Immunotherapy for bowel (colorectal) cancer can cause rash, diarrhoea and fatigue, but it can also make the immune system inflame healthy organs. New diarrhoea, abdominal pain, blood or mucus in stool, increased stoma output, breathing problems, jaundice, severe weakness or confusion should be reported promptly. Side effects can start during treatment or months or years after the last dose, so use the contact and emergency instructions supplied by your oncology team even when treatment has finished.
Which bowel-cancer immunotherapies are involved?
Checkpoint inhibitors help immune cells recognise and attack cancer. The National Cancer Institute (NCI) lists pembrolizumab, nivolumab and ipilimumab among immunotherapy drugs used for some colon cancers. An ipilimumab–nivolumab combination is used for some advanced colorectal cancers with high microsatellite instability (MSI-H) or deficient mismatch repair (dMMR). These examples do not mean every person with bowel cancer is eligible for, or receives, immunotherapy. The drug, combination, dose, treatment schedule and your other health conditions affect both benefit and risk.

Unlike a conventional chemotherapy reaction that is usually tied to a treatment day, checkpoint-inhibitor toxicity can appear at any point. An isolated symptom does not prove that immunotherapy is the cause: infection, the cancer itself, another medicine or a separate illness can look similar. The important action is to report a new or changing symptom so the team can assess it.

Common effects and immune-related inflammation
Rash, diarrhoea and tiredness are commonly reported with checkpoint inhibitors. Some people also have flu-like symptoms or a reaction where a medicine is injected. More significant problems happen when activated immune cells attack normal tissue. Inflammation can affect the bowel, lungs, liver, kidneys, heart, pancreas, hormone-producing glands, skin or nervous system.

| What you notice | What it could represent | What to do |
|---|---|---|
| Itchy or spreading rash, blistering, peeling skin, or sores in the mouth | Skin or mucosal inflammation; severity ranges from mild to serious | Contact the oncology team promptly, especially if it is painful, rapidly spreading or associated with fever or facial swelling |
| Loose stools, urgency, cramps, abdominal pain, mucus or blood | Colitis (immune-related bowel inflammation), although infection and other causes are also possible | Call the team promptly rather than waiting for a routine appointment |
| New cough, chest tightness or shortness of breath | Lung inflammation (pneumonitis) or another condition requiring assessment | Seek urgent advice; severe breathing difficulty requires emergency help |
| Yellow skin or eyes, dark urine, unusual nausea or upper-abdominal pain | Liver inflammation or another liver problem | Contact the team urgently |
| Marked weakness, dizziness, headache, confusion, tingling or numbness | Hormone-gland or nervous-system inflammation, among other possibilities | Report promptly; sudden or severe neurological symptoms are an emergency |
| Chest pain, palpitations, fainting or new swelling | Rare heart or other systemic complications | Use the emergency route supplied by your treatment centre |
This table is a warning guide, not a way to diagnose yourself. The care team may need examinations, blood tests, stool tests, scans or other checks before deciding whether a symptom is immune-related.
Diarrhoea and colitis: how many bowel movements are too many?
For someone receiving a checkpoint inhibitor, diarrhoea is not automatically a minor stomach upset. Cancer Research UK advises contacting the cancer team when you have two or more loose bowel movements than usual in 24 hours. That is an example patient-facing trigger, not a universal rule. Your treatment card or oncology service may tell you to call sooner, particularly if your normal bowel pattern is already frequent or you have a stoma.
Changes that need prompt advice
- A clear increase in frequency or watery stools compared with your personal baseline.
- New abdominal cramps or persistent abdominal pain.
- Mucus, red blood or black, tar-like stool.
- Stoma output that is higher, more watery or harder to replace with fluids than usual.
- Vomiting, dizziness, very little urine or an inability to drink enough to keep up with losses.
Colitis can worsen quickly. Do not wait for the next infusion or clinic appointment to mention it, even if you feel otherwise well. A clinician may need to rule out infection and assess dehydration or intestinal inflammation before recommending treatment.

Warning signs outside the bowel
Lungs
A new cough, breathlessness, chest discomfort or reduced ability to walk normally can signal lung inflammation. Tell the team promptly, even if symptoms are mild. Severe breathlessness, blue lips, inability to speak in full sentences or rapidly worsening chest symptoms call for emergency help.
Liver
Yellowing of the skin or whites of the eyes, dark urine, pale stools, unusual nausea, profound tiredness or pain beneath the ribs on the right side can accompany liver inflammation. Liver injury may develop before you feel very ill, which is why scheduled blood tests matter. Report new symptoms rather than waiting for the next blood test.
Hormone-producing glands
Inflammation of the thyroid, pituitary or adrenal glands can cause unusual tiredness, weight or temperature changes, dizziness, headaches, thirst, frequent urination or marked weakness. These symptoms are non-specific, but a new or worsening cluster deserves prompt assessment.

