Before an AI system reviews mortgage files, define exactly what it will do, identify the laws and supervisory expectations that apply, demand task-specific evidence, test for unfair outcomes, assign human accountability, and contract for ongoing oversight. An AI tool that flags defects is not regulated identically to an automated valuation model (AVM) that informs a credit decision, so the diligence starts with a precise use-case map.
Start by defining the use case precisely
“AI for mortgage QC” can describe very different activities: extracting data from documents, finding missing conditions, checking underwriting calculations, reviewing appraisals, ranking files for manual review, recommending a disposition, or materially informing a credit decision. Regulatory obligations, validation needs and acceptable risk differ across those activities.

Document the task and its boundaries
Create a written intended-use statement before evaluating vendors or building a model. It should identify:
- The mortgage lifecycle stage: application, underwriting, closing, post-closing, servicing or secondary-market review.
- The products, channels, geographies, occupancy types and borrower or property populations in scope.
- Every input, including application fields, credit data, income documents, appraisal data, correspondence and metadata.
- The output: a defect flag, confidence score, queue priority, recommendation or decision input.
- Whether the output only prioritizes a human review, recommends a disposition, or makes or materially informs a credit decision.
- Which employee can override the result, who remains accountable, and which downstream systems receive the output.
- What happens when the model, an upstream data feed or the vendor service is unavailable.
Write down what the system is not allowed to do. A document-classification tool restricted to identifying missing pages has a narrower risk profile than an engine that recommends whether a loan should be approved. Do not let a broad marketing description substitute for this boundary.

Map the current control process
Draw the existing QC workflow, including prefunding checks, approval-condition reviews, underwriting and appraisal reviews, post-closing sampling, defect escalation and remediation. Fannie Mae’s Chapter D1-1, Lender Quality Control Process (page publication shown as October 7, 2026) organizes lender QC across programs and processes, staffing and outsourcing, prefunding review and post-closing activities. Its Chapter D1-3, Lender Post-Closing QC Mortgage Review (page publication shown as September 2, 2026) addresses reviews of approval conditions, underwriting decisions, data and documentation. These chapters describe operational contexts; they do not prescribe one human-review percentage for every AI deployment.
Identify which rules apply
Determine whether the system is a covered AVM
The interagency rule on Quality Control Standards for Automated Valuation Models applies to a defined use of an AVM by a covered mortgage originator or secondary-market issuer. The covered context involves estimating collateral value for a mortgage on a consumer’s principal dwelling when the estimate is used in certain credit decisions or securitization determinations. The rule became effective October 1, 2025.
For a covered AVM, the lender must have policies, practices, procedures and controls designed to ensure a high level of confidence in estimates, protect against data manipulation, seek to avoid conflicts of interest, require random sample testing and reviews, and comply with applicable nondiscrimination laws. Confirm with counsel and compliance whether your transaction, entity and use fall within the rule. Do not describe this AVM rule as a universal AI-in-mortgage-QC requirement: a different AI function may be governed by different laws and supervisory expectations.

If an output influences a credit decision, include the Equal Credit Opportunity Act (ECOA) and Regulation B in the assessment. The Consumer Financial Protection Bureau’s ECOA baseline review procedures, updated July 23, 2026, give examiners current context for identifying and analyzing ECOA risks. The CFPB’s mortgage origination examination procedures, updated December 22, 2021, include appraisal and underwriting modules that are relevant when an automated tool touches those processes.
Ask compliance to identify the exact decision points affected, the prohibited-basis risks, adverse-action obligations and recordkeeping requirements. A system used only to locate a missing signature may still create risk if its errors cause certain applicants’ files to be escalated or delayed disproportionately.

