What to Ask Your Doctor After an Alzheimer’s Biomarker Test

A positive Alzheimer’s biomarker result is evidence of Alzheimer’s-related pathology, not a diagnosis by itself. At your appointment, ask exactly what was measured, which cutoff was used, how the result fits your symptoms and examination, whether another test is needed, and what it means for treatment and monitoring.

Bring the complete laboratory report—not just a portal summary—and ask your clinician to record the result category, assay name, reference range, limitations and next appointment. Blood, cerebrospinal-fluid (CSF) and amyloid-PET results are not interchangeable, and treatment eligibility depends on clinical stage, confirmed amyloid and current local labeling.

1. What did the test measure, and what category is my result?

Identify the analytes and assay

Ask the clinician to name every analyte reported, such as amyloid beta, phosphorylated tau (p-tau), or a ratio that combines them. The same word—“positive”—can mean different things on different platforms. Ask for the laboratory’s cutoff or reference interval, the units or ratio displayed on your report, the assay’s regulatory status, and the population in which it was validated.

A test may estimate the likelihood of amyloid pathology without measuring memory, independence or the many other causes of cognitive symptoms. The U.S. Food and Drug Administration (FDA) describes its Lumipulse review as an aid to clinical assessment, not a screening or stand-alone diagnostic test.

Understand negative, indeterminate and positive ranges

For the FDA-reviewed Lumipulse assay, the reported value is classified as follows:

Reported value FDA-reviewed category What that category can and cannot tell you
≤ 0.00370 Negative Consistent with a negative amyloid result on this assay; it does not explain every possible cause of cognitive change.
0.00371–0.00737 Indeterminate Does not provide a clear classification; your doctor may discuss CSF or amyloid PET if the answer would change care.
≥ 0.00738 Positive Indicates likely amyloid pathology, but does not establish an Alzheimer’s clinical diagnosis.

Those numerical boundaries apply only to that assay and its validated use; do not apply them to another laboratory’s test. In the FDA’s 2025 multicenter study of 499 participants, 91.7% of positive Lumipulse results matched amyloid pathology and 97.3% of negative results matched a negative PET or CSF result. Fewer than 20% of results were indeterminate. These are study findings, not a guarantee for an individual patient.

2. How does the biomarker fit my symptoms and daily function?

Ask your doctor to interpret the result alongside your symptom history, cognitive testing, neurological examination, medicines, medical conditions, family observations and brain imaging. The National Institute on Aging (NIA) states that blood results alone should not be used to diagnose dementia; they may be considered with other tests.

Be specific about when symptoms began, whether they are worsening, and which activities have changed. Examples include managing finances, driving, cooking, taking medicines, navigating familiar places, working or maintaining personal hygiene. A biomarker can support the presence of Alzheimer’s pathology while your current impairment is caused partly or entirely by something else.

Amyloid plaques can occur in people without symptoms, and amyloid can coexist with vascular disease or another neurodegenerative disorder. Ask, “What diagnosis best explains my pattern of symptoms today, and what evidence supports it?” That question keeps a pathology result from being mistaken for a complete clinical explanation.

3. Do I need a confirmatory test?

Ask whether a second test would change diagnostic confidence, treatment eligibility or another concrete decision. Confirmation is most useful when the first result is indeterminate, the clinical picture is unusual, the result conflicts with symptoms or imaging, or an anti-amyloid drug is being considered.

Blood, CSF and amyloid PET compared

Modality What it involves Access and practical issues Radiation Typical decision use
Blood biomarker Venous blood draw measuring an assay-specific amyloid and/or p-tau signal Least invasive and often easiest to obtain; performance and validated population vary by platform None Initial or supportive evidence when the assay is validated for the clinical situation
CSF Lumbar puncture measuring amyloid beta 42, tau and p-tau More invasive; availability, clinician expertise and patient preference matter None High-confidence biochemical confirmation when blood results or symptoms are discordant
Amyloid PET Intravenous tracer followed by a specialized PET scan Requires an imaging center, tracer scheduling and insurance review; cost varies by health system Yes, from the radiotracer Recommended when the result will directly affect diagnosis or care

Ask which specific CSF values or PET interpretation would count as positive, how long results take, and what you would do if the confirmatory test disagrees with the blood test. A positive result on one modality does not erase the need to interpret the entire clinical picture.

