There is no single age-based checklist. In the United States, screening decisions depend on your age, anatomy, smoking exposure, weight, family and personal history, previous results, medicines, other conditions, life expectancy and preferences. Most adults should routinely review blood pressure, diabetes risk, cancer screening, bone health and vaccinations with a clinician, then add risk-specific services such as lung CT or earlier cancer surveillance.
How to use an age-and-risk screening plan
Screening looks for disease before symptoms appear. A recommendation for an average-risk group may not fit someone with symptoms, a previous abnormal result, a strong family history, an immune disorder, a prior cancer or a procedure that changes anatomy. Conversely, an older adult with limited life expectancy or a high burden of illness may reasonably choose fewer tests after a shared decision with a clinician.

The guidance below summarizes current US recommendations from the US Preventive Services Task Force (USPSTF) and the Centers for Disease Control and Prevention (CDC). It is a discussion guide, not a diagnosis or individualized schedule. An abnormal screen is not the same as a diagnosis and often requires a second test or specialist evaluation.
Screenings to review throughout adulthood
Blood pressure
The USPSTF recommends screening adults age 18 or older who do not already have diagnosed hypertension with office blood-pressure measurement. Screening is suggested every year for adults 40 and older and for people at increased risk. Adults ages 18–39 who are at low risk and previously had a normal reading may be screened every three to five years.

An elevated office reading should be confirmed outside the clinic before treatment. The USPSTF specifically recommends ambulatory monitoring or home measurements for diagnostic confirmation. A validated, accurate upper-arm monitor can help collect readings when a clinician directs you to do so, but a consumer device does not diagnose hypertension or replace medical care. Ask which device, cuff size, schedule and reporting method to use.
Diabetes and prediabetes
The USPSTF recommends screening asymptomatic adults ages 35–70 who have overweight or obesity. Screening can identify prediabetes or type 2 diabetes before symptoms develop. If prediabetes is found, the recommendation is to offer or refer the person to effective preventive interventions, which may include structured nutrition, physical-activity and weight-management programs.
People outside this age or weight range may still need testing because of symptoms, pregnancy history, medications, family history or other medical risks. Ask what test is appropriate and when it should be repeated rather than assuming that a normal result eliminates future risk.

The USPSTF recommendation index includes adult depression screening and other services for defined risk groups. Tell your clinician about persistent sadness, loss of interest, anxiety, sleep or appetite changes, substance use, safety concerns and major life changes. Screening should lead to a way to discuss results and obtain evaluation or treatment when needed; it should not be treated as an isolated questionnaire.

Cancer screening by age, anatomy and exposure
Colorectal cancer
The USPSTF recommends screening average-risk adults ages 45–75. You and your clinician can compare stool-based strategies with visual examinations. The best choice depends on eligibility, preparation, test burden, repetition schedule, access, cost and what you would do after an abnormal result.
| Decision point | What to discuss |
|---|---|
| Eligibility | Age, symptoms, family history, prior polyps or cancer, inflammatory bowel disease and previous test results can change the recommendation. |
| Test type | Stool-based and visual examination strategies are available; the specific options and intervals vary by test and should be confirmed in the full current recommendation. |
| Preparation and burden | Ask about diet or bowel preparation, time away from work, sedation, transportation and the possibility of repeat testing. |
| After a positive result | A positive stool test requires follow-up colonoscopy. An at-home test is therefore not a one-time substitute for diagnostic follow-up. |
| Ages 76–85 | Screening is selective. Consider overall health, prior screening, expected benefit and personal preferences with a clinician. |
For adults older than 85, do not assume that routine screening continues; the balance of benefit and burden generally requires individualized medical judgment.
Breast cancer
The current USPSTF recommendation is biennial (every two years) screening mammography for women ages 40–74. Discuss whether this applies to you based on personal and family history, breast symptoms, prior biopsies or abnormal imaging, and any relevant genetic or medical factors. The cited USPSTF index does not establish a routine recommendation for women age 75 and older, so continuation beyond 74 should be an individualized conversation rather than an automatic interval.
New breast symptoms—such as a lump, skin or nipple change, unusual discharge or persistent focal pain—need diagnostic evaluation and should not wait for the next screening mammogram.

