What Predictive Models Can—and Can’t—Tell Us About Nanoparticle Drug Delivery

Predictive models can help scientists choose nanoparticle formulations and estimate defined measurements such as particle size, drug loading, encapsulation efficiency, pharmacokinetic profiles and, in some cases, biodistribution. They cannot, on their own, prove that a carrier will reach the intended tissue in a new patient, be safe, work clinically or be manufactured consistently. The useful question is not whether a model is “accurate” in the abstract, but what it predicts, on which data, within what applicability domain and for what decision.

What a predictive model can actually predict

A machine-learning model learns statistical relationships between input variables and a specified endpoint. In nanoparticle drug delivery, inputs may include carrier composition, process conditions, dose, route, drug or nucleic-acid properties and assay conditions. The output is only as broad as the endpoint used to train it.

Formulation attributes

The 2026 review Artificial Intelligence in Nanopharmaceutical Development: From Predictive Design to Clinical Translation describes models for poly(lactic-co-glycolic acid) (PLGA) nanoparticles that estimate particle size, polydispersity, drug loading and encapsulation efficiency. Such predictions can rank candidate formulations before laboratory work. They are most useful for reducing the number of experiments or identifying combinations worth testing next.

A predicted diameter or encapsulation value is still a formulation result, not a biological result. Particle size can influence circulation and uptake, but the same nominal size can behave differently when surface chemistry, protein adsorption, aggregation, dose or biological environment changes.

Pharmacokinetics and biodistribution

AI-assisted physiologically based pharmacokinetic (PBPK) models extend the prediction problem from a formulation measurement to concentration or distribution over time. They can organize information about compartments, clearance and tissue exposure and compare a simulated profile with measured pharmacokinetic data. Biodistribution models may estimate how much material appears in organs or target tissues under the conditions represented in their training data.

These are useful decision-support tools. They can expose a formulation that is unlikely to meet a pre-set exposure target, help prioritize sampling time points and reveal which parameters most affect a simulated profile. They do not establish that the same profile will occur after a change in species, disease state, manufacturing process, dose or patient population.

What the published performance numbers mean

The 2026 review reports metrics for one AI-assisted PBPK example. For the endpoints labeled DE24, DE168 and DEmax, it reports the following values:

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
Reported endpoint R² RMSE How to read it
DE24 0.83 3.01 A result for this particular model, endpoint and dataset; the RMSE unit and scale must be interpreted from that study.
DE168 0.56 2.27 Lower R² than the other reported endpoints; it is not evidence of a general nanoparticle accuracy rate.
DEmax 0.82 3.51 A separate endpoint with its own error scale and validation conditions.

The same review says that 133 of 288 experimentally measured pharmacokinetic datasets achieved R² of at least 0.70. That fraction describes the datasets and evaluation described in the review. It cannot be converted into a universal probability that a new nanoparticle prediction will be correct. R² and RMSE are not interchangeable across endpoints, concentration ranges or studies, and a strong average can hide failure in a particular formulation class.

For lipid nanoparticles carrying nucleic acids, a 2024 preprint by Gaurav Kumar and Arezoo M. Ardekani describes a framework built from a literature-curated dataset of 6,398 formulations. The authors emphasize that useful prediction requires complete composition information, including constituent structures, molar ratios, nucleic-acid type and dose. Because this is a preprint, its methods and conclusions should be treated as provisional rather than as established clinical evidence. The dataset size is also not the same as the number of independent experiments: multiple records can come from related formulations, laboratories or publication conditions.

Can a model tell whether a nanoparticle will reach a tumor?

It can estimate tumor exposure only to the extent that tumor delivery is represented by suitable data and biological context. A model may identify features associated with accumulation in a particular animal model or assay. That is different from demonstrating that a carrier will penetrate a human tumor, release its payload in the right cells and improve a patient outcome.

The 2026 review reports a median tumor-delivery efficiency of approximately 0.7% of the injected dose. This is a figure reported by that review, not a universal rate for every nanoparticle medicine or tumor type. It illustrates why optimizing size, charge or encapsulation does not automatically solve the delivery problem: only a small fraction of the administered material may reach the measured tumor compartment, and even that fraction may not enter the relevant cells or release active drug.

Biology that a formulation-only model may miss

  • Protein corona: proteins adsorb to a particle in biological fluids and can change the identity recognized by cells.
  • Cell uptake and trafficking: entry into a cell does not guarantee escape from endosomes or delivery to the required intracellular site.
  • Immune recognition: complement activation, antibodies, macrophages and other immune processes can alter circulation and clearance.
  • Tissue architecture: vascular permeability, extracellular matrix and interstitial pressure affect penetration.
  • Drug release: payload release depends on carrier chemistry, local conditions and degradation, not just loading at manufacture.
  • Patient and disease variation: the same carrier can behave differently with route, dose, immune status, tumor biology and organ function.

Why promising predictions fail to translate to people

Preclinical evidence is generated in a defined species, strain, cell system, dose and protocol. Human patients introduce variation in immunity, prior treatment, organ function, tumor vascularization, stroma and macrophage populations. The 2020 review Challenges in nanomedicine clinical translation explains that response depends on pharmacokinetics, tissue distribution, target accumulation and penetration, and drug release at the target site. In cancer, those properties can vary both between patients and among regions of the same tumor.

