Do these 3 things before closing this tab:
1Clear out junk files and repair common Windows errors2Fix the driver behind crashes, sound loss and screen glitches3Repair Windows errors before they cause bigger problemsThe best tour questions test three things at once: whether the community can provide the older adult’s care now, whether it can adapt as needs change, and what daily life will actually feel like. Bring the older adult into the conversation as much as they wish and are able. Before each visit, write down current help needs, likely future needs, personal preferences, and a complete list of costs you need explained in writing.
Prepare a question sheet before you arrive
Use a fresh copy of the same checklist for every community. Record the name and role of the person who answered, the exact answer, any document they promised, and questions that still need follow-up. A consistent sheet makes differences visible after several tours instead of leaving you with general impressions.

Start with the older adult’s priorities
- Which daily tasks require help now, such as bathing, dressing, toileting, transferring, eating, or taking medication?
- What health, therapy, nursing, hospice, or memory-support needs might arise later?
- Which preferences are non-negotiable: meals, wake and sleep times, religious connection, activities, pets, outdoor time, or proximity to family?
- How important are a private room, a particular neighborhood, transportation, or frequent visitors?
If the appropriate level of care is unclear, ask the person’s health care provider to describe the services and supervision required. The National Institute on Aging (NIA) recommends matching present and foreseeable needs to the facility’s capabilities rather than choosing by appearance alone.
Clarify what kind of setting you are touring
“Senior living” can mean assisted living, a nursing home, memory care, or another residential long-term-care arrangement. Licensing, inspections, staffing rules, and disclosure requirements vary by state and facility type. Medicare’s visit checklist is written for nursing homes that are Medicare- or Medicaid-certified; it is not a complete regulatory checklist for every assisted living residence. Ask the community which license and certification apply to the building and which unit your relative would occupy.

Questions about care fit and future needs
Services provided on site
- What help with daily activities, nursing, physical or occupational therapy, hospice, and memory care is available here?
- Which services are provided by your own employees, and which come from outside agencies?
- What does a new-resident assessment cover, who performs it, and how are the resident and family involved—with the resident’s permission?
- How often is the care plan reviewed, and who can request a change?
- Is there an opening for the specific room, service, or care unit needed? If not, how does the waiting list work and how long is the current estimate?
What happens when needs increase
- Can a resident move to a higher-care unit without leaving the community?
- What changes in supervision, staffing, room type, or price accompany a move?
- Which conditions require transfer to another facility, and who helps arrange it?
- How are urgent changes—falls, weight loss, repeated infections, medication problems, or new confusion—communicated to family?
Ask for specific examples rather than a general assurance that the community is “full service.” A clear answer identifies the trigger for a change, the people responsible, the timeline, and the additional charge, if any.
Questions about staffing and medical support
Coverage on every shift
- How many care providers are scheduled for residents on day, evening, overnight, weekend, and holiday shifts?
- How many staff members are trained to provide medical care, and what credentials or dementia training do they hold?
- Are staffing levels adjusted for the number and acuity of residents, or is there a fixed minimum?
- How are absences and agency coverage handled?
Do not treat one staffing number as a guarantee of quality. The appropriate level depends on residents’ needs, the services offered, the layout of the building, and whether staff are available where residents are spending time.
Leadership stability and accountability
- How long have the executive director, nursing director, and department heads worked here?
- How often have key positions changed during the past year?
- Who is the first contact for a care concern, and how quickly should a family expect a response?
- How are complaints documented, investigated, and resolved?
Clinicians, appointments, and illness
- Is a physician, nurse practitioner, physician assistant, or other clinician available to see residents regularly? How often?
- Who coordinates care when a resident becomes ill or leaves for a hospital?
- How are prescriptions ordered, administered, monitored, and reconciled after a hospital stay?
- How are residents transported to medical appointments, and who accompanies someone who needs assistance?
Ask to see the written policy for emergency transfers and family notification. Find out whether transportation is included, billed separately, or arranged by the family.

Questions that reveal daily life, dignity, and choice
Observe a normal day
- What does a typical weekday look like? What changes on weekends?
- Can we observe or join an activity rather than viewing only an activity calendar?
- How can residents who stay in their rooms participate?
- Can residents choose when to wake, sleep, bathe, eat, receive visitors, or go outside?
- How are individual routines, cultural practices, and religious connections accommodated?
Watch how staff address residents. Do they knock before entering, explain what they are doing, wait patiently, and use the resident’s preferred name? Residents should appear clean, appropriately dressed, and comfortable—not necessarily dressed alike or gathered only for the tour.
Building condition and accessibility
- Are hallways, bathrooms, elevators, and common areas accessible for walkers, wheelchairs, and other equipment?
- Are handrails, lighting, call systems, and bathroom supports maintained and easy to use?
- Is the building clean, comfortable, odor-controlled, and in good repair?
- Is outdoor space safe and accessible, and can staff help residents use it?
- What happens during power failures, severe weather, fire alarms, or other emergencies?
If possible, visit a resident room and a shared bathroom, not only the model apartment. Ask which furnishings are supplied, what families may bring, and how personal belongings are labeled and protected.
Meals and social connection
- Can you observe a meal or sample the food?
- How are allergies, swallowing problems, religious diets, and personal preferences handled?
- Are snacks and drinks available between meals?
- How are transportation, visitors, and trips outside the community arranged?
- What are the after-hours visitor procedures?
Ask about leaving for a few hours or days, including sign-out procedures, medication management, and whether an overnight absence affects billing.

