What to Ask an IVF Clinic About Its Embryo-Lab Environment

Ask the clinic to explain, in plain language, how its embryology laboratory controls air, equipment, staffing, identification, cryostorage and emergencies. The most useful answers are specific and documented: who is responsible, what is measured, how often it is reviewed, what limits apply and what happens when a result is out of range. No single filter, certificate, machine or marketing label proves that a laboratory delivers better treatment outcomes.

The questions below draw on professional guidance from the American Society for Reproductive Medicine (ASRM) and the European Society of Human Reproduction and Embryology (ESHRE). They are prompts for a conversation with your clinic, not a substitute for a jurisdiction’s regulatory advice or an inspection.

Why the embryo laboratory deserves detailed questions

An IVF laboratory is a specialized clinical workspace, not simply a room containing incubators. ASRM describes a laboratory that requires deliberate design, trained personnel, suitable equipment and quality systems. Eggs, sperm and embryos may pass through several workstations and storage systems, so safety depends on the entire process remaining controlled and traceable.

Ask the clinic to describe its own procedures rather than assuming that a particular room design or technology guarantees success. The guidance reviewed does not provide an outcome statistic for an individual environmental feature. A well-run laboratory should be able to explain its responsibilities, records, escalation routes and contingency plans without promising that any one feature will increase your chance of pregnancy.

Laboratory leadership and staffing

Who is accountable?

Start with: “Who directs the embryology laboratory, and what qualifications and experience does that person have?” ASRM’s minimum-standards guidance assigns the laboratory director responsibility for operations, personnel, equipment, quality control and contingency planning. Ask whether the director is on site, how often they review performance and who has authority to stop work when a safety limit is breached.

Is staffing matched to workload?

Ask: “How is staffing matched to the laboratory’s workload, and who covers essential duties if a key staff member is absent?” A clinic should be able to describe the number and roles of qualified embryology staff, how competency is maintained and how coverage works during illness, holidays or unusually busy treatment days. ESHRE’s recommendations address laboratory organization, staffing and direction; ASRM calls for sufficient qualified personnel and continuity of operations.

Useful follow-up questions include:

  • Which staff members can perform each critical procedure, and how is competency documented?
  • Who reviews quality-control results when the director is unavailable?
  • What is the escalation path for an incident involving an egg, sperm sample or embryo?

Air quality, materials and room conditions

Ask about incoming air and volatile compounds

Embryos can be affected by the laboratory environment, so ask: “What controls are used for incoming air and volatile compounds, and how are those conditions monitored and recorded?” ASRM describes dedicated clean air with suitable filtration, low-volatile-organic-compound (VOC) materials and positive air pressure. ESHRE guidance also addresses HEPA filtration, VOC control, cleanroom materials and environmental monitoring.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Ask the clinic to distinguish between the equipment it has installed and the performance it verifies. A statement such as “we have a HEPA filter” does not tell you whether airflow, pressure, filter condition or VOC levels are checked routinely. Ask:

  • Is air supplied through a dedicated system, and what filtration stages are used?
  • How is positive pressure maintained relative to adjacent rooms?
  • Which building materials, paints, sealants and cleaning products are approved as low-VOC for the laboratory?
  • How often are pressure, particle or VOC measurements taken, and where are the results recorded?
  • What happens if monitoring shows a deterioration or an alarm?

Temperature and humidity

Ask: “What are the laboratory’s accepted ranges for room temperature and humidity, how often are they checked, and who reviews the readings?” These values should be part of routine environmental quality control. Request an explanation of the response to an out-of-range result, including whether work is paused, equipment is checked, a supervisor is notified and the event is documented.

Controlled access is another sensible question. ASRM describes restricted access as part of laboratory safety. Ask who may enter the embryology area, how visitors and contractors are supervised and how the clinic prevents construction, maintenance or cleaning work from introducing contaminants.

Daily quality control and corrective action

What is checked every day?

ASRM lists routine checks that include incubator gas concentrations and temperature, gas supply, heated surfaces, room temperature and humidity, and liquid-nitrogen tanks. Ask the clinic to show you, or describe, the daily checklist and the person responsible for signing it.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

A precise question is: “Which environmental and equipment readings are checked each day, what limits do you use, who reviews them and what is the documented response when a value is outside its limit?” The answer should cover both detection and action. A number written on a sheet is not a quality system unless someone evaluates it and follows a defined procedure when it is abnormal.

How are deviations handled?

Ask whether the laboratory records incidents, investigates causes, assesses any affected specimens and verifies that corrective action worked. You can ask for an anonymized example of the process, without requesting another patient’s confidential information:

  1. An alarm or measurement identifies a value outside the accepted range.
  2. A designated staff member confirms the reading and protects specimens from further exposure.
  3. The director or delegated quality lead is notified and decides whether procedures must stop or move to validated backup equipment.
  4. The event, decision, affected equipment and corrective work are recorded.
  5. The laboratory verifies recovery before returning the system to service and reviews whether preventive changes are needed.

Clinics may use different forms and thresholds. What matters is that the limits, responsibilities and escalation route are explicit and consistently documented.

Incubators, heated surfaces and other equipment

Ask: “How are incubators and other equipment maintained, calibrated and taken out of service if needed?” The clinic should be able to explain preventive maintenance, calibration schedules, service records and how it confirms that repaired equipment performs correctly before specimens are returned.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Follow up on equipment redundancy: “If an incubator, gas supply or heated surface fails, what validated backup is available, and how quickly can specimens be moved?” Ask whether backup power supports critical systems and how the laboratory tests it. ASRM’s laboratory design guidance includes backup power and operational planning, but the exact configuration varies by facility and jurisdiction.