Nervous system
New numbness, tingling, muscle weakness, confusion, severe headache, visual change, trouble speaking or difficulty walking can have neurological causes. Sudden weakness, seizure, collapse or confusion is an emergency. Otherwise, contact the oncology service urgently.
Heart, kidneys, pancreas and skin
Chest pain, fainting, a racing or irregular heartbeat, new swelling, reduced urine, severe thirst, persistent upper-abdominal pain or repeated vomiting should be reported urgently. A rapidly spreading rash, blistering, skin peeling, mouth or eye sores, or facial swelling also needs urgent advice. These signs are uncommon but potentially serious.
Can side effects start after treatment ends?
Yes. Checkpoint-inhibitor effects can begin during treatment or after the final dose. Cancer Research UK notes that diarrhoea can arise months or years later. Tell every clinician who evaluates a new symptom that you have received immunotherapy, even if your cancer treatment ended long ago. Keep the drug name, treatment dates and oncology contact details with your medicines or medical records.

When to call your care team versus emergency services
| Situation | Best next step |
|---|---|
| New bowel-pattern change, abdominal pain, blood or mucus, higher stoma output, or difficulty maintaining fluids | Call the oncology advice line or care team promptly using the number on your treatment card |
| New cough or breathlessness, jaundice, severe weakness, numbness, confusion, chest symptoms, or symptoms that are clearly worsening | Seek urgent clinical advice; follow the centre’s instructions about whether to call the team first or go directly to emergency care |
| Severe or rapidly worsening breathing trouble, collapse, seizure, heavy bleeding, severe dehydration or sudden neurological change | Use the written emergency plan or your local emergency service immediately |
Contact arrangements and emergency numbers differ by country and treatment centre. This general guidance cannot replace the instructions from your own service. If you cannot reach the oncology team and symptoms are severe, do not keep waiting for a callback; use local emergency care.
What to tell the oncology team
- Give the exact immunotherapy name or combination and the date of the last dose.
- Describe when the symptom started, how quickly it changed and what your usual baseline is.
- For diarrhoea, report frequency, watery consistency, pain, fever, blood or mucus, and any stoma change.
- Explain whether you can drink and urinate normally, and mention dizziness or vomiting.
- List all medicines, supplements and recent antibiotics, including anything taken for diarrhoea or pain.
- Follow the team’s instructions about testing, clinic review, hospital assessment or changes to treatment.
NCI advises keeping the drug name and doctor or hospital contact information readily available. A photo of your treatment card in your phone can help when you are away from home.

- Do not assume diarrhoea is harmless because it started several days after an infusion.
- Do not start or stop steroids, anti-diarrhoeal medicines or other prescription treatment unless your oncology team tells you to. The correct approach depends on the severity and the cause, and infection may need to be excluded.
- Do not stop, delay or restart immunotherapy on your own. The treating clinician must make that decision.
- Do not conceal symptoms from other healthcare professionals. Previous immunotherapy can remain relevant to a diagnosis long after the last treatment.
When immune toxicity is confirmed, clinicians may use medicines to calm the overactive response. The NCI explains: “Sometimes these side effects are managed with medicines, such as steroids, that work to slow down an overactive immune response.” Whether that is appropriate, and how it is given, requires professional assessment.

Why early reporting matters
Mild symptoms can progress to dehydration, organ injury or hospital-level illness. Early contact gives the team a chance to check for infection or another cause, monitor organ function and treat inflammation before it becomes more difficult to control. Conversely, not every symptom is caused by immunotherapy, so reporting it does not mean treatment will automatically be stopped. It means the right clinician can weigh the symptom, your cancer, the drug or combination, the time since dosing and your overall condition.
A practical monitoring checklist
- Save the oncology advice-line and emergency numbers in your phone.
- Keep your immunotherapy name, last-dose date and treatment card accessible.
- Know your normal bowel pattern and, if applicable, normal stoma output.
- Check for new diarrhoea, pain, blood or mucus, cough, breathlessness, jaundice, unusual weakness, neurological changes or severe rash.
- Call at the first concerning change according to your centre’s instructions; do not wait for a routine visit.
- After treatment ends, continue to mention your immunotherapy history whenever a new symptom is assessed.
The safest rule is simple: any new or worsening symptom during or after bowel-cancer immunotherapy deserves a call, and severe or rapidly worsening symptoms require the emergency route provided by your care team or local emergency service.
