Before ordering an aging or longevity biomarker test, ask four questions: what does it measure, has it been validated for people like you, does it predict a health outcome that matters, and what decision would change because of the result? A number labeled “biological age” is not a diagnosis, and current reviews find unresolved standardization and clinical-utility problems.
Use the appointment to define the test, its evidence, possible actions, follow-up, privacy terms and total cost. Do not postpone established screening or evaluation while waiting for an aging score.

Start with the decision, not the number
Tell your doctor what you hope to learn. “What is my biological age?” can mean very different things: curiosity about an epigenetic estimate, concern about a specific disease, interest in preventive care, or a desire to measure whether a lifestyle change is working. Those questions do not necessarily require the same test.

Ask: “What would we do differently if this result were high, low or inconclusive?” If no answer leads to a specific, evidence-supported action, the test may add anxiety or expense without improving care. A result should not replace routine prevention, diagnosis or treatment.

The 2024 Nature Medicine review describes aging biomarkers as promising predictors still under development, with unresolved validation and clinical translation (Moqri et al., “Validation of biomarkers of aging,” 2024). The 2023 Cell review likewise says that a lack of standards and consensus on reliable aging biomarkers hinders development and clinical use (“Biomarkers of aging for the identification and evaluation of longevity interventions,” 2023).
Clarify exactly what the test measures
“Longevity test” is a marketing category, not one standardized assay. Ask the clinician or laboratory to identify the specimen, signals and algorithm in plain language.
| Possible test description | Questions to ask |
|---|---|
| Known disease marker | Is this a clinically established marker for a particular condition? What diagnosis or management decision does it inform? |
| Panel of routine biomarkers | Which measurements are included, and are they being interpreted individually or combined into a new score? |
| Genetic variant or panel | Is the result an inherited risk signal, and what does it establish or fail to establish about disease? |
| Epigenetic signal | Which molecular measurements and reference population generate the age estimate? How stable is the estimate on repeat testing? |
| Proprietary composite score | Are the inputs, algorithm, comparison group and validation data available for independent review? |
A score expressed in years is an estimate produced by measurements and an algorithm. Its meaning depends on the comparison group and the outcome the developers studied; it does not automatically mean that your body is literally the stated number of years old.
Ask for the test’s intended use in writing. A tool developed to predict mortality in one research cohort may not be validated to guide an individual’s exercise plan, medication, cancer screening or insurance decisions.
Separate the three kinds of evidence
Use three distinct questions rather than accepting “clinically validated” as a complete answer.

1. Analytical validity: does it measure its target reliably?
Analytical validity concerns the assay itself: specimen collection, laboratory method, quality controls, calibration and repeatability. The FDA-NIH Biomarker Working Group explains that a test must measure what it was intended to measure (BEST Resource, “Validation”).
Ask who performs the assay, what controls are used, how samples are stored and shipped, and how much variation is expected if the same person tests again. A technically precise measurement can still be clinically unhelpful.
2. Clinical validity: does the result relate to the claimed health concept?
Clinical validity asks whether the biomarker predicts or measures the relevant clinical concept or outcome. Ask which outcome was studied—such as a diagnosed disease, functional decline or mortality—and over what time frame. Also ask whether the association was replicated in independent populations.
Request the population details: age range, sex, ancestry, health status, medications and geographic setting. A model trained in a narrow cohort may not generalize to you. The aging-biomarker review identifies comparability, generalizability and measurement variability as major challenges.
3. Clinical utility: does using it improve care?
Clinical utility is the hardest question: does acting on the result improve a meaningful outcome compared with usual care? The 2024 review says prospective clinical trials demonstrating improved patient outcomes are needed to establish utility for aging biomarkers. A test can be analytically reliable and statistically associated with an outcome without proving that ordering it helps patients.

Ask whether any intervention based on the score has been tested prospectively, or whether the evidence only shows correlation. If the proposed response is already recommended for everyone—such as adequate physical activity, blood-pressure control or age-appropriate screening—the score may not add a useful decision.

Validation is not portable by default. Ask:
- Which populations were used to develop and validate the test?
- Were the findings replicated by independent groups?
- What outcomes, and what follow-up period, were used?
- How do age, sex, ancestry, chronic conditions, medications, smoking, menopause, recent illness or exercise affect the estimate?
- How much change is larger than ordinary day-to-day or laboratory variation?
- What is the expected uncertainty around my individual result?
Ask the clinician to distinguish a meaningful change from noise. A later score that differs by a small amount may reflect specimen handling, normal biological fluctuation or a changed algorithm rather than a real alteration in aging biology. If the company updates its model, results from different dates may no longer be directly comparable; ask how version changes are disclosed.
Know what the result can—and cannot—say
Do not treat a biological-age number as a diagnosis, prognosis or proof that you are aging faster or slower. It may summarize selected signals, but it cannot by itself rule in or rule out disease.
For direct-to-consumer genetic testing, the American Heart Association states: “DTC-GT should not replace traditional screening and diagnostic methods for health risks” (AHA scientific statement, 2025). That statement concerns consumer genetic testing; oversight and clinical claims for a non-genetic aging test depend on its exact intended use, laboratory and sales model. Do not generalize one product’s regulatory status to every test.
Ask whether an unexpected result requires confirmation with a clinical assay. A concerning score may lead to unnecessary testing if it is not confirmed; a reassuring score may create false reassurance if it does not cover the disease you are worried about.

