After the drug is administered, the molecule carries radiation to cells bearing that target, damaging their DNA and nearby tumor cells. Targeting can reduce radiation to healthy tissue compared with nonspecific delivery, but it cannot eliminate exposure or side effects, and eligibility depends on the exact drug, cancer, target expression and treatment history.
What radiopharmaceutical therapy means
The National Cancer Institute defines targeted radionuclide therapy as “A type of radiation therapy in which a radionuclide (a radioactive chemical) is linked to a cell-targeting molecule, such as a monoclonal antibody, and injected into the body.” Read the NCI definition.

A radiopharmaceutical therefore has two functional parts:
- A targeting molecule that recognizes a molecular feature, receptor or tissue change associated with a cancer.
- A radionuclide that emits therapeutic radiation once it is delivered to that location.
The targeting component is not universal. One medicine may bind a protein on prostate-cancer cells, another may bind a receptor on a neuroendocrine tumor, and radium-223 may concentrate in areas of bone affected by tumor-related changes. These approaches share a delivery principle but are different medicines with different indications, doses, schedules and risks.

Radiopharmaceutical therapy is systemic: the drug travels through the bloodstream rather than being aimed from outside the body at a single field. The radiation is nevertheless intended to be concentrated where the targeting feature is present. Radiation can damage the bound cell and, depending on the radiation type and range, nearby cancer cells as well. Some normal tissues can also take up the drug or receive radiation as the medicine circulates and is cleared.
How the targeting process works
1. A tumor-associated target is identified
Before treatment, clinicians determine whether a patient’s tumors display the feature that the proposed radiopharmaceutical recognizes. The target may be a cell-surface protein, a receptor or a characteristic bone environment. A cancer diagnosis by itself does not establish that the target is present at a level sufficient for treatment.
2. The drug binds or accumulates at the target
After administration, the carrier molecule circulates and attaches to its matching target or concentrates in the relevant tissue. In prostate cancer, a PSMA-directed ligand binds prostate-specific membrane antigen (PSMA) on tumor cells. In peptide receptor radionuclide therapy (PRRT), a radioactive peptide binds receptors such as somatostatin receptors on selected neuroendocrine tumors. Radium-223 follows a different biological route and collects in areas of bone undergoing tumor-associated change.

3. Radiation damages cancer cells
The radionuclide emits radiation at or near the target. That energy can produce lethal damage to cellular DNA. Radiation may affect neighboring tumor cells as well, which is useful when not every malignant cell is perfectly reached. The treatment is targeted, not perfectly selective: organs involved in drug processing or excretion, bone marrow and other tissues may receive clinically important exposure.

4. The body clears the medicine
Unbound or metabolized material leaves through normal biological pathways, often including urine. The nuclear-medicine team gives product-specific instructions about hydration, toileting, contact with others and handling body fluids. These instructions vary by radionuclide, dose, patient condition and local regulations; they should come from the treating center rather than from a generic online checklist.
Examples of radiopharmaceutical therapy
| Approach | Target or route | Typical disease context | Key qualification |
|---|---|---|---|
| Lutetium-177 PSMA-directed therapy | A ligand binds PSMA on prostate-cancer cells. | Selected metastatic prostate cancer. | Target expression and the product’s current indication must be confirmed; PSMA-positive imaging is used for selection in specified settings. SNMMI prostate fact sheet |
| Radium-223 | Accumulates in areas of bone undergoing tumor-associated change. | Prostate cancer with bone metastases in appropriate patients. | It is not interchangeable with PSMA-directed drugs, and blood-count monitoring is important. SNMMI prostate fact sheet |
| Peptide receptor radionuclide therapy (PRRT) | A radioactive peptide binds receptors such as somatostatin receptors. | Selected neuroendocrine tumors. | Receptor expression, organ function and the specific protocol determine suitability. NCI PRRT definition |
| Other radiolabeled agents | Varies by carrier, radionuclide and tumor biology. | Depends on the licensed product and jurisdiction. | Do not infer an approved use from the mechanism alone. |
The Society of Nuclear Medicine and Molecular Imaging describes these examples in its patient information on prostate cancer and neuroendocrine tumors. Read the neuroendocrine-tumor fact sheet.
Why a diagnostic scan may come first
Some radiopharmaceutical programs use a diagnostic tracer to show whether the tumor carries the treatment target. This is a form of theranostics: imaging helps identify people who may receive a related therapeutic agent, while the treatment product delivers a therapeutic radiation dose. The two products should not be described as the same medicine.
For example, PSMA PET imaging can show PSMA expression in prostate-cancer lesions. In a July 31, 2026 announcement, the U.S. Food and Drug Administration said adults receiving lutetium Lu 177 vipivotide tetraxetan (Pluvicto) with androgen-receptor pathway inhibitor therapy for the newly approved metastatic androgen-pathway modulation-naïve or -sensitive population should be selected using Locametz (gallium Ga 68 gozetotide) or another approved PSMA PET product based on PSMA expression. That announcement is a U.S. approval for a defined population and date. It does not mean every person with prostate cancer, every PSMA scan result or every country has the same indication. Labels and availability can change, so the current local prescribing information controls.
What treatment usually involves
The exact pathway is arranged by an oncology and nuclear-medicine team. A typical course includes the following decisions and checks:
- Confirm the cancer and treatment goal. The team reviews pathology, scans, disease distribution, prior therapies and whether the proposed product is appropriate for the cancer’s current state.
- Demonstrate the target. A PET scan, other imaging or laboratory assessment may be used to establish target expression. The required test depends on the medicine.
- Check organ and marrow reserve. Blood counts, kidney function, liver-related tests and other evaluations may be needed because clearance and radiation sensitivity differ among patients.
- Review medicines and precautions. Clinicians consider other cancer treatments, pregnancy potential, kidney disease, urinary problems, ability to follow radiation-safety instructions and the need for support at home.
- Receive the radiopharmaceutical. Administration is commonly an intravenous injection or infusion in a controlled clinical setting. Protective measures and observation are determined by the product and facility.
- Follow the center’s radiation-safety instructions. These may address fluids, bathroom use, cleaning spills, laundry and temporary distance from particular people. Instructions are individualized and time-limited according to measured or expected radiation exposure.
- Return for monitoring and later cycles. Blood tests, symptoms, kidney function, imaging and tumor response guide whether and when another dose is given.
Not every patient receives all of these steps, and some products require additional medicines or supportive infusions. The prescribing information and the treating center’s protocol take priority over a generalized schedule.
Schedules are specific to the product
There is no single radiopharmaceutical-therapy timetable. As examples, the SNMMI prostate fact sheet describes the named lutetium-177 PSMA-617 therapy as six injections every six weeks and describes radium-223 as six injections every four weeks. Those intervals belong to the treatments covered by that fact sheet; they are not a prescription for all lutetium, radium or PRRT regimens.

