Security teams need an evidence trail that can answer five questions for every consequential agent action: which agent acted, whose authority it used, what it attempted, which policy or approval allowed it, and what happened. Build that trail around identity, delegation, action context, independent authorization, approval, outcome, and tamper-resistant handling. Do not assume that retaining every prompt is necessary or that one universal event schema or retention period already exists.
The evidence model: identity, authority, action, decision and result
An agent log is useful only when an investigator can reconstruct a decision and its effects. A model-generated explanation is not an authority record. The execution path should create its own security events and preserve the control decisions that governed the request.

Identify both the agent and the authority behind it
Record a durable agent identity for the software instance, workload, or deployment that made the request. Also record the human principal or calling service whose authority the agent is using. If the agent received delegated authority, preserve the delegation context: who granted it, what scope was granted, when it starts and expires, and any constraints attached to it.

This dual attribution matters because “the agent did it” is not enough for an access review, incident investigation, or non-repudiation claim. NIST’s National Cybersecurity Center of Excellence (NCCoE) identifies agent identification, authorization, auditing, and non-repudiation as relevant issues in its February 5, 2026 announcement about a concept paper on software-agent identity and authority.

Correlate the entire execution
Give each session, task, or workflow a correlation identifier. Reuse it across the planner, policy service, approval interface, tool gateway, downstream service, and result handler so an analyst can reconstruct events in sequence. A separate event identifier and a parent-event or step identifier help distinguish retries, parallel tool calls, and compensating actions.
No reviewed guidance prescribes a universal correlation-ID format. Treat the identifiers as an implementation choice, but make them stable enough to join records across agent and tool boundaries and searchable in the organization’s monitoring and incident-response systems.
Capture the action in a normalized form
Log the capability or tool invoked, the target resource, the operation, and normalized parameters. Normalization prevents semantically identical requests from appearing different because of formatting and makes approval matching possible. Store only the parameter elements needed to establish what was authorized; a raw prompt or complete payload can contain credentials, personal data, or retrieved confidential material.
A practical audit-event record
The following fields form a useful baseline. They are an implementation pattern, not a mandatory global schema.
| Event area | What to record | Why it matters |
|---|---|---|
| Time and linkage | Timestamp with an agreed time standard; event ID; session, task, workflow, and parent-event IDs | Places the action in order and links activity across components |
| Actor and authority | Agent identity and version or deployment reference; human principal or calling service; delegation or authorization reference | Shows which software acted and whose authority it used |
| Action | Tool or capability; target resource; operation; normalized parameters or a protected digest of them | Defines the exact request that was evaluated |
| Control state | Policy decision; requested and effective scope; privilege level; approval or denial; approver identity; step-up check; approval expiry | Explains why execution was permitted, refused, or escalated |
| Execution | Dispatch time; receiving service; retry or idempotency reference; execution status | Separates an approved proposal from what actually reached a system |
| Outcome | Allowed, denied, completed, partially completed, cancelled, or failed; result code; affected-object count or safe summary; error category | Establishes what happened without copying sensitive content |
| Protection metadata | Data classification; redaction status; integrity or signing metadata; retention class; access policy | Helps downstream systems handle the record safely and prove it was not altered |
Use protected references or cryptographic digests when investigators need to prove that parameters or an artifact were unchanged but do not need the content in the central log. Keep the verification material under a separate access policy.

Separate proposal, approval and execution
Record what the agent proposed
For a high-impact request, preserve a concise, normalized representation of the intended operation: for example, “disable account 1842,” “transfer ownership of repository X,” or “send message to customer group Y.” Include the target and parameters needed to distinguish that action from a broader or different one. Do not treat the agent’s natural-language rationale as a substitute for this record.
Bind approval to the exact action
OWASP’s AI Agent Security Cheat Sheet recommends binding an approval record to the actor, tool, target, normalized parameters, timestamp, and expiry. The approval record should also show whether the request was approved, denied, or sent for a stronger check, and identify the approver or approval service. A valid approval for one target or parameter set must not silently authorize another.

Validate independently at execution time
The service that executes the action should independently validate identity, scope, privilege, target, parameter constraints, and approval state. Do not rely on a model output that says an action is safe, and do not let a stale approval survive its expiry or a change in delegation. OWASP’s guidance says to fail closed if policy lookup, approval validation, or audit logging fails.
For inter-agent calls, OWASP states: “Authenticate communicating agents and enforce the sender’s permissions at the receiving service before executing a request.” The sentence addresses requests between agents: the receiving service still needs its own authorization decision and an event that records that decision.
Log outcomes, denials and control failures
Make the result unambiguous
Use a small, documented outcome vocabulary such as allowed, denied, completed, partially completed, cancelled, and failed. Store a safe result code and enough metadata to show the effect—for example, an object identifier or count—without duplicating a secret document or personal record. Distinguish “policy denied before dispatch” from “downstream system rejected after dispatch.”

