Focused ultrasound is an awake, MRI-guided procedure that creates a permanent lesion; it is not a cure, and different targets have different indications and uncertainties.
Start with the exact procedure, not just the words “focused ultrasound”
Focused ultrasound for Parkinson’s disease uses sound waves guided by magnetic resonance imaging (MRI) to heat a small area of brain tissue. The heat creates a lesion that interrupts movement-related circuits. There is no incision, implanted electrode, or general anesthetic, and the patient remains awake so the team can monitor effects during treatment.

“Non-invasive” describes the lack of an incision or implanted hardware. It does not mean that the brain is unchanged: the treated tissue is intentionally and permanently damaged. The lesion cannot be programmed or enlarged later in the way an implanted stimulation system can be adjusted.
Ask your neurologist to write down the proposed procedure and target. Focused ultrasound may refer to thalamotomy, pallidotomy, or subthalamotomy, and those procedures are not interchangeable.

Why the brain target and symptom goal matter
Thalamotomy: usually a tremor-focused discussion
A thalamotomy targets the thalamus, a relay area involved in tremor circuits. The Parkinson’s Foundation describes it primarily as an option for tremor. It may be considered when tremor remains disabling despite medication, but the expected benefit depends on the type, distribution, and medication response of your tremor.
Ask: “Is my main problem the kind of tremor this target is intended to treat, and what symptoms would remain untreated?” A unilateral treatment generally addresses symptoms on the opposite side of the body; it is not a treatment for every Parkinson’s symptom.
Pallidotomy: motor complications and dyskinesia
Pallidotomy targets the globus pallidus and may be discussed for slowness, stiffness, dystonia, tremor, troublesome “off” time, or dyskinesia. The U.S. Food and Drug Administration’s October 29, 2021 supplement for the Exablate Neuro system specifies unilateral pallidotomy for people at least 30 years old with advanced idiopathic Parkinson’s disease and medication-refractory, moderate-to-severe motor complications, as an adjunct to Parkinson’s medication. The globus pallidus internus (GPi) must be identifiable and accessible for ablation.
That label applies to a particular device, target, indication, and jurisdiction. It is not a universal eligibility rule for every focused-ultrasound procedure or every country. Ask which current device label or local authorization covers the treatment being offered to you.

Subthalamotomy: a more restricted and uncertain option
Subthalamotomy targets the subthalamic nucleus and may improve slowness and stiffness. The Parkinson’s Foundation characterizes focused-ultrasound subthalamotomy as experimental and not FDA approved on its patient information page. The procedure has also been studied in selected people with markedly asymmetric disease, rather than in everyone with Parkinson’s.
The 2022 European Academy of Neurology/Movement Disorder Society–European Section guideline recommended unilateral MRI-guided focused ultrasound of the subthalamic nucleus only within studies or registries at that time. NICE’s public information page likewise says there is not enough evidence to be sure how well MRI-guided focused-ultrasound subthalamotomy works or how safe it is. Those statements concern subthalamotomy specifically; they should not be applied automatically to a different target or later regulatory decision.
Questions that establish whether you are a plausible candidate
Eligibility is individualized. A movement-disorders neurologist and the treating multidisciplinary team should review your diagnosis, symptom pattern, medication response, imaging, and overall health. Use these questions to make the discussion concrete:

- Which exact procedure and brain target are you proposing?
- Which one symptom is the primary treatment goal: tremor, stiffness, slowness, dystonia, dyskinesia, or “off” time?
- How do my diagnosis, symptom asymmetry, medication response, imaging, and medical history affect candidacy?
- Which of my symptoms are unlikely to improve with this target?
- Could treating one side leave me with a meaningful imbalance between sides?
- Is the treatment approved for my indication where I live, or is it being offered through a clinical study or registry?
- What current device labeling applies, and who can show me the relevant indication in writing?
Do not treat a general online checklist as a candidacy decision. The same diagnosis can lead to different recommendations depending on the dominant symptom, progression, priorities, and available alternatives.

What the evidence actually shows
Ask your neurologist to distinguish evidence for your proposed target from evidence for focused ultrasound in general. Small trials, selected participants, short follow-up, and different outcome measures limit how directly published results predict an individual result.
| Procedure and study | What was measured | Result or finding | Important limitation |
|---|---|---|---|
| Unilateral focused-ultrasound thalamotomy; double-blind, sham-controlled pilot trial in JAMA Neurology (2017) | Medication-refractory, tremor-dominant Parkinson’s disease; treated-hand tremor score at three months | Among 27 participants, median improvement was 62% after treatment versus 22% after sham | Small, early trial with short follow-up; the authors called efficacy preliminary and noted a substantial placebo response |
| Same thalamotomy pilot | Safety | Two treated participants (8%) developed mild hemiparesis related to internal-capsule heating early in the study | The investigators added temperature monitoring in another axis; this figure is study-specific, not a general complication rate |
| Unilateral focused-ultrasound subthalamotomy; sham-controlled trial in the New England Journal of Medicine (2020) | Selected people with markedly asymmetric Parkinson’s disease | Reported adverse events included speech and gait disturbances, weakness on the treated side, and dyskinesia | Results should not be generalized to bilateral or broadly distributed disease |
| Focused-ultrasound evidence reviewed by the 2022 EAN/MDS-ES guideline | Several targets and procedures | Evidence was considered limited; the guideline described one randomized controlled trial for unilateral subthalamotomy with one-year data | The assessment reflects evidence available in 2022 and may not match every later publication or authorization |
| Focused-ultrasound pallidal ablation trial in the New England Journal of Medicine (2023) | Target-specific pallidal treatment | Provides additional clinical evidence for pallidal ablation | Ask how its participants, target, outcome, and follow-up compare with your situation; the cited sources do not establish a personal success probability |
The 62% figure is therefore not a promise that your tremor will improve by 62%. It came from a small, unilateral, three-month comparison in tremor-dominant disease. Ask how many people were studied, how long they were followed, what “improvement” meant, and whether the study population resembled you.
Understand permanent risks before discussing benefits
The Parkinson’s Foundation lists headache, dizziness, nausea, balance or walking problems, numbness or tingling, weakness on one side, and speech or swallowing problems after focused ultrasound. Less common serious risks include stroke and seizure. Some gait or balance difficulties may persist.
Because the lesion is permanent, a side effect cannot be corrected by turning down a setting or removing an electrode. The Foundation notes that bilateral treatment carries a higher side-effect risk. Ask the treating center for rates specific to the proposed target, whether the rates include temporary and lasting problems, how complications are monitored, and what rehabilitation is available.

