Medication may be considered as one part of a weight-control program when lifestyle changes have not been enough. Your clinician should weigh your health conditions, other medicines, family medical history, likely benefit, possible side effects, and cost—not BMI alone. For each option under consideration, ask about:
- Expected benefit: What improvement is realistic, and how will we measure whether it is helping?
- Side effects and warnings: What side effects are common, and which serious symptoms need prompt medical attention?
- Dose and route: How is it taken or administered, how often, and how might the dose change?
- Interactions and contraindications: Could it conflict with my medicines, supplements, or health conditions?
- Monitoring: Will I need examinations, pregnancy tests, or laboratory tests, and how often?

For example, the FDA-approved Wegovy label revised in March 2026 includes a boxed warning about thyroid C-cell tumors and says to discontinue the medicine at least two months before a planned pregnancy when it is used for weight reduction. Qsymia’s FDA labeling calls for pregnancy testing before treatment and monthly during therapy for patients who can become pregnant, along with laboratory monitoring. These examples illustrate why you need the current instructions for the specific medicine being considered; they are not a substitute for your clinician’s advice.

NIDDK advises people who are pregnant or planning pregnancy not to take weight-management medicines and says they are not recommended while breastfeeding. If you are considering over-the-counter alli (orlistat), ask about its product-specific safety information. In a June 10, 2026 communication, the FDA described updated labeling warning of acute kidney injury and kidney stones, including symptoms that should prompt stopping the product and contacting a doctor. This warning concerns alli; do not assume it applies to every weight-management medicine. Read the FDA safety communication on alli.
What should I ask about compounded products?
If a compounded GLP-1 medicine is proposed, ask: “Who prescribed and dispensed this exact product? Is it FDA-approved for this use, and how are the product and dosing instructions verified?” A compounded product should not be described as FDA-approved. The FDA has raised concerns about unapproved compounded GLP-1 products, including dosing and administration issues. How will we know whether treatment is working?
Ask: “When will we reassess, what response should we expect, and what will we do if it is not helping or I cannot tolerate it?” Agree on what you and your clinician will track, when you will follow up, which symptoms warrant a call, and whether the plan would be to continue, adjust, switch, or stop.

NIDDK says treatment duration depends on whether the medicine helps and whether serious side effects occur; a clinician may recommend ongoing treatment when it is helping and tolerated. Its guidance says clinicians may reassess when a patient has not lost at least 5% of starting weight after 12 weeks on the full dose.