Keep blocked attempts
Blocked and expired requests are valuable evidence of abuse, prompt injection, misconfiguration, or an overly broad policy. Record the attempted target and operation in a minimized form, the control that blocked it, and the identity involved. The reviewed guidance does not define a mandatory event taxonomy; this is a practical design choice consistent with approval validation and continuous monitoring.
Do these 3 things before closing this tab:
1Fix the driver behind crashes, sound loss and screen glitches2Repair Windows errors before they cause bigger problems3Scan for outdated or missing drivers - takes under a minuteRecord failures in the logging path
An event should show when an action could not be logged, when a policy service was unavailable, or when an approval could not be verified. Configure the execution path to stop rather than proceed without those controls. Alert on repeated logging or validation failures because an attacker could exploit a “fail open” gap or intentionally exhaust the audit pipeline.
Protect logs as sensitive security data
Minimize what is collected
Auditability does not require retaining every prompt, retrieved document, chain-of-thought-style reasoning trace, or full tool payload. Define the minimum content needed to substantiate identity, authority, action, decision, and outcome. Prefer structured summaries, references, or hashes over raw content when the latter adds exposure without improving accountability.
Redact secrets and personal data
Never place passwords, API keys, session tokens, private keys, or other credentials in plain-text logs. Scan and redact personal data where it is not necessary for the investigation, and classify fields that may contain confidential business information. OWASP recommends data-classification and retention policies and warns against logging personally identifiable information and credentials in plain text.
Control access and integrity
Restrict log reading and searching to roles that need it, separate routine operations access from security-investigation access, and log access to the logs themselves. Protect records against alteration with write-once or append-only controls, integrity checks, or signed events appropriate to the environment. Keep key material and verification services under a separate administrative boundary where practical.

Set retention from obligations and risk
There is no single retention duration that applies to every organization. Sector rules, jurisdiction, contracts, litigation requirements, and incident-response needs can change the answer. Define retention classes, deletion procedures, legal holds, and backup treatment with applicable legal or compliance advice. A longer period is not automatically safer if it creates unnecessary privacy and breach exposure.

Compare execution with authorization
For high-impact actions, review whether the executed target and normalized parameters exactly matched the approved record. Check that the acting identity was entitled to the requested scope at execution time, that the approval had not expired, and that no policy changed between approval and dispatch. Investigate any mismatch as a control failure even if the downstream result appears harmless.
Look for unexpected behavior
Use continuous monitoring to surface unusual tool sequences, access to new resource classes, repeated denials, privilege escalation attempts, unusually large data reads, or actions outside an established task pattern. Correlation IDs let analysts follow a suspicious sequence across the agent, tool gateway, identity provider, and target service.
Test the controls regularly
Exercise the complete path: valid approval, expired approval, changed parameters, revoked delegation, unavailable policy service, unavailable audit sink, replayed request, and a prompt-injection attempt that tries to redirect the agent. Verify that each case produces a useful event and that unsafe cases fail closed. Reassess after model, tool, identity, or policy changes.
Constrain autonomy and privilege
Grant agents only the tools, resources, data scope, and time window needed for a task. Separate read, propose, approve, and execute permissions where possible. Require human oversight or a step-up approval for destructive, financial, externally visible, or difficult-to-reverse operations. An approval screen should display the normalized target and parameters, not just the agent’s prose summary.
Best Value
CISA’s May 1, 2026 summary of the joint guidance “Careful Adoption of Agentic Artificial Intelligence (AI) Services” recommends constrained autonomy, strong identity management, layered defense, oversight, threat modeling, continuous monitoring, and regular security assessments. The NSA’s April 30, 2026 summary emphasizes explicit accountability, rigorous monitoring, human oversight, and incremental deployment because agent opacity creates accountability risk.
Implementation sequence for a security team
- Map authority. List every agent, deployment, human or service principal, delegation mechanism, tool, target system, and high-impact operation.
- Define the minimum event. Agree on identity, authority, correlation, normalized action, policy state, approval, outcome, and protection metadata before choosing a storage platform.
- Instrument the enforcement points. Emit events at request creation, policy evaluation, approval, dispatch, receiving-service authorization, completion, denial, and failure.
- Remove sensitive content by design. Classify fields, redact credentials, minimize personal data, and use references or digests for large payloads.
- Make controls fail closed. Block execution when authorization, approval, policy lookup, or required audit emission is unavailable.
- Centralize correlation and alerting. Route events to the organization’s monitoring workflow, preserve cross-service identifiers, and alert on anomalous sequences and logging gaps.
- Exercise recovery. Run tabletop and technical tests that reconstruct an action from the records, prove whether it was authorized, and identify the affected resources.
- Review and tune. Revisit privileges, retention, redaction, detection rules, and approval thresholds as agents, tools, threats, and obligations change.
How to evaluate a logging approach
When selecting or designing an implementation, compare approaches on the dimensions below rather than on a vendor label or an assumed standard.
| Evaluation axis | Questions to ask |
|---|---|
| Identity and delegation | Can the record distinguish agent identity, human or service authority, and delegation scope? |
| Event and outcome completeness | Does it capture proposals, policy decisions, approvals, execution, denials, failures, and safe outcomes? |
| Integrity and access | Are records protected from alteration, and are log readers and log administrators separately controlled? |
| Privacy and minimization | Can teams redact secrets, classify fields, and avoid retaining unnecessary prompts or payloads? |
| Cross-boundary correlation | Can analysts follow one task across agents, tools, identity systems, and target services? |
| Operational fit | Does the data feed existing monitoring, alerting, investigation, and incident-response workflows? |
Where standards stand in 2026
NIST announced its AI Agent Standards Initiative on February 17, 2026, with priorities that include standards, open protocols, and research in agent security and identity. It is an emerging initiative, not a finalized comprehensive logging standard.

These developments support designing for attributable, reviewable actions, but they do not establish one mandatory schema or retention rule for all deployments. Organizations should document their own event contract, control objectives, and evidence requirements while tracking applicable standards and sector obligations.