Use model-risk guidance as a control framework, not as a safe harbor
The Office of the Comptroller of the Currency’s Model Risk Management: Revised Guidance, Bulletin 2026-13 discusses model development and use, testing, validation and monitoring, governance and controls, and third-party products. It is supervisory guidance, not an enforceable or prescriptive technical standard. It expressly excludes generative and agentic AI from its scope. If your system uses those technologies, you still need a risk framework, but you cannot claim that the OCC document validates the deployment or supplies a complete rule set.
Demand evidence of accuracy and reliability
Request a complete evidence package
Require the vendor or internal development team to provide:
Do these 3 things before closing this tab:
1Clear out junk files and repair common Windows errors2Fix the driver behind crashes, sound loss and screen glitches3Repair Windows errors before they cause bigger problems- An intended-use description, model architecture at an appropriate level, version history and known limitations.
- Data lineage showing where each material input originated, how it was transformed and how long it is retained.
- Label definitions and adjudication rules for defects, exceptions, approvals and other outcomes used in training or testing.
- Test design, datasets, sampling methods, confidence intervals where used, results and error analysis.
- Performance broken out by relevant loan types, property types, channels, geographic areas and borrower or applicant segments.
- Procedures for investigating exceptions, corrupted or manipulated data, missing fields and out-of-distribution cases.
- Post-launch monitoring thresholds, alert owners, retraining or recalibration rules and rollback procedures.
For a covered AVM, evidence should specifically address the rule’s high-confidence requirement, data-manipulation protections and random sample testing and review. For other QC functions, set acceptance criteria for the actual task and its consequences. A document-extraction model might be judged on missed defects and false escalations; a prioritization model might be judged on how effectively it surfaces high-severity defects for finite reviewer capacity.
Reject universal accuracy claims
“AI is more accurate” is not a validation result. Neither is a single accuracy percentage with no definition of the label, population, review process or error cost. Ask what a false negative means in dollars, customer impact, compliance exposure and remediation effort. Set separate tolerances for missed defects, unnecessary escalations, unsupported recommendations and unavailable outputs. Require evidence that the chosen thresholds work on current data rather than only on a development sample.
Plan for drift and data quality
Mortgage products, documentation patterns, appraisal markets and fraud tactics change. Establish a baseline before launch and monitor input completeness, schema changes, population shifts, confidence distributions, defect rates and reviewer overrides. A sudden drop in flagged defects may indicate improvement, drift, a broken feed or manipulated input; it is not evidence of success by itself. Define who investigates, how quickly service is restricted and how prior decisions are assessed when a problem is found.

Test for unfair outcomes and make results explainable
Evaluate relevant populations and scenarios
Ask which populations and property scenarios appear in training, validation and ongoing evaluation. Examine error rates, escalation rates, confidence scores, processing times and recommendations across relevant borrower, geographic and property segments. Investigate both average performance and tail failures, such as sparse rural data, unusual income structures, limited-language documents or properties with few comparable sales.

A fairness review that cannot be reproduced later is difficult to defend to an examiner or auditor.
Connect explanations to the actual decision
Reviewers should be able to understand why a document was flagged, which data element triggered a concern and what evidence would resolve it. If the tool informs a credit decision, determine how the lender will identify and communicate the principal reasons for adverse action required by applicable law. Explanations should reflect the model’s actual drivers, not a generic list generated after the fact.
FHFA Advisory Bulletin 2022-02, Artificial Intelligence and Machine Learning (AI/ML) Risk Management, identifies fair-lending compliance, explainability, interpretability and adequate decision controls as risk areas. No single explanation technique guarantees legal compliance. Treat explainability as a combination of intelligible outputs, documented logic, reviewer training, traceable evidence and a process for challenging an incorrect result.
Assign human review, accountability and escalation
Design the reviewer’s role before launch
Specify which decisions remain with staff, who may override the system, what evidence an override requires and how disagreements are recorded. Define second-review triggers such as low confidence, conflicting data, a high-severity defect, an unfamiliar document type, suspected manipulation or a material customer complaint. Monitor staff overrides and discretionary patterns; human involvement does not automatically eliminate bias or model risk.