Side effects and safety
Targeting can reduce unnecessary radiation to some healthy tissue, but it does not guarantee that normal organs are spared. Side effects depend on the radionuclide, carrier, dose, tumor locations, kidney function, prior treatments and individual vulnerability.
For the lutetium-177 PSMA-617 therapy described in its prostate fact sheet, SNMMI lists fatigue, dry mouth and decreased blood counts among possible effects. For radium-223, it lists decreased blood counts. These examples are not exhaustive and should not be transferred from one product to another.
- Bone marrow: Lower blood counts can increase risks such as anemia, infection or bleeding and may require repeat testing or treatment changes.
- Salivary glands: Dry mouth can occur with some PSMA-directed therapies because related target expression and drug exposure may affect salivary tissue.
- Kidneys and urinary tract: Clearance and baseline kidney function influence safety; report reduced urine, dehydration or urinary problems promptly.
- General effects: Fatigue and other symptoms may reflect the drug, the cancer or concurrent treatment.
Patients should obtain the current medication guide or prescribing information for the specific product and ask which symptoms require an urgent call. Pregnancy and breastfeeding precautions, contraception advice and contact restrictions must be individualized by the treatment team.

Eligibility is a clinical decision, not a conclusion that can be made from the treatment’s general mechanism. The team normally considers:
Best Value
- the cancer type and whether the disease is metastatic, recurrent, progressive or otherwise within the approved indication;
- demonstrated target expression on the required scan or test;
- prior systemic therapies and whether concurrent treatment is allowed;
- blood counts, kidney function and other organ reserves;
- tumor burden and the location of disease;
- ability to attend repeated treatments and follow safety instructions; and
- the product label, regulatory status and availability in the relevant country.
For prostate cancer, NCI’s treatment overview explains that treatment choices depend on factors such as disease extent and prior treatment. Review the NCI prostate-cancer treatment overview. A PSMA-positive scan alone does not answer every eligibility question, and a negative or heterogeneous scan can change the treatment discussion.
Radiopharmaceutical therapy versus other radiation
External-beam radiation therapy uses a machine outside the body to direct radiation at planned fields. It can be highly localized and is often selected for a specific tumor or symptom-producing site.
Systemic drug therapy distributes medicines through the body according to their pharmacology, but it does not necessarily carry a radioactive payload to a molecular target.

Radiopharmaceutical therapy combines systemic administration with molecular or tissue-directed radiation. Its value is greatest when the target is present in clinically important disease sites and the patient can safely receive the radionuclide. It may complement other treatments rather than replace surgery, hormone therapy, chemotherapy, immunotherapy or external-beam radiation.
Questions to ask the treatment team
- Which target does this product recognize, and how was its presence demonstrated?
- Is the proposed use approved for my cancer, disease state and country?
- How many treatments are planned, and what could delay or stop the next dose?
- Which blood tests and kidney tests are required before each cycle?
- Which side effects are expected, and which require same-day medical advice?
- What radiation-safety instructions apply at home, and for how long?
- How will response be measured, and what alternatives exist if the treatment is ineffective or unsafe?
Frequently Asked Questions
Is radiopharmaceutical therapy the same as a PET scan?
No. A PET agent is used primarily to create diagnostic images, while a therapeutic radiopharmaceutical carries a radiation dose intended to damage cancer cells. Some programs use imaging to select patients for a related treatment, but the products and purposes are distinct.
Does targeted radiation affect healthy tissue?
It can. Targeting may limit exposure compared with nonspecific delivery, but normal organs can receive radiation during circulation, uptake or clearance. The specific risk depends on the product and the patient.
Can a PSMA scan determine treatment eligibility by itself?
No. Target expression is one factor. Cancer type and extent, prior treatment, organ function, blood counts, current labeling and the clinical plan also matter.