Cervical cancer
For average-risk women with a cervix, the listed USPSTF options are:
- Ages 21–29: cytology (a Pap test) every three years.
- Ages 30–65: cytology every three years, high-risk human papillomavirus (HPV) testing every five years, or cotesting with cytology and HPV testing every five years.
These intervals do not automatically apply after an abnormal result, cervical precancer or cancer, a cervical procedure, or in some immune conditions. Ask which test was performed previously, what the result means and exactly when follow-up is due. Screening decisions also depend on whether the cervix is present and on the reason for any prior surgery.
Lung cancer
The USPSTF recommends annual low-dose CT for adults ages 50–80 who have at least a 20 pack-year smoking history and currently smoke or quit within the past 15 years. A pack-year means smoking an average of one pack per day for one year; your clinician can calculate the total from the number of packs or cigarettes per day and the years smoked.
Stop screening once 15 years have passed since quitting, or sooner if a health condition substantially limits life expectancy or the ability or willingness to undergo curative lung surgery. Low-dose CT is for people who meet the smoking and age criteria, not for every person with occasional exposure. Bring an accurate smoking history, including quit date, to the visit.

Prostate cancer
The age- and risk-specific prostate screening recommendation was not established in the material summarized here. Do not infer an interval from another cancer guideline. Ask your clinician to verify the current USPSTF recommendation and relevant specialty guidance, taking into account age, family history, ancestry, prior prostate tests, urinary symptoms and preferences. A symptom such as difficulty urinating, blood in urine or bone pain requires clinical evaluation rather than relying on a screening schedule.
Bone health and fracture prevention
Osteoporosis screening for women
The USPSTF recommends screening women age 65 or older for osteoporosis. It also recommends screening postmenopausal women younger than 65 who are at increased fracture risk, determined through clinical risk assessment. A clinician may ask about a previous fracture, falls, family history, low body weight, smoking, alcohol use, rheumatoid arthritis and medicines such as long-term corticosteroids.
What about men?
The searched USPSTF recommendation index does not establish routine osteoporosis screening for all men. Men should still discuss fracture history, medications, medical conditions and other risks; an individualized assessment may justify testing even when no universal age threshold is stated.

Vaccines are part of preventive care
The CDC adult immunization schedule is organized by age, medical conditions, prior doses and other indications. Have a clinician or pharmacist reconcile your record against the currently posted schedule rather than restarting vaccines unnecessarily. CDC schedule materials instruct providers to administer recommended vaccines when a vaccination history is incomplete or unknown.

How to prepare for a preventive-care visit
Organizing information before the appointment makes a risk-based plan more accurate.
- Bring a current medication and supplement list, including doses and long-term medicines.
- Write down diagnoses, surgeries, major procedures, allergies and any symptoms, even if they seem unrelated.
- Record family history of colorectal, breast, cervical, lung, prostate and other cancers, as well as osteoporosis or early fractures.
- For lung screening, calculate or bring the number of pack-years and the date you stopped smoking, if applicable.
- List the dates and results of prior mammograms, cervical tests, colorectal tests, bone-density tests and blood-pressure evaluations.
- Bring vaccine records or authorize the clinic or pharmacy to retrieve them.
- Ask which screenings you are due for now, which results need follow-up, what preparation is required and when the plan should be revisited.
Questions that make the discussion specific
- “Which screenings fit my age, anatomy, family history and previous results?”
- “Does a symptom or medicine change the usual interval?”
- “What are the available tests, and how do preparation, cost, access and repeat frequency differ?”
- “If this result is abnormal, what test happens next and how quickly?”
- “Should an elevated office blood pressure be confirmed at home or with ambulatory monitoring?”
- “When should we revisit this plan if my health, medications or preferences change?”
When a standard schedule should not be used
Contact a clinician rather than waiting for routine screening if you have bleeding, a new lump, unexplained weight loss, persistent pain, a changing mole, a new cough, severe fatigue, neurologic changes or other concerning symptoms. Screening recommendations are for people without symptoms; symptoms can require diagnostic testing at any age.
