Runtime governance is the set of controls that observe and constrain an AI agent while it is operating. It determines what the agent can access, which tools and actions it may use, when it must pause for a person, what evidence it records, and how an operator can intervene or stop it. For an always-on agent—one that runs persistently or repeatedly rather than only during a supervised session—those controls must work during execution, not just in a pre-deployment review or an occasional audit.
Why always-on agents need runtime governance
A policy document can say that an agent must not disclose confidential data or approve a high-value transaction. Runtime governance answers the operational question: what happens when the agent requests that action at 2 a.m., after receiving new context, calling several tools, or delegating part of the task to another agent?

The answer requires a live control plane. It identifies the agent and its operator, limits permissions, checks consequential actions, records the agent’s trajectory, detects risk signals, escalates uncertainty, and provides a practical way to pause, revoke access, contain damage, or stop execution. These controls complement—not replace—periodic risk assessments, design reviews, audits, and organizational accountability.

A continuously available customer-support agent, a scheduled infrastructure agent, and a workflow agent that repeatedly acts on incoming records may have very different risks and obligations. Governance should therefore be proportionate to the use case, authority, data, and potential impact.

The runtime governance control loop
A useful mental model is a loop that evaluates the agent at every consequential boundary rather than trusting a single approval at deployment.
- Inventory the actors and dependencies. Record each agent, its owner and operator, connected tools, external context sources, credentials, delegated agents, data stores, and environments. Include scheduled jobs and agents created dynamically by another agent.
- Define the allowed action space. Specify which data the agent may read, which tools it may call, which systems it may change, transaction limits, geographic or tenant boundaries, and actions that always require escalation. Express deny rules as well as allow rules.
- Grant least privilege. Use task-scoped identities and short-lived credentials where possible. Separate read, write, administrative, and financial authority. Make it possible to narrow or revoke permissions while the agent is running.
- Inspect execution requests. Place policy checks at consequential boundaries: before a tool call, external message, data export, code execution, privilege change, state transition, or handoff. A check that occurs only around the model prompt cannot govern what a tool later does.
- Validate context and results. Check the source, freshness, sensitivity, and integrity of inputs and tool results. Treat retrieved instructions and delegated-agent messages as untrusted until they pass the same policy and security controls as other external context.
- Record the trajectory. Retain enough structured evidence to reconstruct inputs, selected tools, tool results, policy decisions, delegations, state changes, approvals, failures, and operator interventions. Minimize or protect sensitive content in those records.
- Detect and classify signals. Monitor service health, behavioral anomalies, security events, policy violations, user-impact indicators, and compliance conditions. Set thresholds that reflect the risk of the use case rather than a generic uptime target.
- Escalate and intervene. Route uncertain or high-impact events to a named, competent human with the context and authority to decide. Provide pause, quarantine, credential revocation, rollback, and stop controls that work independently of the agent’s cooperation.
- Learn from incidents. Preserve an incident timeline, investigate the control that failed or was bypassed, update policies and tests, and feed relevant findings into the next lifecycle review.
The OWASP Agent Control Standard describes a related approach in which agent platforms expose middleware hooks and policies are enforced at runtime in a portable, declarative way. What to monitor while an agent is running
Monitoring is broader than uptime, latency, or whether a model’s answer looks plausible. NIST’s March 9, 2026 report groups post-deployment monitoring into six categories and describes it as essential to confident, widespread adoption: “post-deployment monitoring – from incident monitoring to field studies – is a crucial practice for confident, wide-spread AI adoption.” NIST’s report announcement

Check whether the agent still performs its intended task: completion rates, tool-call success, validation errors, goal drift, refusal or escalation rates, and regressions after model, prompt, tool, or policy changes. A system can be available yet functionally unsafe if it silently omits a required check or produces actions outside the task’s scope.
Operational performance
Track infrastructure consistency across regions, queues, workers, model endpoints, and dependency versions. Useful signals include latency, timeouts, retries, resource exhaustion, backlog growth, and differences between deployment environments. Correlate these records with the agent trajectory so an operator can distinguish a model decision from a failing dependency.

