What Regenerative Medicine Can and Cannot Treat: A Patient Guide to Evidence and Risks

Short answer: “Regenerative medicine” is not a single treatment. In the United States, FDA approval applies to a particular product for a particular condition—not to every use a clinic associates with the words stem cell, exosome, cord blood, or regenerative. FDA says the approved stem-cell products covered by its consumer alert are blood-forming cells from umbilical cord blood for disorders affecting blood production. Other regenerative products, including certain engineered tissues, cell therapies, and gene therapies, have narrow, product-specific indications. Many heavily advertised uses—such as injections for arthritis, back pain, neurologic disease, chronic pain, autism, or fatigue—are not FDA-approved stem-cell uses.

The safest way to evaluate an offer is to identify the exact product and indication, verify the regulator’s record, distinguish an approved treatment from an investigational trial, and review risks and follow-up with the relevant specialist.

What “regenerative medicine” includes

FDA defines regenerative medicine as an approach intended to restore, replace, or recreate cells, tissues, or organs to treat or mitigate disease. The category includes cell therapies, therapeutic tissue engineering, combination products, some gene therapies, and human cell and tissue products. These modalities differ in how they are made, delivered, monitored, and regulated.

That breadth is why a field label is not enough to judge a treatment. “Stem cell therapy” might refer to hematopoietic progenitor cells used in blood disorders, an experimental cell preparation in a clinical trial, or an unapproved commercial injection. The cell source, manufacturing process, dose, route, target condition, and eligibility criteria all affect whether evidence applies to a particular patient.

The International Society for Stem Cell Research (ISSCR) puts the distinction plainly: “Stem cell treatments are not one-size-fits-all.” A result for one cell type or disease cannot be transferred automatically to a different condition.

What is FDA-approved in the United States?

Approval is tied to the named product, labeled indication, manufacturing controls, and the patients described in that label. It does not authorize a clinic to promote the same product for unrelated diseases.

Example or category What the federal record establishes What it does not establish
Hematopoietic progenitor cells from umbilical cord blood FDA’s consumer alert identifies these as the approved stem-cell products covered by the alert, for disorders affecting blood production. Approval for arthritis, pain, neurologic disease, anti-aging, or other conditions.
Zevaskyn Listed by FDA’s CBER Regenerative Medicine Advanced Therapy (RMAT) approvals as a product for wounds associated with recessive dystrophic epidermolysis bullosa. A general wound-healing or cosmetic indication, or use in people who do not meet the product’s eligibility requirements.
Symvess Listed in the RMAT approvals as a vascular-conduit use after specified extremity arterial injuries when an autologous vein graft is not feasible. Routine treatment for every vascular condition or a substitute for the labeled surgical circumstances.
Other cell and gene therapies FDA’s RMAT list, dated “Data as of September 30, 2025,” includes products for specific cancers and inherited conditions. A broad license to market those products for wellness, unrelated diseases, or general tissue repair.

The RMAT designation or listing is not itself a universal endorsement of regenerative medicine. Patients must check the current FDA label and whether their diagnosis, disease stage, prior treatment, age, and other criteria match it. If a clinic cannot provide the exact product name and approved indication, you cannot assume the treatment is approved.

Uses FDA says are not approved for the stem-cell products in its consumer alert

FDA’s July 22, 2020 consumer alert says the stem-cell products addressed there are not approved for the following uses:

  • Orthopedic conditions, including osteoarthritis, tendonitis, disc disease, tennis elbow, and back, hip, knee, neck, or shoulder pain.
  • Neurologic disorders, including multiple sclerosis, amyotrophic lateral sclerosis (ALS), Alzheimer’s disease, Parkinson’s disease, epilepsy, and stroke.
  • Cardiovascular or pulmonary disease.
  • Autism.
  • Macular degeneration or blindness.
  • Chronic pain or fatigue.

The same alert says there are no FDA-approved exosome products. This status statement concerns the product classes and uses described by FDA in the United States. It does not mean legitimate scientific work has stopped, and it does not turn participation in a trial into proof that a treatment works.