4. What other causes of my symptoms should we investigate?

Request a review of reversible, coexisting and alternative causes before attributing every symptom to Alzheimer’s disease. Depending on your history, your clinician may consider:

  • Thyroid, vitamin, electrolyte, liver, kidney or other metabolic problems.
  • Other neurodegenerative diseases, including disorders whose symptoms overlap with Alzheimer’s.

Ask whether you need an MRI, additional laboratory testing, sleep evaluation or a specialist assessment. NIA describes medical history, medication review, imaging and other tests as parts of a dementia evaluation; a biomarker is one component, not a replacement for that process.

5. Does this result make me eligible for treatment?

Clarify the disease stage

Ask whether your symptoms fit the early symptomatic stage for which anti-amyloid medicines are considered. A biomarker result in a person with no measurable symptoms is not the same treatment situation as a result in someone with mild cognitive impairment or mild dementia. Ask how your cognitive score and ability to function were classified and what stage the clinician is documenting.

Discuss lecanemab and donanemab realistically

NIA lists lecanemab and donanemab as FDA-approved immunotherapies for early Alzheimer’s disease. Amyloid confirmation is used before prescribing. Ask whether your current test is an accepted confirmation for the proposed medicine, or whether CSF or PET is required by the prescriber, health system or insurer.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Ask what benefit is realistic for someone with your baseline cognition and health, over what period it would be measured, and what the treatment schedule involves. Also ask about infusion-site logistics, missed doses, total monitoring visits and out-of-pocket costs in your country. Approval, coverage and availability can change, so verify the current requirements locally rather than relying on an older eligibility checklist.

6. What safety checks and monitoring will treatment require?

APOE ε4 testing

Ask whether APOE ε4 genetic testing is recommended before treatment and how the result will be protected and explained. The FDA Leqembi label says APOE testing should be performed before treatment to inform amyloid-related imaging abnormality (ARIA) risk. APOE status changes risk information; it does not diagnose Alzheimer’s disease or predict an individual’s exact response.

ARIA, bleeding risk and medicines

ARIA can include brain swelling (edema) and bleeding-related changes visible on MRI. Ask which MRI scans are required before the first dose and during treatment, the schedule your center follows, and who reviews each scan before an infusion. Tell the clinician about anticoagulants, antiplatelet medicines, bleeding disorders, prior brain hemorrhage and every supplement. Do not stop a prescribed medicine on your own; ask the prescribing team and the clinician who manages that medicine to coordinate.

Ask which symptoms require an urgent call or emergency evaluation. New severe headache, confusion, unusual sleepiness, dizziness, trouble walking, vision changes, weakness, seizures or other sudden neurological symptoms should be reported according to the emergency plan your treatment center gives you. Infusion reactions are another topic to cover: ask how they are prevented, what observation is provided afterward and what happens if a reaction occurs.

7. Could a laboratory notice or recall affect my result?

Ask which analyzer, assay version and lot number processed your sample, and whether the laboratory checked current FDA safety notices. In February 2026, the FDA posted a Class II recall involving specific Lumipulse lots because falsely elevated positive or indeterminate classifications could occur. A recall does not mean every result from every lot is wrong; it means the exact platform and lot need to be checked.

If your lot was affected, ask whether the laboratory recommends repeat testing, how a corrected report will be issued and whether any treatment decision made from the original result should be revisited. Keep both the original and amended reports in your medical record.

8. Leave the appointment with a written follow-up plan

Before you leave, ask the team to document the next action and the person responsible for it. Your checklist should include:

  • The analyte names, assay and result category, including the cutoff used.
  • Whether the result is sufficient for the current diagnostic or treatment decision.
  • Any CSF, PET, MRI, laboratory or specialist referral that is planned, and why it would change care.
  • The date for reassessing cognition, daily function and symptoms.
  • Who will call you with results and how to reach the clinic between visits.
  • Symptoms that warrant an earlier call or emergency care.
  • Whether referral to a dementia specialist, genetic counselor, social worker or support service would help.

How to prepare for the conversation

Bring a current medicine and supplement list, dates of previous scans and cognitive testing, a symptom timeline, and a trusted person who has observed day-to-day changes. Write down the laboratory’s exact wording and your most important decision—such as whether to pursue PET, seek a second opinion or discuss an anti-amyloid medicine. Ask for plain-language explanations and request a copy of every report.

The most useful outcome is not a single yes-or-no label. It is a documented explanation of what the biomarker shows, what it does not show, which alternatives remain, and what evidence the next decision will require.