Use this appointment checklist
- Define the question. Explain the health concern or decision you want addressed.
- Name the measurement. Ask whether the test uses a disease marker, routine panel, genetic variant, epigenetic signal or proprietary composite.
- Identify the predicted outcome. Ask what the result is intended to predict, for whom and over what time period.
- Request validation details. Ask for analytical repeatability, independent clinical validation and performance in a population like yours.
- Map every possible result to an action. Write down what would happen after a high, low or inconclusive result.
- Compare with standard care. Ask whether an established screening test or evaluation answers your question more directly.
- Plan confirmation. Find out whether an abnormal result needs a clinical laboratory test or specialist review.
- Assign an interpreter. Confirm who will explain the result and arrange follow-up.
- Check oversight. Ask who performs the assay, what quality controls apply and what regulatory or laboratory oversight covers this exact test and sales model.
- Review data terms. Ask how specimens and data are stored, used, shared and deleted.
- Get the total price. Include the test, clinician visit, confirmation, specialist consultation and follow-up. Ask what insurance may cover.

Compare two tests by evidence, not by marketing
If you and your doctor are considering alternatives, score each option on the same dimensions.
| Comparison axis | What to establish | Warning sign |
|---|---|---|
| Measurement | Specimen, biological signals, laboratory method and algorithm transparency | “Biological age” is presented without naming inputs or methods |
| Validation | Analytical reliability, defined clinical outcome, replication and comparable populations | Only testimonials, correlations or an unnamed “clinical validation” claim |
| Clinical utility | Evidence that acting on the result improves meaningful outcomes | No specific decision changes, or no prospective outcome evidence |
| Actionability | High, low and indeterminate result pathways, including confirmation | A score is used to recommend treatment without a clinical diagnosis |
| Uncertainty | Repeat-test variability, confidence limits and effects of sample handling | Small score changes are treated as definitive |
| Practical burden | Collection, turnaround, clinician access, privacy and complete cost | Follow-up, data use or additional fees are unclear |
Current review literature does not establish a universal best aging biomarker, a common biological-age scale or a generally superior test class for routine care. A 2026 review notes that direct-to-consumer epigenetic age tests are sold online while a shared standard for interpreting consumer outputs remains absent (“An aging essential 8: closing the gap between geroscience and the public it serves,” accessed October 2026).

Interpretation and follow-up
Ask whether your primary-care clinician, a genetic counselor, a laboratory physician or another specialist will review the report. Clarify how quickly you will receive an explanation and what happens if the report is ambiguous. Do not make medication changes or abandon recommended screening on the basis of a consumer score.

Common failure modes and safer responses
The score sounds precise
Problem: A result such as “eight years younger” implies more certainty than the assay supports. Response: Ask for the uncertainty range, validation population and smallest change considered meaningful.
A correlation is presented as a treatment benefit
Problem: A biomarker associated with an outcome is marketed as proof that changing the score extends life. Response: Ask whether an intervention trial showed improved patient outcomes, not merely a changed biomarker.

A consumer result substitutes for screening
Problem: A reassuring score delays evaluation of symptoms or recommended screening. Response: Complete established preventive and diagnostic care independently of the aging result.
An unexpected result triggers a cascade
Problem: A nonstandard score leads to expensive or invasive tests without a defined pathway. Response: Confirm the result with an appropriate clinical test and discuss benefits, harms and alternatives before proceeding.
Different reports cannot be compared
Problem: Two companies, or two software versions from one company, use different inputs and reference groups. Response: Treat scores as test-specific estimates; do not track them as one universal biological-age scale.
When a conventional test is the better answer
If your concern is a symptom, a family history, blood pressure, diabetes risk, cancer, heart disease or functional decline, ask whether a validated diagnostic or preventive assessment addresses it directly. Routine screening and clinical evaluation have defined indications and follow-up pathways; an aging score may not.
Likewise, if your goal is to improve health, discuss interventions with established benefits—sleep, physical activity, nutrition, smoking cessation, vaccinations and management of diagnosed conditions—without waiting for a biological-age number. The score should add decision value, not become a substitute for care that already works.
