No peer-reviewed randomized controlled trial under the exact title “Effectiveness of Vestibular Rehabilitation for Oculomotor Symptoms and Disorientation in Individuals With Cybersickness” has been verified. Related randomized studies provide a more limited answer: vestibular rehabilitation can improve selected dizziness, balance and visual-motion outcomes, and repeated virtual-reality exposure may reduce symptoms in some participants. They do not yet prove that one vestibular-rehabilitation protocol reliably treats cybersickness-related blurred vision, eye strain or disorientation, nor that a particular headset is superior.

What vestibular rehabilitation is—and what cybersickness adds

Vestibular rehabilitation is exercise-based therapy designed to help the brain compensate for inaccurate or conflicting signals from the inner ear, eyes and body. Programs may include gaze-stabilization exercises, balance work, walking tasks and controlled exposure to visual motion. Exercises are commonly customized to the person’s diagnosis, provoking movements and functional goals.

Cybersickness is a different but overlapping problem. A headset or moving visual scene can make the eyes signal motion while the vestibular system and body signal little or none. The resulting symptoms may include nausea, dizziness, visual blurring, oculomotor strain and disorientation. Evidence from people with chronic vestibular disorders, acute vertigo, persistent postural-perceptual dizziness (PPPD) or multiple sclerosis cannot automatically be generalized to otherwise healthy computer users whose main complaint is cybersickness.

What randomized studies have actually found

Study and population Intervention and comparator Main findings Important limits
Frontiers in Neurology, 2021; 47 patients (25 supervised-rehabilitation, 22 control) Six months of supervised vestibular rehabilitation versus control care Movement- and social-activity-related dizziness improved more with supervised rehabilitation. Overall subjective dizziness improved in both groups. The participants had a chronic vestibular disorder, not necessarily cybersickness. The result does not isolate oculomotor or disorientation outcomes.
Randomized pilot, 2017; 24 vestibular-rehabilitation participants and 23 matched head-mounted-device participants A four-week vestibular-rehabilitation protocol with a three-dimensional head-mounted gaming task was compared with rehabilitation without that task. Measures included vestibulo-ocular reflex (VOR) gain, posturography, Dizziness Handicap Inventory, Activities-specific Balance Confidence, anxiety, gait and the Simulator Sickness Questionnaire (SSQ). The head-mounted-device group showed overall post-treatment gains, and SSQ symptoms during home gaming declined over repeated sessions. The sample was small. It did not establish that a consumer headset, game or protocol is better than another.
Randomized comparison of VR-based therapy and customized vestibular physical therapy; sample size not stated Repeated virtual-reality sessions versus customized therapy Virtual-reality participants had reductions in dizziness, visual blurring, disorientation, nausea and oculomotor-stress ratings during repeated sessions. The findings support habituation as a possible mechanism, but they do not identify an optimal cybersickness treatment dose or show that the VR approach works for every cause of symptoms.
PPPD randomized trial, 2021 Customized vestibular exercise plus optokinetic stimulation delivered with mobile VR Three patients (10%) reported slight stimulation sickness on one SSQ item. This was a tolerability signal in PPPD, not proof that the intervention treats cybersickness in the general population.
Internet-based rehabilitation trials Online programs versus usual care in a chronic syndrome, and a later acute-vertigo comparison with written instructions Internet-based rehabilitation outperformed usual care in the chronic-syndrome trial. In acute vertigo, it was not superior to written instructions. Delivery method, diagnosis, timing and comparator changed the result; these studies should not be treated as interchangeable.

Why gaze stabilization and visual exposure might help

Gaze stabilization and VOR adaptation

Gaze-stabilization exercises ask a person to keep a target clear while the head moves. Repeated, controlled practice is intended to improve the vestibulo-ocular reflex, which coordinates eye movement with head movement. The 2017 pilot measured VOR gain and found improvement alongside changes in balance and self-reported disability, but it was not large enough to prove that VOR changes directly removed cybersickness.

Optokinetic and immersive exposure

Optokinetic stimulation and moving virtual scenes deliberately challenge the visual system. Gradual exposure may reduce the mismatch that triggers symptoms, explaining why dizziness, blurring, disorientation, nausea and oculomotor-stress ratings fell across repeated sessions in one randomized comparison. Exposure can also provoke symptoms, so intensity and progression matter.

Customization, supervision and dose

Results vary with the diagnosis, exercise selection, supervision, adherence and exposure dose. A therapist may alter target distance, head speed, visual complexity, stance or walking demands. The available trials do not establish a single number of sessions, headset setting or home schedule that is safe and effective for everyone.

How cybersickness should be measured

The Simulator Sickness Questionnaire is the clearest common monitoring tool in the cited protocols. It contains 16 symptom items divided into three domains:

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • Nausea: gastrointestinal and queasiness-related symptoms.
  • Oculomotor: eyestrain, blurred vision and related visual stress.
  • Disorientation: dizziness, spatial confusion and balance-related sensations.

In a 2021 immersive-VR protocol, SSQ scores of 10–15 were described as significant symptoms and scores above 20 as indicating a simulator problem. Those thresholds are monitoring conventions from that protocol, not universal diagnostic cutoffs. A useful assessment records scores before and after exposure, examines each domain rather than only a total score, and pairs SSQ results with falls, fatigue, adverse events, balance, gait and quality-of-life measures.

A safer way to use rehabilitation with VR

  1. Confirm the problem and diagnosis. A clinician should distinguish cybersickness from vestibular hypofunction, migraine, acute vertigo, PPPD, eye problems and medication effects.
  2. Record a baseline. Document visual blurring, eyestrain, dizziness, disorientation, nausea, balance confidence and relevant SSQ items before an exercise session.
  3. Start with a tailored exercise. Gaze stabilization, balance work or optokinetic exposure should match the person’s findings rather than relying on a generic game.
  4. Increase challenge gradually. Change one factor at a time—head speed, visual motion, stance, walking or session duration—so a clinician can identify what provokes symptoms.
  5. Recheck symptoms and function. Record SSQ domains, recovery time, falls or near-falls, fatigue and ability to perform everyday activities. A short-lived, planned increase may be acceptable; escalating or persistent symptoms require reassessment.
  6. Stop and seek clinical advice for warning signs. New neurologic symptoms, fainting, repeated vomiting, severe headache, falls or symptoms that do not settle are not routine training effects.

A VR headset can be optional hardware for immersive exercises, but the studies do not validate any particular brand or model. A headset should not replace assessment or therapist supervision when symptoms are severe, the diagnosis is uncertain or balance is unsafe.

What a definitive cybersickness trial still needs to answer

A well-designed trial would recruit people whose primary condition is cybersickness, define whether symptoms arise from immersive VR, non-immersive screens or another trigger, and compare a clearly specified vestibular-rehabilitation protocol with an appropriate control. It should report oculomotor and disorientation SSQ subscales separately, along with nausea, dizziness, visual clarity, balance, adherence, recovery time and adverse events. Follow-up would show whether improvement persists after exposure ends.

  • Please Note: To reach 10Gbps speed, keep the cable under 3.3 ft. For USB A Male to USB C adapters, try flipping the USB C connector. USB C Male to USB A adapters support bidirectional 10Gbps transfer within 3.3 ft

Until such a study is available, the strongest defensible conclusion is narrower: vestibular rehabilitation has randomized support for selected dizziness and balance outcomes, while repeated and supervised visual-motion exposure may reduce some cybersickness-like symptoms. The evidence does not establish a universally effective treatment for oculomotor symptoms and disorientation or a winning headset.