Human factors
Observe whether users understand that they are interacting with an agent, whether outputs are clear enough to review, how often people override or ignore recommendations, and whether escalations arrive with actionable context. Measure alert burden and false positives; an intervention path that produces constant noise will be ignored when it matters.
Security
Look for prompt injection, malicious tool results, credential misuse, unauthorized data access, unusual delegation chains, exfiltration attempts, and changes in action patterns. Security monitoring should cover the agent, its tools, protocols, identity layer, storage, and the surrounding infrastructure—not just the model endpoint.
Compliance
Check adherence to applicable laws, contractual duties, internal controls, retention rules, consent requirements, and sector guidance. Record which policy version made a decision and whether a required approval, notice, or human review occurred. The relevant obligation depends on the system, role, use, and jurisdiction.
Large-scale impacts
Examine effects beyond an individual transaction: disparate outcomes, concentration of errors, downstream decisions made from agent outputs, environmental or resource effects, and changes in user or workforce behavior. These impacts may emerge only after deployment and may require field studies or sampling rather than a single automated metric.
Execution boundaries that need explicit controls
Tool calls and external actions
Evaluate the target, parameters, data classification, requested scope, and expected side effects before execution. Apply transaction limits, destination allowlists, rate limits, and confirmation requirements where appropriate. Validate the result afterward; a tool that returns an error, unexpected schema, or instruction-like content should not automatically become trusted context.

Delegation and agent-to-agent handoffs
Pass authority explicitly. The receiving agent should know the task, permitted resources, expiry, and constraints rather than inheriting the full authority of its parent. Log who delegated what, to whom, with which credentials, and whether the handoff changed the risk level.
Do these 3 things before closing this tab:
1Clear out junk files and repair common Windows errors2Scan for outdated or missing drivers - takes under a minute3Repair Windows errors before they cause bigger problemsState changes and memory
Separate temporary working context from durable memory and business records. Require validation before an agent writes to a system of record, changes its own instructions, alters a policy, or persists information about a user. Make rollback or compensating actions possible for reversible changes.
Pause and stop mechanisms
Operators need controls outside the agent’s normal reasoning path: disable a tool, revoke a token, suspend a queue, quarantine an output, or terminate a run. Test these controls under load and during dependency failures. A theoretical kill switch is not governance if the responsible person cannot find it or lacks authority to use it.
Evidence, logs, and what they do not prove
Runtime observability supports accountability and investigation when records are complete, protected, and correlated. At minimum, capture timestamps, agent and operator identities, policy versions, input and output references, tool requests and results, delegation events, state changes, approvals, alerts, and intervention actions. Define retention, access, redaction, and integrity controls before an incident occurs.
These records show observable behavior and control decisions; they do not automatically reveal a faithful account of every internal model computation. OWASP’s agentic-security work emphasizes trajectory-level traceability. Prefer evidence of what the system received, selected, called, changed, and was allowed to do over treating generated chain-of-thought or a narrative explanation as proof of internal reasoning. Human oversight that can actually work
Human oversight is not synonymous with approving every action. Controls should match risk and preserve a realistic opportunity to intervene. A reviewer needs sufficient context, training, time, and authority; otherwise an approval button merely shifts responsibility without reducing risk.
- Define events that require approval, consultation, notification, or automatic containment.
- Assign an owner and backup for each escalation path, including outside normal business hours.
- Show the proposed action, affected data and systems, relevant policy, uncertainty or anomaly signals, and recent trajectory—not just a yes/no prompt.
- Test whether reviewers can reject, amend, pause, or revoke an action without asking the agent to police itself.
- Review overrides and missed interventions for training, policy, and interface problems.