Set a clear escalation path
Route issues to named owners for compliance, model risk, operations, information security and vendor management. Establish service-level targets for:
- Investigating low-confidence or contradictory outputs.
- Quarantining a model version or data feed suspected of causing harmful results.
- Reviewing loans processed during a confirmed incident or drift event.
- Responding to borrower, broker or employee complaints about an automated flag.
- Approving a return to service after remediation and documented testing.
Maintain a manual fallback that can operate without the AI system. Test it periodically, including the staffing and turnaround assumptions, rather than discovering during an outage that no one knows which checks the tool had been performing.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.Scrutinize the vendor and the contract
Obtain operational and technical transparency
Ask for model and system documentation, validation evidence, data-use and retention terms, access controls, encryption details, subcontractor lists, incident-notification commitments and service-continuity arrangements. Confirm whether customer data are used to train a shared model, whether they can be deleted on request and where processing occurs. Require enough information for the lender to investigate a disputed output without relying solely on a vendor’s assurance.
Control changes throughout the relationship
Contracts should require advance notice of material model, feature, data-source, infrastructure or subcontractor changes. Define the lender’s rights to test a new version, defer deployment, obtain parallel-run results, audit relevant records and receive root-cause analyses after incidents. Preserve version identifiers with every output so an auditor can reconstruct which system produced it.

Compare competing systems or deployment approaches consistently
Use the same evidence requests and test protocol for each option. The following comparison axes synthesize the control concerns identified by the OCC, CFPB, FHFA and Fannie Mae; they are not a regulator-issued ranking.
| Comparison area | Questions to ask | Evidence to retain |
|---|---|---|
| Intended task and authority | What stage and products are covered? Does the tool flag, recommend or decide? | Use-case statement, workflow map and authority matrix |
| Performance and validation | How were labels, samples, segments and error costs defined? | Validation report, test data description and error analysis |
| Fair lending and explainability | Which groups and scenarios were evaluated? Can staff explain each material flag? | Segment results, mitigation log and explanation examples |
| Human review and auditability | Who overrides, what triggers escalation and can every action be reconstructed? | Override policy, escalation rules and immutable audit records |
| Integration, privacy and security | What data enter the system, where are they stored and what happens during an outage? | Data-flow diagram, retention terms, security assessment and fallback plan |
| Change management and exit | How are versions, subcontractors, incidents and termination handled? | Contract clauses, change notices, incident reports and export test |
| Ongoing operational burden | Who monitors drift, reviews alerts, retrains staff and pays the effort cost? | Operating procedures, staffing model and monitoring calendar |
Use a staged adoption process
1. Establish the baseline
Measure current defect categories, reviewer capacity, turnaround times, override rates and sampling methods. Preserve representative, legally usable records for testing. Without a baseline, a pilot cannot show whether the tool improves QC or merely changes what reviewers notice.
2. Run a controlled, non-decisional pilot
Start with a bounded population and keep qualified reviewers responsible for the official outcome. Compare AI findings with an independent review, record false negatives and false positives, and test unavailable-service procedures. Do not expand scope because a demonstration looks persuasive.
What’s actually slowing this PC down?
Pick the symptom - the matching free tool is one click away.
3. Complete independent validation and compliance review
Have model-risk, compliance, privacy, security and procurement reviewers challenge the evidence and assumptions. Resolve unexplained segment differences, weak labels, missing data and contract gaps before production. For an AVM use, confirm that every applicable control under the October 1, 2025 rule is addressed.
4. Approve, monitor and reapprove
Set launch conditions, named owners, alert thresholds and a review cadence. Reassess after material data, product, vendor or regulatory changes. Keep a decision log showing why the system remains within its intended use and what corrective action followed each exception.
Warning signs that should stop or narrow deployment
- The vendor cannot state exactly what the model is intended to do or which data trained it.
- Performance is reported only as a headline average, without relevant segments, labels or error analysis.
- There is no documented treatment for missing, corrupted, manipulated or out-of-distribution data.
- Staff cannot see why an item was flagged, challenge the result or record an override.
- The lender has no manual fallback, incident process, version history or way to review loans processed during an outage.
- Contract terms prohibit meaningful validation, audit access, change notice or data return.
- A sales claim treats the AVM rule as covering every mortgage AI use or promises that one explanation method solves fair-lending obligations.
The adoption decision should be tied to the documented task, evidence and controls—not to the novelty of the technology. If those elements cannot be demonstrated, narrow the scope, keep the system advisory or defer deployment until the gaps are resolved.