Special questions for dementia or Alzheimer’s care
Training and unit design
- Do staff receive specialized dementia training before working with residents, and how often is it renewed?
- Is there a dedicated memory-care unit? What services and supervision does it provide?
- How many residents have Alzheimer’s or another dementia, and what does a typical day look like for them?
- How are activities adapted for different abilities and stages of the disease?
CMS’s nursing-home checklist asks, “Does the nursing home offer specialized training for staff to support residents living with dementia?” Use that wording when touring a Medicare- or Medicaid-certified nursing home, then ask the same practical question in any memory-care setting.

Behavior, safety, and outdoor access
- How do staff respond to distress, wandering, falls, refusal of care, or sudden behavior changes?
- What safeguards are used, and how do they preserve dignity and freedom of movement?
- How are medical needs recognized when a resident cannot describe symptoms?
- Is there secure outdoor access, and can staff help residents get outside?
- What happens if the person’s needs exceed the unit’s ability to provide safe care?
Memory-care pricing
Ask whether memory care has a separate base rate or a higher-care fee, which services are included, and which events trigger a price change. Request examples for common needs such as increased supervision, incontinence supplies, transportation, or one-to-one support.
Questions about prices, payment, and the contract
Get the complete cost in writing
- What is the base monthly fee, and exactly which housing, meals, utilities, activities, and care services does it include?
- What costs extra: medication assistance, transportation, therapies, personal care, supplies, laundry, guest meals, or optional activities?
- Is there an admission, community, deposit, or move-in fee? Is any part refundable?
- How are one-time charges, monthly charges, and usage-based charges calculated?
- How and when can prices change, and how much notice will the resident receive?
Ask for the full fee schedule, service description, resident handbook, and contract before making a decision. CMS says Medicare- and Medicaid-certified nursing homes must provide information about services, charges, and fees in writing before move-in. That written-disclosure rule should not be assumed to apply identically to every assisted living residence.

- Please Note: To reach 10Gbps speed, keep the cable under 3.3 ft. For USB A Male to USB C adapters, try flipping the USB C connector. USB C Male to USB A adapters support bidirectional 10Gbps transfer within 3.3 ft
Paying for care
- Which payment sources might apply to this particular service and setting?
- Can the financial office explain what is and is not covered, and provide contact information for an independent benefits counselor?
- What happens if savings run low or the resident can no longer afford the monthly fee?
- Does the contract require a personal guarantor, and what responsibility would that person assume?
Do not assume Medicare pays for a long-term residential stay. Medicare Part A coverage for short-term skilled nursing is a different situation with eligibility conditions; long-term custodial nursing-home care is generally not covered by Part A. Confirm the resident’s situation with Medicare, the state Medicaid office, an insurer, or a qualified benefits counselor.
Read exit and transfer terms
- Under what circumstances can the community terminate the agreement or require a move?
- How much notice is provided, and who pays for relocation?
- What happens after hospitalization or a temporary absence?
- What are the rules for refunds, room holds, belongings, and final billing?
Compare communities on the same decision axes

Make a second visit and verify the answers
- Visit at a different time or day, preferably without calling ahead. Staffing, meals, noise, and resident activity can look different in the evening or on a weekend.
- Speak with the administrator, nursing or care director, and—when permitted—residents and family members. Compare their descriptions of routines and responsiveness.
- Match verbal promises to the contract, fee schedule, policies, and care-plan documents. Mark every unanswered question and request a dated written response.
- For a Medicare-certified nursing home, review official inspection results, staffing information, and quality measures before choosing. These federal comparison tools are designed for certified nursing homes and should not be treated as a complete rating system for every assisted living community.
- Discuss the finalists with people who understand the older adult’s health and care needs, such as a clinician, social worker, or hospital discharge planner.
Limits of checklists and rules
Federal guidance can help families ask better questions, but it cannot replace state licensing information, a contract review, or direct observation. Licensing and oversight depend on the state and the type of facility. Fees, openings, staffing, and services are facility-specific and can change. Verify current details with the community and the relevant state agency.

No single nationwide staffing ratio or standard senior-living price is established by the guidance used here. Treat a community’s own figures as time- and location-specific, and ask what they measure—scheduled staff, employees physically present, or staff assigned to a particular unit.
A practical tour-day checklist
- Bring the older adult, a medication and medical-needs list, and a way to take notes.
- Use the same questions and scoring method at every community.
- Observe residents, staff interactions, meals, activities, cleanliness, accessibility, and outdoor access.
- Ask for written fees, included services, policies, and contract terms before deciding.
- Record the name of every person who answers and the date of each promise or follow-up.
- Return for a second visit before signing, especially when dementia care or a major change in care level is involved.
The strongest choice is the community that can document a workable care plan, explain its staffing and escalation process, support the resident’s preferred daily life, and state the full financial obligation plainly. Judge the fit against the older adult’s priorities and likely future needs—not against one impressive feature or a reassuring tour.