A practical implementation sequence
- Map the deployment. Draw the agent’s data flows, tools, identities, queues, delegated workers, durable stores, and human touchpoints.
- Classify actions by consequence. Separate observation from modification, low-impact from high-impact changes, and reversible from irreversible operations.
- Write executable policies. Convert principles into conditions a middleware or platform hook can evaluate: identity, resource, purpose, scope, limits, expiry, approval, and required evidence.
- Place checks where authority is exercised. Cover tool calls, handoffs, state changes, exports, messages, and privilege changes. Test denied, malformed, replayed, and adversarial requests.
- Design the evidence model. Define event schemas, correlation IDs, retention, redaction, access roles, clock synchronization, and integrity protection.
- Build risk-based monitoring. Select signals from all six NIST categories, set thresholds by use case, and document which alerts are automated versus human-validated.
- Prepare response playbooks. Specify owners, severity levels, notification routes, containment steps, rollback or compensation, legal escalation, and evidence preservation.
- Exercise intervention. Run drills for credential compromise, prompt injection, runaway loops, harmful output, dependency failure, and operator absence. Measure time to detect, decide, contain, and recover.
- Review continuously. Reassess after model, prompt, tool, policy, data, or infrastructure changes; use incidents and near misses to update controls and tests.
How to evaluate a runtime-governance approach
Whether the implementation uses internal middleware, platform hooks, a policy engine, an observability service, or a managed offering, compare capabilities rather than product labels. The following axes are practical evaluation criteria, not a published vendor benchmark.
| Question | Evidence to request |
|---|---|
| Enforcement coverage | Which model, tool, delegation, data, state, and external-action boundaries can be observed or blocked? |
| Policy portability | Can rules move across agent frameworks, model providers, regions, and deployment environments without semantic drift? |
| Authority control | Can permissions be scoped, narrowed, expired, or revoked while a run is active? |
| Traceability | Can a reviewer reconstruct inputs, tool results, delegations, policy decisions, state changes, and interventions? |
| Response capability | Are alerts, escalation, pause, quarantine, stop, rollback, and incident workflows available and tested? |
| Monitoring breadth | Does coverage include functionality, operations, human factors, security, compliance, and large-scale impacts? |
| Human usability | Do operators receive enough context and authority, with controls against alert overload? |
| Evidence integration | What is retained, for how long, under whose access policy, and how does it connect to existing security, risk, and compliance processes? |
Known gaps and failure modes
Assuming telemetry equals governance
Logs make activity visible but do not define acceptable behavior, assign responsibility, or guarantee that anyone can intervene. Pair telemetry with policies, thresholds, owners, and tested response procedures.
Monitoring only uptime and model quality
An agent can be fast and accurate on a benchmark while leaking data, bypassing a control through a tool, confusing users, or creating harmful downstream effects. Use the six monitoring categories instead of a single health dashboard.

Fragmented evidence
Agent traces often span model gateways, tools, identity systems, queues, and business applications. Without shared correlation identifiers and synchronized timestamps, investigators cannot reliably reconstruct the sequence. Design the evidence model across system boundaries.

Static permissions for dynamic work
Long-lived, broad credentials turn a prompt injection or compromised dependency into a large incident. Scope authority to the task, enforce expiry and limits, and test revocation during execution.
Human review without human control
Routing every event to a person can create delay and alert fatigue, while routing none leaves consequential decisions unattended. Set risk-based thresholds and verify that reviewers have competence, context, and a working intervention mechanism.
Inventing a universal monitoring cadence
NIST identifies unresolved questions about how cadence should depend on risk and use case, how to reduce monitoring burden, and how automated monitoring should combine with human validation. Document the rationale for your cadence, then revise it as evidence accumulates rather than presenting it as a universal rule.

Runtime governance turns broad principles into controls at the moment an always-on agent receives context, chooses a tool, delegates work, changes state, or takes a consequential action. A defensible deployment can show who had authority, what policy applied, what the agent did, what was recorded, which signals triggered escalation, and how an authorized person could stop or contain it. Frameworks can organize that work, technical standards can make enforcement portable, and law can impose specific duties—but none removes the need to design and test live controls for the particular agent and use case.