A clinic’s statement that its product “treats” a condition is therefore only a marketing claim until the exact product and indication can be matched to a regulator’s approval record. A different country may have different rules; ask which jurisdiction issued the authorization and whether that authorization covers your condition.

Why a clinical trial is not the same as an approved treatment

A clinical trial tests a hypothesis under a protocol. Investigators may be studying safety, dosing, immune reactions, durability, or a possible benefit that has not yet been demonstrated. Trials can show that a treatment is ineffective or unsafe, and many candidates never reach approval.

For a U.S. investigational product, ask whether the study is conducted under an FDA-issued Investigational New Drug (IND) authorization when one is required. Request the trial identifier, the consent form, the sponsor’s contact information, and a plain-language explanation of monitoring and follow-up. An entry on ClinicalTrials.gov is useful for locating a study, but a listing alone does not prove that FDA has approved the product or that it may be sold.

Likewise, a company’s FDA registration or listing is not marketing approval. Registration identifies a facility or business in a regulatory system; it does not verify the quality, safety, potency, or effectiveness of every product that business offers.

ISSCR summarizes the evidence threshold this way: “Stem cell treatments must be evaluated for efficacy and safety in regulated clinical trials before they receive approval.”

A practical verification process before you agree to treatment

  1. Write down the exact intervention. Get the product’s full name, cell or gene type, source tissue, dose, route of administration, and whether it is manipulated or combined with another device or drug. “Your own cells,” “umbilical,” “placental,” “exosome,” and “regenerative” are descriptions, not sufficient product identities.
  2. Define the intended use. Ask the clinician to state the diagnosis and treatment goal in writing. Confirm whether the proposed use is on the product’s FDA label or is investigational. Approval for one condition does not carry over to another.
  3. Verify the regulator’s record yourself. Use FDA’s product labeling and approval databases, and ask which federal or state authority granted authorization. If the provider cites an RMAT designation, clarify that RMAT is a regulatory pathway and designation, not a blanket approval for all uses.
  4. If it is a trial, obtain the protocol details. Request the ClinicalTrials.gov identifier, the IND information supplied to participants when applicable, eligibility criteria, randomization or control procedures, visit schedule, laboratory monitoring, and the plan for long-term follow-up.
  5. Examine human evidence for this exact product and condition. Look for controlled human data rather than testimonials, before-and-after photographs, animal studies, or results from a different cell type. Ask how outcomes were measured, how long patients were followed, and what adverse events occurred. If no controlled evidence is available, treat the claimed benefit as unproven.
  6. Compare alternatives. Ask your treating specialist what standard treatments are available, what their expected benefits and risks are, and what could be lost by delaying them. A proposed injection should not quietly replace a therapy with established benefit.
  7. Clarify cost and responsibility. Determine every charge, whether insurance pays, who owns the product, who handles complications, and where emergency care would occur. Paying a clinic does not make an investigational intervention an approved treatment.
  8. Get an independent opinion. Bring the consent form, product information, and protocol to a physician who is not financially involved in the clinic. A specialist familiar with your diagnosis can judge whether the proposed biology and risk are plausible for your case.

Potential harms and why manufacturing matters

FDA warns that illegally marketed regenerative products can cause serious harm. Possible problems include:

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • Blindness or other vision loss.
  • Tumor formation or unwanted cell growth.
  • Neurologic events.
  • Bacterial infection, including life-threatening bloodstream infection.
  • Severe reactions at the collection or injection site.
  • Unwanted inflammatory or immune responses.
  • Cells migrating to, or growing in, unintended tissues.
  • Contamination during collection, processing, storage, or administration.
  • Failure to work while an effective approved treatment is delayed or abandoned.

These hazards do not occur at the same frequency in every therapy. Risk depends on the product, processing, route, sterility controls, patient’s immune status, and the quality of clinical monitoring. A laboratory label or a claim that cells are “natural” does not remove those risks.

In a 2025 warning about unapproved human-cell and human-tissue products marketed online, FDA described adverse-event reports that included deaths. The warning discussed a reported death involving an imported placental-tissue product called Laennec. FDA said direct testing of the product administered to that patient was not feasible, and testing of remaining samples did not isolate the pathogen associated with the death. That account should not be rewritten as proof that the product caused the death or that a pathogen was found in the administered dose. FDA’s broader point was that the products had not undergone verified review of quality, safety, purity, or potency.