Identity checks and traceability

Ask how specimens are identified at every handoff

Identity errors are a core patient-safety concern. ESHRE’s 2026 recommendations cover patient identification and traceability of reproductive cells. Ask: “How are identity checks and traceability handled at each step involving eggs, sperm and embryos?”

Request a walkthrough of the chain of custody from collection through fertilization, culture, biopsy or transfer, freezing, storage and later disposition. Clarify:

  • What identifiers are used on containers, worksheets and electronic records?
  • Are two-person checks, barcode systems or another independent verification used at critical handoffs?
  • How are witnessing steps recorded, and can the clinic reconstruct who handled a specimen and when?
  • What procedure applies if a label, record or patient detail does not match?

The clinic should protect your privacy while still explaining the control. You are asking how the system works, not for another patient’s information.

Cryostorage and emergency readiness

Liquid-nitrogen monitoring

Ask: “How are cryostorage tanks, liquid-nitrogen reserves and alarms monitored?” ASRM includes liquid-nitrogen tanks in routine quality control, while ESHRE addresses cryopreservation and emergency procedures. Ask how tank levels and temperature are checked, how alarms are transmitted, who receives them outside normal hours and how backup capacity is maintained.

Power, gas and equipment failures

Ask the clinic to describe its response to a power outage, gas-supply interruption, incubator failure, environmental alarm or cryostorage problem. Specific questions include:

  • Is there tested backup power for critical laboratory and monitoring systems?
  • Are spare gas supplies or alternate connections available, and how is their readiness verified?
  • Who is on call overnight and on weekends?
  • How are patients informed if an event could affect a treatment cycle?
  • When was the emergency plan last tested, reviewed or updated?

Emergency provisions vary by clinic and country. A credible answer identifies named roles, communication methods, backup locations or equipment and a documented decision process rather than simply saying that “the system is redundant.”

Time-lapse imaging and other technology claims

Ask: “Does the clinic use time-lapse imaging, and if so, how does it affect embryo assessment or decisions in my care?” ASRM’s 2022 committee opinion says time-lapse imaging has helped researchers understand early embryo development and can serve as an embryo selection or deselection tool, but it is not standard practice in assisted-reproduction laboratories because it has not been shown to improve pregnancy rates over standard embryo-grading techniques.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

That distinction matters. Ask whether the system is used for continuous observation, for selecting embryos, for staff training or for a research protocol, and whether using it changes your fees or treatment plan. Do not rank clinics simply by whether they own a time-lapse incubator; compare the evidence and the way the clinic explains its decisions.

Regulation, accreditation and quality systems

Ask: “Which regulator or accreditation applies to this clinic in this country, and can you explain your current status?” Request the name of the applicable authority, the scope of the accreditation or license and its current validity. Accreditation is one part of oversight, not proof that every process is perfect. The clinic should also be able to explain its internal quality-management system, document control, staff training, equipment records and incident review.

Because requirements differ by jurisdiction, avoid comparing a certificate from one country directly with a certificate from another. Ask each clinic the same question and record the exact answer and date.

A consistent comparison framework for two or more clinics

Use the same axes so that a polished presentation or a single piece of equipment does not dominate your decision.

Area What to ask each clinic Evidence of a useful answer
Leadership and staffing Who directs the lab? How is absence covered? Named responsibility, qualifications, competency records and a coverage plan.
Air and environment How are filtration, VOCs, pressure, temperature and humidity controlled? Defined limits, monitoring frequency, maintenance and alarm response.
Quality control What is checked daily and who reviews it? Signed or electronic logs, escalation rules and documented corrective action.
Equipment How are incubators and instruments calibrated and removed from service? Preventive-maintenance records, validated backups and return-to-service checks.
Identity and traceability How are eggs, sperm and embryos verified at every handoff? Independent checks, traceable records and a mismatch procedure.
Cryostorage and emergencies How are tanks, alarms, power and gas contingencies managed? Round-the-clock contacts, tested plans and backup capacity.
Technology claims What does time-lapse imaging do in this clinic? A clear clinical purpose without implying a proven outcome advantage.

How to prepare for the appointment

  1. Print or save the question list and leave space for the clinic’s exact answers.
  2. Ask for the laboratory director or a senior embryologist if the first contact cannot explain technical procedures.
  3. Request current, location-specific information; a parent company’s general description may not reflect the site where your specimens will be handled.
  4. Ask which records or summaries can be shared, such as accreditation status, quality-policy excerpts or an anonymized emergency workflow.
  5. Write down unanswered questions and ask who will respond, by what date and in what form.
  6. Compare clinics only after collecting answers on the same axes.

Answers that should prompt follow-up

  • “We have the latest technology” without explaining what it is used for or how it is validated.
  • “The air is clean” without describing filtration, pressure, VOC controls or monitoring.
  • “Our equipment is checked regularly” without a frequency, responsible person or record.
  • “We have a backup plan” without identifying backup power, gas, storage capacity or on-call coverage.
  • Inability to explain how identity is verified at handoffs or what happens after a mismatch.
  • Pressure to treat one feature, certificate or marketing label as a guarantee of success.

These responses do not prove that a clinic is unsafe, but they leave important questions unanswered. Ask for specific procedures and documentation, then discuss unresolved concerns with your fertility specialist or the relevant regulator in your jurisdiction.

What the evidence can—and cannot—tell you

ASRM and ESHRE guidance describes the controls a competent laboratory should consider, including design, staffing, air handling, daily quality checks, traceability, cryostorage and emergency planning. It does not establish the actual practices or performance of any named clinic. The available guidance also does not quantify how much a particular room feature changes an individual’s chance of pregnancy. Use it to make your questions more precise and to evaluate whether a clinic can explain and document its own system.