Questions about the lesion itself
- How large is the intended lesion, and how do you confirm its location during treatment?
- What structures are near the target, and what symptoms could result if heat spreads beyond it?
- What is your center’s rate of lasting gait, balance, speech, swallowing, sensory, or strength problems for this exact procedure?
- What happens if I develop a side effect during treatment?
- What options remain if my Parkinson’s progresses or the lesion does not provide enough benefit?
- Would you ever recommend treatment on the other side, and how would the additional risk be assessed?
Compare focused ultrasound with DBS and medication-based options
Your decision should match the symptom you most need to control and how much flexibility you want later. The sources support discussing deep brain stimulation (DBS), medication changes, and infusion or pump therapies as alternatives, but they do not establish a universal winner or an individualized cost comparison.
| Decision factor | Focused ultrasound lesion | DBS or medication-based advanced therapy |
|---|---|---|
| What is changed? | A permanent lesion in a selected brain target | DBS uses implanted electrodes and a programmable system; medication-based therapies alter drug delivery rather than creating a lesion |
| Adjustability | Cannot be tuned after the lesion is made | DBS settings can be adjusted; medication regimens and infusion settings can be changed, subject to their own limitations |
| Laterality | The proposed procedure may be unilateral; bilateral treatment has greater side-effect risk | Discuss whether the alternative can address both sides and what its own risks and maintenance involve |
| Follow-up demands | Ask about imaging, neurological checks, rehabilitation, and long-term monitoring | DBS requires programming and hardware follow-up; medication-based therapies require ongoing prescribing, device or pump management, and monitoring |
| Evidence for your situation | Depends heavily on target, symptom, study population, and local authorization | Ask for evidence that matches your symptoms, disease stage, goals, and tolerance for procedures or ongoing treatment |
Ask your neurologist: “If my priority is one dominant symptom, which option offers the best balance of expected benefit, reversibility or adjustability, risks, and follow-up for me?” Also ask which symptoms each option will not address.
Questions about the treatment day and recovery
- Medication plan: Ask which Parkinson’s medicines to take, hold, or change before and after treatment. Do not alter medication without instructions from your clinical team.
- Procedure format: Confirm how long you should expect to be in the MRI suite, whether you will remain awake throughout, and how the team will test movement or speech during treatment.
- Immediate observation: Ask how long you will be monitored afterward and which symptoms require urgent contact.
- Recovery: Ask when you can walk independently, drive, return to work, exercise, or resume normal activities.
- Rehabilitation: Find out whether physical, occupational, speech, or swallowing therapy is routinely offered or arranged if needed.
- Follow-up schedule: Get the dates and purposes of early and longer-term visits, including who manages medication adjustments.
Verify the center, authorization, and financial responsibility
Focused ultrasound requires MRI equipment, specialized treatment technology, and an experienced medical team. Ask how often the center performs your exact procedure and target, who makes the final eligibility decision, and whether a multidisciplinary conference reviews cases.

A procedure may have a device label in one setting while another target remains investigational or limited to a study. Ask the center to identify the current authorization and to explain whether your treatment is routine care, a registry, or a clinical trial.
Best Value
Request a written estimate of what insurance will cover and what you could owe. Ask who handles prior authorization, whether imaging and rehabilitation are billed separately, and whom to contact if a claim is denied. Have the insurer confirm coverage in writing rather than relying on a verbal estimate.
A practical appointment checklist
Take a care partner, bring an up-to-date medication list, and write down your three most disabling symptoms. During the visit, record the target, intended benefit, alternatives, and center-specific complication rates. Before consenting, make sure you can answer:
- What exactly will be treated?
- What improvement is realistic for my priority symptom?
- What will probably not improve?
- What are the lasting risks for this target at this center?
- What cannot be changed after treatment?
- What alternatives fit my goals and medical situation?
- What evidence and authorization support this recommendation?
- What are the medication, recovery, follow-up, and out-of-pocket requirements?
These questions do not determine eligibility on their own. They help you and your neurologist make a decision based on the exact symptom, target, evidence, risks, and priorities involved.
