What Retinal Imaging Costs, What Insurance May Cover, and What to Ask Before Booking

There is no single price for retinal imaging. The cost depends on the test—such as a retinal photograph or OCT—why it is being done, the clinic’s fee, how it is billed, and your insurance plan. Published prices of $39–$65 apply to optional optomap wellness-screening images, not every retinal test. Before booking, ask for the test name and billing code, the reason for it, whether it is optional screening or diagnostic care, and a written estimate of what you may owe.

What does “retinal imaging” mean?

“Retinal imaging” is an umbrella term, not one standardized service. Fundus photography uses a retinal camera to photograph structures at the back of the eye. Optical coherence tomography (OCT) creates a different kind of image and may be coded separately depending on whether it examines the retina, optic nerve, or anterior segment. The right test depends on the clinical question.

CMS describes fundus photography as a way to document retinal abnormalities or follow eye disease. In its local coverage determination for the policy’s jurisdiction, effective for services on or after April 1, 2026, CMS says the photographs must be used in medical decision-making. The policy includes conditions such as macular degeneration and diabetic retinopathy, but excludes screening fundus photography from coverage under that local policy. It also says fundus photographs do not replace an annual dilated examination by a qualified professional. This is a local coverage policy, not a universal rule for every Medicare location. Read CMS local coverage determination L33567.

Commercial coverage can differ too. For example, Aetna’s policy discusses medical necessity and whether results may influence management, and lists distinct OCT categories for the anterior segment, optic nerve, and retina. That policy is not evidence of how every insurer or plan will decide a claim. Review Aetna’s OCT coverage policy.

How much does retinal imaging cost without insurance?

The available posted prices are examples, not a national cash-price average. Compare them only when the test and billing arrangement are similar.

Published amount What it applies to Important qualification
$39–$65 Optomap wellness-screening imaging Optos describes this as a typical fee range in a 2026 billing FAQ accessed October 4, 2026. The fee can depend on complexity, whether OCT screening images are included, and local market dynamics. It is vendor guidance for wellness screening, not a national survey or diagnostic-care quote. Optos says fees for noncovered services are at the practice’s discretion. Optos billing FAQ.
$39 Optional optomap add-on to an eye exam Newton Advanced Eye Services in Massachusetts posted this price in July 2026 and says screening is typically not covered by insurance. It is one clinic’s fee, not a national estimate. Newton Advanced Eye Services optomap information.
Not to exceed $39 Optomap offer for eligible VSP members VSP advertises this offer for members with eligible plans. Check eligibility and plan terms; it does not apply automatically to every VSP member or other vision plans. VSP retinal screening offer.

These prices are for optional screening examples. They should not be used to estimate a diagnostic OCT, another imaging modality, or a separately billed interpretation. Ask the clinic for its exact self-pay price for the service being offered.

Does insurance cover retinal imaging?

It depends on the test, the medical reason, the payer’s policy, and your specific plan. Routine vision benefits and medical insurance are different routes: an office may offer optional screening as a self-pay add-on, while imaging ordered to assess or manage a documented medical problem may be submitted to medical insurance if the payer’s rules and plan requirements are met.

For the jurisdiction covered by CMS local coverage determination L33567, screening fundus photography is not covered under that policy; medically relevant documentation and use in decision-making are required for covered fundus photography. Other local Medicare policies, commercial insurers, and individual plans may differ. A clinician’s recommendation alone does not guarantee coverage, and having no symptoms alone does not establish that a claim will be denied.

If Medicare applies, Medicare.gov says patients generally pay 20% of the Medicare-approved amount after the Part B deductible for covered diagnostic non-laboratory tests performed in a physician’s office or independent diagnostic testing facility. Outpatient hospital testing may also involve a hospital copayment. This is general cost-sharing guidance, not a promise that a particular retinal service is covered or a quote for your bill. Other insurance, the provider’s charge, whether the provider accepts assignment, facility type, and location can affect the amount. See Medicare’s diagnostic non-laboratory tests coverage page.

What should you ask before booking?

Ask the office for specific details before agreeing to an optional add-on or scheduling a test. Medicare.gov also advises patients to ask how much a test will cost, since recommended services may not be covered.

  1. What exact test is planned? Ask whether it is fundus photography, ultra-widefield imaging, OCT, OCT angiography, or a combination.
  2. Is this optional screening or diagnostic care? Ask for the clinical reason and, if it is meant to assess or monitor a condition, how that indication will be documented.
  3. What code will be billed, and to whom? Ask whether the office will submit the claim to your vision plan, medical insurance, both, or neither.
  4. Are the clinician and facility in network? If Medicare applies, ask whether the provider accepts assignment. Check whether prior authorization is required.
  5. What is my likely cost? Request the estimated allowed amount and your expected deductible, copay, or coinsurance. Also ask for the exact self-pay fee if the claim is denied or the service is not covered.
  6. What does the quoted fee include? Confirm whether image interpretation or a report and the eye examination are included or billed separately. CMS’s cited LCD requires interpretation/report documentation for covered fundus photography.
  7. Does imaging replace any part of the eye examination? Ask whether a dilated examination may still be needed; fundus photographs are not a substitute for the annual dilated examination described in the cited CMS policy.
  8. Can I decline an optional add-on? If so, ask what examination the clinician will perform instead.

Ask for the estimate in writing, including the test name, code, billing route, and what happens if insurance does not pay. Medicare.gov’s guidance is to ask the provider how much the test, item, or service will cost: Medicare.gov cost guidance.
