance, prior treatment, and individual health. Ask what evidence exists for your exact diagnosis and how response will be tested.

Is targeted therapy automatically safer than chemotherapy?

No. Targeted therapies can cause serious side effects; their risks are different from, not automatically lower than, those of chemotherapy. Use the drug-specific medication guide and your team’s warning plan.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

What if no actionable biomarker is found?

A test may be negative, uncertain, or not treatment-guiding. Ask whether the sample was adequate, whether another test is appropriate, and what standard treatments, trials, or supportive options fit your goals.

What does off-label mean for me?

It means the medicine is being used in a way that is not the regulator’s labeled indication for that specific situation. Ask why your oncologist recommends it, what evidence supports it, how uncertainty compares with alternatives, and how coverage will work.

Should I wait for every test before deciding?

That depends on the urgency of your cancer and the information still missing. Ask which results are essential, which can arrive later, and what risks come with delaying or starting treatment while information is incomplete.