An animal model can therefore provide a valuable test of a mechanism without being a reliable numerical forecast of human benefit. Differences in clearance, protein binding, immune responses and tumor structure can change both exposure and toxicity. An in-vitro uptake assay may show that cells internalize a particle while saying little about circulation time, organ accumulation or release in a patient.

Translation also includes the product that can actually be made. Batch-to-batch variation, raw-material differences, mixing or assembly conditions, storage and sterilization can alter critical attributes. A model trained on laboratory formulations may not remain valid when the process is scaled or when a specification changes.

Manufacturing and safety are outside a model’s shortcut

Nanoparticle safety depends on organ exposure, carrier materials, immune reactions, formulation quality and reproducible manufacture. The 2020 review recommends early pharmacokinetic and biodistribution studies, organ-exposure and toxicity assessment and appropriate controls. Particle size, morphology, drug loading and release are directly relevant to in-vivo performance and therefore belong in quality control, not only in a model’s feature list.

“It is absolutely paramount that critical quality attributes, such as particle size, size distribution, charge and morphology, and drug encapsulation and release, are considered very early on in formulation design, and that narrow specifications are defined within which the formulations show optimal performance. Manufacturing should be robust and scalable and performed under stringent adherence to good manufacturing practice (GMP) guidelines.”

How to judge a nanoparticle prediction claim

Use the following questions before treating a reported score as evidence that a model is ready for a high-consequence decision.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

1. What is the exact endpoint?

Distinguish particle size, polydispersity, encapsulation, cellular uptake, biodistribution, pharmacokinetics, toxicity and clinical response. A model that predicts encapsulation efficiency has not demonstrated prediction of tumor response. If an endpoint label is unexplained, ask how it was measured and on what scale.

2. How were the data collected?

Check whether composition, constituent structures, molar ratios, process conditions, assay protocol, dose, route, species and disease context are recorded consistently. Incomplete or heterogeneous records can make a model appear to learn chemistry when it is actually learning laboratory, publication or batch effects.

3. Was there independent external validation?

Internal train/test splits can overstate performance when related formulations appear in both sets. Stronger evidence comes from independent studies, laboratories, carrier classes, manufacturing batches or biological models. A model should be tested on data collected after development and, ideally, under the conditions in which it will be used.

4. What is the applicability domain?

Every model has a region in which its inputs resemble the training data. A PLGA model should not silently be treated as validated for an unrelated lipid, polymer, surface coating or route of administration. Predictions outside that domain should be flagged rather than presented with the same confidence as interpolation.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

5. Are uncertainty and failure cases reported?

Look for prediction intervals, calibration, error by formulation class and examples of large errors. A single headline R² does not show whether uncertainty is reliable for an individual candidate. Interpretability is also important when a model influences safety, dose or manufacturing decisions: users need to know which inputs drive a result and when the model is extrapolating.

6. What decision will the model control?

Intended use Evidence that should be expected
Exploratory screening Clear endpoint definition, documented data and experimental confirmation of top-ranked candidates.
Process development or quality control Robust analytical measurements, reproducibility across batches and sites, defined operating range and change-control documentation.
Pharmacokinetic or safety decisions External validation, uncertainty analysis, relevant biological models and prospective confirmation before decisions affect dose or exposure.
Patient-specific treatment selection Prospective clinical validation, oversight, transparent intended-use limits and evidence of improved patient outcomes.

A practical workflow for using models responsibly

  1. Define the decision. State whether the goal is to select experiments, forecast a measurable property, support process control or inform a clinical decision.
  2. Specify the endpoint and units. Record how the endpoint is measured, at what time point and with which assay or instrument.
  3. Audit the input data. Capture composition, process, dose, route, biological system and batch information, and mark missing or incomparable records.
  4. Set the applicability domain. Identify which carrier types, payloads, doses, species and manufacturing conditions are represented.
  5. Validate outside development data. Use independent formulations or studies and report performance by relevant subgroup, not only as one aggregate score.
  6. Quantify uncertainty. Provide intervals or calibrated probabilities and define a rule for rejecting predictions that are too uncertain.
  7. Run confirmatory experiments. Test high-priority predictions with measurements that match the intended biological question, including biodistribution, release and toxicity where appropriate.
  8. Document version and changes. Preserve the dataset, preprocessing, model version, feature definitions and decision thresholds so a result can be reproduced after a formulation or process change.

What the field can and cannot claim today

The 2026 review describes prospective clinical validation of AI-guided, patient-specific nanotherapeutic selection as largely absent. It reports that available evidence does not yet establish improved patient outcomes over conventional clinical stratification. That status does not mean no related clinical studies exist; it means the evidence summarized in that review is not sufficient to support a broad claim of patient benefit from individualized AI selection.

The defensible present role is decision support: models can organize complex formulation data, rank experiments and estimate selected endpoints when their data and applicability domain are appropriate. As the intended use moves toward manufacturing release, safety or dose decisions, pharmacokinetic extrapolation or an individual treatment recommendation, the required validation, uncertainty analysis, documentation and human oversight become substantially stronger.

Bottom line for readers evaluating a model

Ask what was predicted, not whether the model is described as “AI.” A reliable estimate of size or encapsulation can save experiments, while a validated PBPK model can help examine exposure under stated assumptions. Neither result proves delivery to the right cells, safe and effective treatment in people or a scalable GMP product. Those claims require independent biological evidence, clinical validation and manufacturing controls that a predictive model can inform but cannot replace.