Warning signs of an unsafe or misleading offer

  • One product for many unrelated diseases: ISSCR cautions against clinics presenting one stem-cell treatment as suitable for conditions affecting different organs and biological systems.
  • Testimonials instead of controlled evidence: Individual stories cannot show how many people failed to improve, how outcomes were measured, or whether improvement came from another treatment.
  • Pressure to pay immediately: High fees, time-limited discounts, or demands for travel before you receive a complete consent document are reasons to pause.
  • Vague product language: Refusal to identify the cell source, handling process, dose, or responsible manufacturer prevents meaningful safety evaluation.
  • Regulatory name-dropping: “FDA registered,” “FDA listed,” RMAT designation, or a ClinicalTrials.gov page is presented as if it were product approval.
  • No plan for complications: A legitimate study or treatment should identify emergency contacts, adverse-event reporting, monitoring visits, and long-term follow-up.
  • Promises of certainty: Claims of guaranteed regeneration, a cure, or no side effects are inconsistent with responsible clinical evidence.

How to compare an approved treatment, a trial, and a commercial clinic offer

Question FDA-approved treatment Authorized clinical trial Unverified commercial offer
What is authorized? A named product for a labeled condition and eligible population. A protocol to investigate a product or use; effectiveness may be unknown. Often only a business or facility registration, not authorization to market the treatment.
Evidence expectation FDA has reviewed evidence required for the approval pathway. Evidence is being collected; results may be positive, negative, or inconclusive. May rely mainly on testimonials, uncontrolled case series, or claims that cannot be independently verified.
Patient protections Labeling, manufacturing requirements, and post-market safety obligations apply. Informed consent, protocol oversight, eligibility rules, and specified monitoring should apply. Protections and accountability may be unclear; quality and potency may not have been verified.
Payment Coverage depends on the product and insurance policy. Ask which procedures are covered by the study and which costs the participant must pay. A fee does not establish approval, effectiveness, or legal marketing status.

When a comparison cell cannot be answered by the provider or a regulator’s record, treat that missing information as a decision-relevant risk rather than filling the gap with assumptions.

Questions to take to your doctor or study team

  • What is the exact product, source, cell or gene type, and manufacturing process?
  • What specific diagnosis and eligibility criteria does it address?
  • Is it approved for me, or is it investigational?
  • Which regulator authorized it, and where can I read the current label or protocol?
  • If it is investigational, what is the trial identifier and what FDA-issued IND information is available to participants?
  • What controlled human evidence supports this exact product and use?
  • What benefits are realistic, and what remains uncertain?
  • What short- and long-term risks have been observed or are biologically plausible?
  • What standard alternatives exist, and what happens if I decline or delay this option?
  • Who reports adverse events, who pays for complication care, and how long will follow-up continue?

What to do if something goes wrong

Seek urgent medical care for symptoms such as sudden vision changes, weakness, confusion, severe headache, high fever, spreading redness, breathing difficulty, chest pain, or signs of a serious allergic reaction after a cell or tissue procedure. Tell the emergency team exactly what was administered, when, where, and by whom; bring the consent form, product label, and lot information if available.

Patients and clinicians can report suspected adverse events to FDA MedWatch. Keep copies of invoices, treatment records, laboratory results, consent documents, and advertising. These records help your treating physician and regulators determine what product was used and whether other patients may be at risk.

Bottom line for a treatment decision

Regenerative medicine can be legitimate, but legitimacy belongs to a specific product, indication, manufacturing process, and evidence base. In the United States, FDA’s approved stem-cell products identified in its consumer alert are cord-blood blood-forming cells for blood-production disorders; other regenerative products have narrowly defined approvals such as those listed for Zevaskyn, Symvess, and certain cell or gene therapies. Most clinic promises involving orthopedic pain, neurologic disease, chronic pain, autism, fatigue, or exosomes are not approvals for those uses. Verify the product and indication, distinguish a regulated trial from a marketed treatment, insist on a complication and follow-up plan, and involve the specialist who manages your condition before proceeding.
