What to Ask Your Doctor Before Starting a GLP-1 Weight-Loss Drug

Before starting a GLP-1 weight-loss drug, ask your doctor which specific medicine and formulation fits your health history and treatment goal, what risks or interactions apply to you, and how dosing and follow-up will work. “GLP-1” is not one interchangeable prescription: approved uses, warnings, and instructions differ by product. Bring a complete list of your medicines and supplements, and use the checklist below to guide the appointment.

Which medicine is being considered, and why?

Ask for the exact drug, formulation, and FDA-approved indication being considered for you. The phrase “GLP-1 weight-loss drug” can refer to products with different mechanisms and product-specific directions. For example, Wegovy is semaglutide and has FDA-approved injection and tablet formulations; Zepbound is tirzepatide, which acts on GIP and GLP-1 receptors. Their indications differ by product and patient group. Check the current Wegovy prescribing information and Zepbound prescribing information for the product under discussion.

  • Which indication applies to my circumstances, and what makes me eligible?
  • What is the treatment goal: weight management, a related health outcome, or another approved use?
  • What benefit is reasonable to expect for that goal, and which aspects of my health affect that expectation?

Do not assume that FDA approval of one medicine for an indication means every medicine in the class is approved for it. The FDA announced in March 2024 that Wegovy became the first weight-loss medication also approved to help prevent certain serious cardiovascular events in adults with cardiovascular disease and obesity or overweight. That is a product- and population-specific indication, not a class-wide approval. Read the FDA announcement.

What parts of my medical history matter?

Tell the prescriber about your own health history and relevant family history, including thyroid cancer syndromes, pancreatitis, gallbladder problems, kidney disease, diabetes-related eye disease, and severe allergies. Ask which concerns apply to the particular medicine, whether they change the decision, and what monitoring would be needed.

Some warnings are specific and important: Wegovy is contraindicated for people with a personal or family history of medullary thyroid carcinoma (MTC) or with multiple endocrine neoplasia syndrome type 2 (MEN 2). Its FDA label states: “WEGOVY is contraindicated in patients with a personal or family history of MTC or in patients with MEN 2.” Ask your clinician how the selected product’s current label applies to you; do not assume another product has identical contraindications or warnings.

Could it interact with my medicines or supplements?

Bring a current list of prescription medicines, over-the-counter medicines, and supplements, with doses if possible. In particular, tell your doctor if you use insulin or medicines that increase insulin release: combining these with certain medicines in this area can raise concerns about low blood sugar. These drugs can also delay gastric emptying, which may affect absorption of some oral medicines.

What side effects need a plan?

Ask what to expect, what you can do if symptoms occur, and which symptoms should prompt a call to the office, urgent care, or other immediate action. Nausea, diarrhea, vomiting, and constipation are among the common effects identified in FDA materials. Safety concerns include pancreatitis, gallbladder disease, and kidney injury related to volume depletion, such as when vomiting or diarrhea contributes to dehydration.

  • Which symptoms are expected, and how should I manage them?
  • What symptoms mean I should contact you promptly or seek urgent care?
  • Should I hold a dose while waiting for advice if a concerning symptom occurs?

Ask your clinician to distinguish advice for the exact medicine from general information about the drug class. FDA communication in February 2026 said it found no increased risk of suicidal behavior or ideation with GLP-1 receptor agonists and requested removal of that warning from affected labels. This updates the FDA position for affected drugs; it does not replace discussion of your personal mental-health concerns or the current labeling for the prescription being considered. See the FDA communication.

How will dosing, changes, and follow-up work?

Before the first dose, make sure you understand how the particular formulation is taken and how the prescriber plans to start and change the dose. Ask what to do if you miss a dose or cannot tolerate the current dose, and whom to contact with administration questions.

What if I may become pregnant, breastfeed, or need surgery?

Tell your clinician if you are pregnant, breastfeeding, planning a pregnancy, or could become pregnant, and ask how those circumstances affect the specific prescription. Do not carry timing instructions from one product over to another. For Wegovy, the current FDA label says to stop at least two months before a planned pregnancy when it is used for weight reduction or cardiovascular-risk reduction. Confirm the current label and your plan with your clinician. Wegovy prescribing information.

Also ask whether the medicine affects planning for surgery or anesthesia, and what the surgical team needs to know. Get instructions for your actual prescription rather than relying on a general rule for the whole class.

Is the product FDA-approved, and who will answer practical questions?

Ask whether the proposed medicine is an FDA-approved product and which pharmacy will dispense it. If someone offers compounded injectable semaglutide, ask who made and dispensed it, what the concentration and dose instructions are, and whom to contact to resolve administration questions. FDA has warned about dosing errors with compounded injectable semaglutide; compounded products should not be described as FDA-approved. Read the FDA alert.

Use this checklist to compare options

If your clinician discusses more than one medicine, compare the products on the same points rather than assuming they are interchangeable:
